Erfelijke darmkanker - Kosteneffectiviteit van onderzoek bij CR

In aanvulling op de gebruikersvoorwaarden, die in deze onverkort van toepassing zijn, geldt hetgeen hierna staat. Deze richtlijn is eerder geplaatst geweest op Oncoline en is vooruitlopend op het actualiseren ervan, nu in deze database opgenomen. De richtlijn zoals die nu is opgenomen voldoet nog niet aan alle kwaliteitseisen die aan publicatie in de Richtlijnendatabase worden gesteld en is daarom als PDF geplaatst. Onderbouwing

Er zijn voor het medische gedeelte geen onderzoeken gevonden die uitsluitsel geven over de meerwaarde van MSI en/of IHC versus géén MSI of IHC voor overleving.

In de geïdentificeerde economische literatuur is geen bewijs gevonden over welke benadering het meest kosteneffectief is voor het bepalen van Lynch syndroom bij patiënten met colorectaal carcinoom.

Er zijn 15 economische evaluaties geïdentificeerd [Debniak 2000 (1), Ramsey 2001 (2), Reyes 2002 (3), Ramsey 2003 (4), Kievit 2005 (5), Pinol 2005 (6), Engel 2006 (7), Barrow 2008 (8), Yan 2008 (9), Mvundura 2010 (10), Gudgeon 2011 (11), Ladabaum 2011 (12), Brazowski 2012 (13), Wang 2012 (14), Pigatto 2004 (15)]. De evaluaties werden uitgevoerd voor de Verenigde Staten (7), het Verenigd Koninkrijk (2), Polen (1), Spanje (1), Duitsland (1), China (1), Israël (1) en Nederland (1). Een aantal studies werden uitgevoerd door onderzoeksequipes met gemeenschappelijke auteurs: Ramsey is de hoofdauteur van twee publicaties [Ramsey 2001 (2), Ramsey 2003 (4)] en Ladabaum en Wang zijn ook co-auteurs [Lambadaum 2011 (12), Wang 2012 (14)] De (afwezigheid van) potentiële belangenconflicten zijn niet altijd duidelijk vermeld.

Er is een relatief groot aantal publicaties geïdentificeerd die een economische evaluatie hebben uitgevoerd over de kosteneffectiviteit van MSI/IHC onderzoek om Lynch syndroom te bepalen bij patiënten met colorectaal carcinoom. De 15 publicaties kunnen opgesplitst worden in twee grote groepen:

  1. korte termijn analyses die een ‘cost-consequences analysis' hebben uitgewerkt, en
  2. lange termijn analyses die een ‘cost-effectiveness analysis' of ‘cost-utility analysis' hebben uitgevoerd.

In de eerste groep worden kosten en effecten in kaart gebracht zonder dit uit te drukken in een allesomvattende ratio. In de laatste groep worden de resultaten uitgedrukt in de zogenaamde ICER (incremental costeffectiveness ratio) die de extra kosten weergeeft per gewonnen levensjaar al dan niet aangepast voor levenskwaliteit (de zogenaamde (quality-adjusted) life-years gained (QALYs)).

In het algemeen kan gesteld worden dat de resultaten van geïdentificeerde onderzoeken uiteenlopend zijn. Zowel opties inclusief/exclusief klinische criteria, met IHC (al dan niet aangevuld met methylering en/of BRAF) of MSI apart, of een combinatie van beide worden naar voren geschoven als optimale test door de verschillende studies. Op basis van de geïdentificeerde economische literatuur is het niet éénduidig welke benadering het meest kosteneffectief is voor het bepalen van Lynch syndroom bij patiënten met colorectaal carcinoom. Het literatuuroverzicht geeft wel aan welke de belangrijkste variabelen zijn die deze kosteneffectiviteit sterk kunnen beïnvloeden en waarop toekomstig onderzoek zich zou kunnen focussen.

Samengevat zijn de resultaten van de literatuur ‘limited, but promising'.
