Behandeling recidief
Uitgangsvraag
Wat is de optimale behandeling voor een graad 1/laag risico recidief meningeoom na eerstelijnsbehandeling?
Aanbeveling
Overweeg chirurgie of radiotherapie bij patiënten met een recidief meningeoom (WHO graad 1/low risk), indien klinisch en technisch mogelijk.
Bij patiënten die asymptomatisch zijn en bij wie groei zonder klinische consequenties wordt verwacht kan een wait-and-scan beleid worden voorgesteld (zie Module Behandelindicaties asymptomatisch meningeoom).
Bespreek de keuze tussen chirurgie, gefractioneerde radiotherapie en radiochirurgie multidisciplinair en met de patiënt.
Pas systemische therapie of radionuclidetherapie alleen bij uitzondering toe, wanneer mogelijkheden voor chirurgie en radiotherapie zijn uitgeput en/of binnen onderzoeksverband, omdat er onvoldoende bewijs is voor effectiviteit.
Overwegingen
Waarom deze aanbeveling?
De aanbevelingen zijn gebaseerd op beperkt bewijs met zeer lage bewijskracht, aangevuld met expert opinie, omdat voor recidief intracranieel meningeoom geen overtuigend vergelijkend bewijs beschikbaar is voor een voorkeursbehandeling. Daarom richten deze aanbevelingen zich op het expliciet afwegen van lokale behandelopties in multidisciplinair verband en op terughoudendheid met andere behandelingen buiten studieverband.

Zwakke aanbeveling voor.
Balans tussen gewenste en ongewenste effecten
Het beschikbare bewijs is zeer onzeker en laat, op basis van de beschikbare studie, geen duidelijk verschil zien tussen herhaalde radiotherapie, chirurgie en de combinatie van chirurgie met herhaalde radiotherapie. De cruciale uitkomstmaat progressievrije overleving (PFS) werd onderzocht in één retrospectieve studie (Rubino, 2023). Hierin werd chirurgie gebruikt als referentie. De vergelijking van repeat RT met chirurgie liet een hazard ratio van 1.005 (95% CI 0.538–1.876) zien, met een mediane PFS van 32 maanden. Dit resultaat valt binnen de vooraf vastgestelde grenzen van klinische irrelevantie, wat erop wijst dat repeat RT geen klinisch relevant verschil oplevert ten opzichte van chirurgie.
Voor de vergelijking van chirurgie in combinatie met repeat RT versus chirurgie alleen werd een hazard ratio van 0.784 (95% CI 0.349–1.759) gevonden, met een mediane PFS van 28 maanden. Ook dit resultaat valt binnen de grenzen van klinische irrelevantie, wat betekent dat de combinatie van chirurgie en repeat RT geen klinisch relevant voordeel biedt ten opzichte van chirurgie alleen.
Voor andere belangrijke uitkomstmaten, zoals algehele overleving, kwaliteit van leven en neurologisch functioneren, is binnen de PICO-studie geen of onvoldoende informatie beschikbaar. Op basis van direct bewijs kan daarom geen voorkeursbehandeling worden aangewezen bij een recidief meningeoom.
Aanverwante literatuur, bestaande uit retrospectieve cohortstudies en patiëntseries, beschrijft uitkomsten van re-resectie, gefractioneerde (re-)radiotherapie en radiochirurgie bij geselecteerde patiënten met een recidief meningeoom (Apra, 2018; Imber, 2019; Magill, 2019; Schmutzer, 2023; Corniola, 2022). Deze studies suggereren dat zowel chirurgie als radiotherapie bij zorgvuldig geselecteerde patiënten kan leiden tot lokale controle, maar laten tevens zien dat behandeluitkomsten sterk variëren afhankelijk van tumorgraad, lokalisatie, eerdere behandelingen en patiëntkenmerken. Hoewel deze studies niet zijn beoordeeld op bewijskracht en geen directe vergelijkingen tussen de twee behandelopties mogelijk maken, ondersteunen zij de expert opinie dat op basis van het huidige bewijs geen voorkeursmodaliteit kan worden aangewezen.
Ten aanzien van ongewenste effecten geldt dat zowel herhaalde chirurgie als (re-)radiotherapie gepaard kunnen gaan met relevante risico’s, waaronder neurologische uitval, complicaties en cumulatieve behandelingstoxiciteit. De aard en ernst van deze risico’s verschillen sterk per patiënt en zijn afhankelijk van factoren zoals tumorlokalisatie, eerdere behandelingen, interval sinds eerdere therapie en de algemene conditie van de patiënt. Vergelijkende gegevens over deze ongewenste effecten ontbreken.
Gezien de zeer lage bewijskracht, het ontbreken van duidelijke verschillen in gewenste effecten en de potentieel relevante ongewenste effecten bij de twee behandelopties, concludeert de werkgroep dat behandelkeuzes bij een recidief intracranieel meningeoom niet primair op verwachte effectiviteit kunnen worden gebaseerd. Dit ondersteunt de aanbeveling om zowel chirurgie als radiotherapie te overwegen en om de uiteindelijke behandelkeuze te laten afhangen van een multidisciplinaire afweging van klinische haalbaarheid, eerdere behandeling, tumorkenmerken en patiëntfactoren en diens waarden en voorkeuren.
Voor andere behandelopties, zoals systemische behandelingen of radionuclidetherapie is binnen deze richtlijnmodule geen PICO opgesteld en is geen systematische literatuursearch uitgevoerd. Daardoor ontbreekt een gestructureerde beoordeling van de effectiviteit en veiligheid van deze behandelingen bij recidief intracranieel meningeoom. Gezien deze lacune in het bewijs en de onzekerheid hierover binnen de werkgroep op basis van expert opinie, worden andere behandelingen zoals systemische therapie of radionuclidetherapie eventueel (in studieverband) aanbevolen indien de andere twee behandelingsopties uitgeput zijn.
In het licht van het ontbreken van overtuigend bewijs voor de effectiviteit van systemische of radionuclidetherapieën bij recidief meningeoom is recent de gerandomiseerde fase II-studie EORTC-2334-BTG (LUMEN-1, NCT06326190) gestart, waarin bij patiënten met een recidief meningeoom na eerdere lokale behandeling (chirurgie en/of radiotherapie) [^177Lu]Lu-DOTATATE wordt vergeleken met systemische standaardbehandeling. Deze studie kan in de toekomst meer duidelijkheid geven over de plaats van radionuclidetherapie bij patiënten bij wie lokale behandelopties zijn uitgeput.
Kwaliteit van bewijs
De overall kwaliteit van bewijs is zeer laag. Dit betekent dat we zeer onzeker zijn over het gevonden geschatte effect van de cruciale uitkomstmaten.
Er is afgewaardeerd vanwege:
- Risk of Bias: Er is sprake van methodologische beperkingen. De studie van Rubino (2023) betreft een retrospectieve observationele studie zonder gerandomiseerde opzet, waardoor het risico op selectiebias en confounding aanzienlijk is.
- Imprecisie: De betrouwbaarheidsintervallen van de gerapporteerde hazard ratio’s voor progressievrije overleving zijn breed en omvatten zowel mogelijke voordelen als nadelen van de interventie, wat leidt tot onzekerheid over het effect.
Er zijn geen andere vergelijkende studies beschikbaar die deze uitkomstmaat bij deze populatie evalueren. Door het ontbreken van aanvullende data op andere cruciale en belangrijke uitkomstmaten (zoals overleving, cognitie, kwaliteit van leven en functionele status) kan de zekerheid over de effecten van herhaalde radiotherapie en chirurgie niet worden verhoogd.
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
Bij patiënten met een recidief intracranieel meningeoom kunnen waarden en voorkeuren sterk verschillen. Sommige patiënten hechten meer waarde aan maximale lokale controle, terwijl anderen het risico op neurologische uitval, behandelbelasting of herhaalde interventies zwaarder laten meewegen. Ook eerdere behandelervaringen, functioneren, leeftijd en comorbiditeit spelen een rol in deze afweging.
Omdat het beschikbare bewijs geen duidelijke voorkeursmodaliteit ondersteunt en de balans tussen gewenste en ongewenste effecten per patiënt kan verschillen, acht de werkgroep het essentieel dat de beslissing tot behandeling en de behandelkeuzes expliciet multidisciplinair en samen met de patiënt (en naasten) worden besproken. Dit maakt het mogelijk om individuele waarden en voorkeuren mee te nemen in situaties waarin meerdere behandelopties klinisch aanvaardbaar zijn en de onzekerheid over uitkomsten groot is.
Kostenaspecten
Er zijn geen studies beschikbaar die de kosteneffectiviteit van re-resectie, gefractioneerde (re-)radiotherapie of (re-)radiochirurgie of combinaties daarvan bij recidief intracranieel meningeoom rechtstreeks vergelijken. Wel is aannemelijk dat de kosten van de verschillende behandelopties aanzienlijk kunnen verschillen, onder meer door variatie in opnameduur, inzet van gespecialiseerde technieken, complicatiebehandeling en intensiteit van follow-up.
Bij het ontbreken van overtuigende verschillen in effectiviteit acht de werkgroep het relevant dat kosten en inzet van zorgmiddelen worden meegewogen in de behandelkeuze, naast klinische en patiëntgerichte overwegingen. Dit ondersteunt een benadering waarin intensieve of herhaalde behandelingen proportioneel worden ingezet, met aandacht voor te verwachten gezondheidswinst en belasting voor de patiënt.
Gelijkheid ((health) equity/equitable)
De aanbevelingen beogen bij te dragen aan meer uniforme besluitvorming bij recidief intracranieel meningeoom door expliciet te benoemen dat meerdere behandelopties overwogen kunnen worden en dat multidisciplinaire afweging noodzakelijk is bij de uiteindelijke keuze van beleid, inclusief de mogelijkheid van geen verdere interventie. Hierdoor wordt het risico verminderd dat behandelkeuzes primair worden bepaald door lokale behandeltradities of beschikbare expertise.
De aanbevelingen vereisen geen specifieke vaardigheden of middelen van patiënten en leiden naar verwachting niet tot een toename van gezondheidsongelijkheid.
Aanvaardbaarheid:
Ethische aanvaardbaarheid
De aanbevelingen zijn ethisch aanvaardbaar, omdat zij expliciet rekening houden met de grote onzekerheid in het beschikbare bewijs en ruimte laten voor individuele afwegingen. Het vermijden van een dwingende voorkeursbehandeling en het benadrukken van multidisciplinaire en patiëntgerichte besluitvorming sluiten aan bij principes van autonomie, proportionaliteit en transparantie. De werkgroep voorziet geen specifieke ethische bezwaren.
Duurzaamheid
Bij de behandeling van een recidief intracranieel meningeoom spelen duurzaamheidsaspecten in beperkte mate een rol bij de keuze tussen de verschillende behandelopties. Zowel chirurgie als gefractioneerde radiotherapie en radiochirurgie maken gebruik van bestaande ziekenhuisinfrastructuur en energie-intensieve voorzieningen, maar er zijn geen gegevens beschikbaar die wijzen op relevante verschillen in milieu-impact tussen deze behandelstrategieën. Duurzaamheid is daarom geen doorslaggevend criterium in de afweging tussen de behandelopties, en speelt geen expliciete rol in de formulering van de aanbevelingen.
Haalbaarheid
Chirurgie, gefractioneerde radiotherapie en stereotactische radiotherapie (inclusief stereotactische radiochirurgie, waarmee de behandeling in één fractie bedoeld wordt) zijn beschikbare behandelopties binnen de Nederlandse neuro-oncologische zorg. Multidisciplinaire bespreking en individuele afweging maken reeds deel uit van de standaardzorg bij recidief intracranieel meningeoom.
De aanbevelingen vragen geen nieuwe infrastructuur, maar expliciteren bestaande praktijk en zijn daarom haalbaar. Regionale verschillen in expertise en beschikbaarheid van behandelmodaliteiten kunnen invloed hebben op de uiteindelijke behandelkeuze, hetgeen het belang van multidisciplinaire beoordeling benadrukt.
Onderbouwing
A recurrent meningioma, meaning a tumor that reappears after first-line treatment such as surgery or radiotherapy, presents a therapeutic challenge. The management may include a wait-and-scan policy or a treatment. Possible interventions include surgery and fractionated radiotherapy/radiosurgery, and in some cases emerging treatment options as systemic therapy and radionuclide therapy can be considered. The treatment plan should be tailored to the individual patient and requires a multidisciplinary approach.
We aim to compare the widely considered standard treatment options and provide a recommendation on the most effective approach.

Description of studies
A total of one study was included in the analysis of the literature. Important study characteristics and results are summarized in table 2. The assessment of the risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).
Rubino (2023) performed retrospective study following 48 patients with recurrent and previously irradiated meningiomas. The study was conducted between January 1995 and July 2021. There were 35 patients graded as WHO I grade meningiomas, and after the first radiotherapy course, 33 patients remained WHO I. Patients had to be 18 years or older, with a diagnosis of recurrent skull base meningioma. Patients with irradiation-induced meningiomas were excluded. Progression-free survival (PFS) was defined as the time from the last radiotherapy session or salvage surgery to the date of confirmed recurrence on MRI or last follow-up date for patients not experiencing recurrence. PFS was the primary outcome, but overall survival rate was also reported. KPS-scores were also collected as a measure for general functional abilities.

Relevant outcome measures
The guideline panel considered overall survival and progression-free survival as critical outcome measures for decision making; and cognition, quality of life, general functional abilities as important outcome measures for decision making.
A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.
For progression-free survival, the guideline panel defined a difference of 23%, with a Hazard Ratio (HR) of >1.54 and <0.65 as a minimal clinically (patient) important difference.
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 2014 to December 13, 2024 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from two primary search concepts: (1) grade 1/ low risk recurrent meningioma; (2) radiotherapy/ surgery. Duplicates were removed using EndNote software. After deduplication a total of 741 records were imported for title/abstract screening.
Initially, 45 studies were selected based on title and abstract screening. After reading the full text, 44 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and one study was included.

Ik ben betrokken bij het ontwikkelen van een van de moleculaire risicopredictie modellen op basis van histologische en moleculaire parameters voor menigeomen. Dit werk is in 2021 gepubliceerd (Maas et al. Journal of Clinical Oncology) met een folow-up artikel in 2022 (Hielscher et al. Brain Pathology). Op deze techniek zijn geen patenten aangevraagd, het betreft puur intellectuele erkenning. Mijn expertise met dit model en vergelijkbare modellen, is tevens de reden waarom ik gevraagd ben om bij te dragen aan de richtlijn. |

Inbreng patiëntenperspectief
De werkgroep besteedde aandacht aan het patiëntenperspectief door afgevaardigden namens patiëntenorganisatie Hersenletsel in de werkgroep. De richtlijn is tevens voor commentaar voorgelegd aan Hersenletsel en de eventueel aangeleverde commentaren zijn bekeken en verwerkt.
Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule voerde de werkgroep conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).
