Uitgangsvraag

Welke behandeling gericht op sarcopenie heeft de voorkeur bij ouderen met een kwetsbare gezondheid in het ziekenhuis?

Aanbeveling

Aanbeveling-1

Adviseer ouderen met een kwetsbare gezondheid en sarcopenie een behandeling tegen sarcopenie die aansluit bij de medische behandeldoelen, levensverwachting, wensen, behoeften en mogelijkheden van de patiënt diens (wettelijk) vertegenwoordiger.

 

Behandel de modificeerbare oorzakelijke factoren (die uit het Comprehensive Geriatric Assessment (CGA) komen) van sarcopenie, zoals bijvoorbeeld behandeling van ziekten gepaard gaande met inflammatie en ondervoeding (zie module Screening ondervoeding).

 

Adviseer een trainingsinterventie met in ieder geval een component progressieve weerstandstraining 2-3x per week, aangepast aan de fysieke en cognitieve mogelijkheden, onder begeleiding van een ter zake deskundig fysio-/oefentherapeut gedurende 10-12 weken. Voorbeelden van beweegprogramma’s voor ouderen met een kwetsbare gezondheid zijn: ProMuscle voor thuiswonende ouderen, het Otago oefenprogramma voor thuiswonende ouderen, Coach2Move voor thuiswonende ouderen of ouderen in een verpleeghuis; of het Vivifrail programma voor thuiswonende ouderen en in ziekenhuis opgenomen patiënten.

 

Adviseer daarnaast het algemene belang van bewegen en verwijs naar de Beweegrichtlijn volwassenen en senioren.  

 

Aanbeveling-2

Combineer een beweeginterventie met voldoende eiwitinname:

  • Bij patiënten met aandoeningen met inflammatie 1,2 tot 1,5 gram per kilogram per dag verdeeld over drie hoofdmaaltijden.
  • Bij patiënten zonder aandoeningen met inflammatie 1,0 tot 1,2 gram per kilogram per dag verdeeld over drie hoofdmaaltijden.

Overweeg het betrekken van een diëtist of geef passende informatie ten aanzien van eiwitverrijkte voeding. Adviseer dat per maaltijd een voldoende hoeveelheid eiwitten (25-30g) en leucine (minimaal 2,5-3 gram per maaltijd) wordt ingenomen. De diëtist bepaalt de samenstelling van eiwitten zodat het van voldoende hoge kwaliteit is.

 

Overweeg in overleg met de diëtist toevoeging β-hydroxy-β-methylbutyrate (HMB) 3 gram per dag bij patiënten die in het ziekenhuis liggen, tekenen van inflammatie hebben en niet in staat zijn om voldoende hoge kwaliteit eiwitten binnen te krijgen.

 

Controleer de nierfunctie en de nutritionele status van de patiënt en monitor op indicatie. Bij een GFR <30 ml/minuut dient de hoeveelheid eiwit te worden verlaagd.

 

Adviseer om de voedingsnormen van de Gezondheidsraad voor vitamine D voor ouderen vanaf 70 jaar te volgen.

Overwegingen

Samenvatting literatuursearch

Er is literatuuronderzoek verricht naar het effect van voedings- en bewegingsinterventies vergeleken met standaardzorg in patiënten van 65 jaar en ouder met sarcopenie.

Aanvankelijk was het literatuuronderzoek gericht op systematische reviews, meta-analyses en gerandomiseerde studies die de EWGSOP-2 criteria gebruikten voor sarcopenie. Hieruit kwamen geen studies naar voren die gebruikt konden worden.

 

Vervolgens werd in de werkgroep besloten om systematische reviews te includeren met ouderen met sarcopenie waarbij verschillende criteria voor sarcopenie werden gebruikt. Door de heterogeniteit in de studies is de bewijskracht laag.

 

Er werden drie systematische reviews geïncludeerd, die ieder een andere vergelijking onderzochten. Een trainingsinterventie, een trainingsinterventie gecombineerd met HMB of een interventie van vitamine D alleen of met eiwitsuppletie. Vervolgens worden nog relevante reviews benoemd die buiten de selectiecriteria vielen maar wel relevant zijn voor de aanbevelingen.

 

Trainingsinterventie: progressieve weerstands- of gecombineerde training

De eerste systematische review en meta-analyse die is geïncludeerd betreft de studie van Peng. Hierin zijn 13 RCT’s geïncludeerd met 2080 ouderen met sarcopenie, waarbij diverse criteria voor sarcopenie werden gehanteerd. De meeste studies bevatten 2-3 keer per week een trainingssessie gericht op progressieve weerstandstraining, balanstraining, conditietraining of een combinatie. Hiernaast werd in enkele studies ook oefeningen voor thuis begeleid. Studie duur varieerde van 12 weken tot 2 jaar. Van de beschreven uitkomstmaten waren er geen verschillen tussen de interventie en controle groep in loopsnelheid, balans, kwaliteit van leven en in de meeste maten voor spiermassa. Er werd alleen een effect beschreven in de skeletal muscle mass index in 4 studies met in totaal 144 deelnemers (interventie n=74 en controle n=70) (MD 1,67 kg/m2; 95% CI 0,15-0,48).

 

Er werd wel een duidelijk verschil beschreven in meerdere maten voor spierkracht.

 

In 8 studies met 1863 ouderen met sarcopenie, was de handknijpkracht na een trainingsinterventie (n=936) hoger dan in de controlegroep (n=927) (MD 1.67 kg, 95% CI 0,3-3,04, p =0.02). Een verschil in spierkracht wordt ook gevonden in 6 studies waarin de maximale isometrische spierkracht met beendrukken (leg press) wordt gemeten in de interventiegroep (n=164) versus de controlegroep (n=162) (SMD=0,53; 95% CI 0,28-0,78 p =0,02), dit verschil wordt als klinisch relevant beschouwd op basis van de SMD. Ook met de chair stand test of de 30-s chair stand test wordt een verbetering gezien na interventie in 6 studies in de interventiegroep (n=117) versus de controlegroep (n=107) (SMD=0.40; 95% CI 0,13-0,67, p =0,02). Dit laatste verschil is statistisch significant maar valt net niet onder een klinisch relevant verschil.

De bewijskracht voor de gevonden verschillen is matig.

 

Progressieve weerstandstraining in combinatie met HMB

De tweede systematische review is van Feng (2024). In deze review krijgt zowel de interventiegroep als de controlegroep een trainingsinterventie en wordt het additionele effect van 3-hydroxy-3-methylbutaanzuur (HMB) onderzocht. HMB is een metaboliet van het aminozuur leucine en wordt onderzocht als supplement voor mogelijke bevordering van spierherstel en vermindering van spierafbraak. Er zijn 5 studies geïncludeerd met 257 ouderen met sarcopenie waarvan 4 studies met de AWGSOP criteria (Chen, 2020) en 1 met de EWGSOP 2010 criteria. De interventie duurde ongeveer 3 maanden, trainingssessies waren 2-3 keer per week. Voor de loopsnelheid wordt een verschil beschreven tussen de interventiegroep met HMB (n=71) en de controle groep (n=69) in het voordeel van de HMB groep (SMD 0.48; 95% CI 0,15 to 0,82 p= 0.005). Dit verschil is statistisch significant maar volgens de afkapwaarde voor een klinisch relevant verschil (SMD 0.5) net niet klinisch relevant. De handknijpkracht in de groep met HMB (n=114) versus placebo (n=114) was net niet significant verschillend (SMD 0.55, 95% CI -0,00 - 1.11, p=0.05). Dit verschil kan wel klinisch relevant genoemd worden met de afkapwaarde van SMD 0.5, in het voordeel van de groep met de trainingsinterventie gecombineerd met HMB. Er waren geen verschillen in spiermassa, chair stand test en BMI gevonden tussen beide groepen.

 

Gecombineerd vitamine D en eiwitsuppletie

De derde systematische review en meta-analyse van Gkekas onderzoekt het effect van vitamine D alleen of gecombineerd met eiwit versus de controlegroep. Er waren geen recente systematische reviews die het effect van vitamine D als monotherapie onderzochten. In 2017 verscheen wel een systematische review van 15 studies die bij ouderen (65+) het effect van vitamine D alleen op handknijpkracht en Timed-up-and go test onderzochten. Hier werd geen effect op handknijpkracht gevonden, en een zeer gering negatief effect op de Timed-up-and go test in 5 studies (n=638) van 0.3 sec langere testduur (95% CI 0,1-0,5 sec) (Rosendahl-Riise et al, 2017).

 

In de studie van Gkekas zijn RCT’s geïncludeerd van ouderen met sarcopenie volgens de EWGSOP criteria of de AWGS criteria. Er zijn 8 RCT’s geïncludeerd met 776 ouderen met sarcopenie, gemiddelde leeftijd 79,0 (±6,9 jaar). In alle studies werd een gecombineerde interventie toegepast van vitamine D in wisselende doseringen met een eiwit of aminozuur. Duur van interventie wisselde van 2-6 maanden. In 4 studies werd voor zowel de controle als de interventiegroep ook een trainingsinterventie toegepast. Er werd geen verschil beschreven in de vitamine D en eiwit groep versus de controle groep in loopsnelheid en spiermassa. Er werd wel een verschil beschreven in de handknijpkracht in het voordeel van de groep met vitamine D en eiwit (n=366) versus controlegroep (n=364) met SMD 0,384 (95% CI 0,014 to 0,754; p=0,04), dit verschil is echter niet klinisch relevant op basis van de SMD. In subgroep analyse van 5 studies met dagelijkse eiwitsuppletie van 10-44 g en vitamine D 100-1404 IE, was de toename van handknijpkracht wel klinisch relevant met een SMD 0.57 (95% CI -0.08-1.06, p = 0.02).

 

Er was tevens een verschil in de chair stand test in het voordeel van de groep met vitamine D en eiwit (n=223) versus de controlegroep (n=234) met een SMD van 0,253 (95% CI 0,069 to 0,437; p= 0,007). Dit verschil is niet klinisch relevant.

 

Er is een zeer lage bewijskracht gezien ook grote heterogeniteit van de studies.

 

Overige studies/richtlijnen
Trainingsinterventie: progressieve weerstands- of gecombineerde training

Shen (2023) heeft een systematische review en netwerk meta-analyse (42 studies, 3728 deelnemers) uitgevoerd, door het onderzoeksdesign viel dit onderzoek buiten de eerdere literatuursearch.  In dit onderzoek werd gekeken naar verschillende soorten oefentherapie bij ouderen (≥65 jaar, mediane leeftijd 72,9 jr.) met sarcopenie. Dit onderzoek concludeert dat progressieve weerstandstraining met of zonder voedingsadvies, en de combinatie van progressieve weerstandstraining met aerobe training en balanstraining, de meest effectieve interventies zijn om de kwaliteit van leven van ouderen met sarcopenie te verbeteren. Het toevoegen van verschillende voedingsinterventies (niet gespecificeerd) aan de training had een groter effect op de handknijpkracht dan training alleen, terwijl het een vergelijkbaar effect had op andere maten van fysieke functioneren.

 

Wang (2022) heeft een systematische review en meta-analyse (23 studies, 1252 deelnemers) bij ouderen (leeftijd ≥60 jaar, gemiddelde leeftijd van 63,2±1,4 tot 89,5±4,4 jaar) uitgevoerd. Door de minimale leeftijd van 60 jaar (i.p.v. 65 jaar) is viel dit onderzoek buiten de literatuursearch. In dit onderzoek wordt geconcludeerd dat oefeninterventies de handknijpkracht, spiermassa van de onderste extremiteit, loopsnelheid en fysiek functioneren kunnen verbeteren. Subgroep analyse toont aan dat progressieve weerstandstraining en gecombineerde training de spierkracht verbetert, terwijl enkel aerobe training dit niet doet.

 

Het onderzoek van Bao (2020) is om dezelfde reden buiten de literatuursearch gebleven. In deze systematische review en meta-analyse (22 studies, 1041 personen) bij ouderen (gemiddelde leeftijd tussen 60,5 en 85,9 jaar) wordt geconcludeerd dat oefeninterventies een positief effect hebben op spierkracht, gemeten op de handknijpkracht en de 5x chair stand test. Subgroepanalyse wijst uit dat dit geldt voor zowel progressieve weerstandstraining, gewichtstraining en aerobe training. Er is verbetering van het fysiek functioneren, gemeten in de loopsnelheid en de TUG, maar geen verschil van de spiermassa.

 

In de beweegrichtlijn voor volwassenen en senioren wordt geadviseerd om minstens 150 minuten per week met matige of zwaar intensieve inspanning te bewegen, verspreid over meerdere dagen. Daarnaast minstens 2 keer per week het uitvoeren van spier- en botversterkende activiteiten, aangevuld met balansoefeningen. Dit geldt ook voor ouderen met een kwetsbare gezondheid met sarcopenie. Voor een veilige uitvoering hiervan is het belangrijk een fysio-/oefentherapeut vroegtijdig te betrekken om te beoordelen wat de (on)mogelijkheden en belastbaarheid van de patiënt is. De fysio-/oefentherapeut kan vervolgens een passend beweegplan opstellen die aansluit bij de doelen en belastbaarheid van de patiënt. Dit kan een oefeninterventie zijn op individuele basis, of deelname aan een beweeginterventie in een groep.

 

In de International clinical practice guidelines for sarcopenia uit 2018 (Dent, 2018) voor de behandeling van sarcopenie wordt oefentherapie met progressieve weerstandstraining gezien als de meest effectieve interventie.

 

Naast individuele trainingsinterventies zoals in bovenstaande studies worden beschreven zijn er ook beweeginterventies voor ouderen, sommigen in groepsverband, sommigen individueel. Onderstaande onderzochte programma’s zijn ook geschikt voor ouderen met sarcopenie.

 

Het Vivifrail programma is geschikt voor ouderen (70+) met een kwetsbare gezondheid, voor zowel thuiswonenden als ouderen opgenomen in het ziekenhuis. Het wordt begeleid door een fysio-/oefentherapeut en is een individueel afgestemd oefenprogramma voor ouderen, gericht op kracht-, balans-, flexibiliteits- en duurtraining, met als doel het voorkomen van kwetsbaarheid en valincidenten. Het programma wordt afgestemd op de functionele capaciteit, na uitvoeren van een test (SPPB) wordt een persoon ingedeeld in een categorie, met elk hun eigen oefeningen. Het programma duurt circa 12 weken, met drie tot vijf trainingen van 45–60 minuten per week.

ProMuscle is een leefstijlinterventie voor (per) thuiswonende ouderen (65+) met een kwetsbare gezondheid waarbij progressieve krachttraining (individueel afgestemd) in groepsverband (begeleid door een fysio-/oefentherapeut of sport/beweegaanbieder) wordt gecombineerd met individueel voedingsadvies van een diëtist; de eerste intensieve fase duurt 12 weken, gevolgd door een tweede periode van nogmaals 12 weken gericht op het bestendigen van de leefstijl.

 

Het Otago oefenprogramma is geschikt voor zelfstandig thuiswonende (kwetsbare) ouderen (65+) met een verhoogd valrisico. Het is een op maat gemaakt valpreventie programma begeleid door een fysio-/oefentherapeut of beweegagoog en kan zowel individueel thuis worden uitgevoerd (12 maanden) als in een groep (12 weken). Het bestaat uit kracht- en balansoefeningen en een wandelprogramma, met oefeningen drie keer per week.

 

Coach2Move is specifiek ontwikkeld voor thuiswonende ouderen (70+) met een kwetsbare gezondheid met al bestaande mobiliteitsproblemen of waarvan verwacht wordt dat deze op korte termijn ontstaan ten gevolge van een trauma, chronische aandoening of life event en die beperkt lichamelijk actief zijn (minder dan 30 minuten per dag matig lichamelijk actief), de interventie kan ook worden omgezet voor ouderen in een verpleeghuis. Het is een persoonsgerichte fysiotherapeutische aanpak voor  ouderen met een kwetsbare gezondheid, waarin via een intake, motivational interviewing en op maat gemaakte doelen wordt gewerkt aan duurzame verhoging van mobiliteit, lichamelijke activiteit en zelfredzaamheid. Afhankelijk van het behandelprofiel wordt de duur van het programma bepaald, dit varieert van 2-4 sessies bij profiel 1, 5-9 bij profiel 2 en 9-18 sessie bij profiel 3a/b. De programmaduur is niet vastomlijnd en wordt afgestemd op de individuele situatie, waarbij vaak een periode van ongeveer zes maanden wordt aangehouden.

 

Achtergrond ontstaan sarcopenie en relatie spieropbouw en afbraak met eiwitten

Sarcopenie ontstaat door een combinatie van factoren zoals spierveroudering, inactiviteit, verlies van motor units, energie en eiwitondervoeding en door korte perioden van inactiviteit of immobilisatie door ziekte, ongeval, en/of operatie.

 

De skeletspier is een dynamisch orgaan waarbij dagelijks 2% van de spiereiwitten worden afgebroken en aangemaakt. De aanmaak wordt o.a. gereguleerd via de mTOR‑pathway, waarbij het essentiële aminozuur (EAZ)  leucine een belangrijke directe activator is en andere EAZ versterken dit effect, vooral in combinatie met weerstandstraining (Atherton, 2010; Murphy, 2018). De afbraak vindt o.a. plaats via de ubiquitine-proteasoom pathway en wordt versterkt door ziekten gepaard gaande met ontstekingsfactoren (zoals CRP, IL‑6, TNF‑α). Bij sarcopenie overheerst de spierafbraak de aanmaak en bij ouderen met een kwetsbare gezondheid met sarcopenie wordt dit veroorzaakt door uiteenlopende factoren die invloed hebben op de aanmaak en/of afbraak van de spiereiwitten.

 

Voor een goede spieropbouw zijn er dus voldoende eiwitten nodig. Door de aanwezigheid van anabole resistentie (deels veroorzaakt door een gestoorde mTORC1 signalering), die bij ouderen en toeneemt bij de aanwezigheid van inflammatie, is er extra eiwit nodig om voldoende spieropbouw te krijgen. ESPEN en PROT-AGE richtlijnen adviseren 1,2 tot 1,5 gram eiwit per kilogram per dag voor patiënten met sarcopenie waarbij sprake is van inflammatie. Bij ontbreken van ziekten met inflammatie wordt 1,0-1,2 gram eiwit per kilogram per dag aanbevolen.

 

Belangrijk is hierbij dat niet alleen de dagelijkse hoeveelheid eiwit voldoende is, maar ook dat de kwaliteit van de eiwitten en verdeling over de dag goed is. De eiwitkwaliteit wordt bepaald door de aminozuursamenstelling, de verteerbaarheid en absorptie van de aminozuren in de darm (bio-beschikbaarheid) en de vorm waarin het eiwit wordt aangeboden. Een kwalitatief goed eiwit is een eiwit dat goed wordt opgenomen in de darm én de essentiële aminozuren bevat in hoeveelheden die overeenkomen met de menselijk behoefte aan aminozuur. Deze aminozuurbehoefte kan variëren per leeftijd, fysiologische situatie en gezondheidsstatus.(https://nvdietist.nl/artikelen/de-kwaliteit-van-eiwit/). Studies hebben aangetoond dat er per maaltijd 25–30 g hoogwaardig eiwit met minimaal 2,5–3 g leucine nodig is voor een maximale spieropbouw. Hierbij is de combinatie met voldoende andere EAZ het meest effectief (Katsanos, 2006; Hamarsland, 2017). Ook wordt aanbevolen extra eiwit in te nemen na beweging ( van Loon 2016).  Combinatie inspanning en voeding is essentieel, de anabole resistentie is grotendeels toe te schrijven aan inactiviteit (meer dan aan leeftijd per se).

 

Er zijn aanwijzingen dat bij patiënten met ziekten die gepaard gaan met ernstige inflammatie dat naast de extra eiwitinname ( tot 2g/kg/dag) HMB van toegevoegde waarde kan zijn. HMB is een metaboliet van leucine (5,5% conversie van leucine), heeft een duaal effect met stimulatie van de spieropbouw via mTOR-activatie, maar ook remming van de spierafbraak door onderdrukking van de ubiquitine-proteasoom pathway (Smith, 2005; en Oktaviana, 2019). Bij ziekten gepaard gaande met inflammatie is het effect van leucine slechts matig en niet uniform significant (Yoshimura, 2019; Komar, 2015). Dit heeft ermee te maken dat leucine primair de spieropbouw stimuleert, maar niet de spierafbraak remt.

 

Veel ziekenhuispatiënten halen de aanbevolen eiwitinname van 1,2–1,5 g/kg/dag niet (ook niet bij afwezigheid van bijvoorbeeld slikstoornissen en andere factoren) m.n. door onvoldoende inname tijdens ontbijt en afgenomen eetlust als gevolg van inflammatie,  waardoor vaak suppletie nodig is (Tieland, 2015; van Loon, 2016). Weijzen et al. (2020) laat zien dat in zelfs electieve orthopedische patiënten de inname tijdens hospitalisatie rond de 0.5 g eiwit per kg ligt. Eiwitsupplementen kunnen echter verzadigend werken, wat de totale inname verder verlaagt (de proteïneparadox). Derhalve is belangrijk dat de supplementen hoogwaardig zijn met laag volume en liefst aan de maaltijden worden toegevoegd, na de beweging en voor de nacht (van Loon, 2016). 

 

Kwaliteit van bewijs

De overall kwaliteit van bewijs is zeer laag. Dit betekent dat we zeer onzeker zijn over het gevonden geschatte effect van de cruciale uitkomstmaten.

Er is afgewaardeerd vanwege (zeer) ernstige:

  • Risk of Bias: methodologische beperkingen (onvolledige data en uitval van patiënten).
  • Indirectheid: indirectheid van het bewijs, door verschillen in populatie en interventies.
  • Imprecisie: onnauwkeurigheid, omdat het betrouwbaarheidsinterval [de grens/beide grenzen. 

Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)

Door grote heterogeniteit in comorbiditeit bij ouderen met een kwetsbare gezondheid zijn er veel factoren die het vermogen om goed te kunnen bewegen en veilige deelname aan beweeginterventies beïnvloeden. Denk aan pijnklachten en bewegingsbeperkingen die bij artrose kunnen bestaan, dyspnoe d’effort bij cardiale of pulmonale problematiek, of beweegangst, cognitieve beperkingen.

 

Het ontvangen van begeleiding door een fysio-/oefentherapeut bij het bewegen wordt door patiënten als waardevol beschouwd.

 

Patiënten benadrukken het belang van een interventie op maat, waarbij keuzes gezamenlijk met de patiënt worden gemaakt (shared decision making). Het is daarom van belang om samen met de patiënt te onderzoeken wat mogelijk is voor het individu om trainingsprincipes te bereiken. De aangewezen beweegspecialist hierbij is de fysio-/oefentherapeut, bij voorkeur een geriatriefysiotherapeut, indien deze niet beschikbaar is een fysio-/oefentherapeut met affiniteit met de geriatrie. Deze kan samen met de patiënt een veilig en geschikt trainingsplan opstellen, rekening houdend met de individuele kenmerken en wensen van de patiënt. Het uitvoeren van de trainingsinterventie kan onder verschillende omstandigheden, denk hierbij aan individuele begeleiding, in groepsverband, of zelfstandig oefenen. Dit kan, evt. met coaching op afstand van de (geriatrie) fysio-/oefentherapeut in de thuissituatie, of (in een later stadium) bij een sportschool. Welke vorm van begeleiding het meest geschikt is kan worden bepaald door de (geriatrie) fysio-/oefentherapeut, rekening houdend met de voorkeuren en beperkingen van de patiënt.

 

Patiënten vinden zelfstandigheid belangrijk. Fysio-/oefentherapeuten kunnen dit ondersteunen door patiënten te begeleiden richting het zelfstandig uitvoeren van oefeningen en een schakel te zijn in de zoektocht naar passende bewegingsactiviteiten in het sociale domein.

 

Toevoeging van voedingssupplementen, specifiek eiwit en eventueel met HMB zal door de meeste ouderen met een kwetsbare gezondheid goed ingenomen kunnen worden. Ouderen met slikproblemen zullen hier meer moeite mee hebben. Indien er al sprake is van veel orale medicamenten kan het bijdragen aan het gevoel veel pillen te moeten slikken.

 

Kostenaspecten

Aan de inzet van een (geriatrie) fysio/oefentherapeut zijn kosten verbonden. Er is voor het ontwikkelen van deze richtlijn geen onderzoek gedaan naar de mogelijke besparing van zorgkosten door de het preventieve effect van de behandeling van sarcopenie.

Eiwitsupplementen met eventueel HMB brengt kosten met zich mee.

 

Gelijkheid ((health) equity/equitable)

De inzet van een (geriatrie) fysio/oefentherapeut, eiwitsupplementen en HMB kunnen gepaard gaan met aanvullende kosten, afhankelijk van de (aanvullende) verzekering van de patiënt. Niet elke Nederlander heeft een voldoende ruime aanvullende ziektekostenverzekering om de kosten van de interventie door een fysio-/oefentherapeut te dekken, dit kan tot gevolg hebben dat de trainingsinterventie niet, of met een kortere frequentie/ mindere begeleiding dan gewenst wordt ingezet, waardoor de effectiviteit van de interventie kan verminderen. Hierdoor zal de gezondheidsgelijkheid verschillen en afnemen onder minder begunstigde Nederlanders.

 

Aanvaardbaarheid:

Ethische aanvaardbaarheid

De interventie / diagnostiek lijkt aanvaardbaar voor de betrokkenen. Er zijn geen ethische bezwaren.

 

Duurzaamheid

De werkgroep beoordeelt dat de inzet van een oefentherapeutische behandeling geen verschil in de ecologische voetafdruk zal maken. Wel kan de fysio- of oefentherapeut gebruik maken van de checklist in zijn praktijk (zie checklist). 

 

Haalbaarheid

Uit de praktijk blijkt dat individuele progressieve weerstandstraining haalbaar is om in te zetten en dus te bespreken op de polikliniek en afdeling geriatrie/ouderengeneeskunde, of in consulten/medebehandelingen waarin de geriatrie/ouderengeneeskunde betrokken is. Na een klinische opname en polikliniek bezoek zal de interventie moeten worden voortgezet in de eerstelijns zorg, het daarom van belang dat ook de huisarts en wijkverpleegkundige bekend zijn met de interventies voor sarcopenie.

 

In de dagelijkse ziekenhuiszorg voor opgenomen oudere patiënten met een kwetsbare gezondheid wordt ondanks screening en vroegtijdige betrokkenheid van de diëtist de eiwitbehoefte vaak niet gehaald. Hier is extra aandacht en evaluatie voor nodig met opvolging door de diëtist.
Er zijn een aantal specifieke aandachtpunten wat betreft de trainingsinterventie:

  • De toenemende druk op de toegankelijkheid van fysiotherapeutische zorg maakt het organiseren van intensieve programma’s, zoals drie keer per week trainen, steeds lastiger. Dit vraagt om creatieve oplossingen, zoals groepssessies of het versterken van zelfmanagement, om de benodigde frequentie en kwaliteit van training te kunnen waarborgen.
  • Een interventie heeft vaak een duidelijk begin en einde, maar het is essentieel dat patiënten na afloop structureel blijven bewegen. Het duurzaam doorzetten van beweeggedrag in het reguliere sport- en beweegdomein (bijvoorbeeld via sportverenigingen, buurtsportcoaches of beweeggroepen) is cruciaal om de behaalde gezondheidswinst te behouden. De fysio-/oefentherapeut kan hier een informatieve en verbindende rol in spelen.
  • Het is belangrijk om zowel bij verwijzers als bij fysiotherapeuten kennis over sarcopenie te vergroten, zodat patiënten tijdig passende interventies krijgen aangeboden.
  • Intensieve trainingsprogramma’s zijn niet voor iedereen praktisch uitvoerbaar, bijvoorbeeld door mobiliteitsproblemen, beperkte motivatie of andere gezondheidsproblemen. Maatwerk, laagdrempelige instapmogelijkheden en het benutten van bestaande lokale beweeginitiatieven vergroten de kans op succes.

Rationale van aanbeveling-1: weging van argumenten voor en tegen de interventies

Zodra sarcopenie wordt vastgesteld of vermoed bij ouderen met een kwetsbare gezondheid is het goed om na te gaan of er behandelbare oorzaken zijn en behandeling te bieden die aansluit bij de medische behandeldoelen, levensverwachting, wensen, behoeften en mogelijkheden van de patiënt en eventueel diens naasten.

 

Bij het literatuuronderzoek voor deze richtlijn kwamen weinig geschikte onderzoeken naar voren, met slechts een zeer lage bewijskracht. Redenen hiervan zijn wellicht de leeftijdsgrens van 65 jaar en ouder en het primair onderzoeken van ouderen met een kwetsbare gezondheid die nog thuis wonen.

 

In de overwegingen zijn daarom nog enkele studies beschreven waarbij bredere selectiecriteria zijn toegepast met bredere criteria voor sarcopenie en ruimere leeftijdscriteria.

 

Samengevat wordt in deze studies wel bewijs beschreven voor het effect van een trainingsinterventie van 2-3 keer per week gedurende minimaal 10 weken bij ouderen met sarcopenie op de uitkomstmaat spierkracht. Interventies kunnen variëren van begeleid in een groep of thuis oefenen, en dienen in iedere geval een component progressieve weerstandstraining (waar weerstandstraining staat kan ook krachttraining gelezen worden) te bevatten. Daarnaast zijn er positieve effecten gezien van gecombineerde programma’s met progressieve weerstandstraining, balanstraining en conditietraining. In de International clinical practice guidelines for sarcopenia uit 2018 (Dent, 2018) voor de behandeling van sarcopenie wordt oefentherapie met progressieve weerstandstraining gezien als de meest effectieve interventie. Vanwege heterogeniteit in studies moet dit effect met voorzichtigheid worden geïnterpreteerd. Het is nog niet duidelijk of het gevonden verschil in spierkracht ook invloed heeft op belangrijke lange termijn uitkomstmaten zoals complicaties, vallen en sterfte aangezien deze niet worden gerapporteerd in de geïncludeerde systematische reviews.

 

Indien een beweeginterventie passend is, dan is de aangewezen beweegspecialist hierbij de fysio-/oefentherapeut, bij voorkeur een geriatriefysiotherapeut, indien deze niet beschikbaar is een fysio-/oefentherapeut met affiniteit met de geriatrie. Deze kan samen met de patiënt een veilig en geschikt trainingsplan opstellen, rekening houdend met de individuele kenmerken en wensen van de patiënt. Het uitvoeren van de trainingsinterventie kan onder verschillende omstandigheden, denk hierbij aan individuele begeleiding, in groepsverband, of zelfstandig oefenen. Dit kan, evt. met coaching op afstand van de (geriatrie) fysio-/oefentherapeut in de thuissituatie, of (in een later stadium) bij een sportschool. Welke vorm van begeleiding het meest geschikt is kan worden bepaald door de (geriatrie) fysio-/oefentherapeut, rekening houdend met de voorkeuren en beperkingen van de patiënt.

 

Specifieke beweegprogramma’s waaraan kan worden gedacht bij ouderen zijn de ProMuscle voor thuiswonende ouderen, het Otago oefenprogramma voor thuiswonende ouderen, Coach2Move voor thuiswonende ouderen, of ouderen in een verpleeghuis of het Vivifrail programma voor thuiswonende ouderen en in ziekenhuis opgenomen patiënten.

 

Naast de beweegprogramma’s geldt over het algemeen dat er een zo actief mogelijke leefstijl nagestreefd dient te worden, waarbij de oudere gestimuleerd wordt om ook zelf te oefenen al dan niet samen met een mantelzorger (zie ook Beweegrichtlijn volwassenen en senioren).

 

Rationale van aanbeveling-2: weging van argumenten voor en tegen de interventies

Ten aanzien van de studies waarbij de suppletie van HMB, eiwit en vitamine D onderzocht is, geldt dat er ook sprake is van een lage bewijskracht en grote heterogeniteit tussen studies

 

De studies die het gebruik van HMB met weerstandstraining op de spierkracht onderzoeken geven ook een gering additioneel effect op de handknijpkracht. Gezien een effect met lage bewijskracht van HMB op spierkracht, is suppletie met HMB te overwegen.

 

In de studie met gecombineerd vitamine D en eiwitsuppletie blijkt dat de combinatie van eiwit en vitamine D suppletie wel een gering gunstig effect kan hebben op de handknijpkracht. Volgens de gezondheidsraad wordt aanbevolen om bij ouderen boven de 70 jaar 20 µg vitamine D per dag te gebruiken.

 

Het is nog niet duidelijk of het gevonden verschil in spierkracht ook invloed heeft op belangrijke lange termijn uitkomstmaten zoals complicaties, vallen en sterfte aangezien deze niet worden gerapporteerd in de geïncludeerde systematische reviews.

 

Op basis van de beschreven pathofysiologische mechanismen en overige studies en richtlijnen die zijn bestudeerd door de werkgroep, is de werkgroep van mening dat het inzetten van eiwit verrijkte voeding en voedingssupplementen nodig zal zijn om een minimale hoeveelheid aminozuren met juiste verdeling over de dag te halen om sarcopenie tegen te gaan.

Onderbouwing

When sarcopenia - or the likelihood of sarcopenia - is diagnosed, several interventions are available to treat or stabilize the condition. However, it remains unclear which treatment options are most appropriate for frail older adults and geriatric patients. In recent years, various studies have explored potential interventions, including dietary strategies, pharmacological treatments, and exercise-based programs. The central question now is: what is the most effective treatment in relation to relevant outcome measures when sarcopenia is diagnosed according to the EWGSOP2 criteria? Key outcomes include improvements in physical functioning (including independence), reduced risk of falls, enhanced quality of life, and increased survival.

Description of studies

Three systematic reviews were included in the descriptive analysis of the literature. literature. Important study characteristics and results are summarized in table 2.

 

Peng (2024) performed a systematic review and meta-analysis to examine the effects of more than ten weeks of resistance training (RT) on muscle structure, muscle function, and bone mineral density (BMD) in older people, over 70 years old, with sarcopenia. Different criteria for sarcopenia were accepted in analysis. Studies were searched for up to December 23, 2023. The review included randomized controlled trials involving participants aged 70 years or older (or with a mean age above 70). Eligible studies compared an intervention group receiving resistance training (RT) or a combined exercise and nutrition program lasting at least 10 weeks with a non-exercising control group. Outcomes had to include measures of muscle structure (e.g., muscle or fat mass, muscle cross-sectional area, skeletal muscle mass index, or BMI), muscle function (e.g., strength, gait speed, balance, or SPPB), bone mineral density, or quality of life. Only studies published in English were included. In total, thirteen randomized controlled trials were included in the systematic review.

 

Feng (2024) performed a systematic review and meta-analysis to assess the effects of exercise with or without β-hydroxy-β-methylbutyrate supplementation on muscle mass, muscle strength, physical performance and body composition in patients with sarcopenia. Studies were searched for up to April 2024. To be eligible for inclusion, the individual studies had to meet to the following criteria: participants had a confirmed diagnosis of sarcopenia according to established consensus definitions (e.g., AWGSOP or EWGSOP), without restrictions on sex, ethnicity, socioeconomic status, or country. The intervention consisted of exercise combined with β-hydroxy-β-methylbutyrate (HMB) supplementation in any form (including Ca-HMB or free acid). The comparator was exercise alone or exercise with placebo. Primary outcomes included measures of muscle mass (e.g., skeletal muscle mass index), muscle strength (e.g., handgrip strength), and physical performance (e.g., gait speed, chair-stand test), while secondary outcomes involved body composition parameters such as BMI, fat-free mass, and fat mass. Only randomized controlled trials published in English or Chinese were eligible. In total, five randomized controlled trials were included in the systematic review.

 

Gkekas (2021) performed a systematic review and meta-analysis to determine the effect of vitamin D alone or with protein supplementation on muscle strength, mass, and performance in patients with sarcopenia. Studies were searched for up to March 31, 2020. The review included randomized controlled trials that enrolled participants diagnosed with sarcopenia according to the EWGSOP criteria (for European studies) or the AWGS criteria (for Asian studies). Eligible studies evaluated the effects of vitamin D supplementation - either alone or in combination with protein - on sarcopenia-related outcomes, and provided sufficient data for extraction and analysis. In total, eight randomized controlled trials were included in the systematic review.

Results

Three systematic reviews reported the outcome measures (Peng, 2024; Feng, 2024; Gkekas, 2021). Due to differences in comparisons, the results were not pooled.

 

Resistance training vs. no exercise

Physical performance (critical)

Peng (2024) reported usual gait speed and maximum gait speed (see Figure 1). The SMD in usual gait speed between the patients who received resistance training (n=154) and the patients who received no exercise (n=145) was 0.38 (95% CI -0.20 to 0.95), in favour of the patients who received resistance training. This difference is not considered clinically relevant.


The SMD in maximum gait speed between the patients who received resistance training (n=79) and the patients who received no exercise (n=80) was 0.41 (95% CI -0.04 to 0.86), in favour of the patients who received resistance training. This difference is not considered clinically relevant.

Muscle strength (important)

Peng (2024) reported handgrip strength (see Figure 2). The mean difference between the patients who received resistance training (n=936) and the patients who received no exercise (n=927) was 1.67 (95% CI 0.30 to 3.04), in favour of the patients who received resistance training. This difference is considered clinically relevant.

Peng (2024) reported chair stand test (see Figure 3). The SMD in chair stand test score between the patients who received resistance training (n=117) and the patients who received no exercise (n=107) was 0.40 (95% CI 0.13 to 0.67), in favour of the patients who received resistance training. This difference is not considered clinically relevant.

Muscle mass (important)

Peng (2024) reported muscle mass (see Figure 4). The SMD between the patients who received resistance training (n=115) and the patients who received no exercise (n=113) was 0.14 (95% CI -0.12 to 0.40), in favour of the patients who received resistance training. This difference is not considered clinically relevant.

Peng (2024) also reported appendicular skeletal muscle mass (see Figure 5). The mean difference between the patients who received resistance training (n=912) and the patients who received no exercise (n=909) was 0.20 (95% CI -0.08 to 0.48), in favour of the patients who received resistance training. This difference is not considered clinically relevant.

The standardized mean difference (SMD) in gait speed between the patients who received exercise with HMB supplementation (n=71) and the patients who received exercise with placebo (n=69) was 0.48 (95% CI 0.15 to 0.82), in favour of the patients who received exercise with HMB supplementation. This difference is not considered clinically relevant.

Feng (2024) reported handgrip strength (see Figure 7). The SMD between the patients who received exercise with HMB supplementation (n=114) and the patients who received exercise with placebo (n=114) was 0.55 (95% CI -0.00 to 1.11), in favour of the patients who received exercise with HMB supplementation. This difference is considered clinically relevant.

Feng (2024) reported skeletal muscle index, which was defined in the study as the ratio of skeletal muscle mass to height squared (see Figure 8). The SMD between the patients who received exercise with HMB supplementation (n=113) and the patients who received exercise with placebo (n=112) was 0.06 (95% CI -0.20 to 0.32), in favour of the patients who received exercise with HMB supplementation. This difference is not considered clinically relevant.

Vitamin D with protein supplementation vs. vitamin D alone

Gkekas (2021) reported walking speed (gait speed) (see Figure 9). The SMD in walking speed between the patients who received vitamin D with protein supplementation (n=256) and the patients who received placebo/no vitamin D supplementation (n=264) was 0.130 (95% CI -0.042 to 0.303), in favour of the patients who received vitamin D with protein supplementation. This difference is not considered clinically relevant.

Muscle strength (important)

Gkekas (2021) reported handgrip strength (see Figure 10). The SMD between the patients who received vitamin D with protein supplementation (n=366) and the patients who received placebo/no vitamin D supplementation (n=364) was 0.384 (95% CI 0.014 to 0.754), in favour of the patients who received vitamin D with protein supplementation. This difference is not considered clinically relevant. In subgroup analyses of 5 studies that reported daily protein doses of 10-44 g and vitamin D 100-1404 IE, handgrip strength improves with SMD 0.57 (95% CI -0.08-1.06, p = 0.02). This is considered clinically relevant.

Gkekas (2021) reported sit to stand time (see Figure 11). The SMD in sit to stand time between the patients who received vitamin D with protein supplementation (n=223) and the patients who received placebo/no vitamin D supplementation (n=234) was 0.253 (95% CI 0.069 to 0.437), in favour of the patients who received vitamin D with protein supplementation. This difference is not considered clinically relevant. 

 

In subgroup analysis of 3 studies with daily protein suppletion of 10-44 g and vitamin D 800-1404 IU, chair stand test improved  with SMD 0.25 (95% CI 0.06-0.43, p = 0.007) which is not considered clinically relevant.

Gkekas (2021) reported skeletal muscle index (see Figure 12). The SMD between the patients who received vitamin D with protein supplementation (n=130) and the patients who received placebo/no vitamin D supplementation (n=106) was 0.254 (95% CI -0.006 to 0.514), in favour of the patients who received vitamin D with protein supplementation. Relevant outcome measures

The guideline panel considered physical performance as critical outcome measure for decision making; and muscle strength, muscle mass, complications, quality of life, falling and mortality as important outcome measures for decision making.

 

A priory, the guideline panel defined physical performance as follows: Mobility, self-reliance, activities of daily living (ADL)

 

For the other outcome measures the guideline panel used the definitions used in the studies.

 

The guideline panel defined the following minimal clinically (patient) important differences (MCID):

  • For continuous outcome measures (physical performance measures, muscle strength, muscle mass and quality of life) no anchor based MCID was available. Therefore, 0,5 standard deviation (SD) between the intervention and control group was chosen as a threshold. Similarly, when meta-analyses reported standardized mean differences (SMDs) after pooling, an SMD of 0.5 (in absolute value) was defined as the threshold for clinical relevance. This approach corresponds to the thresholds for clinical relevance in the absence of an anchor based MCID according to the GRADE methodology (Guyatt, 2011).
  • For dichotomous outcomes (complications, falls and mortality), we judged any between-group difference that reached conventional statistical significance (p < 0.05) as clinically relevant, recognizing that even relatively small relative risk reductions may be meaningful given the seriousness of these outcomes.

Search and select (Methods)

A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 2019 to 8 August 2025 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Entree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from three primary search concepts: (1) elderly/frailty; (2) sarcopenia; (3) nutrition/exercise/medication intervention. Duplicates were removed using EndNote software. After deduplication a total of 854 records were imported for title/abstract screening. Initially, 75 studies were selected based on title and abstract screening using ASReview. After reading the full text, all studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), as none met the inclusion criteria defined in our PICO framework. Consequently, the working group reached a consensus to include a descriptive summary of the evidence focusing on the three most relevant treatment options. For each treatment option, one systematic review was selected based on consensus within the working group.

Er werd aandacht besteed aan het patiëntenperspectief door de Patientenfederatie Nederland en de KBO-PCOB uit te nodigen voor de schriftelijke knelpunteninventarisatie, maar beide hebben geen input gegeven. Daarnaast is de KBO-PCOB uitgenodigd voor de werkgroep, maar heeft afgezien van deelname. De conceptrichtlijn is tevens voor commentaar voorgelegd aan de Patientenfederatie Nederland en de KBO-PCOB en de eventueel aangeleverde commentaren zijn bekeken en verwerkt.

 

Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz

Bij de richtlijnmodule is conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uitgevoerd om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema op de Richtlijnendatabase).
