Eerstelijnsbehandeling symptomatisch meningeoom
Uitgangsvraag
Wat is de eerstelijnsbehandeling van voorkeur voor patiënten met een symptomatisch intracranieel meningeoom indien meerdere behandelmodaliteiten mogelijk zijn?
Aanbeveling
Bij patiënten met een symptomatisch intracranieel meningeoom met relevante massawerking en/of snelle radiologische progressie* heeft chirurgische resectie van het meningeoom de voorkeur.
Bespreek bij patiënten met een symptomatisch intracranieel meningeoom zonder massawerking en/of zonder snelle groei zowel chirurgische resectie als stereotactische radiotherapie (gefractioneerd, of eenmalig ‘radiochirurgie’) als mogelijke primaire behandelopties.
- Er is onvoldoende bewijs om één van beide behandelmodaliteiten als voorkeursbehandeling aan te wijzen.
Overweeg om bij de keuze voor de primaire behandelmodaliteit gebruik te maken van de in Tabel 4 beschreven afweging van patiënt- en tumorkenmerken.
- Gebruik deze tabel ter ondersteuning van de multidisciplinaire besluitvorming (MDO) om te komen tot een passende behandelstrategie voor de individuele patiënt.
Evalueer patiënten met een symptomatisch meningeoom in een MDO met vertegenwoordiging vanuit minimaal radiologie, neurologie, (schedelbasis) neurochirurgie, radiotherapie en eventueel een neuro-oncologische verpleegkundige/verpleegkundige specialist.
Bespreek de voorkeuren van de patiënt en de achterliggende overwegingen. Besteed bijzondere aandacht aan verwachtingen over prognose en risico’s van de behandelopties.

Overwegingen
Waarom deze aanbeveling?
Wanneer er bij patiënten met een symptomatisch intracranieel meningeoom zonder klinisch relevant massa-effect en zonder snelle groei behandeling is geïndiceerd, ontbreekt overtuigend bewijs voor de superioriteit van de behandelmodaliteiten chirurgische resectie, gefractioneerde radiotherapie en/of radiochirurgie. De beschikbare literatuur is niet-vergelijkend en van zeer lage bewijskracht, waardoor aanbevelingen primair zijn gebaseerd op expert opinion. Daarom is gekozen voor een gestructureerde afweging van patiënt- en tumorkenmerken. De weging van deze kenmerken is samengevat in Tabel 4. Deze tabel kan dienen ter ondersteuning van multidisciplinaire besluitvorming. Vanwege de onzekerheden in de wetenschappelijke onderbouwing en de verschillende behandelkarakteristieken, is het van extra belang dat de uiteindelijke behandelkeuze in overleg met de patiënt wordt gemaakt.

Van bewijs naar aanbeveling
De overwegingen en aanbevelingen in deze module hebben uitsluitend betrekking op patiënten met een symptomatisch intracranieel meningeoom zonder klinisch relevant massa-effect en zonder aangetoonde snelle groei. Deze afbakening betreft een specifieke en relatief beperkte patiëntengroep, aangezien chirurgie bij symptomatische patiënten in het algemeen de standaard eerstelijnsbehandeling blijft.
Patiënten waarop deze module is gericht, zijn bijvoorbeeld patiënten met een klein meningeoom en presentatie met uitsluitend epilepsie, patiënten met een klein, centraal gelegen meningeoom in de sinus cavernosus, peri-orbitaal of achterste schedelgroeve en patiënten op hoge leeftijd, kwetsbaarheid of relevante comorbiditeit. Voor patiënten met relevante massawerking, snelle neurologische achteruitgang of aangetoonde snelle tumorgroei blijven de bestaande behandelprotocollen onverkort van toepassing; dit vormde geen knelpunt voor deze richtlijnmodule.
Het knelpunt binnen deze module betreft de vraag in welke situaties gefractioneerde radiotherapie en/of stereotactische radiotherapie een passend alternatief kan zijn voor chirurgie, wanneer directe decompressie niet noodzakelijk is en meerdere behandelopties redelijk worden geacht. Onder stereotactische radiotherapie valt ook stereotactische radiochirurgie (kortweg: radiochirurgie), waarmee de behandeling in één fractie bedoeld wordt.
Balans tussen gewenste en ongewenste effecten
Er werden geen studies gevonden die de effectiviteit van eerstelijns stereotactische radiotherapie en chirurgie vergeleken bij patiënten met een symptomatisch meningeoom.
De aanbevelingen in deze module zijn daarom primair gebaseerd op expert opinion en consensus binnen de werkgroep. Ter onderbouwing hiervan is de beschikbare niet-vergelijkende literatuur gebruikt om inzicht te krijgen in factoren die samenhangen met behandeluitkomsten, risico op morbiditeit en symptoomverloop na chirurgie of stereotactische radiotherapie afzonderlijk (Aichholzer, 2000; Dufour, 2001; Schmieder, 2010; Lippitz, 2020; Corniola, 2022; Daza-Ovalle, 2022). Deze literatuur is gebruikt om klinische en radiologische risicofactoren te identificeren die relevant zijn voor de behandelkeuze.
Op basis van deze risicofactoren heeft de werkgroep een overzicht opgesteld dat kan dienen als ondersteunend hulpmiddel bij de multidisciplinaire besluitvorming. Dit overzicht is samengevat in een tabel die bedoeld is voor gebruik in het multidisciplinair overleg (MDO) en ondersteunt een gestructureerde afweging tussen chirurgie en stereotactische radiotherapie bij individuele patiënten.
Chirurgie geldt als de standaard eerstelijnsbehandeling bij het symptomatische intracraniële meningeoom. Dit geldt vooral, maar niet uitsluitend, wanneer veilige resectie mogelijk is en snelle decompressie gewenst is voor klachten en/of snelle groei. De gewenste effecten zijn directe volumevermindering, histologische bevestiging en in veel gevallen verbetering van symptomen, zoals beschreven in niet-vergelijkende chirurgische cohortstudies (Schmieder, 2010; Daza-Ovalle, 2022). Ongewenste effecten betreffen het risico op operatieve morbiditeit, in het bijzonder bij tumoren op of nabij de schedelbasis en kritieke neurovasculaire structuren (Dufour, 2001; Corniola, 2022).
Stereotactische radiotherapie kan in geselecteerde situaties een passend alternatief zijn, met als gewenst effect langdurige tumorcontrole en symptoomstabilisatie zonder chirurgische interventie. Dit wordt met name beschreven bij tumoren op complexe locaties of bij een verhoogd chirurgisch risico (Lippitz, 2020; Corniola, 2022). Ongewenste effecten bestaan uit het ontbreken van directe decompressie en het risico op late stralingsgerelateerde toxiciteit. Bij afwezigheid van massawerking en snelle groei kan dit risico echter acceptabel zijn wanneer het chirurgisch risico hoog wordt geacht (Dufour, 2001; Lippitz, 2020).
Bij patiënten met een schedelbasismeningeoom met massa-effect, dat niet volledig te reseceren is, is een subtotale resectie gevolgd door (gefractioneerde) stereotactische radiotherapie te overwegen. Alternatief is om na de subtotale resectie af te wachten en pas bij groei of toename van symptomen te bestralen, maar van directe postoperatieve radiotherapie is wel een gunstiger PFS beschreven (Ohba, 2011).
Afweging van risicofactoren bij keuze voor behandeling
Om behandelkeuzes te ondersteunen heeft de werkgroep, op basis van expert consensus en de beschikbare niet-vergelijkende literatuur, een overzicht opgesteld van klinische en radiologische factoren die richtinggevend kunnen zijn bij de afweging tussen chirurgie en stereotactische radiotherapie. Deze tabel is bedoeld als hulpmiddel voor het multidisciplinair overleg en dient nadrukkelijk niet als beslisboom.

De werkgroep benadrukt dat chirurgie als de voorkeursbehandeling geldt bij aanwezigheid van massawerking, snelle groei of neurologische achteruitgang. Stereotactische radiotherapie kan worden overwogen als alternatief wanneer het chirurgisch risico verhoogd is.
Kwaliteit van bewijs
De overall kwaliteit van bewijs is zeer laag. Dit betekent dat we zeer onzeker zijn over de gevonden geschatte effecten van de cruciale uitkomstmaten.
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
Gezien het ontbreken van overtuigend bewijs voor superioriteit van één behandelmodaliteit en de zeer lage bewijskracht, acht de werkgroep het essentieel dat patiënten actief worden betrokken bij de keuze tussen chirurgie en stereotactische radiotherapie in de specifieke situatie van symptomatische meningeomen zonder klinisch relevant massa-effect en zonder aangetoonde snelle groei. Juist in deze context, waarin directe decompressie niet noodzakelijk is en meerdere behandelopties redelijk zijn, weegt de individuele afweging tussen gewenste en ongewenste effecten zwaar.
De werkgroep is van mening dat behandelkeuzes alleen zorgvuldig kunnen worden gemaakt wanneer patiënten adequaat worden geïnformeerd over de onzekerheden in de beschikbare evidence, het te verwachten beloop van symptomen, en de mogelijke korte- en langetermijnrisico’s van beide behandelopties. Besluitvorming dient daarom plaats te vinden in gezamenlijk overleg tussen patiënt en behandelteam, bij voorkeur na multidisciplinaire bespreking.
Kostenaspecten
Er zijn geen studies die de kosteneffectiviteit van chirurgie en stereotactische radiotherapie in deze specifieke patiëntengroep vergelijken. Chirurgie en stereotactische radiotherapie verschillen in kostenstructuur en inzet van zorgcapaciteit, maar de werkgroep acht het niet passend om de behandelkeuze primair op kosten te baseren. De keuze dient te worden bepaald door klinische overwegingen en patiëntvoorkeuren.
Gelijkheid ((health) equity/equitable)
De interventie leidt niet tot een relevante toename of afname van gezondheidsgelijkheid. Binnen de Nederlandse zorgcontext zijn zowel chirurgie als stereotactische radiotherapie breed toegankelijk en vergoed binnen het basispakket. Hoewel stereotactische radiotherapie geconcentreerd is in gespecialiseerde centra, wordt dit niet gezien als een onaanvaardbare bron van ongelijkheid, mede doordat verwijzing en multidisciplinaire besluitvorming standaard onderdeel zijn van de zorg voor patiënten met intracraniële meningeomen.
Aanvaardbaarheid:
Ethische aanvaardbaarheid
De interventie lijkt aanvaardbaar voor de betrokkenen. Er zijn geen ethische bezwaren tegen het overwegen van stereotactische radiotherapie als alternatief voor chirurgie in deze specifieke situatie, mits patiënten adequaat worden geïnformeerd over de onzekerheden in de beschikbare evidence en de mogelijke voor- en nadelen van beide behandelopties. Het expliciet betrekken van patiëntvoorkeuren in de besluitvorming wordt door de werkgroep als ethisch wenselijk beschouwd.
Duurzaamheid
Bij stereotactische radiotherapie spelen duurzaamheidsaspecten een rol doordat de behandeling doorgaans poliklinisch plaatsvindt en geen ziekenhuisopname vereist, wat kan leiden tot een lagere inzet van zorgmiddelen. Bij chirurgie is sprake van een hogere inzet van operatiefaciliteiten, materialen en ziekenhuisopname, wat gepaard kan gaan met een grotere milieu-impact. Deze verschillen zijn echter niet doorslaggevend voor de behandelkeuze en worden door de werkgroep niet als primair criterium gehanteerd.
Haalbaarheid
De beschreven behandelopties zijn haalbaar binnen de Nederlandse zorgpraktijk. Zowel chirurgie als stereotactische radiotherapie zijn bestaande en ingebedde behandelmodaliteiten, en multidisciplinaire besluitvorming is gangbare praktijk.
Onderbouwing
Intracranial meningioma (IM) is a tumor arising from the meninges, the membranes surrounding the brain. These lesions occur in approximately 1% of the population. At present, surgical resection is the first-line treatment of choice in patients with a symptomatic meningioma, with the aim of removing as much of the tumor as possible without causing unacceptable damage to the surrounding brain tissue. Stereotactic radiotherapy (including stereotactic radiosurgery as a single-fraction treatment), on the other hand, offers a minimally invasive alternative and is of particular interest in patients for whom surgery carries higher risks. This module focuses on a specific subgroup of patients with a symptomatic intracranial meningioma without clinically relevant mass effect and without proven rapid tumor growth. Absence of clinically relevant mass effect refers to tumors that do not cause compression of, or an imminent threat to, critical neurovascular structures such as the brainstem, optic apparatus or major vessels, and therefore do not require urgent surgical decompression. Tumors may be located near critical structures, provided there is no current compression or functional compromise attributable to mass effect. Absence of proven rapid growth refers to the lack of radiological growth patterns that would mandate immediate intervention according to established wait-and-scan criteria, such as marked or consistent growth on serial imaging. In this research question, we aim to explore whether, and for whom, stereotactic radiotherapy may be an equally effective or even superior option compared to surgery. In this specific clinical context—where immediate decompressive surgery is not obligatory and multiple treatment strategies may reasonably be considered—the key question is whether, and for which patients, stereotactic radiotherapy may represent an equally effective or potentially preferable alternative to surgery.

Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 2000 to December 3, 2024 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from two primary search concepts: (1) meningioma; (2) first-line radiotherapy. Duplicates were removed using EndNote software. After deduplication a total of 759 records were imported for title/abstract screening. Initially, 27 studies were selected based on title and abstract screening. After reading the full text, all 27 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and no studies could be included.

De herziening van deze richtlijnmodule werd ondersteund door het Kennisinstituut van de Federatie Medisch Specialisten (www.demedischspecialist.nl/kennisinstituut) en werd gefinancierd door de Stichting Kwaliteitsgelden Medisch Specialisten (SKMS).
De financier heeft geen enkele invloed gehad op de inhoud van de richtlijnmodule.
Samenstelling werkgroep

Ik ben betrokken bij het ontwikkelen van een van de moleculaire risicopredictie modellen op basis van histologische en moleculaire parameters voor menigeomen. Dit werk is in 2021 gepubliceerd (Maas et al. Journal of Clinical Oncology) met een folow-up artikel in 2022 (Hielscher et al. Brain Pathology). Op deze techniek zijn geen patenten aangevraagd, het betreft puur intellectuele erkenning. Mijn expertise met dit model en vergelijkbare modellen, is tevens de reden waarom ik gevraagd ben om bij te dragen aan de richtlijn. |

Inbreng patiëntenperspectief
De werkgroep besteedde aandacht aan het patiëntenperspectief door afgevaardigden namens patiëntenorganisatie Hersenletsel in de werkgroep. De richtlijn is tevens voor commentaar voorgelegd aan Hersenletsel en de eventueel aangeleverde commentaren zijn bekeken en verwerkt.
Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule voerde de werkgroep conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).
