Uitgangsvraag

Wat is de plaats van transcatheter arteriële embolisatie bij een recidief bloeding na initiële endoscopische behandeling van een ulcusbloeding?

Aanbeveling

Aanbeveling-1

Verricht geen preventieve embolisatie na een succesvolle initiële endoscopische hemostase bij patiënten met een acute bovenste tractus digestivus bloeding.

 

Aanbeveling-2

Verricht bij een recidief bovenste tractus digestivus bloeding een tweede gastroscopie tenzij er al een over-the-scope clip (OTSC) geplaatst is.

 

Verricht een embolisatie bij een recidief bloeding met een OTSC in situ of na de tweede gastroscopie.

 

Plaats laagdrempelig een hemoclip ter markering (indien OTSC niet is geplaatst) ten behoeve van een eventuele embolisatie.

Overwegingen

Balans tussen gewenste en ongewenste effecten

Een systematische review is uitgevoerd om het effect te vergelijken tussen een herhaalde endoscopische behandeling of radiologische transcatheter arteriële embolisatie (hierna kortweg embolisatie genoemd) bij een recidief bloeding bij patiënten met een endoscopisch behandelde bloeding van de bovenste tractus digestivus. Er werden geen studies gevonden die aan de PICO criteria voldeden. Daarom kunnen geen conclusies getrokken worden over het effect van embolisatie na het falen van de eerste endoscopische behandeling vergeleken met een herhaalde endoscopische behandeling.

 

Op basis van de PICO-criteria zijn er geen studies gevonden met een directe vergelijking tussen een radiologische behandeling en een tweede/ herhaalde endoscopie. Het behandelalgoritme voor ulcusbloedingen zoals beschreven in de 2017 NVMDL richtlijn Bloedingen tractus digestivus hoeft op dat punt niet aangepast te worden. Echter, de implementatie van de OTSC (ook bij recidief bloedingen), zoals beschreven in module Behandeling met OTSC, heeft wel consequenties voor het algoritme. Indien bij initiële gastroscopie gekozen wordt voor een OTSC en deze blijkt niet succesvol, dan zijn er bij een 2e gastroscopie geen definitieve endoscopische hemostatische opties meer (de OTSC zit immers over het wondbed) en wordt geadviseerd direct te kiezen voor embolisatie (meestal middels coils). Indien de 1e en eventueel 2e gastroscopie niet succesvol blijken is het van belang dat er een radiopake markering geplaatst is ter hoogte van de bloedingsfocus. Dit kan een niet succesvolle OTSC of een hemoclip betreffen (zie Stroomschema Algoritme voor de behandeling van ulcusbloedingen).

 

Een meerfasen CT-angiografie scan kan worden overwogen voorafgaand aan de transarteriële embolisatie, als de toestand van de patiënt dit toelaat, om de vasculaire anatomie in kaart te brengen, te bepalen of sprake is van een actieve bloeding en de locatie van de bloeding nader te bepalen. De nadelen van embolisatie betreffen non-target embolisatie (migratie van bijvoorbeeld een coil) of klinisch-relevante ischemie. Een niet te stoppen gastro-intestinale bloeding daarentegen is een levensbedreigende situatie, die medische behandeling vergt.

 

Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)

Voor patiënten is het vooral belangrijk dat een recidief bloeding snel en effectief wordt behandeld, met een zo klein mogelijke kans op overlijden en opnieuw bloedverlies. Zij willen dat transcatheter arteriële embolisatie wordt ingezet wanneer dit een bewezen en passende vervolgstap is na een mislukte of onvoldoende effectieve endoscopische behandeling. Daarbij hechten patiënten veel waarde aan veiligheid en het voorkomen van ingrijpendere behandelingen, zoals spoedchirurgie. Duidelijke uitleg over de reden voor deze keuze en de risico’s en voordelen en betrokken worden bij de besluitvorming helpt patiënten en naasten om vertrouwen te hebben in de behandeling.

 

Kostenaspecten

Binnen het voorgestelde algoritme voor de behandeling van ulcusbloedingen is het niet mogelijk om de totale kosten exact te berekenen.

 

Een potentieel voordeel van het gebruik van OTSC binnen het algoritme is een vermindering van het aantal endoscopische interventies en een mogelijke verkorting van de opnameduur, hetgeen kan leiden tot een reductie van zorgkosten. Ook kan succesvolle primaire hemostase herhaalde endoscopieën, aanvullende interventies en intensieve zorg voorkomen.

 

Daartegenover staan nadelen in de vorm van hogere directe kosten van de OTSC-set en van alternatieve interventies zoals embolisatie. Tevens brengen herhaalde endoscopieën, inclusief materiaal- en personeelskosten, aanvullende uitgaven met zich mee. Indien hemostase niet direct wordt bereikt, kan dit resulteren in extra ligdagen en in sommige gevallen opname op de intensive care, wat de totale zorgkosten verder verhoogt.

Gelijkheid ((health) equity/equitable)

De interventie leidt niet tot een mogelijke afname van gezondheidsgelijkheid.

 

Aanvaardbaarheid

De interventie lijkt aanvaardbaar voor de betrokkenen. Er zijn geen ethische bezwaren.

 

Duurzaamheid

Bij de interventie spelen de duurzaamheidsaspecten mogelijk een rol. Embolisatie is een techniek waarbij relatief veel materiaal wordt gebruikt en röntgen contrast. Aangezien deze ingrepen vaak met spoed in de dienst plaatsvinden, zijn er ook extra reisbewegingen van de radioloog en röntgenlaboranten. Zie ook de module Behandeling met OTSC.

 

Haalbaarheid

De interventie lijkt haalbaar. De meeste ziekenhuizen hebben een 24/7 endoscopie achterwacht en hiermee de beschikking over spoed endoscopieën. De overige ziekenhuizen hebben afspraken met een verwijscentrum waar ze in de dienst patiënten naar kunnen verwijzen. In alle ziekenhuizen waar interventieradiologen werkzaam zijn is een 24/7 dekking aanwezig. In ziekenhuizen waar geen interventieradioloog werkzaam is, dienen vaste afspraken te zijn waar patiënten naar toe kunnen worden verwezen, die in aanmerking komen voor een transcatheter embolisatie.

 

Rationale van de aanbeveling-1: weging van argumenten voor en tegen de interventies

Op basis van de beschikbare literatuur concludeert de werkgroep dat er geen algemene uitspraak kan worden gedaan of bij een recidief bloeding na een endoscopisch behandeld bloedingsfocus in de bovenste tractus digestivus, direct overgegaan moet worden tot embolisatie of dat een tweede gastroscopie moet worden gepoogd. Voor preventieve embolisatie na een succesvolle initiële gastroscopie lijkt geen plaats (Lau, 2019).

Rationale van aanbeveling-2: weging van argumenten voor en tegen de interventies

Indien er bij de eerste gastroscopie een OTSC is geplaatst, is aanvullende endoscopische behandeling niet mogelijk en zal er bij een recidief bloeding een embolisatie plaats moeten vinden. Indien er bij de eerste gastroscopie geen OTSC is geplaatst, adviseren we een tweede gastroscopie (tenzij de scopist in het verslag van de initiële gastroscopie heeft aangegeven dat er technisch gezien geen endoscopische opties zijn), zo mogelijk met OTSC. Bij een recidief bloeding na de 2e gastroscopie, is de kans op definitieve hemostase middels embolisatie groter en zal over het algemeen hiertoe over worden gegaan.

Onderbouwing

In case of suspected ulcer bleeding, early gastroscopy is indicated. The success rate for achieving endoscopic hemostasis is high, between 75 and 92% (Spiegel, 2003; Zed, 2001; Lau, 1999; Lau 2013). If the patient starts bleeding again (re-bleeding), repeated endoscopic therapy is often attempted. The alternatives are a radiological intervention (transcatheter arterial embolization) or surgery. Surgery is now considered the last resort, because radiological techniques improved in recent years and are readily available. Introduction of the over-the-scope clips (OTSCs) as primary or secondary endoscopic treatment option, as described in module ‘Behandeling met OTSC’, will further impact the sequence of different treatment modalities.

 

We performed a systematic literature search to identify studies evaluating radiological intervention versus repeated endoscopic therapy after unsuccessful initial endoscopic treatment of non-variceal upper gastrointestinal bleeding. Furthermore, we updated the flowchart for the treatment algorithm of upper non-variceal gastrointestinal bleeding, based on the 2017 Dutch guideline ‘Gastro-intestinal bleeding’.

Relevant outcome measures

The guideline panel considered mortality, re-bleeding and clinical success rate as critical outcome measures for decision making; and re-intervention, surgery, blood transfusion length of hospital stay, length of ICU stay as important outcome measures for decision making.

 

A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.

 

The guideline panel defined the following thresholds as a minimal clinically important difference per outcome measure:

  1. mortality: risk difference (RD) ≥ 0.05 (5%);
  2. re-bleeding: risk difference (RD) ≥ 0.20 (20%);
  3. clinical success: risk difference (RD) ≥ 0.25 (25%);
  4. re-intervention: risk difference (RD) ≥ 0.25 (25%);
  5. surgery: risk difference (RD) ≥ 0.25 (25%);
  6. blood transfusion: risk difference (RD) ≥ 0.25 (25%);
  7. length of hospital stay: mean difference (MD) ≥ 1 day;
  8. length of ICU stay: mean difference (MD) ≥ 1 day.

Search and select (Methods)

A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 01-01-2010 to 10-12-2024 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from three primary search concepts: (1) ulcer bleeding; (2) failed gastroscopy/ endoscopy; (3) coiling/ embolization. The detailed search strategy is listed under the tab ‘Literature search strategy’. Duplicates were removed using EndNote software. After deduplication a total of 514 records were imported for title/abstract screening.

 

Initially, 9 studies were selected based on title and abstract screening. After reading the full text, 9 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and 0 studies were included.

Samenstelling werkgroep

Voor het ontwikkelen van de richtlijnmodule is in 2023 een multidisciplinaire werkgroep ingesteld, bestaande uit vertegenwoordigers van relevante specialismen (zie hiervoor de Samenstelling van de werkgroep) die betrokken zijn bij de zorg voor patiënten met (acute) tractus digestivus bloedingen.

De werkgroep besteedde aandacht aan het patiëntenperspectief door het uitnodigen van Patiëntenfederatie Nederland voor de schriftelijke knelpunteninventarisatie. Het verslag hiervan is besproken in de werkgroep. De conceptrichtlijn is tevens ter commentaar voorgelegd aan Patiëntenfederatie Nederland. De paragraaf ‘waarden en voorkeuren van patiënten’ binnen de overwegingen is door Patiëntenfederatie Nederland opgesteld.

 

Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz

Bij de richtlijnmodule voerde de werkgroep conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).

Hoewel uit de toetsing volgt dat de aanbeveling(en) breed toepasbaar zijn (>40.000 patiënten), volgt ook uit de toetsing dat het geen nieuwe manier van zorgverlening of andere organisatie van zorgverlening betreft, het geen toename in het aantal in te zetten voltijdsequivalenten aan zorgverleners betreft en het geen wijziging in het opleidingsniveau van zorgpersoneel betreft. Er worden daarom geen substantiële financiële gevolgen verwacht.
