Uitgangsvraag
Zijn er specifieke maatregelen op basis van fysisch-chemische en farmacologische kenmerken ten aanzien van extravasaties?
Aanbeveling
Behandel een vasopressor extravasatie met een warm, niet-drukkend kompres (zie module Algemene adviezen). Beslis op basis van verbleking op de plaats van extravasatie en het vasoconstrictieve werkingsmechanisme of antagoneren met intradermaal fentolamine noodzakelijk is. Snellere inzet van het antidotum lijkt de gunstigste effecten te hebben.
Behandel een abusievelijke digitale adrenaline injectie (ten gevolge van verkeerd gebruik van een adrenaline autoinjector ‘epi-pen) met een warm, niet-drukkend kompress eventueel gecombineerd met intradermaal fentolamine indien een warm kompres tot onvoldoende lokale perfusie leidt.
In geval van kinderen of neonaten: overweeg topicaal nitroglycerine bij tekenen van distale ischemie (ongeacht gebruik van vasoactieve geneesmiddelen).
Overweeg een warm, niet-drukkend kompres bij extravasaties van hyperosmolaire formuleringen (>600 mOsmol/kg) en sterk zure (pH<5) of sterk basische (pH>9) formuleringen.
Hanteer bij extravasaties van intraveneuze eiwitformuleringen een afwachtend beleid. Beoordeel of het toedienen van intradermaal hyaluronidase wenselijk is op basis van (pijnlijke) zwelling. Beoordeel of toediening van een aanvullende (gedeeltelijke) dosis van het geneesmiddel bij deze indicatie in deze situatie noodzakelijk is omdat de tijd tot absorptie van de subcutane fractie van het geneesmiddel in het bloed (Tmax) minimaal enkele dagen tot een week is.
Weeg bij extravasatie van calcium- en fosfaat-oplossingen af of behandeling met intradermaal hyaluronidase en/of natriumthiosulfaat topicaal wenselijk is. Neem in de afweging mee dat anorganische calcium-zouten een hoger risico geven op een ernstig beloop dan organische calcium-zouten.
Wees bij extravasatie van neuromusculair blokkerende stoffen alert op vertraagde inwerkingssnelheid en een verlengde duur van werking. Monitor op deze effecten en anticipeer op verlengde beademing, sedatie en observatie noodzaak. Waar mogelijk kan het effect met een antidotum worden geantagoneerd.
Dien dexrazoxaan toe bij een anthracycline extravasatie.
Overwegingen
Balans tussen gewenste en ongewenste effecten
Er is een brede search verricht naar extravasaties, om een overzicht te kunnen geven van beschikbare interventies voor preventie, behandeling van extravasaties en lokaal beleid. Er is gezocht naar alle mogelijke interventies. Er zijn 26 studies met betrekking tot de behandeling van extravasaties in het overzicht opgenomen. De gevonden studies zijn heterogeen in onderzochte interventies, studiepopulaties en eigenschappen van het toegediende middel. Er is gekozen om een overkoepelend overzicht te geven en geen individuele PICO over één interventie uit te werken. De gevonden studies worden beschouwend beschreven en gecombineerd met klinische ervaringen.
Kwaliteit van bewijs
Er is een brede, overkoepelende search uitgevoerd naar alle mogelijke interventies. Er is geen beschrijving van de bewijskracht per uitkomstmaat (per studie, per interventie, per toegediend middel).
De geincludeerde studies betreffen voornamelijk observationele studies met lage incidentie en grote heterogeniteit. Gezien de acute aard van deze complicatie, wat het uitvoeren van grootschalige, gecontroleerde studies bemoeilijkt was dit in de lijn der verwachting. Hierdoor is de huidige evidentie grotendeels gebaseerd op observationeel onderzoek en klinische ervaring (en volgt de uitwerking niet de structuur van tabel).
Aanvullende literatuur
Wanneer een geneesmiddel extravasaal lekt, kan dit op verschillende manieren schade veroorzaken aan het omliggende weefsel. In module Algemene adviezen zijn algemene maatregelen besproken die van toepassing zijn bij extravasatie. In dit hoofdstuk gaan we in op specifieke situaties waarin extra voorzichtigheid geboden is, omdat het risico op weefselschade groter is of omdat aanvullende behandelingen noodzakelijk zijn. Deze risicovolle situaties vragen soms om een afwijking van het standaardbeleid. In dit hoofdstuk beschrijven we per categorie: Het (veronderstelde) mechanisme van schade, voorbeelden van geneesmiddelen, eventueel beschikbare antidota of aanvullende interventies en een korte samenvatting of conclusie per categorie. Naast eigenschappen van het geneesmiddel zelf, kunnen ook bepaalde patiëntkenmerken het risico op schade verhogen. Deze situaties worden eveneens besproken.
Schade door extravasatie kan ontstaan door: mechanische schade, bijvoorbeeld bij hoge infusiedruk of een slechte katheterpositie, farmacologische schade, zoals directe cytotoxiciteit of vasoconstrictieve werking en/of fysisch-chemische schade, zoals bij middelen met een afwijkende pH of hoge osmolaliteit. In de onderstaande paragrafen worden risicogeneesmiddelen en hun specifieke eigenschappen besproken die deze vormen van schade kunnen veroorzaken. Per groep wordt de aanbevolen aanpak toegelicht.
1. Vasoconstrictiva
Vasoconstrictoren zijn verbindingen die de vernauwing van bloedvaten veroorzaken door de contractie van de gladde spiercellen in de vaatwand. Voorbeelden van sympaticomimetische vasopressoren zijn (nor)adrenaline, dopamine en fenylefrine. Daarentegen heeft dobutamine, een racemisch mengsel, voornamelijk een effect op de β2-receptor, waarbij de (-)-isomeer een effect heeft op de α1-receptor. Ook methyleenblauw kan tot deze groep gerekend worden omdat uiteindelijk binnen het moleculaire werkingsmechanisme cAMP wordt vrijgezet dat vervolgens α-adrenerge receptoren activeert. Er zijn ook geneesmiddelen die vasoconstrictie kunnen veroorzaken via andere routes dan de sympaticomimetische.
Vasopressine-1 (V1)-agonisten geven met name arteriële vasoconstrictie. Er zijn verscheidene post-marketingmeldingen bij terlipressine van cutane ischemie en necrose en ook ischemie aan de extremiteiten wat ongerelateerd aan de toedieningsplaats was. Dit wordt gerelateerd aan het werkingsmechanisme zelf. Het is dus belangrijk om onderscheid te maken tussen de effecten van het geneesmiddel zelf versus de complicaties bij extravasatie. Voorbeelden van geneesmiddelen die V1-receptoren agoneren zijn vasopressine en terlipressine. Desmopressine is selectiever voor de V2-receptor maar heeft in hogere doses een zwak V1-effect. Angiotensine II kan ook vasoconstrictie veroorzaken (Stefanos, 2023). Angiotensine II veroorzaakt vasoconstrictie door binding aan de AT1-receptor op de gladde spiercellen van bloedvaten, wat leidt tot de activering van de PLC/IP3-DAG-signaleringsroute. Dit verhoogt de intracellulaire calciumconcentratie, wat de contractie van de gladde spiercellen bevordert. Bij extravasatie van angiotensine II of intradermale toediening werd echter geen lokale ischemische weefselschade waargenomen.
1.1 Mechanisme en complicaties
Extravasatie van vasoconstrictiva kan leiden tot ischemie ten gevolge van vasoconstrictieve effecten op de perifere vasculatuur. Daaropvolgende ischemie kan leiden tot weefselnecrose zoals beschreven is in case reports (Kahn, 2002; van der Wal, 2013). Perifere vasoconstrictie wordt met name veroorzaakt door α1-agonisten, i.e. sympaticomimetische vasopressie.
Het verbleken (“blanching”) van de plek van extravasatie is een opvallend verschijnsel bij extravasatie van vasoconstrictoren. Dit is typerend voor de vasoconstrictieve effecten in het weefsel (Bey, 1998). Weefselschade is met name afhankelijk van de duur en in mindere mate van het volume van de geëxtravaseerde vasoconstrictor. Centraal bij de behandeling van extravasatie van een geneesmiddel met vasoconstrictieve eigenschappen staat het beperken c.q. voorkómen van weefselischemie door lokale vasoconstrictie te beperken. Gerandomiseerde studies hierover ontbreken echter.
1.2 Specifieke adviezen
Er zijn geen vergelijkende studies gedaan naar de toepassing van koude versus warme kompressen bij extravasaties met vasopressoren. Veelal werd in de studies een warm, niet-drukkend kompres toegepast. Er wordt verondersteld dat een warm, niet-drukkend kompres vanwege de lokale vasodilatatie tot sneller herstel van de fysiologische osmolaliteit zal leiden vanwege de lokalisatie strategie. Aanvullend hieraan kan een antidotum gegeven worden. Deze worden hieronder besproken.
1.3 Antidota
Fentolamine
Op basis van farmacologische eigenschappen lijkt de (specifieke) α1-antagonist fentolamine een geschikt antidotum bij extravasatie van sympathicomimetische vasopressoren. Dierstudies lieten de effectiviteit op het beperken van weefselschade zien van fentolamine toediening bij extravasatie van bijvoorbeeld dopamine (Bey, 1998). In deze studie werd fentolamine binnen tien minuten na extravasatie toegediend en werd dit vergeleken met fysiologisch zout.
Fentolamine is in Nederland geregistreerd voor de indicatie “Preventie van dermale necrose en het ontstaan van korsten na extravasatie van noradrenaline” waarbij geadviseerd wordt om binnen 12 uur na de extravasatie subcutaan 5-10 mg fentolamine in 10 ml fysiologisch zoutoplossing voor injectie toe te dienen in het gebied waar de extravasatie is opgetreden (SmPC Regitine, 2024). De SmPC van fentolamine vermeldt niet specifiek dat de genoemde indicatie of dosering alleen voor volwassenen geschikt is. Het Nederlandse kinderformularium beschrijft een dosering van 0,1 mg/kg, éénmalig, met een maximum van 10 mg bij kinderen vanaf 1 maand. Ook bij neonaten is fentolamine (succesvol) toegepast in case reports (Subhani, 2001) waarbij de blanching werd opgeheven. Evidence-based doseerschema’s zijn niet aanwezig. Een gebruikte dosering van 0,1 mg/kg (Kleinman, 2020) met een maximum dosering van 2,5 mg fentolamine intradermaal/subcutaan lijkt veilig toegepast te kunnen worden bij neonaten. Op basis van expert opinion is er bij neonaten meer ervaring met nitroglycerine (zie hieronder).
De werking van fentolamine is niet onmiddellijk. Een placebo-gecontroleerde studie (Nodwell, 2003) in de plastische chirurgie liet zien dat fentolamine bijna 90 minuten nodig had om het effect van adrenaline in een vinger te antagoneren terwijl na toediening van fysiologisch zout dit meer dan 6 uur duurde (480 minuten).
Terbutaline
Fentolamine lijkt de voorkeur te hebben bij de behandeling van deze extravasaties. Er zijn ook gegevens over andere behandelopties (Bey, 1998), bij afwezigheid van toegang tot fentolamine. De β2-agonist terbutaline veroorzaakt lokale vasodilatatie na subcutane injectie rondom het aangedane gebied. Echter, een parenterale formulering van terbutaline is in Nederland niet verkrijgbaar waardoor deze optie hier verder niet wordt besproken.
Nitroglycerine
Er is ook ervaring met topicale toediening van nitroglycerine. (Bey, 1998; Stefanos, 2023) Nitroglycerine is een organisch nitraat dat vrije radicalen stikstofoxide laat ontstaan wat er uiteindelijk voor zorgt dat gladde vasculaire spieren zich ontspannen, wat weer resulteert in veneuze (en arteriële) vaatverwijding. De meeste ervaring is opgedaan met een 2% nitroglycerine zalf welke elke 8 uur op de aangedane plek wordt aangebracht. Ook bij neonaten is deze formulering toegepast (in een dosering van 4mm zalf per kg lichaamsgewicht). In Nederland is deze formulering niet commercieel verkrijgbaar. Er is zeer beperkte ervaring beschreven met een alternatieve toedieningsvorm: een nitroglycerine pleister (Generali, 2001). Nadeel hierbij was dat deze case niet een vasoconstrictor extravasatie betrof. Wat deze case report wél liet zien was dat extravasatieschade beperkt werd onder de pleister, maar niet daarnaast. Deze pleister lijkt niet geschikt voor behandeling. In Nederland is echter wel een nitroglycerine spray commercieel verkrijgbaar. Neonaten met digitale ischemie, necrose en zelfs gangreen als gevolg van per ongeluk aangebrachte arteriële cannulatie bij intentie van een intraveneuze cannulatie, kunnen volgens enkele richtlijnen worden behandeld met topicaal nitroglycerine zalf. Deze zalf heeft bewezen effecten om perifere ischemie bij neonaten die niet reageren op conservatieve maatregelen om te keren. Een case series (Mosalli, 2021) laat zien dat toepassing van een nitroglycerine spray, bij afwezigheid van een nitroglycerine zalf, een therapeutische mogelijkheidd is die te overwegen is. Echter, deze case series betreft neonaten met ischemie die niet door extravasatie van een vasopressor werd veroorzaakt. Op basis van deze case series zou aangenomen kunnen worden dat ook voor de behandeling van vasopressor extravasaties, nitroglycerine spray een alternatief zou kunnen zijn echter er zijn geen beschrijvingen bij vasopressor extravasatie beschikbaar. Op basis van expert opinion geeft de NVK aan dat er ervaring is met topicaal nitroglycerine bij tekenen van distale ischemie ongeacht gebruik van vasoactieve geneesmiddelen.
De toepassing van fentolamine lijkt uitsluitend bij extravasatie van sympathicomimetische vasopressoren te zijn onderzocht in cases die passen bij het farmacologische werkingsmechanisme van fentolamine. De behandeling c.q. het antagoneren van vasoconstrictie via andere pathways is nauwelijks beschreven. Diverse internationale medische centra gebruiken fentolamine als eerstekeus, ongeacht het onderliggende mechanisme van vasoconstrictie. Gezien de meeste ervaring met fentolamine, het werkingsmechanisme, de potentiële ernst van de complicaties van vasopressor-extravasatie en de beperkte risico’s van fentolamine, lijkt laagdrempelige inzet hiervan van toepassing. Topicaal nitroglycerine 2% lijkt in principe een mogelijkheid indien fentolamine geen of onvoldoende effect geeft, maar is gecompliceerd doordat het in Nederland niet commercieel verkrijgbaar is.
De dierstudies en case reports (Plum, 2017; Bey, 1998; Stefanos, 2023) schetsen over het algemeen het beeld dat de effectiviteit van de behandeling afhankelijk is van de snelheid waarmee het antidotum ingezet wordt na het optreden van de extravasatie, bij voorkeur binnen 2 uur. Snelle interventie lijkt daardoor gewenst.
1.4 Patiëntkenmerken bij het gebruik van vasoconstrictiva
Bij het beoordelen en behandelen van extravasatie is het essentieel om ook rekening te houden met de klinische toestand van de patiënt. Wanneer vasoconstrictieve geneesmiddelen worden toegediend, is de patiënt in de meeste gevallen hemodynamisch instabiel. Deze instabiliteit heeft directe gevolgen voor het beleid bij extravasatie. Doordat de perifere doorbloeding vaak al verminderd is bij deze patiëntengroep, is het risico op ernstige weefselschade verhoogd. Niet alleen kan de extravasatie zelf snel tot ischemie leiden, maar ook het herstel van de doorbloeding is vertraagd of beperkt, waardoor necrose sneller kan optreden en het effect van lokale interventies verminderd kan zijn.
In deze context is het belangrijk om:
- Snelle herkenning en beoordeling van extravasatie te waarborgen.
- Zo nodig tijdig specialistische ondersteuning in te schakelen (zoals vaatchirurgie, plastische chirurgie of pijnteam).
- Het beleid waar mogelijk proactief aan te passen, bijvoorbeeld door het gebruik van centrale katheter voor vasoconstrictiva te overwegen in risicopatiënten.
- Bij twijfel over de effectiviteit van lokale maatregelen, het risico op uitgebreide weefselschade in de afweging mee te nemen.
Het klinisch beeld van de patiënt moet dus integraal deel uitmaken van de besluitvorming bij extravasatie van vasoconstrictieve middelen.
1.5 Foutieve toediening adrenaline auto-injector
Tot slot, een relatief vaak voorkomende abusievelijke toediening van een sympathicomimetische vasopressor is het injecteren van adrenaline vanuit een auto-injector in de duim van de toediener wanneer deze injector op een onjuiste wijze (i.e. in omgekeerde positie) gehanteerd wordt. Dit is formeel geen extravasatie maar het beleid is wel in lijn met extravasatie literatuur. Er is literatuur beschikbaar over het toedienen van fentolamine, terbutaline, nitroglycerine pasta of een combinatie daarvan, waarbij uiteindelijk in alle gevallen volledig herstel optrad. Veelal werd dit gecombineerd met een , niet-drukkend kompres. Gezien praktische overwegingen lijkt een warm, niet-drukkend kompres als eerste maatregel aangewezen. De werkgroep is van mening dat wanneer de doorbloeding onvoldoende verbetert, behandeling met fentolamine overwogen moet worden.
2. Neuromusculaire blokkerende stoffen
Neuromusculaire blokkerende stoffen (neuromuscular blocking agents, NMBAs) worden met name gebruikt tijdens chirurgische ingrepen voor algehele anesthesie voor het verkrijgen van spierrelaxatie en om kunstmatige beademing te vergemakkelijken.
2.1 Mechanisme en complicaties
Case reports (Nietvelt, 2024) met betrekking tot extravasatie van deze middelen laten zien dat subcutaan een depot gevormd kan worden waardoor de werking minder snel intreedt en tevens langer kan aanhouden. De plaats van extravasatie draagt bij aan de snelheid tot intrede en herstel, waarbij extravasatie in sterker doorbloed weefsel (over het algemeen is de bloedflow door de huid in de bovenste lichaamshelft hogere dan in de onderste lichaamshelft) een relatief snellere intrede en herstel hebben (Iwasaki, 1992; Tsuchida, 1987).
2.2 Specifieke adviezen
Op basis van praktijkervaring wordt geadviseerd om bij (vermoeden op) extravasatie van een NMBA stof tijdens algehele anesthesie om (Nietvelt, 2024):
- Een nieuwe intraveneuze katheter te plaatsen om een veilige anesthesie te garanderen.
- Voorbereid te zijn op een verlengde en onvoorspelbare duur van de neuromusculaire blokkade, met name wanneer extra intraveneuze doseringen zijn toegediend.
- De neuromusculaire blokkade te monitoren, met name via train-of-four (TOF) monitoring.
- Mogelijkheid creëren voor verlengde beademingsduur en sedatie.
- Zonodig het effect van een aminosteroide-NMBA (rocuronium, pancuronium, vecuronium) antagoneren met sugammadex, en van een non-aminosteroide NMBA (suxamethonium) met neostigmine.
- Na extubatie nog frequente monitoring van de neuromusculaire blokkade en verlengd verblijf op de PACU voor minimaal 4-5 uur.
- De plastisch chirurg in consult te vragen.
3. Osmotisch actieve middelen
De fysiologische osmolaliteit van menselijk plasma is ongeveer 300 mOsm/kg.
De osmolariteit heeft betrekking op de totale concentratie osmotisch actieve deeltjes in een oplossing. Dit omvat zowel de permeabele (bijvoorbeeld ureum) als de niet-permeabele deeltjes (bijvoorbeeld natrium, glucose). Of een oplossing schadelijk voor cellen is, is afhankelijk van de niet-permeabele deeltjes omdat alleen deze deeltjes osmotische waterverplaatsing beïnvloeden.
De toniciteit beschrijft het effect van een oplossing op de waterverplaatsing door een celmembraan. Toniciteit wordt bepaald door effectieve osmotische deeltjes – dus deeltjes die niet vrij door de celmembraan kunnen bewegen en daardoor waterverplaatsing veroorzaken. Het kan dus voorkomen dat een oplossing hyperosmolair is, maar niet per se een hypertoon, zoals mannitol 20% (circa 1100 mOsm/kg). Echter, om eenduidigheid te creëren wordt er in dit hoofdstuk vanuit gegaan dat wanneer een oplossing hyperosmolair is, deze mogelijk ook hypertoon is, omdat niet voor alle middelen eenvoudig na te gaan is in hoeverre de osmolaliteit voornamelijk wordt bepaald door permeabele of door niet-permeabele deeltjes in de oplossing.
3.1 Mechanisme en complicaties
Hyperosmolaire middelen vormen een aanzienlijk risico op flebitis en extravasatieletsels. De verplaatsing van vocht over de celmembraan bij blootstelling aan hypertonische middelen kan leiden tot cellulaire dysfunctie en oxidatieve cellulaire stress, wat ontsteking en necrose veroorzaakt. Daarnaast trekt extravasatie van hyperosmolare middelen vocht aan in de weefselcompartimenten, wat kan leiden tot ernstige compressie (Park, 2015) en hypoperfusie, met als gevolg ischemie en ook hypovolemie doordat vocht onttrokken wordt naar de derde ruimte. Hoewel dit zelden voorkomt is monitoring op een (dreigend) compartimentsyndroom aan te raden, met name bij extravasaties met grote volumina of vloeistoffen, waaronder hyperosmolaire, welke vocht kunnen aantrekken. Een compartimentsyndroom ontstaat wanneer de druk binnen een spiercompartiment (een afgesloten ruimte omgeven door fascia) zó hoog wordt dat de bloedtoevoer naar de spieren wordt beperkt en zenuwen bekneld raken. Dit kan leiden tot weefselbeschadiging, ischemie en soms blijvend functieverlies. Hoewel het extravasaat zelf niet direct in de spier gaat, kan het wél de druk in een compartiment verhogen via indirecte zwelling. Subcutane zwelling kan de fascia naar binnen drukken als de ruimte beperkt is, in gebieden waar de huid en onderhuidse weefsels strak over de fascie liggen (bijvoorbeeld onderarm of scheenbeen) zonder de mogelijkheid te kunnen zwellen. Met name wanneer de zwelling circulair is, kan deze als een tourniquet de fascia omsloten spiercompartimenten indirect onder druk zetten. Dit is dus vooral relevant bij grote volumes extravasatie of bij blaartrekkende middelen, die veel lokaal oedeem veroorzaken, met een circulair effect op het ledemaat als resultaat.
Totaal Parentale Voeding (TPV, of Totaal Parenteral Nutrition: TPN) en contrastmiddelen behoren tot de meest hyperosmolaire formuleringen. Geconcentreerde electrolytoplossingen kunnen ook hyperosmolair zijn en er zijn ook geneesmiddelformuleringen met sterk hyperosmolaire eigenschappen, zoals valproïnezuur injectie.
Er wordt verondersteld dat de mate van weefselschade afhankelijk is van de grootte van de afwijking van de osmolaliteit van de formulering ten opzichte van die van het menselijke plasma. Er is geen onderzoek gepubliceerd over de te accepteren grenzen. Over het algemeen wordt wel gesteld dat perifeer toegediende vloeistoffen bij voorkeur een osmolariteit onder de 800-1000 mOsmol/kg mogen hebben omdat een hogere osmolaliteit is geassocieerd met een grotere kans op complicaties waaronder flebitis (Dugan, 2014; Roethlisberger, 2017) en crenatie (i.e. soort inkarteling) van rode bloedcellen. Dit laatste is bij osmolaliteiten van 600 mOsm/kg al waargenomen waardoor sommige richtlijnen een grens van 600 mOsm/kg hanteren (Pittiruti, 2021). Ook kan de grootte van bloedflow door het bloedvat waarin de toediening plaats vindt bepalend zijn voor de grens. In deze richtlijn worden formuleringen met een osmolaliteit van >600 mOsm/kg als ‘sterk hyperosmotisch’ geclassificeerd. Voorbeelden van producten met een osmolaliteit van >600 mOsm/kg zijn onder andere natriumchloride ≥3%, glucose ≥20%, mannitol ≥20%. Het is overigens van belang om op te merken dat in de literatuur zowel van osmolariteit (mOsm/kg) als van osmolaliteit (mOsm/L) gebruik wordt gemaakt. In deze richtlijn is bij het ontbreken van gegevens over de osmolaliteit de osmolariteit gebruikt, waarbij is aangenomen dat deze ongeveer gelijk is aan de osmolaliteit, aangezien bij verdunde oplossingen (zoals in fysiologische omstandigheden) de dichtheid dicht bij 1 kg/L ligt en het verschil tussen beide daardoor verwaarloosbaar is. Met andere woorden, de osmolaliteit is gelijkgesteld aan de osmolariteit.
3.2 Specifieke maatregelen
Er zijn geen vergelijkende studies gedaan naar de toepassing van koude versus warme kompressen bij extravasaties met hyperosmolaire formuleringen. Er wordt verondersteld dat een warm, niet-drukkend kompres vanwege de lokale vasodilatatie tot sneller herstel van de fysiologische osmolaliteit zal leiden vanwege de ‘verspreiden en verdunnen’ strategie.
Hyaluronidase is succesvol toegepast bij hyperosmolaire formuleringen (Wiegand, 2010; Kumar, 2003). Dit is in lijn met de verwachtingen van het werkingsmechanisme dat verdunning van de oplossing tot minder complicaties leidt.
4. Zure en basische formuleringen
De zuurgraad (pH) van bloed ligt in fysiologische omstandigheden tussen de 7,35 en 7,45 en wordt gereguleerd door enkele buffersystemen. Ook de extracellulaire, zoals de interstitiële, vloeistof heeft een vergelijkbare pH.
4.1 Mechanisme en complicaties
Extravasatie van oplossingen met een (sterk) afwijkende pH zal er in resulteren dat er, althans tijdelijk, in het weefsel een afwijking van de fysiologische pH zal plaatsvinden. Hoe groter het verschil tussen de fysiologische pH van het weefsel en de pH van de geëxtravaseerde vloeistof, in des te grotere mate zal de pH in het weefsel (tijdelijk) veranderen. Mogelijk kan ook het volume van de geëxtravaseerde vloeistof een rol spelen bij de duur van deze tijdelijke pH verandering. Om deze reden is het van belang om de weefselperfusie te optimaliseren opdat de duur verkort wordt om de fysiologische pH weer te kunnen instellen door de buffersystemen.
Zure oplossingen kunnen vasoconstrictie, oedeem en ulceraties veroorzaken. Basische, c.q. alkalische oplossingen kunnen eiwitten denatureren. Basische oplossingen hebben relatief een sterker beschadigend effect dan zure oplossingen (Reynolds, 2014).
Er zijn geen studies die de relatie tussen de (afwijking van de) pH van de oplossing en weefselschade bij extravasatie hebben onderzocht. In de praktijk wordt gesteld dat kleine hoeveelheden van intraveneuze oplossingen een vrij brede pH-range mogen hebben van zelfs 3-11. Indien er kans is op instillatie van het subcutane weefsel wordt bij voorkeur een nauwere pH-range gehanteerd: over het algemeen vaak een grens van pH<5 voor zure oplossingen en pH>9 voor alkalische oplossingen als sterk afwijkend gehanteerd (Barone, 2023; Pittiruti, 2021; Roethlisberger, 2017), wat overeenkomt met 1.5-2 log-eenheden afwijking van de fysiologische pH waarbij een kleinere afwijking bij basische oplossingen wordt geaccepteerd.
De pH van een oplossing speelt dus mogelijk een rol bij de mate van weefselschade bij een extravasatie. Hiervoor dient niet alleen naar de pH van de formulering gekeken te worden maar ook naar de mate van verdunning van het geneesmiddel in de dragervloeistof (bijvoorbeeld fysiologisch zout of een glucose-oplossing). De pH van een fysiologisch zout oplossing ligt ongeveer tussen de 5 en 7. De pH van een glucose-oplossing is van zichzelf zuurder met een pH veelal tussen 3,5 en 6,5.
4.2 Specifieke adviezen
Case reports (Russell, 2006; Fox, 2017; Ledingham, 2019; Simoni, 2011; Hoelen, 2007; Fortuna, 2012; Jha, 2017; Bohm, 2012; Nanjappa, 2017; Kanno, 2022; Zenati, 2021) van extravasatie met sterk zure oplossingen beschrijven weefselschade bij extravasatie, waaronder oedeem en soms zelfs necrose. Daarnaast kan de extravasatie pijnlijk zijn.
Warme kompressen werden veelal toegepast in het kader van ‘verspreiden en verdunnen’. Een case report (Russell, 2006) beschrijft de toepassing van hyaluronidase bij een extravasatie met amiodaron waarbij de pijn verbeterde. Bij extravasaties van basische oplossingen werden vesiculaire erupties beschreven veroorzaakt door het neerslaan van eiwitten (Buck, 1993; Armingaud, 2000; Sarica, 2012; Neocleous, 2017; de Zouza, 2002; Powell 2018).
Zeer sterke basische oplossingen kunnen zelfs leiden tot weefselnecrose (Hasija, 2004). Ook bij basische oplossingen lijkt de ‘verspreiden en verdunnen’ strategie het meest logisch om het pH verschil zo snel mogelijk op te lossen, maar vergelijkende studies ontbreken. Het opheffen van een pH-verschil bij extravasatie door het toepassen van een exogeen zuur of base wordt afgeraden omdat dit resulteert in een exotherme chemische reactie waarbij warmte vrijkomt welke de weefselschade kan verergeren (Reynolds, 2014).
5. Eiwitgeneesmiddelen
Er zijn vele intraveneuze formuleringen van eiwitgeneesmiddelen op de markt omdat intraveneuze formuleringen over het algemeen hogere doses van eiwitten toestaan en omdat er onmiddelijke effectiviteit is. Tegenwoordig worden van steeds meer eiwitgeneesmiddelen subcutane toedieningen ontwikkeld. Dit veronderstelt dat extravasatie van dit eiwit an sich niet schadelijk zou zijn bij extravasatie, maar er zijn wel specifieke aspecten om mee rekening te houden bij een eiwitgeneesmiddel.
5.1 Mechanisme en complicaties
Eiwitgeneesmiddelen, voornamelijk monoclonale antilichamen, veroorzaken bij extravasatie over het algemeen geen lokale toxiciteit gezien de specificiteit van het geneesmiddelen tegen een receptor die veelal niet in de huid lokaal aanwezig is. Uitzonderingen hierop zijn de antbody-drug conjugates, dit is een aparte categorie, omdat hier een cytostaticum aan het eiwit gekoppeld is. Bovendien kan de hoge eiwitconcentratie op de extravasatieplaats vocht aantrekken, waardoor het risico op een dreigend compartimentsyndroom vergroot door indirecte zwelling. Een toenemend aantal monoclonale antilichamen is als subcutane formulering beschikbaar en illustreert de lokale tolerantie. Echter, subcutane formuleringen van eiwitgeneesmiddelen, met name monoclonale antilichamen, bevatten hyaluronidase (veelal in een concentratie van ongeveer 2.000 IU/mL) om toediening van een relatief groot injectievolume binnen een korte tijd mogelijk te maken. Intraveneuze formuleringen bevatten daarentegen geen hyaluronidase. Hierdoor kan extravasatie van een intraveneus preparaat leiden tot aanzienlijke lokale zwelling.
5.2 Specifieke adviezen
Op theoretische grond kan hyaluronidase toegediend worden om de zwelling te verminderen en de lokale druk in het weefsel te verlagen, welke in 5.1. werd beschreven. Gebruik van een koud, niet-drukkend kompres om te lokaliseren of een warm, niet-drukkend kompres om te verspreiden lijkt niet noodzakelijk. Het is belangrijk om te realiseren dat er bij extravasatie sprake is van een substantieel veranderde farmacokinetiek (en daarmee ook farmacodynamiek). Monoclonale antilichamen worden na subcutane toediening via de lymfe geabsorbeerd, waarbij de maximale plasmaconcentratie doorgaans pas na ongeveer een week wordt bereikt. Bij Fc-fusie-eiwitten is deze tijd korter, gemiddeld een halve week. Dit is van toepassing voor alle eiwitten met een grootte van meer dan 16kDa (Jarvi, 2021). Dit heeft implicaties voor het moment waarop het geëxtravaseerde eiwitgeneesmiddel werkzaam wordt. Het is daarom belangrijk om te beoordelen of onmiddellijk effect van het geneesmiddelvereist is en of een substantieel deel van de dosis extravasaal is terechtgekomen. In acute situaties kan op basis daarvan overwogen worden om (een deel van) de dosering opnieuw toe te dienen. Deze afweging dient per geneesmiddel, per indicatie en per patiënt te worden gemaakt.
6. Calcium-oplossingen
Parenterale calciumoplossingen zijn beschikbaar in diverse zout-vormen, zoals organische (bijvoorbeeld calciumgluconaat) en anorganische (bijvoorbeeld calciumchloride).
6.1 Mechanisme en complicaties
Extravasatie van een calcium-oplossing (Rumancik, 2020) kan leiden tot een iatrogene calsinosis cutis, ofwel kalkdepositie in de huid (cutis en subcutis). Calcium kan neerslaan in de huid waar het een onoplosbaar zout vormt waarbij ook mogelijk intracellulair calcium wordt vrijgezet. In dierproeven (Leonard, 1972) worden namelijk calciumdeposities gezien die meer calcium bevatten dan de oorspronkelijke hoeveelheid van het extravasaat. Collageen faciliteert de vorming van de (sub)cutane calciumfosfaat afzetting mogelijk (Moss, 2006; Goldminz, 1988).
Er zijn diverse case reports die het ontstaan van kalkdeposities in de huid na extravasatie van een calcium-oplossing beschrijven. Na een initiële roodheid en zwelling kan de calcinose tot twee weken na de extravasaties ontstaan als geel-witte pustels, plakken of nodules eventueel met necrose (Rumancik, 2020). In een studie met 103 neonaten werd een mediane tijd tot ontstaan van de calcinosis cutis van 7.7 dagen gezien met een range van 5-11 dagen (Mu, 1999). Extravasatie van anorganische calciumzouten (zoals calciumchloride) lijkt een ernstiger beloop te hebben dan van organische calciumzouten (zoals calciumgluconaat) omdat de ionen van anorganische calciumzouten makkelijker dissociëren (Heckler, 1976; Semple, 1996). Een artikel waarin 60 neonatale extravasatie cases van calciumgluconaat worden besproken beschrijft dat in deze populatie calciumdeposities optraden welke na 3-6 maanden waren genezen. Er worden verschillende farmacologische behandelingen uitgevoerd maar deze worden niet geëvalueerd. In de helft van de patienten zijn chirurgische interventies uitgevoerd (Pacheco Compaña, 2018).
6.2 Specifieke adviezen
Een dierstudie met muizen heeft het effect onderzocht van natriumthiosulfaat, hyaluronidase, triamcinolonacetonide en fysiologisch zout als antidotum bij een calciumgluconaat extravasatie op kalkdeposities. Hierbij werd gekeken naar zichtbare lesies, radiografische controle en histologische controle. Toediening van natriumthiosulfaat had overall het beste resultaat, gevolgd door hyaluronidase. Triamcinolonacetonide had relatief het minste effect. Daarentegen liet een andere dierstudie in konijnen wel een gunstig beloop zien van triamcinolonacetonide ten opzichte van afwachtend beleid (Ahn, 1997). Gezien nadelige effecten van intradermale toediening van triamcinolonacetonide, zoals atrofie, lijkt dit antidotum minder geschik (Pacheco Compaña, 2018).
Een dierstudie met ratten heeft aangetoond dat extravasatie met calciumchloride necrose veroorzaakt en dat de ernst is geassocieerd met de calcium-concentratie van het extravasaat (Heckler, 1976). Deze studie liet zien dat toediening van hyaluronidase verdund in fysiologisch zout effectief was in het voorkomen van necrose. Men gaf hierbij aan effectiviteit te zien wanneer een antidotum volume van minimaal tien maal het geextravaseerde volume werd toegediend. Een studie in konijnen bevestigt de effectiviteit van hyaluronidase (Laurie, 1984).
Thiosulfaat wordt gecombineerd met calciumzouten om calciumthiosulfaat te vormen, een veel beter oplosbare molecuul, wat de heropname vergemakkelijkt. De oplosbaarheid neemt toe met een factor tussen de 250 en 100.000 in vergelijking met andere calciumzouten (Wolf, 2008; Yatzidis, 1985).
Ervaring met deze antidota bij extravasatie met calcium-oplossingen in mensen is beperkt.
Één case series (Casanova, 2001) beschrijft de toediening van hyaluronidase bij twee neonaten met een calcium-oplossing extravasatie. Bij de ene neonaat werd dit toegepast binnen 5 uur na extravasatie, bij de ander binnen 12 uur, waarbij bij de eerste positieve effecten werden gezien en bij de laatste necrose.
Een case report (Raffaella, 2009) van een 5-jarig kind met calcinosis cutis ten gevolge van een calciumgluconaat extravasatie met roodheid, pijn, oedeem en tenslotte ulceratie gedurende bijna drie maanden beschrijft de toepassing van intraveneus natriumthiosulfaat (435 mg/kg, drie keer per week) vanaf dag 100 na de extravasatie. Drie maanden na de start van de natriumthiosulfaat toediening was de plek volledig hersteld. De effectiviteit van intraveneus natriumthiosulfaat bij andere oorzaken van calcinosis cutis is zeer beperkt (Song, 2018; Howard, 2020) maar het is niet bekend in hoeverre de bevindingen uit deze studies te extrapoleren zijn naar de vormen die veroorzaakt worden door extravasatie van een calcium-oplossing.
Bewijs met betrekking tot intralesionale toediening van natriumthiosulfaat bij calcinosis cutis zijn er alleen bij patiënten met dystrofische calcinosis cutis. In een dubbelblinde, placebo-gecontroleerde studie met n=4 personen werd gezien dat een maandelijkse intralesionale toediening van een 4% natriumthiosulfaat oplossing in een dosering van 0,1 mL/cm2 met een maximum van 0,4 mL totaal (16 mg) bij (slechts) één van de vier personen effectief was (Winter, 2020).
7. Fosfaat-oplossingen
Fosfaat wordt in parenterale oplossingen toegediend met als doel om fosfaat te suppleren maar kan ook voorkomen als tegen-ion in oplossingen met andere elektrolyten. Voorbeelden van fosfaat-bevattende parenterale oplossingen zijn: dinatriumwaterstoffosfaat, glycerofosforzuur en kaliumdiwaterstoffosfaat.
7.1 Mechanisme en complicaties
Extravasatie van een fosfaat-oplossing kan leiden tot de vorming van gele plaques en ulceraties van weefsel. Waarschijnlijk bestaan deze plaques uit calciumfosfaat neerslagen ten gevolge van intracellulaire vrijzetting van calcium in aanwezigheid van extracellulair (geëxtravaseerd) fosfaat. Dit proces vertoont overeenkomsten met het ontstaan van calcinosis cutis bij de extravasatie van calcium-oplossingen (Verykiou, 2017; Xie, 2021).
7.2 Specifieke adviezen
Een case report (Xie, 2021) beschrijft een gunstig effect van topicale toepassing van een 25% natriumthiosulfaat crème (tweemaal daags gedurende drie weken) op kalkdeposities na extravasatie van een fosfaat-oplossing in vergelijking met natuurlijk beloop.
8. Cytostatica
Cytostatica zijn geneesmiddelen die interfereren met de celdeling waardoor de celdeling stopt en een cytotoxisch effect wordt veroorzaakt. Cytostatica werden van oudsher onderverdeeld in blaarvormend (‘vesicant’), irriterend (‘irritant’) en niet-blaarvormend (‘non-vesicant’), maar dit wordt in mindere mate toegepast omdat het onderscheid niet altijd duidelijk is tussen deze groepen. Welke schade optreedt kan van verschillende factoren afhankelijk kan zijn, waaronder de concentratie (bijvoorbeeld bij cisplatine).
8.1 Anthracycline
Extravasatie van anthracyclines kan een zeer ernstig beloop hebben met weefselschade die binnen enkele uren tot dagen kan ontstaan en kan verergeren gedurende weken met risico op pijn, infecties, gewrichtsbeschadiging, zenuwbeschadiging en zelfs permanente dysfunctie en cosmetische schade (Mouridsen, 2006; Langer, 2009). In twee prospectieve (single-arm) studies is aangetoond dat toediening van dexrazoxaan, zelf ook een topoisomerase-II remmer en ijzer-chelator, bij slechts 1 van de 54 patienten (1.8%) resulteerde in necrose waarvoor chirurgie geindiceerd was. Op basis van andere literatuur wordt verondersteld dat dit percentage zonder dexrazoxaan hoger zou zijn. Op basis van deze twee studies is dexrazoxaan geregistreerd “ in volwassenen voor de behandeling van anthracycline-extravasatie”. De indicatie is anthracyline breed, maar in de twee betreffende registratiestudies zat slechts één patient (1.8%) met daunorubicine en één patient (1.8%) met gepegyleerd liposomaal daunorubicine waarbij het merendeel van de patiënten epirubicine (54.4%) of doxorubicine (42.1%) had ontvangen. In kinderen ontbreken studies betreffende de effectiviteit van dexrazoxaan bij extravasatie van een anthracycline.
Ondersteunend aan de dexrazoxaan kan een kompres worden gebruikt. In de twee registratiestudies was lokale koeling toegestaan, maar er is in de publicaties niet aangegeven in hoeverre dit ook daadwerkelijk is toegepast. Een preklinische studie (Dorr, 1985) in muizen heeft aangetoond dat koelen van de huid minder doxorubicine-geïnduceerde huidtoxiciteit gaf, terwijl verwarmen geen effect hierop had. Een studie in varkens (Harwood, 1987) liet zien dat verlengde koeling. Echter niet alle (preklinische en klinische) studies konden het effect van koelen bevestigen (Soble, 1987; Langer, 2001; Larson, 1982; Larson, 1985; Barlock, 1979; Harwood, 1994). Data met verwarmen is beperkt maar lijkt wel meer geassocieerd te zijn met ernstige uitkomsten, zoals necrose en blaarvorming (Reilly, 1977; Snyderman, 1986).
Overige maatregelen die specifiek bij een anthracycline extravasatie onderzocht zijn zijn onder andere flushen (Gault, 1993; Scuderi, 1994; D'Andrea, 2004; Khan, 2002), toediening van corticosteroïden, bicarbonaat, DMSO, hyaluronidase, alfa-tocoferol en groeifactoren (Langer, 2009). Zij concludeerden dat dat flushen ongetwijfeld een deel van de geëxtravaseerde vloeistof kan terughalen maar dat dit intensief en invasief is met onzekerheid over de hoeveelheid vloeistof die nog achter blijft. Topicaal DMSO, als verhoger van de permeabiliteit van de huid waardoor verhoogde absorptie zal plaatsvinden en tevens scavenger van vrije radicalen die door anthracyclines ontstaan, kan mogelijk effect hebben maar de gepubliceerde case reports en kleine studies bieden onvoldoende mogelijkheid om effectiviteit vast te stellen en routinematig toe te passen (Langer, 2009). Een studie met topicaal DMSO (in combinatie met koelen) waarin 53 evalueerbare patienten zaten met een anthracycline extravasatie had als resultaat dat slechts één patient blaren had (Bertelli, 1995).
8.2 Overige oncolytica
Voor de overige oncolytica is minder bewijs in de literatuur beschikbaar voor adequate behandelingsstrategieën bij extravasaties. De ESMO publiceerde in 2012 een behandelalgoritme voor cytostatica extravasaties, waarbij de potentiele eigenschappen van het geneesmiddel op weefselschade (onderverdeeld als ‘vesicant’, ‘irritant’, of nonvesicant’) de strategie bepaalt (Pérez Fidalgo, 2012). In geval van een ‘nonvesicant’ wordt gekozen voor lokalisatie met behulp van een koud, niet-drukkend kompres. Bij een ‘vesicant’ of ‘irritant’ extravasatie is het beleid (lokaliseren m.b.v. een koud, niet-drukkend kompres of verspreiden m.b.v. een warm, niet-drukkend kompres) afhankelijk van de klasse van het cytostaticum.
Voor taxanen en vinca-alkaloiden wordt geadviseerd om een warm, niet-drukkend kompres toe te passen en hyaluronidase. Antidota worden geadviseerd voor anthracycline en mitomycine C (topicaal DMSO) (Alberts, 1991). Ook de recentere richtlijn van de Japanse oncologische beroepsverenigingen hanteert min of meer vergelijkbare aanpak van extravasaties (Matsumoto, 2024). In Nederland zijn specifieke adviezen opgenomen in de Crashkaart cytostatica die wordt onderhouden door de NVZA. Over het algemeen wordt voor cytostatica de lokalisatie strategie toegepast en aangepast op basis van case reports en commentaar uit het veld.
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
De werkgroep heeft geen specifieke literatuursearch uitgevoerd naar de waarden en voorkeuren van patiënten bij de behandeling van extravasaties.
Over het algemeen geldt dat voor patiënten comfort, pijnvermindering en het voorkomen van langdurige huidschade of chronische pijn het belangrijk is. Patiënten willen graag begrijpen wat het doel van de behandeling is.
Hierbij dient opgemerkt te worden dat de bewijslast achter de meest voorkomende maatregelen (warme en koude kompressen) zeer beperkt is en voornamelijk op theoretische grond is gebaseerd. Echter gezien de zeer lage belasting van deze maatregelen die niet-invasief is, is dit tegen de achtergrond van voorkomen van blijvende schade te verantwoorden als primaire maatregel. Ook een maatregel als intradermaal fentolamine heeft een beperkte sterkte van het klinische bewijs maar is op basis van het werkingsmechanisme te overwegen. Gezien potentieel ernstig beloop van een vasopressor extravasatie wordt deze behandeling ter overweging geadviseerd.
Kostenaspecten
De werkgroep heeft geen specifieke literatuursearch uitgevoerd naar de kostenaspecten van de behandeling van extravasaties. In het algemeen kan gesteld worden dat behandeling van extravasaties gepaard gaat met meer zorgkosten omdat het tijd van zorgpersoneel vraagt. Echter het doel van deze behandeling is het voorkomen van (esthetische) schade en daaruit volgende kosten voor de samenleving.
Gelijkheid ((health) equity/equitable)
De werkgroep voorziet geen bezwaren met betrekking tot gelijkheid bij de preventie van extravasaties. De toepassing van uitzonderingen op het beleid – zoals antidota – mag niet afhankelijk zijn van de locatie of beschikbaarheid van middelen. Gelijke toegang tot de geneesmiddelen waar het meeste bewijs voor is (hyaluronidase fentolamine) moet nagestreefd worden binnen regio's om ongelijkheid in zorguitkomsten te voorkomen.
Aanvaardbaarheid
Ethische aanvaardbaarheid
De inzet van potentieel toxische of experimentele middelen vraagt een zorgvuldige afweging. Toepassing moet altijd plaatsvinden op basis van gedeelde besluitvorming, waarbij risico’s en onzekerheden helder worden gecommuniceerd aan de patiënt of diens vertegenwoordiger.
Duurzaamheid
Door selectief gebruik van geneesmiddelen zoals hyaluronidase en door standaardisatie van indicaties, wordt verspilling van geneesmiddel en onnodige belasting van de zorgketen voorkomen.
Haalbaarheid
De aanbevolen uitzonderingen, zoals het gebruik van hyaluronidase, zijn technisch uitvoerbaar binnen ziekenhuizen waar dit middel beschikbaar is. De werkgroep voorziet dat beschikbaarheid en voorraadbeheer per instelling kunnen verschillen, wat afstemming vereist. De maatregelen kunnen bewerkelijk zijn en kosten tijd, maar zijn passend binnen de huidige zorg. Scholing en duidelijke protocollen kunnen de toepassing van uitzonderingen verder ondersteunen.
Rationale van de aanbeveling: weging van argumenten voor en tegen de interventies
Aanbevelingen zijn opgesteld op basis van de in de literatuur meest beschreven risicovolle geneesmiddeleigenschappen en –formuleringen. Er is rekening gehouden met de beschikbaarheid van aanbevolen behandelingen op de Nederlandse markt.
Neonaten en kinderen vallen binnen de scope, maar vereisen populatie-specifieke interpretatie van aanbevelingen.

A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 1996 to July 9th 2024 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from one primary search concept: (1) extravasation. Duplicates were removed using EndNote software.
The detailed search strategy is depicted under the tab Methods. The systematic literature search resulted in 6831 hits. Titles and abstracts were screened using the ASReview software. The settings TF-IDF and Naïve bayes were used [Ly, 2017; Marsh, 2020; Gopalakrishnan, 2017; Thomas, 1997; Sagheb, 2022; Corbett, 2019; Corbet, 2018; Roy, 2022; Dionyssiou, 2011; Molas-Ferrer, 2015; Hermitte-Gandoliere, 2018; Plum, 2017; Ye, 2022; Mu, 1999] was used as prior knowledge for inclusion [Lee, 2018; Cvetkovic, 1999; Julka, 2014; Sabe, 2009; Vadi, 2022] was used as prior knowledge for exclusions. The first 10% of hits were screened by the working group and the guideline methodologist. The remaining articles were subsequently screened by the guideline methodologist, using the following stopping rule: stop after 200 subsequent exclusions.
Studies were thereafter screened for 1) intervention aiming prevention or treatment of extravasation, or local policy 2) primarily investigating extravasation. Based on title and abstract, 612 articles were initially selected. After screening the full-text articles, a total of 64 studies were included, and 548 studies were excluded. The complete overview of excluded studies from ASReview is available upon request from the Knowledge Institute.
During the screening, the types of publications and interventions were considered. Due to the broad clinical question, it is relevant to mention all identified interventions. The module was deliberately kept broad in order to provide an overview, rather than elaborating on individual PICOs. Studies are presented in the table. No level of evidence is described. The considerations provide a deeper analysis of the results and place them in the context of Dutch healthcare.

Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule voerde de werkgroep conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).

Hoewel uit de toetsing volgt dat de aanbeveling(en) breed toepasbaar zijn (>40.000 patiënten), volgt ook uit de toetsing dat het overgrote deel (±90%) van de zorgaanbieders en zorgverleners al aan de norm voldoet en/of het geen nieuwe manier van zorgverlening of andere organisatie van zorgverlening betreft, het geen toename in het aantal in te zetten voltijdsequivalenten aan zorgverleners betreft en het geen wijziging in het opleidingsniveau van zorgpersoneel betreft. Er worden daarom geen substantiële financiële gevolgen verwacht. |
