Genetische diagnostiek bij sporadische cerebellaire ataxie
Uitgangsvraag
Is genetische diagnostiek bij personen met sporadische cerebellaire ataxie op volwassen leeftijd (SAOA, sporadic adult onset ataxia) zinvol?
Aanbeveling
Bied elke persoon met sporadische cerebellaire ataxie, genetische diagnostiek aan, of verwijs naar een klinisch geneticus.
- Deze genetische diagnostiek omvat minimaal het testen van repeat expansies in ATXN1 (SCA1), ATXN2 (SCA2), ATXN3 (SCA3), CACNA1A (SCA6), ATXN7 (SCA7), TBP (SCA17), FGF14 (SCA27B), FXN (Friedreich) en RFC1 (CANVAS), én sequentieanalyse van het SPG7-gen*.
- Bespreek met uw klinisch genetisch laboratorium of alle genen uit de minimale set daadwerkelijk wordt aangeboden, of verstuur indien nodig naar een ander academisch genetisch laboratorium.
Debuutleeftijd:
Voer bij een debuutleeftijd onder de 50 jaar aanvullend op bovenstaand een genpanel ataxie/Bewegingsstoornissen* uit.
Bij een debuutleeftijd boven de 50 jaar kan een aanvullend genpanel ataxie/ bewegingsstoornissen overwogen worden. Houd hierbij rekening met de volgende aspecten:
- Debuutleefijd én huidige leeftijd van een persoon; let op dat bij hogere leeftijd segregatieonderzoek bij familieleden lastig is.
- Familieleden met vragen over herhalingsrisico.
- Kinderwens bij familieleden.
Overleg bij twijfel over het uitvoeren van genetisch onderzoek met een expertisecentrum (zie module organisatie van zorg).
*Deze genen/ repeatexpansies zijn in sommige laboratoria ondergebracht in één ataxie/bewegingsstoornissenpanel. Eén aanvraag kan dan volstaan. Zie de VKGL-website voor aanbod van genetische diagnostiek.
Overwegingen
Waarom deze aanbeveling?
Er is, ongeacht de debuutleeftijd, wel reden voor genetische diagnostiek bij personen met sporadische ataxie op volwassen leeftijd omdat gebleken is dat bij 10-30% een genetische oorzaak wordt aangetoond. De voordelen van het weten van de genetisch oorzaak zijn: duidelijkheid omtrent oorzaak en beloop, gevolgen voor familieleden, kinderwens in de familie en eventuele behandeling. Nadelen in sommige gevallen kan zijn het krijgen van een onduidelijke uitslag of negatieve psychosociale gevolgen. Bij een debuutleeftijd onder de 50 jaar wordt een aanvullende brede genetische analyse geadviseerd, terwijl bij een hoge debuutleeftijd deze keuze afhangt van persoonlijke aspecten. De inhoud van de genpanels voor ataxie en bewegingsstoornissen verschilt per academisch ziekenhuis.
Eindoordeel:
Sterke aanbeveling voor genetische diagnostiek bij sporadische, cerebellaire ataxie.
Balans tussen gewenste en ongewenste effecten
Er werd een literatuursearch uitgevoerd naar studies die de prevalentie van gevonden (waarschijnlijk) pathogene genetische varianten rapporteren bij mensen met sporadische, late onset cerebellaire ataxie (zie tabel 2 in de bijlage).
Veel studies geven een incompleet beeld van de diagnostische opbrengt van het genetisch onderzoek. Een deel van de studies keek alleen naar de opbrengst van één gen (Barghanini, 2022; Cortese, 2019; Kartanou, 2024; Kontogeorgious, 2021; Montaut, 2021; Ouyang, 2024; Pellerin, 2023; Satolli, 2024; Traschutz, 2021; Wan, 2020), includeerde slechts één specifiek fenotype (Genis, 2025), maakte geen onderscheid in gevonden resultaten tussen familiaire en sporadische ataxie of rapporteerde deze karakteristiek niet of niet volledig (Genis, 2025; Iruzubieta, 2023; Kontogeorgio 2021; Ouyang, 2024; Pellerin, 2023; Traschutz, 2021). Daarnaast zijn niet in alle studies volledige karakteristieken van de cohorten beschreven. Vaak vond al een selectie plaats o.b.v. eerder genetisch onderzoek. Hier moet rekening mee worden gehouden in de interpretatie van de resultaten. Omdat deze studies wel enigszins een indruk geven hoe vaak bepaalde genetische diagnosen worden gesteld, zijn deze wel opgenomen in de tabel.
Uiteindelijk zijn er 2 studies die wat betreft de werkgroep het beste voldeden aan de PICO waarbij de opbrengt van genetisch onderzoek wordt beschreven in een opeenvolgend cohort van sporadische personen met late onset cerbellaire ataxie (Bogdan, 2022; Koutsis, 2024). Deze studies zijn hieronder in meer detail beschreven.
De studie van Bogdan (2022) rapporteert in een opeenvolgend cohort van 205 personen met sporadische late onset ataxie (gemiddelde beginleeftijd 57 jaar) een genetische diagnose in 20 personen (10%). Hierbij dient opgemerkt te worden dat in dit cohort SCA27b niet was getest. Tevens komt uit de methode niet geheel duidelijk naar voren of elke persoon met cerebellaire ataxie elke genetische test heeft ondergaan. Dit kan dus een onderschatting geven.
Het cohort van Koutsis (2024) bestaat uit 206 opeenvolgende personen met ataxie (57 familiair en 149 sporadisch) met een beginleeftijd boven de 30 jaar (gemiddelde beginleeftijd 52 jaar) die in de periode 1995-2023 het landelijk referentiecentrum voor erfelijke ataxie in Athene bezochten. Een genetische diagnose werd gesteld bij 34 van de 149 sporadische ataxie personen (23%). Ook hier waren de meest voorkomende diagnoses SCA27b en CANVAS, gevolgd door Friedreich’s ataxie. Deze studie verrichtte echter alleen het testen van de bekende repeat expansies en additioneel (Next Genereation) sequencing werd niet verricht. Dit kan dus een onderschatting geven.
Omdat na het voltooien van de literatuursearch van de richtlijn nog een belangrijke studie is uitgekomen, zal deze hier nog kort worden besproken. In de studie van Beijer (2025) worden de resultaten beschreven van een groot Europees multicenter cohort van sporadische ataxie personen met beginleeftijd > 40 jaar die negatief waren getest op SCA1, 2, 3, 6, Fragiele X en Friedreich’s ataxie. In totaal werden 337 personen met cerebellaire ataxie geïncludeerd (n=229 SAOA en 148 MSA-C). In de SAOA-groep (gemiddelde beginleeftijd 57 jaar) werd bij 67 van de 229 (29%) personen een genetische diagnose gesteld, waarvan SCA27b het meest frequent was gevolgd door CANVAS. In het cohort met klinische diagnose MSA-C was de genetische opbrengst 18 van de 148 (12%) personen.
Bovenstaande studies tonen aan dat de prevalentie van genetische oorzaken bij volwassenen met cerebellaire ataxie aanzienlijk is (10-30%). Dit kan enerzijds een onderschatting zijn omdat, in de gebruikte literatuur, niet alle oorzakelijke genen getest zijn. Er kan ook sprake zijn van een overschatting omdat de gebruikte cohorten geen afspiegeling zijn van een naïeve persoon met ataxie in de spreekkamer, d.w.z. dat researchcohorten soms een selectie zijn van personen met ataxie die beter klinisch zijn onderzocht en tertiaire expertisecentra vaak referral bias kennen. Daarnaast hanteerden de studies verschillende inclusiecriteria voor late-onset.
Bovenstaande studies (Bogdan, 2022; Koutsis, 2024; Beijer 2025) tonen tevens aan dat de gemiddelde beginleeftijd in alle studies boven de 50 jaar was, waardoor de werkgroep van mening is dat het hanteren van een leeftijdsgrens als bovengrens voor genetisch diagnostiek obsoleet is geworden. Bij een debuutleeftijd onder de 50 jaar wordt een brede genetische analyse geadviseerd. Bij cerebellaire ataxie op hoge leeftijd is het aan de aanvrager om in samenspraak met een persoon te beoordelen of het inzetten van brede genetische diagnostiek zinvol en gewenst is. Deze keuze hangt af van persoonlijke aspecten, zoals vragen rondom herhalingskans of kinderwens bij familieleden. Men dient zich ervan bewust te zijn dat bij een hoge leeftijd segregatieonderzoek van onduidelijke genetische varianten vaak lastig is omdat ouders niet meer in leven zijn.
De inhoud van de genpanels voor ataxie en bewegingsstoornissen verschilt per academisch ziekenhuis. In sommige centra is het (nog) niet mogelijk om bepaalde genen met repeat expansies te testen. De verwachting is echter dat dit met de komst van nieuwe genetische technieken (long read whole genome sequencing) de komende jaren zal veranderen.
Kwaliteit van bewijs
Niet van toepassing voor deze module.
Waarden en voorkeuren van personen met ataxie (en eventueel hun naasten/verzorgers)
Het aantonen van een genetische oorzaak heeft verschillende voordelen:
- het geeft een verklaring voor de symptomen bij de persoon met ataxie;
- het geeft informatie over de (wijze van) overerving en daarbij het risico voor andere familieleden;
- het biedt mogelijkheden voor voorspellend genetisch onderzoek van gezonde familieleden;
- het biedt reproductieve mogelijkheden zoals prenataal onderzoek of pre-implantatie genetische test (PGT);
- het leidt in sommige gevallen (bijvoorbeeld bij SCA27b) tot mogelijkheden voor behandeling.
Ongewenste effecten van genetische testen:
- soms kan genetisch onderzoek tot een onduidelijke uitkomst leiden, zoals een repeat lengte in het gebied van incomplete penetrantie, of een variant van onbekende betekenis (z.g.n. ‘VUS’, variant of unknown significance);
- er bestaat een kleine kans op een nevenbevinding bij uitgebreid onderzoek d.m.v. een breed genpanel. Zie de VKGN-website voor up-to-date richtlijnen;
- het aantonen van een genetische oorzaak kan soms tot negatieve psychosociale effecten leiden voor persoon met ataxie en/of diens familieleden (angst, schuldgevoelens, onzekerheid, veranderde familiedynamiek, ongerustheid rondom levensverzekering of arbeidsongeschiktheidsverzekering);
- het aantonen van een genetische oorzaak leidt niet altijd tot mogelijkheden voor behandeling.
Een goede voorlichting in de pre-test counseling is hierbij essentieel. Zorgprofessionals kunnen de leidraad ‘aanvragen genetische diagnostiek in de kiembaan door niet klinisch genetische zorgprofessionals’ raadplegen op de website van de VKGN (https://www.vkgn.org/). Kostenaspecten
Het gebruik van genetisch onderzoek leidt tot een aanzienlijke kans op het stellen van een onderliggende diagnose. Door de komst van nieuwe genetische technieken, wordt het steeds goedkoper om meerdere genen met dezelfde test te onderzoeken. Het aantonen van een genetische oorzaak levert een completer beeld op over de oorzaak en prognose van de ataxie, en daarmee potentieel tot minder ziekenhuisbezoeken van de persoon met ataxie.
Gelijkheid ((health) equity/equitable)
De interventie leidt tot een mogelijke afname van gezondheidsgelijkheid, omdat uit eerder onderzoek is gebleken dat genetisch onderzoek minder toegankelijk is voor personen met een lager opleidingsniveau, sociale status, afkomst en geloofsovertuiging (Gold, 2026).
Aanvaardbaarheid
Ethische aanvaardbaarheid
De genetische diagnostiek bij SAOA lijkt aanvaardbaar voor de betrokkenen. Er zijn geen medisch ethische bezwaren indien adequate genetische counseling plaatsgevonden heeft door een klinisch geneticus of neuroloog met expertise in erfelijke ataxie.
Duurzaamheid
Niet van toepassing.
Haalbaarheid
Genetische diagnostiek is bij personen met ataxie over het algemeen al standaardzorg in de praktijk en behoeft slechts een aanpassing in de selectiecriteria.
Onderbouwing
In patients with suspected genetic causes of cerebellar ataxia, DNA testing is typically performed. For patients with sporadic adult onset ataxia (SAOA), this has traditionally depended on age. However, in recent years, new genes have been identified that can cause symptoms even later in life. This raises the question of whether DNA testing should be considered for all individuals with SAOA. To address this, it is important to determine how often a genetic cause is identified in people with SAOA.
Description of studies
A total of fourteen studies were included in the analysis of literature. Since no comparative studies were included, no summary of findings table and risk of bias tables were constructed, and no GRADE assessment was performed. Only a descriptive table was provided. Table 2 describes the characteristics of the included studies and total (likely) pathogenic variants reported. More detailed descriptions of the prevalence of the type of variants reported can be found in table 3.

A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all. Both databases were searched from 2019 to March 10th, 2025. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) ataxia; (2) SCA27B, FGF14, CANVAS, RFC1. Duplicates were removed using EndNote software. After deduplication a total of 272 records were imported for title/abstract screening.
Studies were selected based on the following criteria:
- Systematic review (searched in at least two databases, and detailed search strategy, risk of bias assessment and results of individual studies available); randomized controlled trials or observational studies about the prevalence of (likely) pathogenic variants in DNA-tests.
- Adults with sporadic cerebellar ataxia (onset >30 years).
- Inclusion of at least CANVAS or SCA27B screening.
- Full-text English language publication; and
- Studies according to PICO.
Initially, 43 studies were selected based on title and abstract screening. After reading the full text, 29 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and initially fourteen studies were included. Finally, two of these studies fulfilled all criteria and were used for this guideline.

Inbreng patiëntenperspectief
De werkgroep besteedde aandacht aan het patiëntenperspectief door de Ataxie Vereniging Nederland uit te nodigen voor de schriftelijke knelpuntenanalyse en een patiëntvertegenwoordiger namens Ataxie Vereniging Nederland was betrokken bij de ontwikkeling van de modules. De verkregen input is meegenomen bij het opstellen van de uitgangsvragen, de keuze voor de uitkomstmaten en bij het opstellen van de overwegingen. De conceptrichtlijn is tevens voor commentaar voorgelegd aan de Ataxie Vereniging Nederland en de eventueel aangeleverde commentaren zijn bekeken en verwerkt.
Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule voerde de werkgroep conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).

A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all. Both databases were searched from 2019 to March 10th, 2025. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) ataxia; (2) SCA27B, FGF14, CANVAS, RFC1. Duplicates were removed using EndNote software. After deduplication a total of 272 records were imported for title/abstract screening. |
