Medicatie bij niet-immuungemedieerde ataxie
Uitgangsvraag
Wat is de plaats van medicamenteuze interventie in de behandeling van personen met genetische of neurodegeneratieve (niet immuun-gemedieerde) ataxie?
Wat is effectiviteit van 4-aminopyridine bij mensen met SCA27B/FGF14 ataxie?
Aanbeveling
Bespreek de mogelijkheid tot een proefbehandeling met 4-aminopyridine (4-AP) bij symptomatische personen met SCA27b/FGF14-GAA ataxie, waarbij tenminste sprake is van (episodische):
- balansproblemen,
- visusklachten of
- spraakproblemen,
- en er geen contra-indicaties (nierfunctiestoornissen, hartritmestoornissen, epilepsie) bestaan.
Bespreek dat eerste studies wijzen op een positief effect bij een deel van de personen met ataxie, RCT’s ontbreken echter. Het betreft vooralsnogeen off-label indicatie betreft waarbij dit middel niet door alle verzekeraars wordt vergoed.
Informeer de persoon met ataxie over mogelijke bijwerkingen en vraag toestemming voor het off-label gebruik.
Indien besloten wordt tot een proefbehandeling, evalueer het effect na 4–6 weken op basis van vooraf gestelde patiëntgerichte doelen op bovengenoemde domeinen. Besluit op basis hiervan of de behandeling voortgezet wordt. Gezien puur symptomatisch effect is bij uitblijven van resultaat op de gestelde doelen voortzetting van behandeling niet zinvol.
Verricht voor de start van de behandeling een ECG en bepaal de nierfunctie.
Controleer de nierfunctie regelmatig (elke 6 maanden) tijdens de behandeling.
Overleg met cardioloog en of nefroloog indien er (lichte) nierfunctie- of hartritmestoornissen zijn.

Waarom deze aanbeveling?
Uit de literatuur blijkt uit retrospectieve studies een mogelijk positief effect van 4-aminopyridine. Gezien de beperkte, reversibele bijwerkingen bij dit middel kan een proefbehandeling overwogen worden.

Balans tussen gewenste en ongewenste effecten
De bewijskracht voor positieve dan wel negatieve effecten van 4-AP is laag op basis van de beschikbare studies. Dit komt met name doordat de studies een kleine studiepopulatie omvatten, de studies niet gerandomiseerd en geblindeerd zijn en omdat de symptoomverbetering niet gestandaardiseerd is gemeten en resultaten niet structureel worden gepresenteerd. Desondanks zou 4-AP zou een positieve werking hebben op het verminderen van symptomen. Dit wordt gerapporteerd in 75% (Abou Chaar, 2024) en 84% (Wilke, 2023) van de personen met ataxie. De mogelijk gunstige effecten van 4-AP hebben onder andere invloed op balans, spraak, en verbeterde looppatroon. Ook verbetering van visuele symptomen kan mogelijk zijn bij behandeling met 4-AP. Over het algemeen wordt 4-aminopyridine (4-AP) beschouwd als een veilige behandeling voor SCA27b/FGF14-GAA ataxie (Abou Chaar, 2024). Daarnaast is er met dit (geregistreerde) medicijn ervaring opgedaan bij behandeling van multiple sclerose en andere vormen van episodische ataxie. Ook hier wordt 4-AP over het algemeen als veilig gezien met een beperkt bijwerkingenprofiel.
Ook uit nog ongepubliceerde data van een retrospectief groot Nederlands cohort met SCA27b/FGF14 personen werd een positief effect gevonden in ongeveer 50% van de personen die 4-AP hebben gebruikt of gebruiken.
Een kleiner deel van de personen (27%) rapporteerde een neutraal of negatief effect op de klachten. In dit cohort stopte ongeveer 16% van de SCA27b personen die 4-AP hadden gebruikt in verband met mogelijke bijwerkingen of uitblijven van effect.
Mogelijk is het effect van 4-AP behandeling afhankelijk van het genetisch profiel van FGF14-GAA ataxie, waarbij het effect gunstiger lijkt voor personen met een GAA >250 expansie (Pellerin, 2024).
Op basis van de eerste ervaringen en studieresultaten, lijkt 4-AP al met al een effectieve symptomatische behandeling in een deel van de personen met SCA27b. Gezien de afwezigheid van andere medicamenteuze symptomatische behandelingen, is het voorschrijven van een behandeling met 4-AP te overwegen. Aangezien 4-AP nog niet standaard vergoed wordt zou het mogelijk aangeboden kunnen worden gedurende een korte proefperiode, waarbij een goede monitoring van effectiviteit als ook eventuele bijwerkingen plaats dient te vinden. Evaluatie van het effect op basis van een objectieve meting van de ernst van de ataxie (met bijv. de SARA score) is niet altijd toepasbaar, omdat effecten ook binnen andere domeinen merkbaar kunnen zijn. Het vooraf stellen van individuele patiëntgerichte doelen (“goalsetting”) en evaluatie op basis van deze doelen lijkt beter aan te sluiten.
Als termijn van de proefbehandeling kan 4-6 weken worden aangehouden. Contra-indicaties voor de start van 4-AP zijn nierfunctiestoornissen en hartritmestoornissen. In dat kader dient voor de start een ECG verricht te worden en de nierfunctie gecontroleerd te worden. Wees terughoudend bij (lichte) nierfunctiestoornissen. Controleer de nierfunctie ook gedurende de behandeling (bijvoorbeeld iedere 6 maanden).
Kwaliteit van bewijs
Er is geen GRADE toegepast. De bewijskracht voor 4-AP in de behandeling van cerebellaire ataxie is dan ook niet duidelijk.
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
Gezien de afwezigheid van andere medicamenteuze symptomatische behandelingen zullen personen met ataxie waarschijnlijk geïnteresseerd zijn in een proefbehandeling, gezien de potentieel gunstige effecten en het beperkte bijwerkingenprofiel. Hierbij moet echter goed besproken worden dat het een off-label indicatie betreft en dat er monitoring van effecten en bijwerkingen plaatsvindt. Tevens dient een persoon zich bewust te zijn dat er (mogelijk) een eigen bijdrage nodig is voor het gebruik van 4-AP, aangezien het nog niet standaard vergoed wordt. Dit vormt een belangrijke beperkende factor, aangezien het structureel gebruik van dit medicijn daarmee niet voor alle personen financieel haalbaar zal zijn.
Kostenaspecten
Het voorschrijven van 4-AP levert voor een persoon extra kosten op aangezien het een middel is dat niet door alle zorgverzekeraars standaard vergoed wordt. De kosten bedragen ongeveer 8.50 euro per dag.
Vermindering van symptomen zou uiteindelijk echter kunnen leiden tot verbetering van kwaliteit van leven, en daardoor minder zorgkosten en maatschappelijke kosten (arbeidsverzuim, mantelzorg). Een goede inschatting hiervan is op dit moment echter lastig te geven.
Gelijkheid ((health) equity/equitable)
De interventie leidt tot een mogelijke afname van gezondheidsgelijkheid want het medicijn wordt momenteel in wisselende mate vergoed door zorgverzekeraars. Doordat het een off-label indicatie betreft gaan verzekeraars hier wisselend mee om. Daardoor zal een deel van de personen waar dit middel aan wordt voorgeschreven een eigen bijdrage moeten betalen.
Voor personen met een lagere socio-economische status zal deze eigen bijdrage een grotere impact hebben, danwel reden zijn om af te zien van de start van het medicijn. Vanuit het gelijkheidsprincipe is deze ongelijkheid erg onwenselijk en ethisch onjuist. Er dient snel meer bewijs te komen m.b.t. de effectiviteit van 4-AP. Dit zou gesprekken over vergoedingen met zorgverzekeraars makkelijker maken.
Aanvaardbaarheid
De interventie lijkt aanvaardbaar voor de betrokkenen. De ongelijkheid in vergoeding en daarmee het wel/ niet kunnen voorschrijven plaatst de medisch specialist echter wel voor een ethisch dilemma.
Duurzaamheid
Duurzaamheid is niet van beperkte invloed op de keuze tussen interventie of de controle behandeling omdat de productie van de medicijn waarschijnlijk niet fors zal toenemen gezien de relatief beperkte patiëntpopulatie.
Haalbaarheid
De werkgroep voorziet belemmeringen gezien bovengenoemde beperkingen in vergoeding bij het voorschrijven van 4-AP.
Onderbouwing
No disease-modifying drug treatment is available for patients with genetic or neurodegenerative ataxia. Currently, treatment is purely symptomatic and mainly relies on non-pharmacological intervention or on symptomatic drug treatment if specific non-ataxia signs are present. The effectiveness and risks of pharmacological treatment for the symptomatic treatment of (non-immune-mediated) ataxia are unknown.

Description of studies
A total of 3 studies were included in the analysis of the literature. Important study characteristics and results are summarized in table 2. The assessment of the risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).
Abou Chaar (2024) performed a retrospective cohort study including 102 participants from 5 academic centers in the United States. Patients were included if they had genetically confirmed SCA27B or expressions of GAA-FGF14 ≥250 repeats. No further restrictions were used for age or clinical presentation. Patients with other common genetic causes of cerebellar ataxia were excluded. Phenotypic profile, symptom onset, overall survival, imaging findings, disease severity, pathological findings, and 4-aminopyridine (4-AP) treatment response were reported.
Pellerin (2024) performed a retrospective cohort study including 170 participants with idiopathic downbeat nystagmus (DBN). Patients were retrospectively enrolled between 2012 and 2020. Exclusion was based on: no DNA available for genetic screening, DBN was not confirmed during examination, another competing aetiology was found for DBN. The authors reported 4-AP treatment response, phenotype and disease progression. The latter included measures of severity of ataxia, quality of life, adverse events, symptom improvement and change in oculomotor function. Treatment response to 4-AP was reported based on an open-label real-world treatment response data and a small double-blind placebo-controlled clinical trial. For the open-label study 4-AP treatment was given as routine clinical care to 83 patients, stratified in 3 groups based on their GAA repeat length (i.e. ³ 250 GAA repeats, 200-249 GAA repeats and <200 GAA repeats). This measured the clinician reported responses (ocular motor signs and gait) and patient reported benefits in 57 (36 GAA ³ 250, 5 GAA 200-249 and 16 GAA <200) and 73 patients (49 GAA ³ 250, 5 GAA 200-249 and 19 GAA <200) respectively. And the placebo-controlled trial on 4-AP was performed in four patients.
Wilke (2023) conducted a multi-modal cohort study including 50 patients with genetically confirmed GAA-FGF14 expansions of ≥250 repeats. These patients were identified from a larger cohort after genetic screening. The authors reported on genetic profile and phenotyping, disease progression, neuropathology, serum neurofilament light levels, and 4-AP treatment effect. The 4-AP treatment effect was investigated in 7 of the 50 (14%) patients in a retrospective cohort analysis. Additionally, in three subjects a prospective n-of-1 treatment was used with an intra-individually controlled ON/OFF protocol also assessing the effects of 4-AP treatment. The latter reported ataxia severity, yet these results were not incorporated in our analysis.

Results
1. SARA (severity) (critical)
Abou Chaar (2024) reported disease severity as SARA scores. Over a median follow-up of 2.8 (0.3–6) years, the mean SARA scores progressed from 8 (±5.5) at the first visit to 12 (±6.6) at the last visit. Of the included patients, 28 (27%) received 4-AP treatment. The change in SARA scores from 8 to 12 indicate an increase in disease severity during the course of the follow-up. These findings were based on general disease progression/ worsening but weren’t specifically related to 4-AP treatment.
Wilke (2023) described SARA scores to investigate the contribution of ataxia domains to GAA-FGF14 ataxia severity. They reported gait and stance ataxia, as well as lower-limb ataxia, to be drivers of overall severity in GAA-FGF14 ataxia. Sitting and speech disturbances were less common. They did not specify SARA scores for 10 patients treated with 4-AP.
Additionally, they also investigated disease progression based on SARA scores. Disease progression was considered slow as indicated by SARA scores. The gait and stance domains correlated with disease duration. The SARA scores remained within moderate levels, with a maximum of 18 points, even in subject with >20 years of disease duration. Thus indicating moderate level of disease severity in GAA-FGF14 ataxia. Disease progression based on SARA scores was not available for patients on 4-AP treatment.
Pellerin (2024) reported on severity of ataxia and cerebellar signs and disease progression for the different GAA strata. But didn’t report on the SARA specifically, neither did they report on the severity of ataxia specifically related to 4-AP treatment responses.
2. Quality of life (critical)
None of the included studies reported on the quality of life in patients with FGF14 ataxia (SCA27B), either in the general study population or specifically related to treatment response in patients treated with 4-AP.
3. FARS (ADL functioning) (important)
Two studies reported on ADL functioning as measured with the FARS. They both used this scale in a different context. The study of Pellerin (2024) assessed disease progression and Wilkes (2023) focused on functional impairment.
Pellerin (2024) reported disease progression using the FARS (Friedreich Ataxia Rating Scale). For the general cohort: the median FARS score at the last examination was 3 (IQR. 2 – 3), indicating mild disability. Progression was considered slow based on longitudinal data of 40 patients, with an individual increase of 0.10 FARS stage per year of disability. There was no association between FARS stage and disease duration.
The authors investigated the effect of 4-AP treatment on disability on and off treatment (based on the FARS disability stages) and found for nine patients (seven with the (GAA)≥250 allele, 2 with the (GAA)200-249 allele), that FARS stages were lower by up to 2 stages while on 4-AP compared to off 4-AP.
Functional impairment was described by Wilke (2023), and was shown to increase relatively slowly with disease duration. These findings were supported by a positive association of the FARS-FDS disease stage with disease duration. Functional impairment was not investigated specifically in the 4-AP group.
4. Symptom improvement (important)
Abou Chaar (2024) investigated the effect of 4-AP or dalfampridine ER in GAA FGF 14 (SCA-27B) patients in a retrospective/ prospective manner (depending on the institution) in 28 patients. Response were defined as a subjective symptom improvement reported by the patients that sustained for over 1 week. They found that treatment with 4-AP or dalfampridine ER resulted in positive results in twenty one patients (75%), based on subjective patient reports of symptom improvement.
The positive results were mainly in terms of improved balance, speech, diplopia, limb coordination, and frequency and intensity of episodes. Two patients (7%) treated with 4-AP reported worsening of symptoms (imbalance and vertigo) for which drug use was discontinued. The treatment mainly increased imbalance and vertigo. Five patients (18%) reported no effect after at least 2 months of treatment.
Pellerin (2024) reported 4-AP treatment response (before and 1 to 3 months after treatment initiation), stratified by GAA-FGF 14 genotype from the open-label real world treatment response data:
For the (GAA)≥250 group, clinician reports were available for 29 of 36 patients (81%). These reports described improvement of ocular motor signs in 24 patients (83%), and improvements in gait in 14 patients (48%) on 4-AP treatment.
The patient-reported benefit was recorded in 29 of 49 patients in the (GAA)≥250 group (59%), with improved visual symptoms in 14 patients (48%) and improved gait in 17 patients (59%) on 4-AP treatment.
For the (GAA)200-249 group, clinician reports were available for 4 of 5 patients (80%). These reports described improvement of ocular motor signs in 2 patients (40%), and improvements in gait in 3 patients (60%) on 4-AP treatment. For the (GAA)200-249 group, the patients-reported benefit was recorded for 3 of 5 patients (60%), with improved visual symptoms in 2 patients (40%) and improved gait in 2 patients (40%) on 4-AP treatment.
For the (GAA)<200 group, clinician reports were available for 5 of 16 patients (31%). These reports described improvement of ocular motor signs in 3 patients (60%), and improvements in gait in 2 patients (40%) on 4-AP treatment. For the (GAA)<200 group, the patients-reported benefit was recorded for 2 of 19 patients (11%), with improved visual symptoms in 1 patient (50%) and improved gait in 1 patient (50%) on 4-AP treatment.
Patients carrying a (GAA)≥250 or a (GAA)200-249 allele had a significantly greater clinician-reported (80%, 33/41 vs. 31%, 5/16; RR, 2.58; 95% CI, 1.23–5.41; Fisher’s exact test p = 0.0011) and patient-reported (59%, 32/54 vs 11%, 2/19; RR, 5.63; 95% CI, 1.49–21.27; Fisher’s exact test p = 0.00033) response rate to 4-AP treatment compared to patients carrying a (GAA) <200 allele.
Furthermore, they reported the results from a double-blind placebo-controlled clinical trial on 4-AP in four patients. Genotype was not known until after the trial, which showed all four patients were (GAA)≥250 allele positive. Video-oculography data was available for these patients from a previous double-blind trial. Slow phase velocity (SPV) decreased under treatment with 4-AP, but not with placebo. This study however didn’t report on the improvement on other symptoms.
Wilke (2023) reported 7 patients that received 4-AP treatment. A treatment effect relevant for daily living was reported by six patients (86%). These data were collected retrospectively. Additionally, the authors performed a series of prospective n-of-1 experiments in three patients. During ON-periods, where patients were taking the drugs, reduction of symptomatic time per day was documented by the patients. Frequency of days with severe symptoms was also reduced in all three patients along with a reduction in objective SARA scores. During the OFF-periods, these beneficial effects disappeared in all three patients.
5. Adverse events (important)
Adverse events were not systematically measured in included studies.
Abou Chaar reported cessation of drug use due to worsening of symptoms in 2 patients and 1 report of heartburn but stated that most patients tolerated the 4-AP treatment well without side effects. And Wilke (2023) reported no major AEs and stated an acceptable tolerability and minimal side effects.

Relevant outcome measures
The guideline panel considered Severity Ataxia (SARA) and quality of life as a critical outcome measure for decision making; and ADL functioning (PROM-ataxia), adverse events, and symptom improvement as an important outcome measure for decision making.
A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.
The guideline panel defined following differences as a minimal clinically (patient) important:
- Severity Ataxia (SARA): difference of 1.0 (or 1.1) between group and 2.0 within group.
- Quality of life: a change of 7 (to 10) points on the EQ-VAS or 5 (3-10) points on the SF36 (sub) scales.
- Activities of Daily Living:
- FARS: a 1 (to 2) point change on this scale.
- Adverse events: 0.91>RR>1.1 for adverse events.
- Symptom improvement: a change of at least 1-(to 2) point on the Numerical Rating Scale or Patient Global Impression of Change both assessing patient reported improvements, or a 2 (to 6) point change in PROMIS scale on fatigue/ mobility, cognition.
- All other outcome scales: 25% (0.8 ≥ RR ≥ 1.25) for dichotomous outcomes, 0.5 SD for continuous outcomes and standardized mean difference (0.5 ≥ SMD ≥ 0.5) as minimal clinically (patient) important differences.
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all. Both databases were searched from 2005 to April 15, 2025 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) cerebellar ataxia; (2) 4-aminopyridin. Duplicates were removed using EndNote software. After deduplication a total of 136 records were imported for title/abstract screening.
Initially, 18 studies were selected based on title and abstract screening. After reading the full text, 15 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and 3 studies were included.
An additional search strategy was derived on the following primary search concepts: (1) cerebellar ataxia; (2) any pharmacological intervention (no 4-aminopyridin). This resulted in 856 records. These were screened with the aim of providing a complete overview on pharmacological interventions in SCA27B for inclusion in the ‘Overwegingen’. Of these 3 were included.

Samenstelling werkgroep

Inbreng patiëntenperspectief
De werkgroep besteedde aandacht aan het patiëntenperspectief door de Ataxie Vereniging Nederland uit te nodigen voor de schriftelijke knelpuntenanalyse en een patiëntvertegenwoordiger namens Ataxie Vereniging Nederland was betrokken bij de ontwikkeling van de modules. De verkregen input is meegenomen bij het opstellen van de uitgangsvragen, de keuze voor de uitkomstmaten en bij het opstellen van de overwegingen. Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule voerde de werkgroep conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).

A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all. Both databases were searched from 2005 to April 15, 2025 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) cerebellar ataxia; (2) 4-aminopyridin/ other pharmacological intervention. Duplicates were removed using EndNote software. After deduplication a total of 136 records were imported for title/abstract screening. |
