Uitgangsvraag
Bij welke patiënten met intracranieel meningeoom is na een chirurgische behandeling radiotherapie geïndiceerd om recidief te voorkomen?
Aanbeveling
Pas geen radiotherapie toe bij graad 1 meningeomen, tenzij
- Er een verhoogd moleculair risicoprofiel vastgesteld is (zie onder),
- wanneer een meningeoomrest bij geringe groei tot ernstige klachten leidt, met name bij meningeomen van de schedelbasis (overweeg dan gefractioneerde stereotactische radiotherapie/radiochirurgie).
Bespreek patiënten met een histologisch graad 1 meningeoom EN een moleculair ‘intermediair’ of ‘hoog’ risicoprofiel EN een macroscopische resttumor*, in een neuro-oncologisch/schedelbasis MDO, en overweeg hierbij de volgende opties afhankelijk van het individuele patiëntprofiel:
- Watchful waiting met follow-up volgens het graad 2 meningeoom-schema (zie module Primair wait-and-scan beleid bij asymptomatisch meningeoom), met radiotherapie op het moment van objectiveerbare groei.
- Postoperatieve gefractioneerde radiotherapie zoals bij een graad 2 meningeoom.
Pas geen radiotherapie toe bij graad 2 meningeomen na macroscopisch volledige resectie (gross total resection, GTR)**.
Overweeg adjuvante gefractioneerde radiotherapie bij graad 2 meningeomen, gebaseerd op de nieuwe WHO CNS5 classificatie/c-IMPACT-NOW-8 aanbevelingen (zie ook Module Pathologie), wanneer er een macroscopische resttumor* is.
Start postoperatieve adjuvante gefractioneerde radiotherapie bij graad 3 meningeomen, gebaseerd op de nieuwe WHO CNS5 classificatie/c-IMPACT-NOW-8 aanbevelingen (zie ook Module Pathologie).
*Macroscopische resttumor correspondeert met een subtotale resectie, Simpson klasse IV-V.
**GTR correspondeert met Simpson klasse I-III resectie.
Overwegingen
Waarom deze aanbeveling?
De aanbevelingen zijn gebaseerd op observationele literatuur met lage tot zeer lage bewijskracht, in combinatie met brede klinische praktijkervaring en de huidige WHO 2021 CNS5/c-IMPACT-NOW-8-classificatie. Gezien het uiteenlopende biologische gedrag per WHO-graad en resectiestatus acht de werkgroep differentiatie in indicatiestelling voor postoperatieve radiotherapie noodzakelijk. Hiermee wordt beoogd overbehandeling bij laagrisicogroepen te voorkomen en tijdige behandeling bij hoogrisicogroepen te waarborgen.

Balans tussen gewenste en ongewenste effecten
WHO graad 1
Voor histologisch WHO graad 1 meningeomen is geen vergelijkend onderzoek beschikbaar dat de effectiviteit van postoperatieve gefractioneerde radiotherapie na volledige resectie onderbouwt, en is derhalve geen bewijskracht beoordeeld. Het uitgangspunt om bij deze groep geen gefractioneerde radiotherapie toe te passen is gebaseerd op internationale richtlijnen en gangbare klinische praktijk, waarin het doorgaans gunstige natuurlijke beloop en het risico op overbehandeling centraal staan.
Beperkt observationeel onderzoek bij patiënten met een WHO graad 1 meningeoom na subtotale resectie suggereert dat adjuvante radiotherapie in deze groep geassocieerd kan zijn met een langere progressievrije overleving (Oya 2021). De bewijskracht is echter zeer laag, waardoor onzekerheid bestaat over de precieze grootte en klinische relevantie van het effect. Daarnaast ontbreken gegevens over algehele overleving en langetermijntoxiciteit volledig. De werkgroep acht adjuvante stereotactische/gefractioneerde radiotherapie bij WHO graad 1 meningeomen daarom alleen te overwegen in specifieke situaties waarin het risico op klinisch relevante progressie verhoogd is, bij resttumor die bij geringe groei tot ernstige klachten kan leiden, met name bij meningeomen van de schedelbasis. Ook bij patiënten met een gereseceerd graad 1 meningeoom met een moleculair intermediair of hoog risicoprofiel én een macroscopische resttumor (Simpson klasse 4–5), is postoperatieve gefractioneerde radiotherapie te overwegen. Omdat er bij de laatstgenoemde groep onzekerheid bestaat over het optimale beleid, adviseert de werkgroep deze patiënten te bespreken in een neuro-oncologisch of schedelbasis-MDO. Afhankelijk van het individuele patiëntprofiel kan worden gekozen voor een afwachtend beleid met intensievere follow-up volgens het schema voor WHO graad 2 meningeomen, met gefractioneerde radiotherapie bij groei, of voor postoperatieve gefractioneerde radiotherapie analoog aan het beleid bij WHO graad 2 meningeomen. Dit betreft een zwakke aanbeveling, gebaseerd op expert opinion en klinische praktijk.
In dit kader kan adjuvante gefractioneerde radiotherapie ook worden overwogen als onderdeel van het behandelplan waarbij ontlastende chirurgie (debulking/subtotale resectie) wordt gevolgd door radiotherapie van een meningeoomrest die bij geringe groei tot ernstige klachten leidt. Dit geldt met name voor schedelbasismeningeomen, waarbij volledige resectie vaak niet haalbaar is en beperkte tumorgroei klinisch relevante gevolgen kan hebben.
WHO graad 2
Voor WHO graad 2 meningeomen is uitsluitend observationele literatuur geïncludeerd waarin het effect van postoperatieve radiotherapie wordt onderzocht. Op basis van deze geïncludeerde studies is de bewijskracht voor zowel progressievrije overleving als algehele overleving laag.
De geïncludeerde studies suggereren dat het effect van adjuvante gefractioneerde radiotherapie afhankelijk is van de resectiegraad. Met name bij patiënten met een macroscopische resttumor (subtotale resectie, STR; Simpson klasse IV–V) lijkt adjuvante radiotherapie geassocieerd te zijn met een langere progressievrije overleving en mogelijk ook een langere algehele overleving. Bij patiënten na volledige resectie (gross total resection, GTR; Simpson klasse I–III) zijn de resultaten minder consistent en is het voordeel van directe postoperatieve gefractioneerde radiotherapie minder duidelijk. Gegevens over kwaliteit van leven, cognitief functioneren en late toxiciteit zijn beperkt beschikbaar, waardoor ongewenste effecten niet goed kwantitatief kunnen worden meegewogen. De meerwaarde van postoperatieve gefractioneerde radiotherapie na GTR van een graad 2 meningeoom wordt onderzocht in de gerandomiseerde ROAM/EORTC-1308 trial (Jenkinson 2015), waarvan resultaten binnen 1-2 jaar na verschijnen van deze richtlijn verwacht worden.
Bij patiënten met een WHO graad 2 meningeoom na GTR weegt de werkgroep het ontbreken van overtuigend bewijs voor klinisch relevante winst af tegen het risico op overbehandeling en mogelijke langetermijngevolgen van radiotherapie. Dit leidt tot de aanbeveling om geen gefractioneerde radiotherapie toe te passen na GTR bij WHO graad 2 meningeomen (sterke aanbeveling), met een zorgvuldig en gestructureerd follow-upbeleid.
Bij aanwezigheid van een macroscopische resttumor wordt het hogere recidiefrisico zwaarder gewogen en kan adjuvante gefractioneerde radiotherapie bijdragen aan ziektecontrole. In combinatie met de nieuwe WHO CNS5-classificatie en c-IMPACT-NOW-8 aanbevelingen (zie module Pathologie), waarin risicostratificatie nadrukkelijker is verankerd, ondersteunt dit de aanbeveling om adjuvante gefractioneerde radiotherapie te overwegen bij WHO graad 2 meningeomen met een macroscopische resttumor.
WHO graad 3
Voor WHO graad 3 meningeomen is eveneens uitsluitend observationele literatuur beschikbaar. De bewijskracht van deze studies is laag voor algehele overleving en zeer laag voor progressievrije overleving, waardoor de conclusies uit de literatuur met voorzichtigheid moeten worden geïnterpreteerd. De aanbevelingen voor deze patiëntengroep zijn daarom gebaseerd op een combinatie van de beschikbare literatuur en brede klinische ervaring.
In de dagelijkse praktijk worden WHO graad 3 meningeomen gekenmerkt door een agressief biologisch gedrag en een hoog risico op vroege progressie, ongeacht de mate van resectie. De geïncludeerde studies suggereren dat postoperatieve gefractioneerde radiotherapie geassocieerd is met een langere algehele overleving in vergelijking met geen radiotherapie, terwijl het effect op progressievrije overleving minder consistent is. Gezien het ongunstige natuurlijke beloop en de beperkte alternatieven wordt postoperatieve gefractioneerde radiotherapie in de klinische praktijk beschouwd als de meest veilige behandeloptie om het risico op vroege progressie te beperken.
Tegen deze achtergrond weegt de werkgroep de potentiële voordelen van postoperatieve radiotherapie zwaarder dan de onzekerheden in het bewijs en de mogelijke nadelen van behandeling. Postoperatieve adjuvante gefractioneerde radiotherapie is momenteel standaardzorg bij WHO graad 3 meningeomen. Dit leidt tot de aanbeveling om postoperatieve adjuvante radiotherapie te starten bij WHO graad 3 meningeomen.
WHO 2021 CNS5- en c-IMPACT-NOW-8-classificatie
De beschikbare gegevens over de meerwaarde van postoperatieve gefractioneerde radiotherapie zijn gebaseerd op studies met beperkte bewijskracht en vrijwel uitsluitend op classificatiesystemen die dateren van vóór de WHO 2021 CNS5- en c-IMPACT-NOW-8-classificatie. Binnen deze oudere classificatiekaders lijkt een voordeel van postoperatieve gefractioneerde radiotherapie vooral zichtbaar bij hoogrisicogroepen, waaronder WHO graad II meningeomen met een macroscopische resttumor, en WHO graad III meningeomen.
De introductie van de WHO 2021 CNS5- en c-IMPACT-NOW-8-classificatie heeft de prognostische stratificatie van meningeomen verbeterd. Of deze verbeterde prognostische indeling ook gepaard gaat met een betere predictieve waarde voor postoperatieve gefractioneerde radiotherapie is op dit moment echter nog niet vastgesteld. Dit dient te worden onderzocht in prospectieve studies of systematische heranalyses van bestaande cohorten.
Kwaliteit van bewijs
Bij patiënten met WHO graad 2 meningeomen is de algehele kwaliteit van het bewijs voor de uitkomsten algemene overleving en ziekteprogressievrije overleving laag. Dit betekent dat we onzeker zijn over het geschatte effect op overleving. De beoordeling werd verlaagd vanwege methodologische beperkingen (confounding) en het feit dat de optimale steekproefgrootte niet werd bereikt. Publicatiebias werd niet geëvalueerd.
Bij patiënten met WHO graad 3 meningeomen is de algehele kwaliteit van het bewijs voor de uitkomst algemene overleving laag. Dit betekent dat we onzeker zijn over het geschatte effect op algemene overleving. De beoordeling werd verlaagd vanwege methodologische beperkingen (mogelijke selectiebias in de studiepopulatie en onvolledige uitkomstgegevens, afgewaardeerd met twee niveaus). Publicatiebias werd niet geëvalueerd.
Voor de uitkomst ziekteprogressievrije overleving bij patiënten met WHO graad 3 meningeomen is de algehele kwaliteit van het bewijs zeer laag. Dit betekent dat we zeer onzeker zijn over het geschatte effect. De beoordeling werd verlaagd vanwege methodologische beperkingen (mogelijke selectiebias in de studiepopulatie en onvolledige uitkomstgegevens, afgewaardeerd met twee niveaus) en omdat de optimale steekproefgrootte niet werd bereikt. Publicatiebias werd ook hier niet geëvalueerd.
Helaas werden er geen resultaten gerapporteerd over cognitie, kwaliteit van leven of algemeen functioneren.
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
Bij de afweging voor postoperatieve gefractioneerde radiotherapie spelen voor patiënten vooral de balans tussen behandelbelasting en het risico op recidief of vroege progressie een rol. Bij WHO graad 2 meningeomen na volledige resectie kan het vermijden van aanvullende behandeling en mogelijke langetermijn bijwerkingen voor patiënten zwaarder wegen, terwijl bij aanwezigheid van een macroscopische resttumor of bij WHO graad 3 meningeomen het beperken van het risico op vroege progressie vaak prioriteit heeft. Gezien de beperkte bewijskracht en het belang van individuele doelen en belastbaarheid acht de werkgroep het essentieel dat deze afwegingen expliciet worden besproken in multidisciplinair verband en samen met de patiënt.
De kans op neurologische restschade en epilepsie, als gevolg van de tumor zelf en/of radiotherapie, dient expliciet met patiënten te worden besproken, aangezien deze gevolgen een belangrijke impact kunnen hebben op kwaliteit van leven en daarmee op de behandelkeuze.
Kostenaspecten
De interventie gefractioneerde radiotherapie levert meer kosten ten opzichte van de controlebehandeling observatie. Dit weegt bij laagrisicogroepen (WHO graad 1 en WHO graad 2 na GTR) niet op tegen het verschil in effectiviteit. In laagrisicogroepen is geen overtuigend voordeel aangetoond, waardoor de extra kosten en behandelbelasting niet gerechtvaardigd worden door een duidelijk klinisch effect. Bij hoogrisicogroepen (WHO graad 2 met macroscopische resttumor en WHO graad 3) kunnen hogere kosten proportioneel zijn in relatie tot het verwachte klinische voordeel, mede omdat ziekteprogressie kan leiden tot intensieve vervolgbehandelingen, zoals heroperatie of salvage-radiotherapie, die eveneens gepaard gaan met substantiële zorgkosten en belasting voor de patiënt.
Gelijkheid ((health) equity/equitable)
De interventie leidt tot een mogelijke afname van ongelijkheid in zorg, doordat expliciete aanbevelingen per WHO-graad en resectiestatus richting geven aan de indicatiestelling voor postoperatieve radiotherapie. Hiermee wordt het risico verminderd dat behandelkeuzes primair worden bepaald door lokale behandeltradities of beschikbare expertise, en wordt meer uniforme besluitvorming bevorderd.
Postoperatieve gefractioneerde radiotherapie is binnen de Nederlandse zorg toegankelijk en vereist geen specifieke financiële middelen of vaardigheden van patiënten. Wel vraagt de complexiteit van risicostratificatie en besluitvorming om duidelijke en begrijpelijke informatievoorziening, zodat ook patiënten met beperkte gezondheidsvaardigheden volwaardig kunnen participeren in de besluitvorming.
Aanvaardbaarheid:
Ethische aanvaardbaarheid
De interventie wordt binnen de huidige neuro-oncologische praktijk als ethisch aanvaardbaar beschouwd. Er zijn geen specifieke ethische bezwaren geïdentificeerd, mits de indicatiestelling zorgvuldig plaatsvindt en patiënten adequaat worden geïnformeerd over de mogelijke baten en onzekerheden. Het vermijden van onnodige behandeling bij WHO graad I en bij WHO graad II na volledige resectie sluit aan bij het principe van het voorkomen van overbehandeling.
Duurzaamheid
Bij de interventie spelen duurzaamheidsaspecten een rol, aangezien radiotherapie gepaard gaat met energiegebruik en inzet van gespecialiseerde infrastructuur. Het is binnen deze richtlijn niet mogelijk om de milieu-impact van radiotherapie direct te vergelijken met alternatieven, zoals intensieve follow-up of latere salvagebehandelingen. Door gefractioneerde radiotherapie selectief toe te passen bij patiënten met een verhoogd risico op progressie en deze te vermijden bij patiënten met een laag verwacht klinisch voordeel, draagt de richtlijn bij aan doelmatig en terughoudend gebruik van deze behandeling.
Haalbaarheid
De interventie lijkt haalbaar binnen de huidige Nederlandse zorgstructuur. Postoperatieve gefractioneerde radiotherapie is beschikbaar in gespecialiseerde centra en maakt voor hoogrisicogroepen (met name WHO graad 3) reeds deel uit van de gangbare klinische praktijk.
De implementatie van de aanbevelingen vraagt om structurele multidisciplinaire afstemming en adequate pathologische risicostratificatie conform de WHO 2021 CNS5/c-IMPACT-NOW-8-classificatie. De haalbaarheid is daarmee afhankelijk van goed georganiseerde MDO-bespreking en toegang tot de benodigde pathologische expertise. Omdat deze vereisten al deel uitmaken van de huidige infrastructuur in de bestaande neuro-oncologische centra, zijn er naar verwachting geen aanvullende zorgstructuren of nieuwe behandelvoorzieningen vereist.
Onderbouwing
Postoperative radiotherapy is an established component of care in selected patients with intracranial meningiomas, but its indication varies according to tumor grade, extent of resection and perceived risk of recurrence. Recent updates in the WHO CNS5 classification and c-IMPACT-NOW recommendations have refined risk stratification, but comparative studies based on this updated framework are not yet available. As a result, clinical practice shows variation in the use of postoperative radiotherapy, particularly in WHO grade 2 meningiomas and in patients with residual disease. This module aims to reduce unwarranted practice variation by providing structured guidance that extrapolates available evidence and integrates it with clinical expertise.

Patients with WHO grade 2 meningiomas. Based on the 2021 WHO classification of tumors of the CNS, Grade 2 meningiomas are now categorized as atypical, chordoid, and clear cell subtypes.
Inclusion criteria: studies that compare the performance of surgery alone versus surgery plus adjuvant radiotherapy for treating WHO grade 2 meningiomas.

Patients with atypical meningiomas.
In general, GTR was defined as a Simpson Grade I or II tumor resection, or Simpson Grade I, II or III tumor resection, and STR was defined as a Simpson Grade IV tumor resection. In addition, to further clarify the potential impact of GTR plus RT on AM under real-world conditions, a subgroup analysis of the GTR group was performed according to Simpson’s classification. Radiotherapy was considered to include both conventional radiotherapy and stereotactic radiosurgery (SRS).
Inclusion criteria: (1) cohort studies or randomized controlled trials, (2) patients with atypical meningioma verified by pathology, (3) studies that investigated different treatment modalities, including GTR and STR plus RT, and (4) OS and/or PFS data that were provided or allowed for the calculation of hazard ratios (HRs) with corresponding 95% confidence intervals (CIs). Exclusion criteria: (1) reviews, letters, case reports, and database-based studies; (2) studies with a sample size of less than 20; (3) non-English studies, studies with duplicate data; and (4) studies that lacked key information for calculation.

Results
One observational comparative study (Oya, 2021) reported on the effects of postoperative fractionated radiotherapy on progression-free survival among patients with WHO grade I meningeoma. Two systematic reviews (Verly 2024, Song 2021) reported treatment outcome in patients with WHO grade II meningioma in terms of overall survival and progression free survival. Bergner 2022 reported outcomes in patients with WHO grade III meningioma in terms of overall survival, and progression free survival. Addionally, Dobeson (2024) reported on the effects of radiotherapy on overall survival in WHO grade III patients in an observational cohort study (Table 2 (Summary of findings)).

Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 2000 to December 13, 2024 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from three primary search concepts: (1) meningioma; (2) surgery; (3) adjuvant radiotherapy. Duplicates were removed using EndNote software. After deduplication a total of 1022 records were imported for title/abstract screening. Initially, 80 studies were selected based on title and abstract screening. After reading the full text, 77 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and three studies were included.

Ik ben betrokken bij het ontwikkelen van een van de moleculaire risicopredictie modellen op basis van histologische en moleculaire parameters voor menigeomen. Dit werk is in 2021 gepubliceerd (Maas et al. Journal of Clinical Oncology) met een folow-up artikel in 2022 (Hielscher et al. Brain Pathology). Op deze techniek zijn geen patenten aangevraagd, het betreft puur intellectuele erkenning. Mijn expertise met dit model en vergelijkbare modellen, is tevens de reden waarom ik gevraagd ben om bij te dragen aan de richtlijn. |

Inbreng patiëntenperspectief
De werkgroep besteedde aandacht aan het patiëntenperspectief door afgevaardigden namens patiëntenorganisatie Hersenletsel in de werkgroep. De richtlijn is tevens voor commentaar voorgelegd aan Hersenletsel en de eventueel aangeleverde commentaren zijn bekeken en verwerkt.
Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule voerde de werkgroep conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).
