Defaecografie
Uitgangsvraag
Gezondheidsgelijkheid ((health) equity/equitable)
Eindoordeel:
Zwakke aanbeveling voor (Doen).
Onderbouwing
Defecography may influence therapeutic decision-making and clinical care in patients with fecal incontinence. In cases where defecography demonstrates clinically significant external or internal prolapse, this should be addressed (for example, by performing a rectopexy) before considering other interventions.

No studies were found that fulfilled the PICO criteria.
A systematic review of the literature was performed to answer the following question:
What are the favorable and unfavorable effects of using defecography compared to not using defecography (or usual care consisting of digital rectal examination/clinical examination/physical examination) on therapeutic decisions and patient-relevant outcomes in adult patients with fecal incontinence?
Table 1. PICO
| Patients | Adult patients with fecal incontinence |
| Intervention | (MRI-)defecographyy |
| Control | No (MRI-)defecography/ usual care |
| Outcomes | Clinical utility for therapeutic decision-making (e.g. number of treatment alterations), diagnostic yield (e.g. positively tested patients versus number of patients tested) |
| Other selection criteria | Study design: systematic reviews and randomized controlled trials, comparative observational studies |
Relevant outcome measures
The guideline panel considered clinical utility for therapeutic decision-making, diagnostic yield as critical outcome measures for decision making; no important outcome measures were defined.
A priori, the guideline panel did not further define the outcome measures listed above but used the definitions used in the studies.
The guideline panel did not define thresholds of minimal clinically (patient) important difference due to the nature of outcome measures.
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 2005 to the 17th of Juli 2025 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from two primary search concepts: (1) feces incontinence; (2) defecography. Duplicates were removed using EndNote software. After deduplication a total of 587 records were imported for title/abstract screening.
Initially, nine studies were selected based on title and abstract screening. After reading the full text, all nine studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’).
