Uitgangsvraag

Gelijkheid ((health) equity/equitable)

Eindoordeel:

Sterke aanbeveling voor (Doen).

Onderbouwing

Pelvic physical therapy (PPT) focuses on improving the function of the pelvic floor muscles, including the anal sphincter. In case of a decreased tone of the pelvic floor, the focus is on improving strength, endurance, coordination and awareness of the pelvic floor. When there is an increased tone of the pelvic floor and/or dyssynergia, relaxation techniques are also part of the therapy. Biofeedback and electrical stimulation can be used in addition. Biofeedback can be provided through visual, sensory, and/or auditory feedback. Rectal sensitivity training (rectal balloon) is also a form of biofeedback. The pelvic physical therapist also addresses general relaxation, breathing, functioning of the pelvic floor during defecation (toilet habits) and additional lifestyle advice. These methods can be used separately but are often combined (see Guideline Rectumprolaps).

Description of studies

The UEG/ESCP/ESNM/ESPCG guideline (2022) included two randomized controlled trials for the comparison of pelvic physical therapy versus pelvic physical therapy + biofeedback (Norton, 2003; Heymen, 2009). The present analysis added one study (Mundet, 2021).

Pelvic physical therapy versus pelvic physical therapy + biofeedback

The UEG/ESCP/ESNM/ESPCG guideline included two randomized controlled trials (RCT’s), Norton (2003) and Heymen (2009). For Norton (2003), only group 2 (n=32) in which patients received sphincter exercises, group 3 (n=44) in which patients additionally received computed assisted biofeedback during teaching sessions and group 4 (n=35) in which patients additionally received home biofeedback device (DMI Medical Limited, Wigan, UK), were included in the analysis. Heymen (2009) included 108 patients to examine the effects of pelvic physical therapy (PPT, n=63) versus PFPT with manometric biofeedback (n=45).

Mundet (2021) performed a randomized controlled trial to determine the clinical efficacy of 4 rehabilitation techniques amongst which Kegel exercises (K) and biofeedback (BF) + K. Adult patients (>18 years) with >6 months of FI symptoms were included, whereas patients with mild symptoms (Cleveland < 4) were excluded. This parallel design used two intervention periods, in which stage one patients received the assigned treatment for 3 months and in stage 3 patients received only K (control) to evaluate the persistence of the effects of the treatment. Therefore, only results of the first 3 months will be used. The number of required patients (n=45) was met, however only 36 patients in both groups could complete the study. No statistical correction was reported and applied to account for the loss to follow-up.

Important study characteristics and results of the new studies are summarized in Table 2. The assessment of the risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).

Results
Although Heymen (2009) and Norton (2003) were included, these studies mostly did not report comparative results (e.g. mean differences with confidence intervals). Due to lack of data this could not be calculated. Except for the outcome measurement ‘frequency of FI episodes’, results could therefore not be included in our analysis. For further details, see the footnotes in the Summary of Findings (SoF) table.

Frequency of FI episodes

Heymen (2009) reported reduction in mean number of days with FI per week with a mean difference of 0.77 (95% CI 0.11, 1.43) in favor of the biofeedback group. Our update did not include new studies.

Severity of FI symptoms

Mundet (2021) reported Wexner score before and after treatment. The calculated mean difference in the intervention group was -5.00 (95%CI: -7.06 to 2.94) versus -3.46 (95%CI: -5.44 to -1.48) in the control group. The calculated mean difference between groups was
-1.54 (95%CI: -4.30 to 1.22).

Disease-specific and generic quality of life

Mundet (2021) reported both disease-specific (FIQoL) and generic (EQ5D-5L) questionnaires for quality of life.

Disease-specific Quality of Live (FIQoL score)

Mundet (2021) reported FIQoL score with a follow-up period for three months. Mean differences for all four domains were calculated (Lifestyle, Coping behaviour, Depression/self-perception, Embarrassment; Table 3).

Relevant outcome measures

The guideline panel considered frequency of FI episodes, severity of FI symptoms, disease-specific quality of life (FIQoL score) and generic quality of life (SF-36, EQ-5D, proctoPROM score) as critical outcome measures for decision making; and work-related outcomes as important outcome measures for decision making.

The guideline panel used definitions of outcome measures following the UEG/ESCP/ESNM/ESPCG guideline (Assmann, 2022; see below). However, for future updates the guideline panel refers to the recently published core outcome set (COS) for adults with fecal incontinence (Assmann, 2025). In line with this reference, the panel also advises to include patient-reported outcome measures (PROMS), patient-specific complaints (PSK) and global perceived effect in the future.

The guideline panel defined the outcome measures as follows:

1. Frequency of FI episodes, measured by:

a. Number of episodes per week.

b. Number of FI episodes per 2 or 4 weeks.

2. Severity of FI symptoms, measured by:

a. St. Marks incontinence score or Vaizey questionnaire.

St. Marks consists of questions on types of anal incontinence and its impact on daily life. The Vaizey questionnaire is a patient-reported adaptation of the St. Mark’s score. Score for both questionnaires ranges from 0 to 24 with higher score indicating complete incontinence (Roos, 2009).

b. https://meetinstrumentenzorg.nl/wp-content/uploads/instrumenten/Wexner-score-form.pdf influences on the lifestyle. The score ranges from 0 to 20 with a higher score indicating less favorable outcomes (Bols, 2011).

3. Quality of life (QoL):

Disease-specific and generic quality of life, measured by:

a. https://meetinstrumentenzorg.nl/instrumenten/fecal-incontinence-quality-of-life-scale/ of FI. The score ranges from 4 to 16, with higher scores indicating favourable outcomes (Bols, 2011).

b. Generic, measured by:

  • https://www.brandeis.edu/roybal/docs/SF-36_website_PDF.pdffrom 0-100 with higher scores indicating less disability.
  • https://link.springer.com/rwe/10.1007/978-0-387-78665-0_5 scale (EQ-VAS). The EQ-VAS records the subject’s perceptions of their own current overall health status with scores ranging from 0 to 100 (best imaginable health). The questionnaire is a self-reported description of the subject’s current health in 5 dimensions, with which utilities can be calculated.
  • ProctoPROM: a validated patient-reported outcome measure for proctological complaints (Vander Mijnsbrugge, 2020).

4. Work-related outcomes (duration of sick leave): as defined by the authors of included studies.

The guideline panel defined the following thresholds as a minimal clinically (patient) important difference (MID) for each outcome measure:

1. Frequency of FI episodes:

a. Number of FI episodes per week: mean difference of >0.5 or a 50% decrease in number of FI episodes.

b. Number of FI episodes per 4 weeks: mean difference of >0.5 or a 50% decrease in number of FI episodes.

2. Severity of FI symptoms:

a. St Marks/Vaizey questionnaire: mean difference of -3 points (Bols, 2011).

b. Wexner (CCF-FI) score: mean difference of -2 points (Bols, 2011).

3. Disease-specific quality of life

a. FIQoL score: point difference > 1.1 (Bols, 2011).

Search and select (Methods)

We used and updated the literature search strategy of the UEG/ESCP/ESNM/ESPCG Guideline for diagnosis and treatment of fecal incontinence (Assmann, 2022). For the UEG/ESCP/ESNM/ESPCG guideline, a literature search was conducted in Medline (via OVID), Embase (OVID) and Cochrane databases until October 2020. During current adaptation of this guideline, a systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from October 1, 2020, to April 6, 2025 for systematic reviews and RCTs. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) fecal incontinence; (2) pelvic floor physical therapy. Duplicates were removed using EndNote software. After deduplication a total of 548 records were imported for title/abstract screening. The detailed search strategy is listed under the tab ‘Literature search strategy’. More information about the methodology of the international guideline Fecal incontinence can be found in the Methods section of the guideline (supporting documents) on the UEG website.

Four studies were initially selected based on title and abstract screening. After reading the full text, three studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and one study was included.
