Endo-anale echografie
Uitgangsvraag
Gezondheidsgelijkheid ((health) equity/equitable)
Eindoordeel:
Sterke aanbeveling tegen (Niet doen).
Eindoordeel:
Zwakke aanbeveling voor (Doen).
Onderbouwing
Endoanal ultrasound enables visualization of the anatomy and morphology of the anal sphincter. It remains unclear whether morphology is associated with treatment outcomes, or whether morphological findings can be used to predict the most effective therapeutic approach. There is likely substantial variation in clinical practice with respect to the use and interpretation of endoanal ultrasound. Endoanal ultrasound is considered a minimally burdensome diagnostic modality, although it does require a hospital visit. The associated costs are considered acceptable.
Summary of Findings
N.v.t.
Description of studies
The cross-sectional study by Lam (2012) aimed to investigate whether addition of anorectal manometry to endoanal ultrasonography provided additional information to
guide the surgical management of patients with fecal incontinence (FI) who failed conservative treatment. The study included all consecutive patients referred for the evaluation of FI to between 2003 and 2009 at the study site (VU University Medical Center, Amsterdam, The Netherlands). Patients were prospectively assessed by a comprehensive questionnaire regarding their perianal complaints, including the duration of symptoms, frequency of bowel movements, stool consistency and the use of pads. The presence of urinary incontinence, the use of medications and social impact of FI on their lifestyle were assessed. Past obstetric, surgical, and drug history was taken. The severity of incontinence was graded using the Vaizey and Wexner scores.
Patients with inflammatory bowel disease, proctitis, pouches, rectal and prostate carcinomas, (chronic) diarrhea were excluded. Patients were classified into three groups: (1) no anal sphincter defects; (2) small sphincter defects; and (3) large sphincter defects.
Anorectal manometry and endoanal ultrasonography were performed in all patients.
218 patients met the inclusion criteria, 10 patients (5%) were men. Of 208 female patients, 192 had at least one previous vaginal delivery. The mean duration of FI was 4.9 years.
There was no follow-up after the diagnostic tests were performed. The reported outcomes included measurements obtained during endoanal ultrasound and anorectal manometry. No information on changes in therapeutic decisions during follow-up was collected.
Important study characteristics and results are summarized in table 2. The assessment of the risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).

The mean Wexner score was 13 (range 6–20). The Wexner score did not correlate with the MBP, the MSP or the RC. A significant correlation was found with the MSP in patients with soft stool. No correlation was found between Wexner scores and sphincter defects.
A systematic review of the literature was performed to answer the following question(s):
What are the favorable and unfavorable effects of using endoanal ultrasound compared to not using endoanal ultrasound on clinical utility for therapeutic decision-making and patient-relevant outcomes in adult patients with fecal incontinence?
Table 1. PICO
| Patients | Adult patients with fecal incontinence |
| Intervention | Endoanal ultrasound |
| Control | No endoanal ultrasound or another diagnostic test or usual care (digital rectal examination, clinical examination, physical examination) |
| Outcomes | Clinical utility for therapeutic decision-making (e.g. therapeutic decisions), diagnostic yield (e.g. positively tested patients versus number of patients tested), patient-related outcomes (severity of FI episodes: Vaizey score, Wexner scores; quality of life) |
| Other selection criteria | Study design: systematic reviews, randomized controlled trials, comparative observational studies |
Relevant outcome measures
The guideline panel considered clinical utility for therapeutic decision-making, diagnostic yield and severity of fecal incontinence as critical outcome measures for decision making; no important outcome measures were defined.
A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.
The guideline panel did not define thresholds of minimal clinically (patient) important difference for clinical utility for therapeutic decision-making and diagnostic yield due to the nature of outcome measures.
The guideline panel defined the thresholds of minimal clinically (patient) important difference for severity of fecal incontinence:
- Vaizey score: mean difference of -3 points.
- Wexner score: mean difference of -2 points.
- Quality of life: Faecal Incontinence Quality of Life (FIQoL) questionnaire: > 0.4 point difference.
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 2005 to the 12th of August 2025 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from two primary search concepts: (1) fecal incontinence; (2) endoanal ultrasound. Duplicates were removed using EndNote software. After deduplication a total of 1027 records were imported for title/abstract screening.
Studies were selected based on the following criteria: [inclusion criteria]. Titles and abstracts were screened using the ASReview software. The settings ELAS u4 (TF-IDF and SVM) were used. Heitmann (2020) was used as prior knowledge for inclusion. The first 10% of hits were screened by the guideline methodologist in consultation with the working group. The remaining articles were subsequently screened by the guideline methodologist, using the following stopping rule: stop after 10% of the total set subsequent exclusions.
Initially, four studies were selected based on title and abstract screening and were assessed in full text: three studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and one study was included. However, this study only provided observational data, without a control group, and was insufficient as evidence for the recommendations. A concise summary of this study is provided here to illustrate findings of endo-anal ultrasound seen in patients with fecal incontinence. This level of evidence could not be assessed.
