Uitgangsvraag

Welke interventies zijn er om extravasatie te voorkomen?

Aanbeveling

Implementeer een preventieprogramma voor het reduceren van extravasaties. Deze dient tenminste de educatie en training van zorgverleners en patiënten te omvatten.

 

De inhoud van een mogelijk preventieprogramma wordt beschreven in de tabel met maatregelen. Het dient op basis van de haalbaarheid in de praktijk vastgesteld te worden.

Preventieprogramma

Doorlopende scholing van betrokken zorgverleners over de beleidslijnen en protocollen.

Monitoringsafspraken over het optreden van extravasaties.

Specifieke afspraken over registratie en het analyseren van (de ernstige) extravasaties.

Educatie en training

Geef voorlichting aan de patiënt. Adviseer de patiënt om elke mate van pijn, branderigheid, tinteling of jeuk te melden, wat kan wijzen op extravasatie.

Een getrainde, bekwame verpleegkundige is de sleutel tot het voorkomen van extravasatie. Het is belangrijk dat de zorgverlener de vroege tekenen en symptomen van extravasatie kan herkennen.

Vasculaire toegang

Stem de locatie van de katheter af op de therapieduur- en soort:

  • maak een afweging tussen centraal en perifeer waarbij perifeer een grotere kans geeft op extravasatie;
  • punctie ter hoogte van gewrichten en handrug hebben niet de voorkeur vanwege de grotere kans op dislocatie en daarmee extravasatie.

Kies de naaldlengte die past bij de toediening (kort bij palpeerbare zichtbare venen, lang bij dieper gelegen venen).

Kies een diameter die past bij het bloedvat.

Inbrenghoek van 45 graden geeft minder kans op dislocatie t.o.v. kleinere hoek.

Fixeer de veneuze toegang met verbandmateriaal die de punctieplaats zichtbaar houdt.

Kies fixatiemethode op basis van kathetertype, locatie, huidconditie en patiëntmobiliteit.

Minimaliseer trekkracht op de katheter.

Monitor de punctieplaats zowel om de doorlaatbaarheid te beoordelen als voor de snelle visualisatie van mogelijke reacties van vaatbeschadigingen of extravasatie

Beoordeel voortdurend het risico op perifere veneuze extravasatie en pas de frequentie en het moment van de controles hierop aan, bijvoorbeeld na activiteiten zoals verzorging, toiletbezoek of lopen.

Controleer de bloedstroom na punctie en spoel vervolgens met basisvloeistof en controleer op tekenen van extravasatie.

Detectiemethoden

Overweeg het gebruik van aanvullende detectiemethoden. In de praktijk zijn klinische inspectie en echografie de meest gangbare methoden.

Risicofactoren

Houd rekening met of voorkom waar mogelijk patient- en infusiegerelateerde risicofactoren.

Infusiegerelateerde risicofactoren hebben betrekking op:

  • geneesmiddeleigenschappen (voorkom extreme pH of osmolaliteit);
  • het volume van de toediening;
  • toedieningstijd.

Patientgerelateerde risicofactoren hebben betrekking op:

  • toegangspoort (meerdere eerdere veneuze puncties);
  • vaatkenmerken (intraveneuze perifere katheterisatie van dunne en/of fragiele aderen);
  • co-morbiditeiten (obesitas);
  • bewustzijn/cognitie van de patiënt (i.v.m. het niet kunnen aangeven van pijn bij toediening);
  • specifieke patiëntcategorieën (neonaten).

Vermijd punctie in ledematen met verlies van gevoel, chirurgisch gemanipuleerde gebieden en eerder bestraalde ledematen.

Overwegingen

Balans tussen gewenste en ongewenste effecten

Er is een brede search verricht naar extravasaties, om een overzicht te kunnen geven van beschikbare interventies voor preventie, behandeling van extravasaties en lokaal beleid. Er zijn 32 studies met betrekking tot de preventie van extravasaties in het overzicht opgenomen. De gevonden studies zijn heterogeen in onderzochte interventies, studiepopulaties en eigenschappen van het toegediende middel. Er is gekozen om een overkoepelend overzicht te geven en geen specifieke PICO over één preventieve maatregel uit te werken. De gevonden studies worden beschouwend beschreven en gecombineerd met klinische ervaringen.

 

Kwaliteit van bewijs

Er is een brede, overkoepelende search uitgevoerd naar alle mogelijke interventies. Er is geen beschrijving van de bewijskracht per uitkomstmaat (per studie, per interventie, per toegediend middel).

 

De geincludeerde studies betreffen voornamelijk observationele studies met lage incidentie en grote heterogeniteit. Gezien de acute aard van deze complicatie, wat het uitvoeren van grootschalige, gecontroleerde studies bemoeilijkt was dit in de lijn der verwachting. Hierdoor is de huidige evidentie grotendeels gebaseerd op observationeel onderzoek en klinische ervaring (en volgt de uitwerking niet de structuur van tabel).

 

Aanvullende literatuur

In de systematische review van Melo (2020) is een uitgebreide search uitgevoerd naar preventieve maatregelen voor het voorkomen van extravasaties (Melo, 2020). Hieronder worden de categorieën (Preventieprogramma; educatie en training; vasculaire toegang; detectiemethoden en risicofactoren) in paragrafen uitgewerkt. Per categorie wordt een tabel met specifieke maatregelen gegeven uit Melo et al. aangevuld met andere bronnen. De overlappende maatregelen zijn samengevoegd. Niet alle maatregelen uit de literatuur werden relevant geacht voor de Nederlandse praktijk en zijn derhalve verwijderd. Bij de tabellen dient de kanttekening te worden gemaakt dat de preventieve maatregelen die worden genoemd, het voorkomen van een (ernstige) extravasatie als belangrijkste doel hebben, in de praktijk kunnen andere overwegingen een rol spelen om de maatregel al dan niet uit te voeren.

 

Preventieprogramma

Een preventieprogramma is een gestructureerde aanpak voor het beheren van een reeks samenhangende projecten en activiteiten die gericht zijn op het realiseren van strategische organisatiedoelen, namelijk het reduceren van het aantal en de ernst van extravasaties. In de sub paragrafen hierna wordt dieper ingegaan op de maatregelen die in een preventieprogramma opgenomen kunnen worden. Specifieke voorbeelden zijn training en educatie van personeel op herkenning en/of het maken van werkafspraken over de plaatsing van (perifere) katheters. Om de effectiviteit op de uitkomst te vervolgen dienen afspraken gemaakt te worden over de registratie van een extravasatie, meer hierover volgt in module 4. Aan de hand van deze registratie kunnen maatregelen worden op- of afgeschaald. In de literatuur wordt een positief effect gevonden van een preventieprogramma (Sakaida, 2014; Tofani, 2012; Antúnez-Blancat, 2023). Het effect van een enkele specifieke maatregel kan niet gemeten worden omdat er vaak een pakket aan maatregelen geïmplementeerd wordt. Hieronder worden de twee grootste studies en een systematische review verder toegelicht.

 

In de Japanse retrospectieve studie van Sakaida et al. werd het effect van een gestandaardiseerd toedieningsprotocol bij chemotherapie onderzocht met diverse maatregelen waaronder het trainen van verpleging, het niet toestaan van een vlindernaald en controle van de toedieningsplek na toiletbezoek. Implementatie van deze maatregelen resulteerde in een jaarlijks dalende incidentie van extravasaties: respectievelijk 0.11% in 2007, 0.11% in 2008, 0.1% in 2009, 0.08% in 2010, 0.07% in 2011 en 0.01% in 2012 (Sakaida, 2014).

 

In een grote studie Tofani et al. in een Amerikaans kinderziekenhuis is het effect van een preventieprogramma op extravasaties bij kinderen met een perifere katheter onderzocht. Hierbij is onder andere gebruik gemaakt van posters en training. De incidentie daalde na implementatie in geval van graad 2-3 van 4.2 naar 1.8 per 1000 katheterdagen. Dit effect hield echter geen stand omdat één van de maatregelen, het elk uur controleren van katheter, niet haalbaar bleek. De onderzoekers concludeerden dat verder onderzoek nodig is naar effectieve, haalbare maatregelen voor een blijvend effect (Tofani, 2012).

 

Een systematische review van Antúnez-Blancat (2023) onderzocht onder andere de rol van verpleegkundigen bij extravasaties en factoren in effectieve preventieprogramma’s. Er werden 9 artikelen geïncludeerd en de auteurs concludeerden dat er een cruciale rol is voor de verpleegkundigen in het hele proces van toedienen tot preventie en educatie. (Antúnez-Blancat, 2023). Mohammed et al. is een praktisch voorbeeld van een klinische studie hoe training direct invloed kan hebben op het aantal extravasaties. De incidentie van extravasaties op de oncologieafdeling van het geïncludeerde ziekenhuis daalde significant na de training (van 20% naar 8%). Alhoewel de situatie mogelijk niet representatief is voor de Nederlandse situatie geeft het wel een praktisch voorbeeld van een effectief preventieprogramma (Mohammed, 2023).

 

Het implementeren van een preventieprogramma voor het reduceren van extravasaties lijkt effectief te zijn.

Educatie en training

Educatie en training van zorgverleners is een belangrijke preventieve maatregel om extravasaties te verminderen en tevens het aantal ernstige complicaties te verminderen. Door toepassing van de juiste maatregelen en vroege herkenning kan schade voorkomen worden. Lokaal beleid moet een team in staat stellen een veilige intraveneuze toediening te verzorgen met handvaten voor de benodigde controlestappen. Hiervoor is kennis nodig van risicofactoren, herkenning van signalen van extravasatie, vasculaire toegang en preventieve maatregelen (Kreidieh, 2016).

 

Onder educatie valt ook het informeren en instrueren van patiënten. Dit kan een bijdrage leveren aan vroege herkenning omdat een zorgverlener niet constant bij een patiënt is. Er zijn geen studies bekend naar het effect hiervan op uitkomsten, echter het instrueren van patiënten komt terug in verschillende preventieprogramma’s (Melo, 2020). Patiënten kunnen geïnstrueerd worden om te melden als de toediening oncomfortabel voelt, pijn doet, of als er roodheid of zwelling ontstaat rondom de toedienplek, zodat een zorgverlener actie kan ondernemen.

Vasculaire toegang

De keuze en plaatsingsaspecten van de toegangspoort speelt een belangrijke rol in het optreden van extravasaties. De toegang kan in hoofdlijnen onderverdeeld worden in perifere of centrale katheters op basis van de locatie van de kathetertip in het veneuze bloedvat en verder getypeerd op basis van inbrenglocatie en type. Het voorkomen van extravasaties kan niet los worden gezien van andere plaatsing- en toedieningsaspecten. De SRI richtlijn intravasculaire katheters licht dit uitgebreid toe en beantwoordt in verschillende modules vragen die samenhangen met deze richtlijn Extravasaties waarbij de focus ligt op microbiële aspecten.

 

In verschillende handboeken en richtlijnen zoals INS (No authors listed, 2021) WHO (No authors listed, 2024) CDC (O’Grady, 2017) worden meer overwegingen over de toegangspoort gedeeld dan in de Nederlandse SRI richtlijnen aan bod komen. Perifeer veneuze katheters zijn de meest gebruikte katheters, onder andere vanwege de eenvoud en snelheid van plaatsing ten opzichte van een centrale katheter. De WHO schat in dat zo’n 70% van de klinische patiënten tijdens een ziekenhuisopname deze geplaatst krijgt, het aantal centrale katheters is veel kleiner (no authors listed, 2024). In de praktijk blijken extravasaties meer voor te komen bij perifere katheters. Er dient vroegtijdig afgestemd te worden welk type katheter past bij de patiënt, de behandeling en de therapieduur om de kans op complicaties zoals extravasatie te verkleinen. Maak een afweging tussen centraal en perifeer waarbij perifeer een grotere kans geeft op extravasatie. Tevens heeft punctie ter hoogte van gewrichten en handrug niet de voorkeur vanwege de grotere kans op dislocatie en daarmee extravasatie.

 

Een screeningstool, zoals A-DIVA, kan gebruikt worden om een geschikte wijze van inbrengen van de katheter te identificeren die rekening houdt met individuele patiëntaspecten (van Loon, 2019). Voor de ideale locatiebepaling kan gebruik gemaakt worden van de Zone Insertion Method (ZIM) (Dawson, 2011). Voor neonaten zijn andere tools beschikbaar zoals Neo-INS of IEGS PIVIE (No authors listed, 2021). In zichtbare en palpeerbare venen kunnen katheters met naaldlengte van 25 en 32 mm geplaatst worden. Bij dieper gelegen bloedvaten die echogeleide gepuncteerd worden, moet men minimaal 45 mm naaldlengte hebben. (Hoekendijk, 2024). Kies een naalddiameter (Gauge) die past bij het bloedvat, in geval van snelle toediening van grotere volumina kan overwogen worden om voor de grotere naalddiameter te kiezen: katheter-vene ratio advies in de literatuur ligt doorgaans tussen 1:3 en 1:4,5. Specifieke details en adviezen voor m.b.t. de vasculaire toegang worden in de tabel benoemd.

 

In de literatuursearch zijn meerdere studies gevonden die de plaats van de katheter controleren. De klinische controle door de verpleegkundige is van oudsher de gouden standaard. Andere methodes die beschreven worden zijn de o.a. (Color flow) Doppler ultrasound, IVwatch, en het inspuiten van een vloeistof waarna beoordeling. Vaak zijn (technische) hulpmiddelen nodig. De literatuur is onvoldoende overtuigend om dit soort methodes breed in te zetten. Wel kan het voor specifieke patiëntengroepen als extra hulpmiddel worden toegepast.

 

Preventieve maatregelen moeten een correcte plaatsing waarborgen en de kans op extravasatie verkleinen. De standaardprocedure bestaat altijd uit klinische detectie, waaronder visuele inspectie (denk hierbij ook aan links-rechts vergelijking) en palpitatie.  Indien sprake is van een hoog risico patiënt voor moeilijke intraveneuze plaatsing (bv. geïndentifeerd met de A-DIVA schaal) kan gebruik gemaakt worden van: echogeleide plaatsing; Near-infrared (NIR) vein finders (bijv. VeinViewer met CathCompass) of een transilluminator (bijv. de Veinlite). De kans op een succesvolle eerste poging is onder echogeleide plaatsing 78,9%, onder infrarood licht 58,9% en bij standaardprocedure 62,2% (Poulsen, 2023, Yalçınlı, 2021). Het is echter onduidelijk in hoeverre dit ook daadwerkelijk invloed heeft op het extravasatierisico.

Detectiemethoden

Detectie is het herkennen van een extravasatie zodra deze optreedt, waarna er actie kan worden ondernomen om verdere schade te beperken. Dit verschilt van preventie, waarbij juist wordt geprobeerd extravasatie te voorkomen door een correcte plaatsing.

 

Voor de detectie van extravasatie tijdens de toediening zijn verschillende methodes beschikbaar, elk met hun eigen technische benadering en toepassingsgebied. De meeste van deze methodes zijn gericht op het gebruik bij perifere katheters. Deze technieken worden in Nederland beperkt toegepast.

 

In de volgende tabel worden de meest beschreven technieken besproken, variërend van traditionele klinische inspectie tot meer geavanceerde technologieën zoals bio-impedantie, infraroodthermografie en AI-gestuurde monitoring (Hirata, 2023). De praktische toepasbaarheid van deze methodes is mede afhankelijk van de beschikbaarheid van technologie en middelen binnen de zorginstelling.

 

Enkele experimentele methoden die verder onderzoek behoeven zijn: microgolf monitoring, optische sensor, colorflow injectie, flow-rate variability en AI-videomonitoring. Twee andere methoden voor het detecteren van extravasatie bestaan uit het monitoren van de bultjes en zwellingen van de huid van de patiënt rondom de katheterisatieplaats door gebruik te maken van rek- of scintillatiesensoren. Het gebruik van laatstgenoemden is echter beperkt vanwege de radioactieve media die gebruikt moeten worden.

Detectiemethoden

Overweeg het gebruik van aanvullende detectiemethoden. In de praktijk zijn klinische inspectie en echografie de meest gangbare methoden.

Risicofactoren

Risicofactoren voor het optreden van extravasaties kunnen in grote lijnen onderverdeeld worden in infusievloeistof gerelateerd en patiëntfactoren gerelateerd. Deze overlappen deels met de risicofactoren op een ernstig beloop na optreden van een extravasatie, die in module Specifieke geneesmiddelgroepen op basis van fysisch-chemische en farmacologische kenmerken bij extravasaties worden behandeld.

 

Met betrekking tot de infusiegerelateerde risico’s zijn de geneesmiddeleigenschappen, het volume en de toedieningstijd (al dan niet in relatie tot elkaar) van belang. Een hoge flow over een katheter met een kleine diameter zorgt voor meer schuifspanning (shear stress) op de veneuze vaatwand, waardoor er endotheel dysfunctie kan optreden en daardoor dus complicaties als extravasatie. In geval van een lange toedieningstijd zal er meer kans op complicaties zijn. Het is onduidelijk of een hoge flow/korte toedieningstijd of lage flow/lange toedieningstijd veiliger is met betrekking tot extravasatierisico. Wanneer de pH of osmolaliteit van het geneesmiddel sterk verschilt van bloed, is de kans op flebitis en irritatie groter, met mogelijk extravasatie tot gevolg. Er zijn geen harde grenzen gedefinieerd waarbinnen de pH en osmolaliteit van geneesmiddelen moet liggen om perifeer intraveneus toegediend te kunnen worden. Over het algemeen kan aangehouden worden dat pH > 5 en < 9 en osmolaliteit < 600 mOsmol/kg perifeer mag worden toegediend. Bloed heeft een pH tussen 7,35 -7,45 en een osmolaliteit van ca. 290 mOsmol/kg (Pittiruti, 2023). Hier wordt in module Specifieke geneesmiddelgroepen verder op ingegaan.

 

Met betrekking tot de patiënt gerelateerde risico’s zijn van belang: de toegangspoort, vaatkenmerken, co-morbiditeit, bewustzijn/cognitie en specifieke patiëntcategorieën. Belangrijke voorbeelden hierbij zijn respectievelijk: meerdere eerdere veneuze puncties of intraveneuze perifere katheterisatie van dunne en/of fragiele aderen; obesitas); beweging van de patiënt en het kunnen aangeven van pijn bij toediening; neonaten. In de INS wordt het vrouwelijk geslacht ook als risicofactor genoemd evenals als infectie, ziektes die invloed hebben op het vaatstelsel of verminderde bloedsomloop (kanker, diabetes mellitus, lymfoedeem, systemische lupus, Raynaud syndroom, perifere neuropathie, perifeer vaatlijden) (No authors listed, 2021). 

 

In een case-serie van Sakaida (2014) onder 43557 patiënten met cytostaticatoedieningen werd bij 35 (0.08%) patiënten een extravasatie vastgesteld. Hierbij is een associatie gelegd met een langere infusieduur. In 80% van de extravasaties betrof het een infusieduur van meer dan 2 uur. Daarnaast ontstond de extravasatie bij ruim 30% van de patiënten na een toiletbezoek. Hieruit werd de conclusie getrokken dat er verbetering nodig was met betrekking tot het fixeren van de katheter en de instructie dat patiënten zoveel mogelijk stil moeten blijven zitten (Sakaida, 2014).

 

In de retrospectieve studie Park et al. zijn risicofactoren bij kinderen onderzocht onder 1174 klinisch opgenomen kinderen. Hierin bleek een incidentie van infiltraties 7,8% waarbij de volgende risicofactoren waren geïdentificeerd: toediening via onderste ledemaat, fenytoïne toediening evenals glucose 10%, steroïden, vancomycine, hoog geconcentreerde elektrolyten en ampicilline/sulbactam. Het wordt uit de studie niet duidelijk welke extravasaties geregistreerd worden en wat de verdeling is over de graderingen. Of er een causale relatie is met deze geneesmiddelen en het ontstaan van extravasatie is onduidelijk. Deze middelen hebben veelal een extreme pH of osmolaliteit. Een extreme pH zorgt voor instabiliteit van eiwitten en de osmolaliteit voor het verplaatsen van vloeistoffen naar interstitium. De auteurs suggereren dat gebruik van extreme pH en/of osmolaliteit de functionaliteit van de vaatwand zou kunnen aantasten waardoor er sneller een extravasatie plaatsvindt. (Park, 2015)

 

Een retrospectieve studie onderzocht de ernst van extravasaties bij verschillende patiëntengroepen in een Chinees ziekenhuis. De onderzoekers observeerden dat vooral jonge kinderen en ouderen met comorbiditeiten, zoals diabetes, significant vaker een ernstiger beloop van een extravasaties ontwikkelden. De studie benadrukte ook het belang van vroege detectie en preventieve maatregelen, zoals centrale katheters bij risicopatiënten (Zhang, 2017).

 

Bij patiënten met een verhoogd risico op extravasatie (zoals ouderen, obesitas, neuropathie, oedeem, oncologische patiënten, verwarde patiënten en neonaten) (Becker 2011; Vademecum Hematologie, 2023) is extra alertheid aan te bevelen. De werkgroep heeft geen specifieke literatuursearch uitgevoerd naar de waarden en voorkeuren van patiënten in verband met de preventie van extravasaties. Preventie hiervan zal vanwege het discomfort en mogelijk vertraging van medicamenteuze behandeling gewenst zijn. De preventieve maatregelen zijn niet invasief en wegen vanuit patiëntperspectief op tegen de complicatie. Voor preventie van extravasatie en vroegtijdige detectie is het voor patiënten (naasten en verzorgers) van belang om een goede en heldere uitleg te krijgen over het aanleggen van een infuus (katheter).

 

Kostenaspecten

De werkgroep heeft geen specifieke literatuursearch uitgevoerd naar de kostenaspecten van de preventie van extravasaties.

 

Over het algemeen kan gesteld worden dat de preventie van extravasaties gepaard gaat met meer zorgkosten omdat het tijd van zorgpersoneel vraagt. Het voorkomen ervan door standaardisatie en de naleving van aanbevelingen over preventie resulteert naar verwachting in minder extravasaties en andere complicaties, wat vervolgens weer tijd van verschillende zorgverleners (zaalarts, achterwacht specialist, (ziekenhuis)apotheker, (plastisch) chirurg) oplevert omdat er geen extra consulten zijn.

 

Het artikel "Cost effectiveness of a vascular access education and training program for hospitalized emergency department patients" onderzoekt de kosteneffectiviteit van een opleidingsprogramma voor personeel wat perifere veneuze toegangen plaatst in een ziekenhuisomgeving. Hoewel het artikel op meer dan alleen extravasaties in gaat, zijn de bevindingen relevant voor het verbeteren van de zorgkwaliteit en het verminderen van complicaties zoals extravasaties. Conclusie was dat de investeringen in onderwijs en training aanzienlijke financiële voordelen kunnen opleveren en tegelijkertijd de uitkomsten van vasculaire toegang verbeteren (Bahl, 2024).

 

Gelijkheid ((health) equity/equitable)

De werkgroep voorziet geen bezwaren met betrekking tot gelijkheid bij de preventie van extravasaties. Wanneer een zorginstelling de keuze maakt een groot deel van de preventie bij de patiënt neer te leggen, kan er ongelijkheid ontstaan tussen patiënten: bijvoorbeeld bij patiënten die geen pijn kunnen aangeven (sedatie, veel pijnstilling etc.) of bij patienten die laaggeletterd zijn of een taalbarrière hebben waardoor ze de uitleg niet begrijpen.

 

Aanvaardbaarheid

Bij de preventie van extravasaties spelen duurzaamheidsaspecten een rol. Er is een spanningsveld tussen het voorkomen van een extravasatie en de maatregelen die mogelijk zijn om dit te minimaliseren. Als voorbeeld: frequente controles door een zorgverlener. De bewijslast is beperkt, maar er zijn veel maatregelen mogelijk die tijd kosten. Het is aan de praktijk hier een afweging over te maken lokaal beleid voor op te stellen.

 

Haalbaarheid

De werkgroep acht educatie als de belangrijkste preventieve maatregel tegen extravasaties haalbaar en ziet geen belemmeringen voor de implementatie.

 

Rationale van de aanbeveling: weging van argumenten voor en tegen de interventies

Aanbevelingen met betrekking tot algemeen beleid zijn met name gebaseerd op observationele, retrospectieve studies en ervaringen uit de praktijk. Neonaten en kinderen vallen binnen de scope, maar vereisen populatie-specifieke interpretatie van aanbevelingen.

 

Het voorkomen van extravasatie is van belang voor het waarborgen van de veiligheid en het minimaliseren van de risico’s voor patiënten die intraveneuze geneesmiddelen toegediend krijgen. Hierbij is de keuze van de toedieningsweg en -plaats en de juiste techniek bij het inbrengen van infuus of katheter zeer belangrijk. Educatie van zorgprofessionals is cruciaal, zij dienen goed op de hoogte te zijn van (patiëntgebonden) risicofactoren, monitoren de infusieplaats en informeren de patiënten over signalen waarop zij alert moeten zijn.

 

Ontwikkel een preventieprogramma dat gericht is op het voorkomen van extravasaties. Educatie en training van zorgverleners en patiënten is een belangrijke preventieve maatregel die hierin terug zou moeten komen. De preventieve maatregelen zijn uitgebreid en veelal tijdsintensief, maar zullen het optreden van extravasaties niet geheel voorkomen. Derhalve verdient het de aanbeveling om naar haalbaarheid in de eigen praktijk vast te stellen welke maatregelen ingevoerd dienen te worden.

The occurrence of extravasation can have both short- and long-term consequences for the patient. Therefore, it is important to organize care in such a way that extravasation occurs as little as possible. In current practice, it is unclear which actions contribute to preventing extravasation. This module provides an overview of possible interventions. Prevention of extravasation leads to health benefits for the patient.

The level of evidence regarding the outcomes was not graded, as no systemic literature synthesis was performed.

In a routine setting, the standardized management of cytotoxic extravasations by an interdisciplinary task force resulted in a satisfactory outcome. The incidence of thrombophlebitis and extravasation appeared to be influenced by the vein into which the catheters were directed.

Breast cancer chemotherapy via TIVAD is safe and convenient and results in high satisfaction levels, although it involves additional expenditure. Chemotherapy via PIVA is acceptable, albeit with lower satisfaction, more so in those receiving more than six chemotherapy cycles.

Our results suggest that intravenous therapy with MCs is generally safe for prolonged courses that do not exceed 14 days as compared with PICC lines, which can be placed for months. There is still limited evidence for the use of MCs between 14 and 28 days of therapy. This study can help guide our selection of intravenous catheters for the purpose of outpatient antimicrobial therapy.

Although the CICS has a small lumen, it allows a high flow rate; therefore, IV access required for CCTA can be secured with higher vascular attenuation and fewer complications. It is encouraged to use the CIVC system for CT angiographic studies, in particular CCTA.

As no studies were included in the literature summary, no elaborate result section is available. Studies reporting about the treatment of extravasation, are presented in Table 2.

A systematic review of the literature was performed to answer the following question(s):

What is the effect of [intervention x] versus [intervention y or standard of care] on the incidence of extravasation, grade of extravasation (including symptoms according to grade), successful intended treatment, patient satisfaction and health professional satisfaction in patients in whom an extravasation of medication occurs?

Search and select (Methods)

A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 1996 to July 9th, 2024, for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from one primary search concept: (1) extravasation. Duplicates were removed using EndNote software. 

 

The detailed search strategy is depicted under the tab Methods. The systematic literature search resulted in 6831 hits. Titles and abstracts were screened using the ASReview software. The settings TF-IDF and Naïve bayes were used [Ly, 2017; Marsh, 2020; Gopalakrishnan, 2017; Thomas, 1997; Sagheb, 2022; Corbett, 2019; Corbet, 2018; Roy, 2022; Dionyssiou, 2011; Molas-Ferrer, 2015; Hermitte-Gandoliere, 2018; Plum, 2017; Ye, 2022; Mu, 1999] was used as prior knowledge for inclusion [Lee, 2018; Cvetkovic, 1999; Julka, 2014; Sabe, 2009; Vadi, 2022] was used as prior knowledge for exclusions. The first 10% of hits were screened by the working group and the guideline methodologist. The remaining articles were subsequently screened by the guideline methodologist, using the following stopping rule: stop after 200 subsequent exclusions.

 

Studies were thereafter screened for 1) intervention aiming prevention or treatment of extravasation, or local policy 2) primarily investigating extravasation. Based on title and abstract, 612 articles were initially selected. After screening the full-text articles, a total of 64 studies were included, and 530 studies were excluded. The complete overview of excluded studies from ASReview is available upon request from the Knowledge Institute.

 

During the screening, the types of publications and interventions were considered. Due to the broad clinical question, it is relevant to mention all identified interventions. The module was deliberately kept broad in order to provide an overview, rather than elaborating on individual PICOs. Studies are presented in the table. No level of evidence is described. The considerations provide a deeper analysis of the results and place them in the context of Dutch healthcare.

Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz

Bij de richtlijnmodule voerde de werkgroep conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).

Hoewel uit de toetsing volgt dat de aanbeveling(en) breed toepasbaar zijn (>40.000 patiënten), volgt ook uit de toetsing dat het overgrote deel (±90%) van de zorgaanbieders en zorgverleners al aan de norm voldoet en/of het geen nieuwe manier van zorgverlening of andere organisatie van zorgverlening betreft, het geen toename in het aantal in te zetten voltijdsequivalenten aan zorgverleners betreft en het geen wijziging in het opleidingsniveau van zorgpersoneel betreft. Er worden daarom geen substantiële financiële gevolgen verwacht.
