Diagnostiek bij verworven, niet-immuungemedieerde ataxie
Uitgangsvraag
Wat is de waarde van het testen op vitamine E, vitamine B1, vitamine B12, calcium, anti-TPO, TSH bij personen met een verdenking op een verworven cerebellaire ataxie?
Aanbeveling
Voer niet standaard laboratoriumtesten naar calcium, TSH, vitamine b1, vitamine b12 en vitamine E uit bij een persoon met een (geïsoleerde) geleidelijk progressieve late-onset cerebellaire ataxie (gedefinieerd als een periode van >6 maanden vanaf ontstaan tot invaliderende klachten)*.
Overweeg de bovengenoemde testen alleen in geval er sprake is van klinische factoren/specifieke patiëntkarakteristieken:
- TSH bij fysieke kenmerken van schildklier disfunctioneren; zoals bij hypothyreoïdie: vermoeidheid, gewichtstoename, koude-intolerantie, obstipatie, cognitieve klachten (concentratie en geheugen); of bij hyperthyreoïdie: angst, slapeloosheid, hartkloppingen, gewichtsverlies en intolerantie voor warmte.
- Vitamine B1 (thiamine) alleen in te zetten indien er indicaties zijn voor een mogelijke malabsorptie of verminderde dietaire inname (o.a. veganistisch dieet)
- Vitamine B12 (cobolamine) alleen in te zetten indien er bijkomend sensibele verschijnselen zijn of bij indicaties voor een mogelijke malabsorptie (o.a. bariatrische chirurgie, pernicieuze anemie, Helicobacter pylori gastritis) of verminderde dietaire inname (o.a. veganistisch/vegetarisch dieet)
- Vitamine E bij personen met een bekende gastro-intestinale aandoening (zoals coeliakie, inflammatoire darmaandoening) of kenmerken die een aanleiding geven om aan een van deze aandoeningen te denken.

Overwegingen
Waarom deze aanbeveling?
Op basis van de huidige literatuur én op basis van uitgebreide ervaring binnen de expertisecentra blijkt de opbrengst (met behandelconsequenties) van routinematige laboratoriumtesten bij personen met geleidelijk progressieve cerebellaire ataxie zeer laag. Gezien de nieuwe ontwikkelingen op het genetische vlak, waarbij in veel meer gevallen ook bij personen met een sporadische late-onset cerebellaire ataxie een causale genetische mutatie wordt gevonden is de plaats van de laboratoriumtesten nog beperkter geworden.

Kwaliteit van bewijs
Er is slechts één studie gevonden waarop geen GRADE beoordeling is uitgevoerd.
Balans tussen gewenste en ongewenste effecten
Er is één studie gevonden die de resultaten van laboratoriumtesten naar vitamine E, vitamine B1, vitamine B12, calcium, anti-TPO, TSH bij 205 personen met een verdenking op een late, verworven cerebellaire ataxie rapporteerde. Deze studie beschreef een diagnostische opbrengst van 0% voor de resultaten van vitamine E en anti-TPO. Andere testresultaten werden niet gerapporteerd.
Gezien het beperkte bewijs voor de uitvoer van diagnostische testen bij verdenking op een verworven cerebellaire ataxie in de gerapporteerde studie en de ervaring binnen de expertisecentra is het advies deze niet standaard in te zetten.
Echter, er zijn wel enige aanvullende opmerkingen te maken over de wijze waarop deze bepalingen worden ingezet. De a priori kans op het vinden van afwijkende testresultaten is nl. groter wanneer er sprake is van een subacute presentatie van cerebellaire ataxie en/of in combinatie met het vinden van andere neurologische verschijnselen of bewegingsstoornissen. Deze aspecten dienen meegenomen te worden in de overweging om wel of niet bepaalde testen uit te laten voeren. Bij een geleidelijk progressieve cerebellaire ataxie zonder bijkomende andere verschijnselen zoals hieronder beschreven is de kans op het vinden van klinisch relevante afwijkingen middels deze diagnostische testen verwaarloosbaar klein.
Bij zowel een hypo- als hyperthyreoïdie is bijv. tremor het meest voorkomende neurologische verschijnsel en slechts zelden (bij minder dan 100 patiënten) werd ooit een cerebellair syndroom vastgesteld (Ercoli, 2019; Schneider, 2023). En dan ook nog eens bij casus die ook duidelijk systemisch kenmerken van schildklierdisfunctioneren vertoonden, zoals vermoeidheid, gewichtstoename, koude-intolerantie, obstipatie, cognitieve klachten (concentratie en geheugen) frequent bij een hypothyreoïdie, en angst, slapeloosheid, hartkloppingen, gewichtsverlies en intolerantie voor warmte bij een hyperthyreoïdie worden gevonden.
Indien een hypercalciëmie leidt tot cerebellaire ataxie ontstaat dit alleen bij een snel ontstane of extreem hoge calciumwaarden (Howard, 2025). Bij een ernstige of snel ontstane hypercalciëmie worden ook kenmerken als misselijkheid, braken, uitdroging, ook desoriëntatie en een gedaald bewustzijn gevonden. Binnen het kader van een chronisch cerebellaire ataxie is er geen afdoende rationale het calcium te bepalen.
Verworven vitamine E deficiënties gaan ondanks een geleidelijk ontstaan en progressie van cerebellaire ataxie vaak gepaard met gastro-intestinale aandoeningen (zoals bijvoorbeeld coeliakie en inflammatoire darmziekten of status na bariatrische chirurgie) die aanleiding geven tot malabsorptie (Aslam, 2004). In de aanwezigheid van deze kenmerken is het onzes inziens geïndiceerd de vitamine E te bepalen.
Al langere tijd is het de clinicus bekend dat een tekort aan vitamine b1 (thiamine) kan leiden tot cerebellaire ataxie. Bij verklaringen leidend tot een thiamine tekort, zoals maar niet alleen bij alcoholabusus, hyperemesis (gravidarum)en bariatrische chirurgie, ontstaan de symptomen vaak subacuut daar thiamine een essentieel onderdeel is de vorming van ATP uit glucose. Meest voorkomende neurologische kenmerken zijn polyneuropathie (paresthesieën en paresen), stemmingsstoornis en geheugenstoornissen. En bij een ernstig tekort ontstaat Wernicke encefalopthie, waar cerebellaire ataxie een van de drie kardinale kenmerken is. Vitamine b1 dient altijd bepaald te worden bij personen met een subacuut ontstane cerebellaire ataxie. Bij chronische cerebellaire ataxie is de aanbeveling vitamine b1 alleen in te zetten indien er indicaties zijn voor een mogelijke malabsorptie of verminderde dietaire inname (o.a. veganistisch).
Vitamine b12 (cobalamine) deficiëntie leidt tot een verscheidenheid aan systemische kenmerken, zoals o.a. vermoeidheid, gewichtsverlies, en anemie. Neurologische symptomen als gevolg van een cobalamine tekort zijn vooral sensibele verschijnselen (verminderde positie- en vibratiezin) in combinatie met spasticiteit (gecombineerde strengziekte). In dat kader wordt ook vaak een sensore ataxie gevonden. Een geïsoleerd cerebellaire ataxie ten gevolge van een cobalamine tekort is uiterst zeldzaam. Bij chronisch ataxie is de aanbeveling vitamine b12 alleen in te zetten indien er bijkomend sensibele verschijnselen zijn of bij indicaties voor een mogelijke malabsorptie (o.a. bariatrische chirurgie, pernicieuze anemie, Helicobacter pylori gastritis) of verminderde dietaire inname (o.a. veganistisch/vegetarisch).
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
Ondanks dat er in de literatuur en in de praktijk weinig bewijs is voor het nut van het uitvoeren van testen bij verdenking op een verworven niet immuun gemedieerde ataxie, bestaat er een kans op het vinden van een behandelbare aandoening. Dit geeft de persoon met ataxie hoop op het vinden van een oorzaak en behandeling van hun klachten, als ook het gevoel dat er alles aan gedaan is om een eventuele oorzaak te vinden. Enig of volledig herstel van klinische verschijnselen is vaak meer dan afdoende motivatie voor de persoon met ataxie te opteren voor oriënterend bloedonderzoek.
Het behandelen van bijvoorbeeld een gevonden schildklier dysfunctie kan bij 57-74% tot een volledig klinisch herstel leiden (Ercoli, 2019).
Overigens betekent het vinden van positieve laboratoriumbepalingen niet altijd dat de symptomen volledig behandelbaar zijn. Zo blijkt het merendeel van de personen met een verworven vitamine E deficiëntie slechts gedeeltelijk te herstellen (Aslam, 2004).
In veel gevallen zou het standaard inzetten van bloedonderzoek kunnen worden ontraden en dient men het zeer beperkte nut en de mogelijkheden van testen in relatie tot de waarden en voorkeuren van de individuele persoon met ataxie te bespreken. De uiteindelijke afweging is voor iedereen anders. Het hangt af van zowel de individuele factoren die de kans op een bepaalde diagnose en daarmee de kans op een positieve lab-bepaling en een passende behandeling bepalen als ook van de individuele voorkeuren van een persoon.
Kostenaspecten
Er zijn bij ons geen kosten-effectiviteitsstudies bekend. De kosten voor bloedonderzoek (alle in de ze paragraaf genoemde bepalingen samen kosten <150 euro) zijn in vergelijking met de kosten van de andere testen (MRI-scan Hersenen 300-480 euro en WES 1000-1500 euro) beperkt te noemen.
Uitgaande van en extrapolerend naar de Nederlandse situatie op de door Muzaimi (2004) genoemde incidentie van op late leeftijd ontstane cerebellaire ataxie van 0,3 per 100.000 inwoners zou het niet inzetten van bloedonderzoek tot een potentiële kostenbesparing van maximaal 7650 euro per jaar kunnen leiden. Dit is maximaal 12% van het totaal aan diagnostiek kosten voor deze patiëntengroep. Naar de mening van de commissie is dit een grote overschatting daar in de praktijk er toch specifieke kenmerken of omstandigheden zijn waarop besloten wordt bloedonderzoek in te inzetten.
Gelijkheid ((health) equity/equitable)
Er zijn geen redenen aan te nemen dat bij de overweging van lab testen ongelijkheid zou kunnen ontstaan die kunnen leiden tot verschillen in gezondheidsgelijkheid.
Personen met ataxie zullen eigen risico verbruikt hebben voordat deze testen overwogen worden.
Aanvaardbaarheid:
Diagnostiek lijkt aanvaardbaar voor alle personen, er zijn geen ethische bezwaren bekend.
Er lijken geen grote duurzaamheidsaspecten een rol te spelen bij het uitvoeren van deze testen, aangezien standaardtesten ontraden worden.
Haalbaarheid
De diagnostiek lijkt haalbaar. De diagnostiek is over het algemeen al standaardzorg in de praktijk.
Onderbouwing
In the case of suspicion of acquired cerebellar ataxia, various antibody tests can be performed, including vitamin E, vitamin B1, vitamin B12, anti-TPO, calcium and TSH testing. The benefit of these tests in diagnosing an acquired CA is not always clear. In practice, tests are still routinely utilized as missing a potentially treatable cause may have grave consequences. The question is how effective are these (antibody) tests for diagnosis and what role should these tests have in the diagnosis of acquired cerebellar ataxia?
Description of studies
One study was included in the analysis of the literature. Important study characteristics and results are summarized in table 2. The assessment of the risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).
Bogdan (2022) performed a monocentric, prospective study aiming to investigate the causes of sporadic late-onset cerebellar ataxia (SLOCA). Patients were included if: cerebellar ataxia (CA) occurred after 40 years, the disease had a chronic and progressive evolution for at least 3 months, and no family history of CA was documented. Patients with a follow-up of less than 1 year were excluded. Patients recruited in Strasbourg University Hospital were included from January 2013 to December 2019.

Bogdan reports diagnostic yield of patients that were tested for: Vitamin E, Anti-TPO and Anti-GAD. They report a yield of 0 to <1% of these tests (Table 3).

Bogdan (2022) diagnosed 135 of 205 patients SLOCA patients (66%): 84 patients (41%) were diagnosed with multiple system atrophy cerebellar type (MSA-C) and 51% were diagnosed with another diagnosis. Of the 205 patients, 70 (34%) patients remained without diagnosis. These patients formed the idiopathic late-onset cerebellar ataxia (ILOCA) group.
Treatment of acquired cerebellar ataxia

‘The ideal body of evidence would include studies that directly compare the test strategies under consideration (i.e., randomized trials) and the resulting interventions and consequences (i.e., patient-important outcomes). Such studies would, by design, address all of the issues in the analytical framework and allow guideline panelists to apply the familiar GRADE approach for interventions’ (Schünemann, 2019). For most tests or test- and treat strategies, however, this direct evidence does not exist.
A systematic review of the literature was performed to answer the following question(s):
What are the (un)beneficial effects of vitamin E (RRR -α-tocoferol), vitamin B1 (thiamin), vitamin B12 (cobalamine), anti-TPO, TSH (thyreotropine) and calcium testing on the diagnosis and consequences (treatment and the course) of ataxia when compared to not testing these antibodies in patients with suspected acquired cerebellar ataxia?

Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all. Both databases were searched from 2005 to April 9, 2025 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) cerebellar ataxia; (2) antibody tests. Duplicates were removed using EndNote software. After deduplication a total of 990 records were imported for title/abstract screening.
Initially, 33 studies were selected based on title and abstract screening. After reading the full text, 32 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and 1 study was included.

Inbreng patiëntenperspectief
De werkgroep besteedde aandacht aan het patiëntenperspectief door de Ataxie Vereniging Nederland uit te nodigen voor de schriftelijke knelpuntenanalyse en een patiëntvertegenwoordiger namens Ataxie Vereniging Nederland was betrokken bij de ontwikkeling van de modules. De verkregen input is meegenomen bij het opstellen van de uitgangsvragen, de keuze voor de uitkomstmaten en bij het opstellen van de overwegingen. De conceptrichtlijn is tevens voor commentaar voorgelegd aan de Ataxie Vereniging Nederland en de eventueel aangeleverde commentaren zijn bekeken en verwerkt.
Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule voerde de werkgroep conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).

A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all. Both databases were searched from 2005 to April 9, 2025 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) cerebellar ataxia; (2) antibody tests. Duplicates were removed using EndNote software.
