O₂-suppletie bij CSA
Uitgangsvraag
Gelijkheid ((health) equity/equitable)
Eindoordeel:
Zwakke aanbeveling voor.
Onderbouwing
Central sleep apnea (CSA) can be treated with (C)PAP, although its effectiveness is variable. Small studies suggest that nocturnal oxygen can improve the apnea-hypopnea index (AHI) in these patients. While the AASM suggests considering oxygen therapy for CSA related to heart failure, the added benefit of combining it with (C)PAP compared to (C)PAP alone remains unclear.
Description of studies
A total of three studies were included in the analysis of the literature. Important study characteristics and results are summarized in table 2. The assessment of the risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).
Chowdhuri (2012) conducted a retrospective chart review over a period of 3 years, performed at a Veterans Affairs sleep disorders center in Detroit. The CSA treatment protocol was designed as a technologist driven protocol that was utilized in all patients with CSA, regardless of its etiology or the presence of concomitant OSA.
Sadeghi (2019) conducted a longitudinal cross-sectional study. A total of 64 patients with CSA (defined as CAI > 5) were recruited consecutively from sleep and respiratory disorder centers in Isfahan, Iran, between April 2017 and January 2018. Patients unresponsive to one treatment progressed to the next in the protocol using a stepwise approach: CPAP, CPAP + supplemental oxygen, and BiPAP.
Schoebel (2018) conducted a longitudinal cross-sectional study. This study included 41 opioid related CSA patients who underwent a step-by-step protocol (including CPAP, CPAP + O₂ and BPAP) in which if the patient was nonresponsive to a treatment, the next therapy was applied. If the patient was nonresponsive to all of these approaches, only oxygen was administered. Male patients presenting at the centers of sleep and breathing disorders in Isfahan during April 2017 to March 2018 were included.

CSA treatment protocol: Positive pressure titration was initiated at CPAP 4-5 cm H2O and titrated upward to 10-14 cm H2O. If frequent central apneas persisted at CPAP pressures of 10-14 cm H2O, then CPAP was not increased further, in order to avoid further hyperinflation and hypocapnia. Instead, supplemental O₂ was added at 2 liters per minute (L/min) to dampen the post-apneic ventilatory overshoot, and increased by 1 L/min to maintain oxygen saturation ≥ 93%, keeping CPAP at the same level. Oxygen saturation was maintained at ≥ 93%, to ensure that it was consistently > 90% even if apnea occurred, and allowing for a 2% device error. Supplemental oxygen at above goal was continued for ≥ 20 minutes. If central apneas persisted despite the addition of adequate supplemental O₂ for ≥ 20 min, CPAP was switched to BPAP while maintaining oxygen saturation ≥ 93%. Initial BPAP setting was adjusted to keep the inspiratory positive airway pressure setting (IPAP) 2-3 cm H2O higher, and the expiratory positive airway pressure setting (EPAP) 2 cm H2O lower than the previous CPAP setting. These were adjusted upward if hypopneas and obstructive apneas in addition to CA appeared, but keeping the IPAP-EPAP difference low (4-6 cm H2O) to prevent further hypocapnia and central apneas. The usual laboratory protocol was to achieve optimal titration in both supine and lateral positions. |

Results
As none of the included studies met the criteria for the GRADE protocol, there is no literature analysis. Below there is a summary of the included studies.
Chowdhuri (2012)
Of the 45 patients with central sleep apnea (CSA), 14 responded to CPAP, while 31 were non-responders. Among these non-responders, 7 showed a response to CPAP + O₂, and 24 remained nonresponsive. This study did not report the raw data of AHI. Other relevant outcomes were not reported.
Sadeghi (2019)
Of the 64 patients, 27 (42.2%) responded to CPAP, while 37 (57.8%) did not. Among the CPAP non-responders, 13 patients (20.3% of total) responded to CPAP + O₂, and 24 (37.5% of total) did not. The mean AHI was comparable in CPAP + O₂ responders compared to CPAP responders (26.20 ± 7.02 vs. 29.11 ± 32.06; p = 0.020). Other relevant outcomes were not reported.
Schoebel (2018)
Of the 41 patients, 17 were responsive to CPAP versus 24 nonresponsive patients. Of these non-responders, 6 patients were responsive to CPAP+ O₂, versus 18 nonresponsive patients. A lower mean AHI was reported for CPAP+ O₂ responders versus the non-responders (21.67±15.87 vs. 34.33±12.17). The mean age of CPAP responders was higher than in CPAP+ O₂ responders (67.18±12.25 vs. 57.68±5.99).
Summary of Findings
As none of the included studies met the criteria for the GRADE protocol, there is no summary of evidence.
A systematic review of the literature was performed to answer the following question(s):
Does the addition of O₂ supplementation to (C)PAP treatment of patients with CSA result in better AHI, QoL, mortality, LVEF and hospitalization outcomes compared to only (C)PAP treatment?

Relevant outcome measures
The guideline panel considered AHI and mortality as a critical outcome measure for decision making; and QoL, LVEF, and hospitalization as an important outcome measure for decision making.
A priori, the guideline panel did not define the outcome measure hospitalization as the number of hospitalizations for cardiac events. The guideline panel did not define the other outcome measures listed above but used the definitions used in the studies.
The guideline panel defined the following values per outcome as a minimal clinically (patient) important difference.
- Apneu-hypopneu index (AHI): decrease by 50%.
- All-cause mortality: risk difference by 5% (0.95≥RR≥1.05).
- Quality of life (QoL): Risk difference of ≥ 10 point on the MLHFQ scale (Zhu 2023).
- Hospitalization (total number of hospitalizations for cardiac events): Risk difference of ≥ 25% (0.75>RR>1.25).
- LVEF: risk difference by 5% point (0.95≥RR≥1.05).
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 2005 to the 15th of April 2025 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from two primary search concepts: (1) central sleep apnea; and (2) oxygen supplementation. Duplicates were removed using EndNote software. After deduplication a total of 203 records were imported for title/abstract screening. Initially, 32 studies were selected based on title and abstract screening. After reading the full text, 29 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and three studies were included. One of the excluded studies is summarized in the ‘overwegingen’ section.
