Uitgangsvraag
Wat is het algemene beleid ten aanzien van extravasaties?
Aanbeveling
Bij alle extravasaties dienen algemene maatregelen genomen te worden:
- Zet het infuus stop
- Extravasaat zo mogelijk aspireren. Daarna kan de katheter worden verwijderd.
- Leg het aangedane gebied hoog.
- Markeer het gebied en maak een foto.
- Volg lokale afspraken en/of vraag de ziekenhuisapotheker in consult voor eventuele geneesmiddel specifieke adviezen.
Bij graad 1 of 2 extravasaties wordt koelen of verwarmen niet als standaardmaatregel geadviseerd.
Verwarmen kan toegepast worden als het geneesmiddel over een groter oppervlak verspreid, en hierdoor verdund moet worden (zie module 3). Dit is het geval bij een graad 3 of hoger extravasatie en bij enkele specifieke geneesmiddelen/formuleringen.
Koelen dient toegepast te worden bij specifieke geneesmiddelen waarbij wordt verondersteld dat verspreiden ervan schade kan brengen aan het omliggende weefsel zoals cytostatica (zie module Specifieke geneesmiddelgroepen).
Pas hyaluronidase toe bij een graad 4 extravasatie. Overweeg toepassing van hyaluronidase bij een graad 3 extravasatie.
De toepassing van intradermale corticosteroïden bij de behandeling van extravasaties wordt afgeraden. Ook topicale corticosteroiden worden niet aangeraden.
Directe chirurgische interventie bij een extravasatie is geen standaardbehandeling, maar kan worden toegepast bij direct ernstig verlopende extravasaties. De keuze voor de toepassing en de keuze voor de techniek worden gemaakt door de plastisch chirurg.
Consulteer een plastisch chirurg bij een graad 4 extravasatie.
Overwegingen
Balans tussen gewenste en ongewenste effecten
Er is een brede search verricht naar extravasaties, om een overzicht te kunnen geven van beschikbare interventies voor preventie, behandeling van extravasaties en lokaal beleid. Er is gezocht naar alle mogelijke interventies. Er zijn 26 studies met betrekking tot de behandeling van extravasaties in het overzicht opgenomen. De gevonden studies zijn heterogeen in onderzochte interventies, studiepopulaties en eigenschappen van het toegediende middel. Er is gekozen om een overkoepelend overzicht te geven en geen individuele PICO over één interventie uit te werken. De gevonden studies worden beschouwend beschreven en gecombineerd met klinische ervaringen.
Kwaliteit van bewijs
Er is een brede, overkoepelende search uitgevoerd naar alle mogelijke interventies. Er is geen beschrijving van de bewijskracht per uitkomstmaat (per studie, per interventie, per toegediend middel).
De geincludeerde studies betreffen voornamelijk observationele studies met lage incidentie en grote heterogeniteit. Gezien de acute aard van deze complicatie, wat het uitvoeren van grootschalige, gecontroleerde studies bemoeilijkt was dit in de lijn der verwachting. Hierdoor is de huidige evidentie grotendeels gebaseerd op observationeel onderzoek en klinische ervaring (en volgt de uitwerking niet de structuur van tabel).
Aanvullende literatuur
Het toepassen van maatregelen bij een extravasatie heeft als doel complicaties en cosmetische of functionele schade te beperken, en verdere complicaties te voorkomen. Bij een extravasatie kunnen aantal maatregelen genomen worden waaronder algemene maatregelen in combinatie met kompressen, farmacologische maatregelen en chirurgische maatregelen. Op basis van de ernst van de symptomen en de farmacologische/fysisch-chemische eigenschappen van een geneesmiddel kan een inschatting worden gemaakt van de ernst van de extravasatie en de noodzaak om over te gaan op farmacologisch dan wel chirurgisch ingrijpen.
Algemene maatregelen
Algemene maatregelen kunnen bij alle extravasaties toegepast worden. Het doel van deze algemene maatregelen is om de hydrostatische intracapillaire druk te verminderen en de diffusie te bevorderen. Daarnaast bevat het stappen om het aangedane gebied op te volgen om nieuw ontstane weefselschade tijdig te herkennen en zo nodig actie te kunnen ondernemen. In de literatuur is geen onderzoek gedaan naar het effect van deze maatregelen.
Voer de onderstaande handelingen uit (Reynolds, 2014; David, 2020; Kim, 2020; Becker 2011):
1. Zet het infuus stop. Aspireer zo veel mogelijk vloeistof via de katheter, zonder druk uit te oefenen op het gebied.
Het terugtrekken van de vloeistof helpt om de extravasatie te beperken. Door geen druk uit te oefenen, wordt verdere schade aan het weefsel voorkomen, zoals toename van zwelling of schade aan de bloedvaten. In de praktijk resulteert aspiratie doorgaans in het terugtrekken van slechts een beperkte hoeveelheid of geen vloeistof.
2. Verwijder vervolgens de katheter.
De katheter verwijderen gezien deze niet meer bruikbaar is. Plaats elders een nieuwe katheter.
3. Leg het getroffen lichaamsdeel hoog. Dit vermindert zwelling.
Door het getroffen lichaamsdeel hoog te leggen, wordt de terugkeer van vocht (en geneesmiddel) naar de bloedvaten bevorderd en de ophoping van vocht in het weefsel (zwelling) vermindert. Zwelling kan de schade verergeren en de genezing vertragen, dus het verhogen van het lichaamsdeel helpt om dit proces tegen te gaan.
4. Markeer en observeer het gebied. Teken de rand van het aangedane gebied met een stift om veranderingen te volgen. Maak eventueel foto’s.
Door het gebied te markeren, kun je nauwkeurig de veranderingen in de omvang en ernst van de extravasatie volgen. Het visualiseren van de veranderingen helpt om tijdig in te grijpen als de situatie verergert. Foto’s zijn nuttig voor medische documentatie en voor overleg met andere zorgprofessionals.
5. Informeer de behandeld arts. Zorg dat een regie behandelaar het incident beoordeelt.
Het is belangrijk dat de arts of een andere bevoegde zorgverlener het incident beoordeelt om te bepalen welke verdere behandelingen nodig zijn. Zij kunnen de ernst van de extravasatie inschatten en besluiten of er aanvullende medische maatregelen getroffen moeten worden, zoals het toedienen van specifieke geneesmiddelen.
6. Beoordeel symptomen en ernst (gradatie) volgens Tabel 1. Gradering volgens Thigpen (2007) naar Nederlandse vertaling.
Let op: pijn, zwelling, verkleuring, blaren, necrose.
Het beoordelen van de symptomen is nodig om te bepalen hoe ernstig de extravasatie is en welke behandeling nodig is. Het kan helpen bij het identificeren van eventuele complicaties, zoals necrose (dood weefsel), die snel ingrijpen vereisen.
7. Volg de lokale afspraken en/of vraag de ziekenhuisapotheker in consult voor eventuele geneesmiddel specifieke adviezen.
Lokale afs zijn een praktische uitwerking van de richtlijn waarin de lokale verantwoordelijkheden zijn beschreven. De ziekenhuisapotheker kan daarnaast advies geven over geneesmiddelen die mogelijk nodig zijn om de extravasatie te behandelen, zoals antidota of andere therapieën.
8. Documenteer zorgvuldig: tijdstip, aard van de vloeistof, concentratie en ingelopen volume, graad van extravasatie, genomen maatregelen en follow-up.
9. Consulteer een plastisch chirurg tenminste bij graad 4 extravasatie, zie tabel 1 inleiding.

Hieronder wordt dieper ingegaan op de specifieke interventies.
Koelen of verwarmen
Het standaard toepassen van koude of warme, droge, niet-drukkende kompressen wordt niet geadviseerd omdat evidente bewijsvoering hiervoor ontbreekt. Op indicatie kan er worden gekozen voor een droog, schoon, niet-drukkend kompres. De overwegingen voor het toepassen voor warm of koud strategie worden hieronder toegelicht. Bij neonaten dient het gebruik van zowel koude als warme kompressen met grote terughoudendheid te gebeuren vanwege risico op thermische en perfusiegerelateerde schade.
Verwarmen
Verwarmen door middel van warme, droge niet-drukkende kompressen, stimuleert de doorbloeding van de weefsels en daardoor wordt het geneesmiddel sneller opgenomen in het bloed. Door het extravasaat over een groot oppervlak te verdelen, wordt het verdund en neemt de schadelijkheid af. Warme niet-drukkende kompressen kunnen worden toegepast bij hyper- en hypotone oplossingen, zure of alkalische oplossingen, TPV’s en oplossingen die kationen bevatten (zie module Specifieke geneesmiddelgroepen). Tevens is er een plaats bij hoge gradering.
In geval van neonaten met zeer kwetsbare huid dient men er extra alert op te zijn dat het kompres niet te warm is. Er zijn in het verleden incidenten hierdoor ontstaan met secundaire brandwondschade tot geval. Tevens is er een plaats bij hoge gradering.
Koelen
Koelen middels koude, droge niet-drukkende kompressen kan worden toegepast om verspreiding tegen te gaan door de doorbloeding van de weefsels te beperken en zo het extravasaat te lokaliseren. Dit kan toegepast worden indien een geneesmiddel zeer ernstige weefselschade kan veroorzaken en het verspreiden van het extravasaat juist leidt tot meer weefselschade, zoals enkele cytostatica. Daarom moet het extravasaat zoveel mogelijk op één plek gehouden worden en, indien mogelijk, met een antidotum geneutraliseerd worden. Voor geneesmiddel specifieke maatregelen en antidota zie module Specifieke geneesmiddelgroepen.
Een ander voordeel van koude kompressen is dat ze pijn en zwelling verminderen (Bleakley, 2004; Schields, 2023). Lokaliseren is bijvoorbeeld beschreven bij anthracyclinederivaten (Mouridsen, 2007).
Miura (2019) bestudeerde het effect van van koude en warme kompressen op de migratie van neutrofielen bij doxorubicine-extravasaties in muizen. De resultaten toonden aan dat koude kompressen de inflammatoire reactie verminderden door de migratie van neutrofielen te beperken en de expressie van ontstekingsmarkers zoals CD88 en IL-8RA te verlagen. Daarentegen verhoogden warme kompressen de inflammatoire reactie en de vasculaire permeabiliteit.
Er zijn geen gegevens bekend over de frequentie en duur van warme en koude, droge, niet-drukkende kompressen bij een extravasatie. In praktijk wordt een koud of warme kompres 15 tot 20 minuten per keer, 3 tot 4 keer per dag, gedurende 24 tot 48 uur toegepast.
Uit klinische expertise blijkt dat een afwachtend beleid in veel gevallen voldoende kan zijn.
Farmacologisch interventies
Naast algemene maatregelen en het toepassen van kompressen, kunnen bij extravasatie ook geneesmiddelen worden toegepast. Dit betreft zowel algemene (ondersteunende) farmacologische interventies als specifieke antidota. Specifieke antidota worden indicatiegericht ingezet, afhankelijk van het type extravasaat. In module Specifieke geneesmiddelgroepen wordt nader ingegaan op geneesmiddelspecifieke maatregelen.
Hyaluronidase
Hyaluronidase kan toegepast worden als hulpmiddel om het extravasaat over een groter oppervlak te verspreiden en te verdunnen. Hyaluronidase breekt enzymatisch hyaluronzuur in bind- en epitheelweefsel af, waardoor de subcutane weefselverbindingen worden verbroken. Dit maakt het mogelijk dat de geëxtravaseerde vloeistof zich over een 3- tot 5-voudig grotere interstitiële ruimte verspreidt (Razska, 1990). In een dierstudie werd gevonden dat hyaluronidase effectiever was dan zoutoplossing in het voorkómen van necrose na extravasatie van een hypertone zoutoplossing en extravasatie met sclerosans natriumtetradecylsulfaat (Zimmet, 1993). Hyaluronidase kan doorgaans worden toegepast bij hyper- en hypotone oplossingen, zure of alkalische oplossingen, totale parenterale voedingen (TPV’s) en oplossingen die kationen bevatten.
Er zijn geen RCTs gedaan naar de toepassing van hyaluronidase als behandeling van extravasaties. In aantal open label studies met zowel neonaten en volwassenen is toepassing van hyaluronidase bij extravasaties beschreven (Sokol, 1998; Fox 2017; Wiegand, 2010). In een open label studie bij 16 neonaten met extravasatie met TPV, calciumchloride en glucose 10% zorgde hyaluronidase toediening voor vermindering van de klachten bij neonaten (Sageb, 2022). Hyaluronidase werd binnen 8 uur na extravasatie toegediend, bij toediening na 72 uur leek het geen effect meer te hebben. Een case report (Fox, 2017) beschrijft de toepassing van hyaluronidase bij een extravasatie met amiodaron waarbij roodheid en pijn verbeterde. Een case-report beschrijft verbetering van roodheid en zwelling na toepassing van hyaluronidase na extravasatie met hypertone glucose 50% (Wiegand, 2010). Ook wordt hyaluronidase beschreven als behandeling van compartimentssyndroom bij hypertone oplossing zoals mannitol (Kumar, 2023; Kaur, 2021).
Er is geen bewijs bij welke graad van extravasatie hyaluronidase ingezet moet worden. Sageb (2022) suggereert vanaf graad 3. De werkgroep is van mening dat met name bij een graad 4 extravasatie toepassing van hyaluronidase geïndiceerd is. Hoewel evidente bewijsvoering ontbreekt, is de werkgroep van mening dat de kans op mogelijk blijvend letsel bij een graad 4 extravasatie groter is dan te verwachten complicaties ten gevolge van hyaluronidase toediening. Bij een graad 3 extravasatie kan hyaluronidase overwogen worden. Let wel, er zijn extravasaties van specifieke geneesmiddelen waarbij er géén plaats is voor hyaluronidase, ongeacht de graad van de extravasatie, zoals extravasaties van sympathicomimetica. Zie hiervoor module Specifieke geneesmiddelgroepen.
In de literatuur wordt gesproken over een dosering van 150 IE in 1 ml verdeeld over meerdere intradermale injecties van bijvoorbeeld vijf injecties van 0.2 ml rondom het extravasatie gebied (met naald van 25- gauge). Bij een groter gebied kan gekozen worden voor 300IE (Fox, 2017). Bij neonaten wordt een dosering van 15 IE genoemd (Yan, 2014) en bij kinderen vanaf een maand in het geval van cytostatica extravasatie een dosis van 150IE (bron: Kinderformularium).
Subcutaan toegepast hyaluronidase is veilig. Ter vergelijking, in subcutane formuleringen van monoclonale eiwitten zitten hoeveelheden die vele malen hoger zijn dat de bij extravasatie gebruikte doseringen. Voorbeeld: de subcutane formulering van Darzalex® (daratumumab), bevat 30.000 IU hyaluronidase per injectie en wordt in onderhoudsfase maandelijks toegepast.
Uit de klinische praktijk blijkt dat intradermale toepassing van hyaluronidase als pijnlijk ervaren wordt.
Intradermale corticosteroiden
De toepassing van intradermale corticosteroïden bij de behandeling van extravasaties is beperkt. Een retrospectieve cohortstudie geeft aan dat het gebruik van subcutane steroïdinjecties na extravasatie van blaartrekkende chemotherapiemiddelen geassocieerd is met een hogere incidentie van huiddefect gerelateerde chirurgie, wat suggereert dat steroïdinjecties mogelijk niet effectief zijn en zelfs schadelijk kunnen zijn (Ohisa, 2022).
Chirurgisch interventies
Uit de praktijk blijkt dat door het nemen van algemene maatregelen, met name het hoog houden van het aangedane lichaamsdeel, en het toepassen van hyaluronidase de klachten bij extravasatie aanzienlijk afnemen. Dankzij goede managementprogramma’s, zie hoofdstuk 1, die gericht zijn op preventie en vroege detectie van extravasatie, zijn acute preventieve chirurgische interventie tegenwoordig geen standaardbehandeling.
Er zijn geen studies waarin onderzocht wordt naar de effectiviteit van chirurgische interventie ten opzichte van een andere behandeling of een controlegroep. In uitzonderlijke gevallen kan een chirurgische ingreep nodig zijn, met name wanneer er ernstige weefselschade is ontstaan. Gegevens over chirurgische interventies na extravasatie is gebaseerd op case-reports of case-series dan wel retrospectieve cohortonderzoeken. De keuze voor een chirurgische techniek wordt door de (plastische) chirurg gemaakt en hangt af van de ernst, het soort geneesmiddel, de snelheid van ingrijpen en de beschikbare expertise.
Hoewel acute preventieve chirurgische interventies tegenwoordig als niet meer standaard worden toegepast, zijn voor de volledigheid van de richtlijn deze technieken uiteengezet in tabel 3 en (zie tabel Chirurgische interventies).
Compartiment syndroom
Een ernstige, maar zeer zeldzame complicatie van een extravasatie is het compartiment syndroom. Terwijl we altijd bedacht moeten zijn op deze ernstige complicatie wanneer ledematen zijn betrokken, komt het bij extravasatie slechts zelden voor. Dit komt omdat het extravasaat over het algemeen subcutaan loopt, met als gevolg een schade aan het subcutane weefsel en het aantrekken van vocht in dit subcutane weefsel. Beiden zullen als effect een gespannen huid met mogelijk blaarvorming tot gevolg kunnen hebben, zonder dat de onderliggende spiercompartimenten betrokken zijn. Een compartimentsyndroom ontstaat pas wanneer de druktoename zich uitbreidt tot een gesloten spiercompartiment met belemmering van de doorbloeding en functie van de daarin gelegen structuren.
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
De werkgroep heeft geen specifieke literatuursearch uitgevoerd naar de waarden en voorkeuren van patiënten bij de behandeling van extravasaties.
Kostenaspecten
De werkgroep heeft geen specifieke literatuursearch uitgevoerd naar de kostenaspecten van de behandeling van extravasaties. In het algemeen kan gesteld worden dat behandeling van extravasaties gepaard gaat met meer zorgkosten omdat het tijd van zorgpersoneel vraagt. Echter het doel van deze behandeling is het voorkomen van (cosmetische) schade en daaruit volgende kosten voor de samenleving.
Gelijkheid ((health) equity/equitable)
De werkgroep voorziet geen bezwaren met betrekking tot gelijkheid bij de preventie van extravasaties.
Aanvaardbaarheid
Ethische aanvaardbaarheid
Er zijn veel case reports in de literatuur te vinden, maar overkoepelend weinig bewijs voor de gewenste strategie. Omdat een extravasatie ernstige gevolgen kan hebben, maar daarentegen in de praktijk ook vaak zonder restschade herstelt dient het beleid met zorg bepaald te worden om geen onnodige tijd en zorgcapaciteit te verspillen. In geval van ernstige gevallen (hetzij snelle schade, hetzij schadelijke geneesmiddelen) dienen passen maatregelen getroffen te worden.
Haalbaarheid
De aanbevolen interventies zijn grotendeels uitvoerbaar binnen de meeste ziekenhuissettings en vereisen geen structurele aanpassingen van de infrastructuur. Benodigde materialen zoals hyaluronidase en kompressen zijn doorgaans beschikbaar, en protocollen kunnen worden geïntegreerd in bestaande werkprocessen. Dankzij goede managementprogramma’s, die zich moeten richten op preventie en vroege detectie van extravasatie, is chirurgische interventies uitzonderlijk. Implementatie is verder vooral afhankelijk van scholing en bewustwording van zorgprofessionals, en kan ondersteund worden met praktische hulpmiddelen zoals checklijsten, protocollen en training.
Rationale van de aanbeveling: weging van argumenten voor en tegen de interventies
Aanbevelingen met betrekking tot algemeen beleid zijn met name gebaseerd uit observationele, retrospectieve studies en ervaringen uit de praktijk. Neonaten en kinderen vallen binnen de scope, maar vereisen populatie-specifieke interpretatie van aanbevelingen.

Onderbouwing
Extravasation can have both short- and long-term consequences for the patient, making timely and adequate treatment essential. In current practice, there is wide variation in how extravasation is managed, and it is often unclear which interventions are most effective. This inconsistency can lead to delayed recognition, inadequate treatment, and increased risk of complications. This module provides an overview of available treatment options to support a more structured and evidence-informed approach.
Summary of Findings
The level of evidence regarding the outcomes was not graded, as no systemic literature synthesis was performed.
Description of studies
No elaborate literature summary was performed. A total of 26 studies were described in Table 2. These studies address various aspects of the general management of extravasation. Information about the study design, number of participants, type of patients, substance, intervention and control condition, outcome measure(s) en the author conclusion is provided.

A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 1996 to July 9th, 2024, for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from one primary search concept: (1) extravasation. Duplicates were removed using EndNote software.
The detailed search strategy is depicted under the tab Methods. The systematic literature search resulted in 6831 hits. Titles and abstracts were screened using the ASReview software. The settings TF-IDF and Naïve bayes were used [Ly, 2017; Marsh, 2020; Gopalakrishnan, 2017; Thomas, 1997; Sagheb, 2022; Corbett, 2019; Corbet, 2018; Roy, 2022; Dionyssiou, 2011; Molas-Ferrer, 2015; Hermitte-Gandoliere, 2018; Plum, 2017; Ye, 2022; Mu, 1999] was used as prior knowledge for inclusion [Lee, 2018; Cvetkovic, 1999; Julka, 2014; Sabe, 2009; Vadi, 2022] was used as prior knowledge for exclusions. The first 10% of hits were screened by the working group and the guideline methodologist. The remaining articles were subsequently screened by the guideline methodologist, using the following stopping rule: stop after 200 subsequent exclusions.
Studies were thereafter screened for 1) intervention aiming prevention or treatment of extravasation, or local policy 2) primarily investigating extravasation. Based on title and abstract, 612 articles were initially selected. After screening the full-text articles, a total of 64 studies were included, and 548 studies were excluded. The complete overview of excluded studies from ASReview is available upon request from the Knowledge Institute.
During the screening, the types of publications and interventions were considered. Due to the broad clinical question, it is relevant to mention all identified interventions. The module was deliberately kept broad in order to provide an overview, rather than elaborating on individual PICOs. Studies are presented in the table. No level of evidence is described. The considerations provide a deeper analysis of the results and place them in the context of Dutch healthcare.

Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule voerde de werkgroep conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).

Hoewel uit de toetsing volgt dat de aanbeveling(en) breed toepasbaar zijn (>40.000 patiënten), volgt ook uit de toetsing dat het overgrote deel (±90%) van de zorgaanbieders en zorgverleners al aan de norm voldoet en/of het geen nieuwe manier van zorgverlening of andere organisatie van zorgverlening betreft, het geen toename in het aantal in te zetten voltijdsequivalenten aan zorgverleners betreft en het geen wijziging in het opleidingsniveau van zorgpersoneel betreft.
