Plaats neurologisch consult/MRI bij CSA
Uitgangsvraag
Gelijkheid (health) equity/equitable)
Eindoordeel:
Sterke aanbeveling tegen.
Onderbouwing
In some patients with central sleep apnea (CSA) an underlying intracerebral anomaly is the cause of this breathing pattern. However, the frequency and risk factors of such causative intracranial lesions are not known. It is therefore unclear which patients should be referred for a neurological examination and/or MRI-cerebrum leading to considerable practice variation and costs.

Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 2005 to the 30th of April 2025 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from two primary search concepts: (1) central sleep apnea; and (2) magnetic resonance imaging (MRI). Duplicates were removed using EndNote software. After deduplication a total of 540 records were imported for title/abstract screening. Initially, 0 studies were selected based on title and abstract screening (see the exclusion table under the tab ‘Evidence tabellen’).
As anticipated, the body of literature in this field is limited, and studies conducted in children were therefore included as part of the literature search. Due to insufficient results, the search was expanded to include studies from 1980 to 30th of April 2025, yielding 82 more articles for screening.
