Uitgangsvraag

Wat is het optimale periprocedurele beleid bij patiënten die een combinatie van antitrombotica gebruiken en een electieve invasieve interventie ondergaan?

Aanbeveling

Overweeg - waar mogelijk - om de interventie uit te stellen, tot de indicatie voor de gecombineerde therapie is vervallen.*

Als dit niet mogelijk of wenselijk is: volg voor het bepalen van de stop - en herstarttijden de aanbevelingen zoals die zijn opgesteld voor de individuele antitrombotica:
- DOACs
- TAR
- VKA

Overleg bij twijfel of onduidelijkheid met de voorschrijver(s)/het specialisme waarvoor de antitrombotica geïndiceerd zijn, of de lokale stollingsexpert.

Overwegingen

Balans tussen gewenste en ongewenste effecten

Er zijn geen studies gevonden die voldeden aan de selectiecriteria. Hierdoor konden er geen conclusies opgesteld worden over het effect van het continueren dan wel stoppen van de combinatietherapie (antistolling én plaatjesremmers) op de cruciale uitkomsten (trombo-embolische en bloedingscomplicaties en mortaliteit) en belangrijke uitkomsten (opnameduur, patiënttevredenheid en uitstel van chirurgie) in patiënten die een electieve interventie ondergaan.

De Richtlijn Regional anaesthesia in patients on antithrombotic drugs Joint ESAIC/ESRA guidelines uit 2022 (Kietaibl, 2022) gaat in op de tijdsintervallen die aangehouden kunnen worden voor het stoppen van combinatietherapie. Zij geven hierbij aan dat combinaties van antitrombotica kunnen leiden tot een hoger bloedingsrisico. Tegelijkertijd is het niet duidelijk of deze combinatietherapie ook zou moeten leiden tot andere aanbevelingen voor het tijdstip waarop de antitrombotica gestopt zouden moeten worden. Het lijkt er vooralsnog niet op dat antistollingsmiddelen en plaatjesremmers direct met elkaar interacteren (Valina, 2019 en Kubitza, 2006). Wanneer antitrombotische geneesmiddelen worden gecombineerd, wordt in de literatuur gesteld dat het therapievrije tijdsinterval van het geneesmiddel met het langste interval moet worden aangehouden (Kietaibl, 2022).

In de Regional Anesthesia in the Patient Receiving Antithrombotic or Thrombolytic Therapy: American Society of Regional Anesthesia and Pain Medicine Evidence-Based Guidelines (Horlocker, 2018) wordt gesuggereerd dat de combinatie van NSAID’s met ongefractioneerde heparine, LMWH, orale antistollingsmiddelen of trombolytica het aantal spontane bloedingen verhoogt. Dit geldt ook voor de combinatie van aspirine en heparine. Zij bevelen daarom aan om voorzichtig om te gaan met neuraxiale technieken bij patiënten die NSAIDs gebruiken in combinatie met andere plaatjesremmers, orale antistolling, LMHW of ongefractioneerde heparine (Horlocker, 2018).

Er is dus geen bewijs beschikbaar over de specifieke situatie van gecombineerde antitrombotica. Daarmee is er geen grond voor het afwijken van de aanbevelingen zoals die gelden voor individuele antitrombotica, in ieder geval niet voor de situatie waarin het individuele antitromboticum gestopt wordt. De geldende aanbevelingen laten de individuele antitrombotica stoppen op farmacokinetische gronden, dus zoveel tijd tevoren dat het effect is uitgewerkt. Het is dus niet rationeel om bij gecombineerde antitrombotica een langere stoptermijn te overwegen: ook bij combinatietherapie wordt elk middel volgens zijn eigen tijdsinterval gestopt. Het is wel belangrijk allereerst te overwegen of de interventie uitgesteld kan worden (gecombineerde antitrombotische therapie is immers in principe tijdelijk).

De enige situatie waarin bovenstaande niet voldoet, is wanneer een enkel antitromboticum (andere middelen dan hierboven genoemd) gecontinueerd kan worden als de patiënt een interventie krijgt. Als de interventie niet uitgesteld kan worden, is ter overweging om één van beide middelen wel te onderbreken. Bij deze keuze moet de voorschrijver betrokken worden.

Voor NSAID’s, dipyridamol, metamizol en SSRI’s gelden geen beperkingen. Deze middelen kunnen ook in combinatie gecontinueerd worden en dan is het niet nodig om de interventie uit te stellen.

Kwaliteit van bewijs

Er zijn geen studies gevonden die voldeden aan de selectiecriteria. De overall kwaliteit van bewijs kon daarom niet bepaald worden.

Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)

Er zijn geen data over de voorkeur van patiënten in de perioperatieve setting. In de populatie patiënten met atriumfibrilleren (AF) zijn er wel studies die laten zien dat patiënten meer waarde hechten aan het voorkomen van een herseninfarct dan aan het voorkomen van een majeure bloeding (Loewen, 2017). Het ligt dus in de rede dat bij gecombineerde antitrombotica, die alleen voorkomen bij complexe patiënten, de afweging tussen risico op bloeding en risico op trombose expliciet in de spreekkamer wordt gemaakt en dat samen met de patiënt een besluit wordt genomen.

Kostenaspecten

De werkgroepleden kennen geen kosteneffectiviteitsstudies op dit onderwerp. Nu er geen studies zijn gevonden over effectiviteit en veiligheid kan er geen uitspraak gedaan worden over kosteneffectiviteit. Zowel bloedingen als trombotische complicaties kunnen tot (hoge) zorgkosten leiden.

Gelijkheid ((health) equity/equitable)

Het al dan niet onderbreken van de antitrombotica heeft geen effect op de gezondheidsgelijkheid. Het is wel belangrijk dat onderbreken van antitrombotica goed gecommuniceerd wordt met patiënten, zodat de informatie ook begrijpelijk is voor mensen met minder gezondheids- of taalvaardigheid.

Aanvaardbaarheid

Ethische aanvaardbaarheid

Het al dan niet onderbreken of wijzigen van antitrombotica, afhankelijk van het toegenomen bloedingsrisico periprocedureel lijkt voor alle betrokkenen aanvaardbaar. De werkgroep voorziet geen ethische bezwaren voor alle betrokkenen.

Duurzaamheid

In de keuze tussen het onderbreken of continueren van antitrombotica speelt duurzaamheid geen rol.

Haalbaarheid

De huidige adviezen voor perioperatief onderbreken van een antitromboticum sluiten aan bij de zorg die reeds geïmplementeerd is voor patiënten die orale antistollingsmiddelen of plaatjesremmers gebruiken. De aanbevelingen lijken daardoor vergelijkbaar en dus haalbaar te zijn in de praktijk.

Rationale van de aanbeveling: weging van argumenten voor en tegen de interventies

Staken van antitrombotica is gebruikelijk rondom (electieve) interventies om zo het bloedingsrisico te reduceren. Anderzijds moet voorkomen worden dat patiënten tijdens het staken van de antitrombotica trombo-embolische problemen ondervinden.

Uit de literatuurstudie zijn geen relevante studies gekomen die voldoen aan de selectiecriteria. Er is dus geen bewijs om een andere aanpak aan te bevelen dan wordt aanbevolen voor de individuele antitrombotica. Het voorstel zou daarom zijn om interventies bij patiënten met gecombineerde antitrombotica zo mogelijk uit te stellen tot de indicatie voor één van beide vervallen is, waarbij overleg met de patiënt belangrijk is Als dat niet mogelijk of wenselijk is, kunnen de richtlijnen voor ieder individueel antitromboticum gebruikt worden voor het bepalen van stop- en herstart beleid van gecombineerde antitrombotica. Hierbij is altijd overleg nodig met de voorschrijver en de patiënt.

Eindoordeel:

Sterke aanbeveling om de aanbevelingen te volgen zoals die zijn opgesteld voor de individuele antitrombotica.

Achtergrond

Patients often use combinations of antithrombotic medications. It is unknown what the best and safest management is when these antithrombotic medications have to be interrupted because of an elective (surgical) procedure. We performed an extensive literature search to inform the best periprocedural management; that is, with the lowest risk of thromboembolic or bleeding complications.

Relevant outcome measures

The guideline panel considered thromboembolic complications (venous as well as arterial), major bleeding, CRNMB and mortality as critical outcome measures for decision making; and length of hospital stay, patient satisfaction and postponement of surgery as important outcome measures for decision making.

The guideline panel defined the outcome measures as follows:

Major bleeding: fatal bleeding, and/or symptomatic bleeding in a critical area or organ, such as intracranial, intraspinal, intraocular, retroperitoneal, intra-articular or pericardial, or intramuscular with compartment syndrome, and/or bleeding causing a fall in hemoglobin levels of 1.24 mmol/L (20 g/L or greater) or more, or leading to a transfusion of 2 U or more of whole blood or red cells, as defined by International Society on Thrombosis and Haemostasis (ISTH).
Clinical relevant non-major bleeding: Any sign or symptom of hemorrhage (e.g., more bleeding than would be expected for a clinical circumstance, including bleeding found by imaging alone) that does not fit the criteria for the ISTH-definition of major bleeding but does meet at least one of the following criteria:
- requiring medical intervention by a healthcare professional;
- leading to hospitalization or increased level of care;
- prompting a face to face (i.e., not just a telephone or electronic communication) evaluation (Kaatz, 2015).

A priori, the guideline panel did not define the other outcome measures listed above but used the definitions used in the studies.

The working group defined the following as a minimal clinically (patient) important difference:

Net clinical benefit, VTE, major bleeding, mortality, CRNMB, ATE: risk difference of 3%*.
For all other outcome measures, the default thresholds proposed by the international GRADE working group were used as a threshold for clinically relevant differences: a 25% difference in relative risk (RR) for dichotomous outcomes (RR <0.8 or RR >1.25), and 0.5 standard deviations (SD) for continuous outcomes.

*Based on the differences applied in the guidelines on thromboprophylaxis in patients with COVID-19. This working group derived the minimal clinically (patient) important differences from the ACCP (2012).

Search and select (Methods)

The databases Medline (via OVID) and Embase (via Embase.com) were searched with relevant search terms from 2019 until September 12^th 2024. The detailed search strategy is listed under the tab ‘Literature search strategy’. The systematic literature search resulted in 1.719 hits (785 guidelines, 934 systematic reviews).

Systematic reviews were selected based on the following criteria:

Comparison between interruption vs continuation of combination therapy in patients undergoing elective surgery/procedure.
Focusing on >1 type of procedure/surgery.
Assessment of risk of bias of the individual studies.
Description of in- and excluded studies.
Complete search strategy.

Initially, thirteen studies were selected based on title and abstract screening. After reading the full text, all studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and no study was included.

Besides, guidelines were checked whether recommendations on periprocedural management on combination therapy were included. Initially, 23 guidelines were selected based on title and abstract screening. After reading the full text, 21 guidelines were excluded and two guidelines were included. These guidelines can not be included in analysis of the literature, but will be incorporated in the evidence to decision framework (see ‘overwegingen’).

Samenstelling werkgroep

Voor het ontwikkelen van de richtlijnmodule is in 2022 een multidisciplinaire cluster ingesteld. Het cluster Antitrombotisch beleid bestaat uit meerdere richtlijnen, zie hier voor de actuele clusterindeling. De stuurgroep bewaakt het proces van modulair onderhoud binnen het cluster. Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz

Bij de richtlijnmodule voerden de clusterleden conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).

Periprocedureel beleid bij combinatie antitrombotica	geen financiële gevolgen	Hoewel uit de toetsing volgt dat de aanbeveling(en) breed toepasbaar zijn (>40.000 patiënten), volgt ook uit de toetsing dat het geen nieuwe manier van zorgverlening of andere organisatie van zorgverlening betreft, het geen toename in het aantal in te zetten voltijdsequivalenten aan zorgverleners betreft en het geen wijziging in het opleidingsniveau van zorgpersoneel betreft. Er worden daarom geen substantiële financiële gevolgen verwacht.
