Slaaptesten voor diagnostiek bij CSA
Uitgangsvraag
- Chronisch opiaat gebruik
- CVA in de voorgeschiedenis
- Werkzaamheidsfalen CPAP therapie
Onderstaande studies geven aanvullend relevante informatie.
Gelijkheid ((health) equity/equitable)
Eindoordeel:
Sterke aanbeveling voor.
Onderbouwing
In-laboratory polysomnography (PSG) including esophageal pressure measurement is considered the gold standard for the diagnosis of CSA and the differentiation of CSA from other sleep disorders, such as obstructive sleep apnea, movement disorders or insomnia, causing central sleep apneas in sleep–wake transitions. However, an esophageal pressure transducer is not easily tolerated, so surrogates of respiratory effort are mostly used during polysomnography. Moreover, in-laboratory polysomnography is expensive, labor and time-consuming. Ambulatory polysomnography and other (type 3) respiratory diagnostic tests, such as conventional polygraphy and PAT technology are available. We wanted to know the accuracy of an (ambulatory or in-laboratory) polysomnography without esophageal pressure measurement in the diagnostic work up of CSA, as compared to an in-laboratory PSG with esophageal pressure measurement. As this search did not yield any relevant papers, we amended the refence test in the selection to in-laboratory polysomnography to reflect clinical practice. We also wanted to know if there is evidence on the accuracy of type 3 respiratory diagnostic tests in the diagnosis of CSA, as compared to polysomnography.
The level of evidence (GRADE method) is based on results from one diagnostic test accuracy (DTA) study and therefore starts at level “high”. The level of evidence was downgraded by three levels because of very serious risk of bias (i.e., due to funding and potential involvement of industry, potential selection bias and no reported prevalence of the disease with the reference test; -3), and imprecision (no report of 95% confidence interval for diagnostic values; -1). The level of evidence for the outcomes ‘sensitivity, specificity, positive predictive value, negative predictive value and area under the curve’ is very low.

Description of studies
One study was included in the analysis of the literature. Important study characteristics and results are summarized in table 3. The assessment of the risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).
Pillar (2020) performed a multicenter study to assess the accuracy of enhanced WatchPAT to differentiate between central sleep apnea (CSA) and obstructive sleep apnea (OSA) compared with simultaneous in-laboratory polysomnography (PSG). This study was performed in 84 patients from 11 study centers, with suspected sleep-disordered breathing (SDB). Inclusion criteria were: age between 17 and 90, ability to read, understand and sign the informed consent form, persons suspected of having SBD with and without cardiac disorders, willingness to sleep with the WatchPAT and PSG simultaneously in the sleep lab. The authors selectively recruited heart failure patients to secure a substantial representation of CSA in this study (selection bias). This resulted in 59.5% of included patients being cardiac patients (congestive heart failure and/or atrial fibrillation). Exclusion criteria were: finger deformity that precludes adequate sensor appliance, and use of alpha blockers or short-acting nitrates (<3h before the study). Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV), overall agreement and kappa level of agreement and area under the receiver operating curve (ROC) were calculated with PSG as gold standard, based on two thresholds of AHI≥10 and AHI≥15.

Results
Pillar (2020) studied 84 patients suspected of having sleep-disordered breathing (SDB), with focus on recruiting patients with congestive heart failure. This was a multi-center study in Israel, Germany, USA and Canada. It is not reported how many of these 84 patients were diagnosed with central sleep apnea (CSA).
The diagnostic values of central apnea-hypopnea index (AHIc) ≥10 events / hour to diagnose CSA are as followed:
- Sensitivity: 71.4%
- Specificity: 98.6%
- PPV: 90.9%
- NPV: 94.5%
- AUC: 0.827
Prevalence of the diagnosis CSA based on AHIc ≥10 events / hour is not presented by Pillar (2020), therefore the underlying values true positive, true negative, false positive and false negative are unknown.
The diagnostic values of central apnea-hypopnea index (AHIc) ≥15 events / hour to diagnose CSA are as followed:
- Sensitivity: 66.7%
- Specificity: 100%
- PPV: 100%
- NPV: 94.7%
- AUC: 0.866
Prevalence of the diagnosis CSA based on AHIc ≥15 events / hour is not presented by Pillar (2020), therefore the underlying values true positive, true negative, false positive and false negative are unknown.
A systematic review of the literature was performed to answer the following question(s):
What is the diagnostic accuracy of in-laboratory PSG / ambulatory PSG / ambulatory conventional polygraphy / PAT technology in patients suspected of CSA?

Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched until the 6th of May 2025 for systematic reviews and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from two primary search concepts: (1) central sleep apnea; (2) sleep study. Duplicates were removed using EndNote software. After deduplication a total of 546 records were imported for title/abstract screening.
Initially, seven studies were selected based on title and abstract screening. After reading the full text, six studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and one study was included.
