Uitgangsvraag
Wat is de gewenste beeldvorming bij patiënten met een verdenking intracranieel meningeoom ter vaststelling van de diagnose?
De uitgangsvraag omvat de volgende deelvragen:
- Wat is de voorkeursmodaliteit in beeldvorming voor primaire diagnostiek van een vermoedelijk intracranieel meningeoom?
- Wat is de toegevoegde waarde van PET-imaging in de diagnostische work-up bij patiënten met verdenking van intracranieel meningeoom?
Aanbeveling
Aanbeveling deelvraag 1
Kies bij voorkeur voor MRI met contrast bij patiënten die verdacht worden van een asymptomatisch intracranieel meningeoom.
Overweeg, bij contra-indicaties voor MRI of beperkte belastbaarheid van de patiënt, een CT met en zonder contrast als alternatief.
Aanbeveling deelvraag 2
Overweeg, als alternatief voor versnelde MRI-follow-up of direct histologisch onderzoek, een SSTR PET bij differentiaaldiagnostische twijfel na eerdere beeldvorming met MRI of CT bij patiënten die verdacht worden van een asymptomatisch intracranieel meningeoom, bijvoorbeeld bij twijfel tussen een intracranieel meningeoom of een meer hooggradige/maligne laesie.
Overwegingen
Waarom deze aanbeveling? deelvraag 1
Voor follow-up van asymptomatische intracraniële meningeomen is de werkgroep van mening dat MRI met contrast de voorkeursmodaliteit is voor primaire diagnostiek naar asymptomatische intracraniële meningeomen vanwege de hogere verwachte diagnostische accuratesse/weefseldiscriminatie en het ontbreken van stralingsbelasting. CT vormt een acceptabel alternatief indien door patiëntfactoren/voorkeuren of contra-indicaties een MRI niet mogelijk is of wanneer additionele informatie omtrent hyperostosis, of intralesionale calcificaties is gewenst.

Waarom deze aanbeveling? deelvraag 2
Er is beperkt beschikbaar bewijs dat SSTR-PET een hogere sensitiviteit en specificiteit biedt dan conventionele MRI/CT bij differentiaaldiagnostiek van meningeomen. Gezien de beperkte literatuur en afwezigheid van gerandomiseerde studies, is de aanbeveling gebaseerd op expert opinion. SSTR-PET kan overwogen worden als aanvullende diagnostiek bij aanhoudende onzekerheid over de aard van een asymptomatisch meningeoom, rekening houdend met extra kosten, stralingsbelasting en beschikbaarheid.

Balans tussen gewenste en ongewenste effecten deelvraag 1
Er werd geen literatuur search naar deze deelvraag uitgevoerd omdat dit in de praktijk niet als knelpunt wordt ervaren/is aangedragen. Vanuit een uniformiteitsperspectief qua werkwijze is de werkgroep van mening dat dit onderdeel wel in de richtlijn moet worden behouden. Omdat er geen systematische literatuur search is vervaardigd is deze deelvraag op basis van expert opinion beantwoord, en is zowel inhoudelijk als qua methodiek in lijn met de vorige richtlijn.
Asymptomatische intracraniële meningeomen worden per definitie als toevalsbevinding gediagnosticeerd, hoofdzakelijk op basis van computed tomography (CT) of magnetic resonance imaging (MRI) onderzoeken. De werkgroep is van mening dat MRI de voorkeur heeft voor diagnostiek en follow-up van asymptomatische intracraniële meningeomen met name vanwege de hogere weke delen discriminatie (Saloner, 2010; Runge, 2015). Dit ondanks dat goede kwaliteit studies met betrekking tot diagnostische accuratesse van MRI vs CT onderzoek bij diagnosestelling van asymptomatische intracraniële meningeomen ontbreken en ondanks dat ossale structuren, intralesionele verkalkingen en hyperostosis op MRI minder goed kunnen worden gevisualiseerd dan op CT onderzoek. De voorkeur voor MRI is in lijn met andere internationale meningeoom richtlijnen (Yarabarla, 2023; NICE 2021). Tevens is het ontbreken van stralingsbelasting als ongewenst neveneffect van CT-onderzoeken een aanvullend argument om MRI de voorkeur te geven boven CT. Indien dit niet haalbaar is (bv. claustrofobie of onmogelijkheid tot lang plat liggen) is CT een redelijk alternatief. CT heeft als voordeel ten opzichte van MRI dat intralesionele calcificaties en hyperostosis van het aangrenzende bot beter kunnen worden waargenomen. De werkgroep is echter van mening dat ondanks deze voordelen MRI, vanwege de eerdergenoemde voordelen ten opzichte van CT, de voorkeur heeft.
Indien er vanwege contra-indicaties of praktische bezwaren wordt gekozen voor CT is de werkgroep van mening dat er zowel een natief CT-onderzoek als een contrast versterkt CT-onderzoek ter evaluatie van calcificaties, aankleuring en een potentiële dural tail met late (post veneuze) contrast timing dient te worden vervaardigd.
Kwaliteit van bewijs
Er werd geen systematisch literatuuronderzoek verricht. De aanbevelingen komen tot stand op basis van expert opinion. Alle eventueel gebruikte wetenschappelijke literatuur werd niet formeel beoordeeld op kwaliteit (risk of bias) en zekerheid (GRADE).
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
De patiëntbelasting voor MRI-onderzoek is hoger dan voor CT-onderzoek, hoofdzakelijk vanwege de hogere fysieke belasting. Hierbij is te denken aan de duur van het onderzoek en de diameter van de gantry, welke bij MRI ten opzichte van CT respectievelijk langer duurt en smaller is. Derhalve zal een CT-onderzoek tot minder claustrofobieklachten leiden en is een CT-onderzoek dermate snel dat van bewegingsonscherpte slechts zelden sprake is. In de praktijk wegen deze factoren niet op tegen de hogere diagnostische accuratesse van MRI, maar kunnen afhankelijk van de patiëntfactoren of -voorkeuren in een specifieke situatie toch doorslag geven richting CT.
CT gebruikt ioniserende straling. Hoewel de stralingsbelasting van een CT van de hersenen laag is, brengt blootstelling aan röntgenstraling een klein, cumulatief risico op het ontwikkelen van kanker op lange termijn met zich mee. MRI gebruikt geen ioniserende straling en verhoogt derhalve ook bij herhaalde MRI scans niet het risico op kanker.
Kostenaspecten
MRI is duurder dan CT met name vanwege de langere duur van het onderzoek en meer complexe, tijdsintensieve beoordeling ten opzichte van CT. De werkgroep is echter van mening dat dit verschil niet opweegt tegen de voordelen van MRI versus CT, waar de diagnostische accuratesse van MRI evident hoger is.
Gelijkheid ((health) equity/equitable)
Beide modaliteiten zijn ruimschoots voorhanden in Nederlandse ziekenhuizen. Voor MRI is de wachttijd doorgaans langer dan voor CT. Gezien asymptomatische meningeomen per definitie geen klinische verschijnselen geven en een doorgaans langzaam groeipatroon hebben, zal deze wachttijd geen relevante problemen opleveren. Beide modaliteiten vallen onder de basisverzekering en worden (los van een eventueel eigen risico) volledig vergoed. Derhalve is van gezondheidsongelijkheid geen sprake.
Aanvaardbaarheid:
Ethische aanvaardbaarheid
Beide diagnostische modaliteiten, zowel CT als MRI lijken aanvaardbaar voor de betrokkenen. Er zijn geen zwaarwegende ethische bezwaren. Beide modaliteiten vallen worden reeds lange tijd gebruikt en vergoed vanuit de basisverzekering. Beide zijn ruimschoots voorhanden. CT heeft echter zoals vermeld een stralingsbelasting waarvan negatieve effecten op lange termijn kunnen worden verwacht afhankelijk van de blootstelling. Mede hierdoor heeft MRI de voorkeur.
Duurzaamheid
Beide modaliteiten vergen een aanzienlijke consumptie van stroom. Tevens is het werken met metaalhoudende contrastmiddelen zoals gadolinium vanuit duurzaamheidsperspectief onwenselijk. Beide contrastmiddelen worden hoofdzakelijk via de urine uitgestoten en komen daarmee in het afvalwater en grondwater terecht. Met name voor gadolinium is extractie uit grond-/drinkwater uitdagend en dient het gebruik vanuit een duurzaamheidsperspectief tot een minimum te worden beperkt.
Haalbaarheid
Beide modaliteiten, zowel de voorkeursmodaliteit MRI als de alternatieve modaliteit CT zijn ruimschoots voorhanden in alle Nederlandse ziekenhuizen. De capaciteit is echter beperkt van beide modaliteiten, hoofdzakelijk voor MRI. Derhalve dat haalbaarheid met name afhankelijk is van scanfrequentie.
Balans tussen gewenste en ongewenste effecten deelvraag 2
Er werden geen gerandomiseerde studies gevonden die het effect van somatostatine receptor (SSTR) positron emission tomography (PET)CT dan wel SSTR PET-MRI als add-on na onduidelijkheid van conventionele beeldvorming op klinische (patiënt-relevante) uitkomstmaten onderzocht. Uit de zoekopbrengst kwamen twee potentieel relevante studies over de diagnostische waarde van add-on SSTR PET-CT (Wang, 2023; Purandare, 2020), maar deze bleken uiteindelijk niet te voldoen aan de selectiecriteria van de literatuuranalyse. Zij werden wel relevant genoeg geacht om ter overweging te bespreken. Gezien het ontbreken van gerandomiseerde studies en beperkte beschikbaarheid van literatuur zijn de overwegingen in deze deelvraag met name gebaseerd op expert opinion.
Wang (2023) includeerde patiënten verdacht van een nieuw of terugkerend intracranieel meningeoom op een complexe locatie of met atypische kenmerken op. Het werd de werkgroep niet duidelijk genoeg wat er werd bedoeld met complexe locaties, wat atypische kenmerken zouden zijn bij beeldvorming en op welke eerdere beeldvorming dit werd vastgesteld. Ook is het niet helder hoeveel patiënten er een verdenking op een complexe locatie hadden en hoeveel patiënten er atypische kenmerken hadden. Hierdoor is het onduidelijk hoe eventuele uitkomsten zich verhouden tot de doelpopulatie van deze richtlijnmodule. Patiënten in de studie ondergingen zowel een CE-MRI (T1-weighted, T2-weighted en post-contrast T1 weighted) met gadopentetate dimeglumine als een PET-CT (low-dose CT) met [68Ga]Ga-DOTA-JR11 (een somatostatine receptor antagonist). Histopathologie was de referentiestandaard. Er werden uiteindelijk 36 patiënten geanalyseerd van de oorspronkelijk 41 gerekruteerden (n=4 hadden geen pathologische uitslag; n=1 had een onduidelijke pathologische uitslag). Zij hadden een gemiddelde leeftijd van 50 jaar (SD: 13, range: 18-69) en bestonden uit 20 vrouwen en 16 mannen. Volgens de histopathologie waren er 32 patiënten met een meningeoom en 4 patiënten met andersoortige intracraniële tumoren (n=1 plasmacytoom, n=1 epithelioid hemangioendothelioom, n=2 fibreuze tumor). Uit de groep patiënten met een meningeoom detecteerde PET-CT 31 patiënten met een meningeoom (SUVmax > 3) tegenover 28 patiënten door CE-MRI. Uit de groep patiënten met een andersoortige tumor werden er 3 patiënten correct zonder meningeoom geclassificeerd door PET-CT (SUVmax < 3) tegenover 2 door CE-MRI. Dit resulteerde in een sensitiviteit van 96.9% (31/32) en een specificiteit van 75% (3/4) voor PET-CT vergeleken met een sensitiviteit van 87,5% (28/32) en specificiteit van 50% (2/4) voor CE-MRI.
Purandare (2020) selecteerde uit een database patiënten met een onduidelijkheid over de differentiatie van een durale laesie op MRI, CT, of FDG-PET-CT (d.w.z. metastase of meningeoom) die ook een 68Ga DOTA-NOC PET-CT ondergingen. Van de 42 patiënten in de analyse bleken er uiteindelijk 31 patiënten een meningeoom te hebben en 9 patiënten durale metastases. Uit de histopathologie bleek dat de twee overige patiënten respectievelijk een inflammatoire pseudotumor en een hemangioblastoom hadden. Vergeleken met een metastase liet een meningeoom een significant hogere SUVmax zien (mediaan 6.0 [IQR: 4,3-8,0] vs. mediaan 12.7 [IQR: 7,3-26,1]; p=0,001). Daarnaast werd aan elke laesie een visuele score toegekend aan de hand van de opname-intensiteit. Van de 31 laesies die als meningeoom werden gediagnosticeerd kregen 30 laesies de visuele score 3 (d.w.z. opname-intensiteit groter of gelijk aan de milt) en 1 laesie de score 2 (d.w.z. opname-intensiteit groter of gelijk aan de lever, maar minder dan de milt). De 9 laesies die als metastasen werden gediagnosticeerd werden visueel beoordeeld als score 2 (n=7) of score 1 (n=2, opname-intensiteit lager dan lever).
De combinatie van zowel FDG-PET en SSTR-PET kan in geselecteerde gevallen complementair zijn, bijvoorbeeld wanneer SSTR-negativiteit wordt vastgesteld bij een verdachte laesie, en FDG-opname juist verhoogd is — wat kan wijzen op een hooggradige of metastatische afwijking. Indien er geen beschikbaarheid van STTR PET is valt een 111indium-octreotide SPECT (eventueel gecombineerd met een FDG-PET) te overwegen indien er radiologisch twijfel is over de aard van een durale laesie (Nathoo, 2007).
Kwaliteit van bewijs
Er werden geen studies gevonden die het effect of de accuratesse van SSTR PET-CT dan wel SSTR PET-MRI als add-on na onduidelijkheid van conventionele beeldvorming onderzochten. Twee studies zijn in de overwegingen besproken (Wang, 2023; Purandare, 2020), maar deze werden niet formeel op kwaliteit (risk of bias) en zekerheid (GRADE) beoordeeld.
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
Het toevoegen van SSTR-PET als aanvullende diagnostische techniek heeft als gewenst effect dat het de diagnostische zekerheid kan vergroten en de duur van het diagnostisch traject kan verkorten. Hierdoor kan sneller en gerichter worden gekozen voor het juiste behandeltraject—of dit nu actief ingrijpen, zoals een operatie, betreft, of juist een afwachtend beleid.
Tegelijkertijd zijn er ook enkele ongewenste effecten die in overweging moeten worden genomen. De inzet van een extra scan brengt aanvullende belasting met zich mee, zowel in termen van kosten als voor de patiënt zelf. Zoals bij elke diagnostische test bestaat er een kans op fout-positieve of fout-negatieve resultaten, die desondanks toch invloed kunnen hebben op het klinisch handelen. Tot slot betekent het gebruik van SSTR-PET ook extra blootstelling aan ioniserende straling, wat met name relevant is bij jongere patiënten of bij herhaalde beeldvorming. Hoewel de stralingsbelasting relatief laag is, brengt herhaald blootstellen aan straling een klein, cumulatief risico op het ontwikkelen van kanker op lange termijn met zich mee.
Kostenaspecten
SSTR-PET brengt hogere directe kosten met zich mee dan standaard beeldvorming (CE-MRI of CT), vanwege de benodigde radiotracers en gespecialiseerde apparatuur. Deze hogere kosten kunnen echter deels worden gecompenseerd door de toegenomen diagnostische nauwkeurigheid. Door betere differentiatie kan sneller een passend behandeltraject worden gekozen, wat mogelijk leidt tot minder onnodige operaties, kortere diagnostische trajecten en efficiëntere inzet van zorg.
Er zijn nog geen kosten-effectiviteitsstudies bekend binnen de werkgroep die deze afweging voor SSTR-PET kwantificeren. Al met al lijkt het toevoegen van SSTR-PET voor het vernauwen van differentiaaldiagnostische overwegingen in bepaalde gevallen op te wegen tegen de extra kosten. Verdere evaluatie van de kosteneffectiviteit is wenselijk.
Gelijkheid ((health) equity)
In Nederland is SSTR-PET beschikbaar in alle universitair medische centra en in toenemende mate ook in perifere ziekenhuizen. Desondanks is de toegankelijkheid nog beperkt in vergelijking met standaard beeldvorming zoals CT en MRI, die vrijwel overal direct beschikbaar zijn. Dit kan leiden tot ongelijkheid in toegang, met name voor patiënten die verder van een centrum met SSTR-PET-afdeling wonen of voor wie reizen belastend of kostbaar is.
Aanvaardbaarheid:
Ethische aanvaardbaarheid
De diagnostiek met SSTR-PET wordt over het algemeen als ethisch aanvaardbaar beschouwd voor de betrokkenen. De procedure is vergelijkbaar met andere gangbare nucleaire beeldvormingstechnieken en wordt uitgevoerd onder medische indicatie. Er zijn geen specifieke ethische bezwaren geïdentificeerd door de werkgroep.
Duurzaamheid
Bij de inzet van SSTR-PET spelen verschillende duurzaamheidsaspecten een rol. In positieve zin kan deze techniek bijdragen aan het verminderen van onnodige vervolgonderzoeken, behandelingen of langdurige follow-uptrajecten, wat zowel de zorgbelasting als verspilling van middelen kan beperken. Ook vermindert het mogelijk de onzekerheid voor patiënten, wat de mentale belasting verlaagt.
Aan de andere kant betekent SSTR-PET wel extra inzet van apparatuur, personeel en middelen. De procedure vereist radioactieve tracers, en brengt een beperkte stralingsbelasting met zich mee voor de patiënt.
Haalbaarheid
De inzet van SSTR-PET is haalbaar. Hoewel de test nog geen landelijke standaardzorg is voor deze specifieke indicatie, wordt deze op indicatie al regelmatig ingezet bij complexe casuïstiek. De aanbeveling om SSTR-PET structureel aan te bieden in de context van aanhoudende twijfel na conventionele MRI/CT zal leiden tot een geringe toename van het jaarlijkse aantal verrichtingen. Het beslag op middelen en menskracht lijkt aanvaardbaar, mede gezien de verwachte voordelen.
Onderbouwing
In the Netherlands, somatostatin receptor (SSTR) positron emission tomography (PET) is not common practice in the diagnostic work-up of intracranial meningioma and the position of SSTR-PET in the work-up varies per hospital. SSTR-PET is usually not applied when conventional imaging (i.e. computed tomography [CT] and/or magnetic resonance imaging [MRI) suffices. However, SSTR-PET may be utilized in cases when there is doubt concerning the differential diagnosis after conventional imaging, for instance when there are multiple dural lesions and metastases are in the differential diagnosis.

Sub-question 2: add-on PET
‘The ideal body of evidence would include studies that directly compare the test strategies under consideration (i.e., randomized trials) and the resulting interventions and consequences (i.e., patient-important outcomes). Such studies would, by design, address all of the issues in the analytical framework and allow guideline panelists to apply the familiar GRADE approach for interventions’ (Schünemann, 2019). For most tests or test and treat strategies, however, this direct evidence does not exist. Therefore, a second search question on test accuracy outcomes is formulated, whereby linked evidence that connects test accuracy to downstream consequences is required for decision-making.

Search and select (Methods)
Sub-question 1: modality of preference for primary diagnosis
A systematic literature review for this sub-question was not performed. Considerations and recommendations will be based on expert opinion.
Sub-question 2: add-on PET
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched for systematic reviews, RCTs, observational and other studies from 2000 to November 20, 2024. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from three primary search concepts: (1) meningioma; (2) SSTR PET-CT/PET-MRI; (3) diagnostic filter. Duplicates were removed using EndNote software. After deduplication a total of 175 records were imported for title/abstract screening. Initially, 19 studies were selected based on title and abstract screening. After reading the full text, all studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), leaving no studies for inclusion.

Ik ben betrokken bij het ontwikkelen van een van de moleculaire risicopredictie modellen op basis van histologische en moleculaire parameters voor menigeomen. Dit werk is in 2021 gepubliceerd (Maas et al. Journal of Clinical Oncology) met een folow-up artikel in 2022 (Hielscher et al. Brain Pathology). Op deze techniek zijn geen patenten aangevraagd, het betreft puur intellectuele erkenning. Mijn expertise met dit model en vergelijkbare modellen, is tevens de reden waarom ik gevraagd ben om bij te dragen aan de richtlijn. |

Inbreng patiëntenperspectief
De werkgroep besteedde aandacht aan het patiëntenperspectief door afgevaardigden namens patiëntenorganisatie Hersenletsel in de werkgroep. De richtlijn is tevens voor commentaar voorgelegd aan Hersenletsel en de eventueel aangeleverde commentaren zijn bekeken en verwerkt.
Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule voerde de werkgroep conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).
