Psychologische en seksuologische behandeling
Uitgangsvraag
Eindoordeel:
Sterke aanbeveling voor (Doen).
Onderbouwing
Fecal incontinence (FI) can have a significant impact on psychological and sexual wellbeing and may lead to fear, shame, social isolation and sexual problems. Fecal incontinence may also perpetuate pre-existing psychological or functional limitations. In clinical practice patients express a need for psychosexual care. It remains unclear whether psychological and sexual interventions are being used and whether they are effective. An increasing body of evidence indicates that psychosocial interventions may not only enhance adherence to therapy and improve quality of life but also contribute positively to the outcome specialist medical care. Nonetheless, it remains to be determined whether these benefits extend to patients suffering from FI (zie o.a. Kwaliteitsstandaard Psychosociale zorg bij somatische aandoeningen).

Relevant outcome measures
The guideline panel considered patient satisfaction, quality of life, and psychological complaints (anxiety and depression) as critical outcome measures for decision making; and work-related outcomes as important outcome measures for decision-making.
A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.
The guideline panel defined the following threshold as a minimal clinically (patient) important difference per outcome measure:
1. quality of life:
- SF-36: mean difference in 0.7 score points (from baseline).
- FIQL score: mean difference in 1.1 score points (from baseline) (Bols, 2011).
- proctoprom score: mean difference in 10 score points (from baseline) (Mijnsbrugge, 2020).
2. work-related outcomes
- sick leave: mean difference > 1 days.
The guideline panel did not define a minimal important difference threshold for patient satisfaction (often measured using the Sexual Self Conscious Scale) or psychological complaints (often measured using the Hospital Anxiety and Depression Scale), since there is no evidence available from practice or research on the minimal important difference (MID) in patients with fecal incontinence. Therefore, the panel recommends that future studies include the outcomes of these questionnaires and define an appropriate MID.
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all and Ovid/ PsycInfo. The databases were searched from 2005 to March 12th, 2025 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) fecal incontinence; (2) cognitive behavioural therapy. Duplicates were removed using EndNote software. After deduplication a total of 614 records were imported for title/abstract screening. Initially, 10 studies were selected based on title and abstract screening. After reading the full text, all 10 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’).
