Medicatie bij immuungemedieerde cerebellaire ataxie
Uitgangsvraag
Hoe dient de toepassing van farmacologische interventies bij de behandeling van ataxie bij volwassenen met immuungemedieerde cerebellaire ataxie er uit te zien?
De uitgangsvraag omvat de volgende deelvragen:
- Hoe dient de toepassing van farmacologische interventies bij de behandeling van ataxie van paraneoplastische oorzaak bij volwassenen met immuungemedieerde cerebellaire ataxie er uit te zien?
- Hoe dient de toepassing van farmacologische interventies bij de behandeling van ataxie van niet-paraneoplastische oorzaak bij volwassenen met immuungemedieerde cerebellaire ataxie er uit te zien?
Aanbeveling
Algemeen
- Stem altijd laagdrempelig het behandelschema af met een expertisecentrum, waarbij onderstaande een leidraad vormt.
- Stem de onderhoudsbehandeling af met een experticecentrum.
Algemeen immuungemedieerd
- Start bij mensen met sterke verdenking op immuungemedieerde ataxie met i.v. methylprednisolon (IVMP; 5 dagen 1000mg/dag). Geef indien de diagnose nog niet duidelijk is alleen kortdurend methylprednisolon (<7 dagen).
- Indien een lymfoom of cerebrale infectie niet zijn uitgesloten, start dan met i.v. immunoglobulines (IVIG; 5 dagen 0.4 g/kg/dag) in plaats van de methylprednisolon.
- Evalueer het effect van de behandeling: bij achteruitgang ondanks behandeling: IVIG of plasmaferese toevoegen.
- Bij ernstig aangedane persoon met ataxie (modified Ranking Scale (mRS) 4-5) of bekende antistof: gelijktijdig met of direct na IVMP, behandelen met IVIG of plasmaferese (in principe 5 cycli in 2 weken). Na behandeling evalueren effect.
- Stabiele/verbeterende persoon met ataxie met mRS 1-3, dan evalueren effect 2 weken na initiatie immuunsuppressie:
- Vooruitgang: afwachtend beleid.
- Achteruitgang: overweeg escalatie in overleg met expertisecentrum.
Paraneoplastisch
- Zet zo snel mogelijk behandeling oncologie/tumor in, ook bij ernstig aangedane persoon met ataxie (ook niet ADL-zelfstandig).
- Geef de tumorbehandeling altijd voorrang op immunosuppressieve therapie, als deze met elkaar interacteren.
- Start oncologische immuuntherapie alleen na overleg met een expertisecentrum. Immuuntherapie houdt een hoog risico in voor paraneoplastische neurologische syndromen.
Niet-paraneoplastisch
- Overweeg tweedelijns therapie (als rituximab of cyclofosfamide) bij
- personen met een antistof tegen een extracellulair antigen en achteruitgang na eerstelijnsbehandeling of
- personen met een aanhoudende mRS van 4-5, ondanks enkele weken behandeling met adequate dosering eerstelijnsbehandeling.
Overwegingen
Algemeen - Waarom deze aanbeveling?
Dit vanwege de lage incidentie van immuungemedieerde cerebellaire syndromen, de verscheidenheid aan geassocieerde antistoffen en de beperkte blootstelling van de gemiddelde neuroloog hieraan.
Eindoordeel: Conditionele aanbeveling voor.
Algemeen immuungemedieerd - Waarom deze aanbeveling?
Bovenstaande adviezen zijn gebaseerd op 1 grote cohortstudie bij personen met een NMDA-receptor encefalitis (Titulaer, 2013), kleinere studies bij personen met encefalitis of ataxie met andere antistoffen, in combinatie met ervaring van de auteurs.
Eindoordeel: Conditionele aanbeveling voor.
Paraneoplastisch - Waarom deze aanbeveling?
Een slechte Karnofsky performance score veroorzaakt door een cerebellaire ataxie dient niet als criterium voor afzien van oncologische behandeling gebruikt te worden, omdat dit potentieel een reversibel symptoom betreft, zeker bij adequate behandeling van de onderliggende tumor.

Niet-paraneoplastisch - Waarom deze aanbeveling?
Voor personen met antistoffen tegen extracellulaire (met name niet-paraneoplastische) antigenen is de meerwaarde vanuit niet-gerandomiseerde studies bij herhaling aangetoond (de Bruijn, 2025).
Eindoordeel: Conditionele aanbeveling voor.
Van bewijs naar aanbevelingen
Er zijn twee literatuuronderzoeken verricht naar de toepassing van farmacologische interventies bij de behandeling van ataxie bij volwassenen met immuungemedieerde ataxie. Eén richtte zich op de effectiviteit van tweedelijnstherapie bij volwassenen met immuungemedieerde ataxie van paraneoplastische oorzaak. De ander richtte zich op immuungemedieerde ataxie van niet-paraneoplastische oorzaak. De cruciale uitkomstmaten waren overleving, ADL/functie en ataxie. Een secundaire uitkomstmaat was complicaties van de behandeling.
Aangezien beide literatuuronderzoeken geen relevante studies opleverden én de overwegingen voor behandeling overlap kennen, is ervoor gekozen om de overwegingen bij deze onderzoeken samen te voegen. De zoekstrategieën en uitkomsten voor beide literatuuronderzoeken zijn te vinden in de respectievelijke submodules. Deze zijn beschikbaar in de bijlage.
Balans tussen voor- en nadelen van de interventie
Er zijn geen studies gevonden die het gebruik van tweedelijnstherapie (rituximab/cyclophosphamide) en/of IVIg/plasmaferese vergelijkt met eerstelijnstherapie prednison of placebo bij volwassenen met immuungemedieerde ataxie van niet-paraneoplastische oorzaak.
Er zijn geen studies gevonden die het al dan niet toevoegen van immunosuppressiva aan tumorbehandeling vergelijkt bij mensen met immuungemedieerde ataxie van paraneoplastische oorzaak.
De literatuur kan daarom geen richting geven aan de besluitvorming, waardoor de aanbeveling is gebaseerd op aanvullende argumenten en expert opinion, waar mogelijk aangevuld met indirect bewijs.
Aanvullende argumentatie
De behandeling van immuungemedieerde ataxie is grotendeels gebaseerd op (kleinere of grotere) cohortstudies van personen met dezelfde antistof, maar andere klinische presentaties ([limbische] encefalitis, stamencefalitis, neuronopathie, myelitis, stiff person syndroom, epilepsie). Ondanks het gebrek aan gerandomiseerde studies is er geen twijfel over het nut van immunosuppressieve behandeling bij antistoffen tegen extracellulaire antigenen (wat met name niet-paraneoplastische antigenen zijn); deze antistoffen zorgen voor directe functionele aanpassing van de werking van hersenen en het aanpakken van de antistoffen is meteen zinvol (de Bruijn, 2025). Ook is er geen twijfel over de fundamentele waarde van tumorbehandeling voor het remmen, of verbeteren, van de klachten veroorzaakt door een paraneoplastische antistof (Graus, 2021). De behandeling van een immuungemedieerde ataxie door middel van steroïden, al dan niet in combinatie met IVIG, wordt over het algemeen als adequaat beschouwd. De tweedelijnsbehandelingen rituximab en cyclofosfamide grijpen beide aan op een ander mechanisme en hebben daarom waarschijnlijk een synergistisch effect op de behandeling.
De tweedelijnsbehandelingen geven echter een verhoogde kans op bijwerkingen als leukopenie of trombocytopenie. Dit dient daarom gereserveerd te worden voor specifieke subpopulaties: bij een aangetoonde antistof tegen een extracellulair antigen en onvoldoende verbetering kan rituximab worden gestart, bij achteruitgang na eerstelijnsbehandeling of bij een aanhoudende mRS van 4-5 na behandeling kan cyclofosfamide worden overwogen.
Waarden en voorkeuren van patiënten
Gezien de complexiteit van de behandeling van een immuungemedieerd cerebellair syndroom en een beperkt tijdswindow waarin keuzes gemaakt dienen te worden, is er vaak een beperkte rol voor de specifieke voorkeuren van de persoon met ataxie of naaste. Dat laat onverlet dat het uiterst zinvol is om de persoon met ataxie mee te nemen in het besluitvormingsproces. Ook kan in sommige gevallen, bij ernstig aangedane personen met ataxie en slechts een kleine kans op verbetering, in overleg met persoon met ataxie afgezien worden van escalatie of continueren van behandeling.
Kostenaspecten
Omdat de beschreven behandelingen worden toegepast in een zeer geselecteerde groep, de potentiële gezondheidswinst aanzienlijk is en de behandeling zelf niet bovengemiddeld duur is, maakt dit het kostenaspect niet relevant voor de keuzes gemaakt in deze richtlijn.
Gezondheidsgelijkheid ((health) equity/equitable)
De behandeling van immuungemedieerd cerebellair syndroom valt onder vergoede zorg, wat maakt dat er geen sprake zal zijn van ongelijkheid in toegang tot behandeling.
Aanvaardbaarheid
Ethische aanvaardbaarheid
De interventie lijkt aanvaardbaar voor de betrokkenen. De werkgroep voorziet geen ethische bezwaren.
Duurzaamheid
De werkgroep/het cluster is van mening dat de milieu-impact geen rol speelt in de richting van de aanbeveling.
Haalbaarheid
De interventie is haalbaar, aangezien het reeds standaardzorg betreft.
Onderbouwing

The treatment of a cerebellar syndrome caused by paraneoplastic antibodies primarily consists of treating the underlying tumor. Additional treatment with immunotherapy may be considered; however, it is currently unclear whether this is beneficial. An intravenous course of methylprednisolone is often added, and if a good response is achieved, this may be followed by an oral course of prednisone (possibly in combination with IVIG).
In severely affected patients, second-line therapy is sometimes considered after a good response to the initial immunosuppressive treatment.

In immune-mediated cerebellar ataxia of non-paraneoplastic origin, treatment generally consists of first-line immunotherapy/immunosuppression in the form of (methyl)prednisolone. In patients with a known antibody or in those who are severely affected, this may be combined with IVIG or plasma exchange, either concurrently or sequentially. Second-line immunotherapy can also be added. The extent to which these treatments provide additional benefit remains unclear.

The guideline panel defined 25% (0.8 ≥ RR ≥ 1.25) for dichotomous outcomes, 0.5 SD for continuous outcomes and standardized mean difference (0.5 ≥ SMD ≥ 0.5) as minimal clinically (patient) important differences.
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all. Both databases were searched from 1999 to July 30th, 2025, for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) autoimmune cerebellar ataxia; (2) immunosuppressive therapy. Duplicates were removed using EndNote software. After deduplication a total of 982 records were imported for title/abstract screening.
Subsequently, titles and abstracts of 982 studies were screened by the guideline methodologist using the ASReview software, using the following stopping rule: stop after 10% of the total set subsequent exclusions. The settings ELAS u4 (TF-IDF and SVM) were used. Shams’ili (2003) was used as prior knowledge for inclusion. The working group screened a subset of 20 randomly excluded studies to verify the study selection.
Initially, 22 studies were selected based on title and abstract screening. After reading the full text, 21 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and no studies were included.

Inbreng patiëntenperspectief
De werkgroep besteedde aandacht aan het patiëntenperspectief door de Ataxie Vereniging Nederland uit te nodigen voor de schriftelijke knelpuntenanalyse en een patiëntvertegenwoordiger namens Ataxie Vereniging Nederland was betrokken bij de ontwikkeling van de modules. De verkregen input is meegenomen bij het opstellen van de uitgangsvragen, de keuze voor de uitkomstmaten en bij het opstellen van de overwegingen. De conceptrichtlijn is tevens voor commentaar voorgelegd aan de Ataxie Vereniging Nederland en de eventueel aangeleverde commentaren zijn bekeken en verwerkt.

A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all. Both databases were searched from 1999 to July 30th, 2025 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) autoimmune cerebellar ataxia; (2) immunosuppressive therapy. Duplicates were removed using EndNote software. After deduplication a total of 982 records were imported for title/abstract screening. |
