Acetazolamide bij CSA
Uitgangsvraag
Gelijkheid ((health) equity/equitable)
Eindoordeel:
Sterke aanbeveling tegen.
Onderbouwing
Central sleep apnea (CSA) encompasses a heterogeneous group of disorders characterized by impaired ventilatory control during sleep. Multiple therapeutic approaches are currently used, targeting different underlying mechanisms. Acetazolamide, a carbonic anhydrase inhibitor, has been proposed as a pharmacological option to stimulate ventilation by inducing mild metabolic acidosis. However, its clinical use remains limited and varies substantially between centers. Uncertainty persists regarding its efficacy, mechanism of action, optimal patient selection, and comparative position relative to other available therapies. Evidence is needed to determine whether acetazolamide provides clinically meaningful benefit in the management of CSA.
CSA related to high-altitude is outside the scope of this guideline, as it is typically transient and not applicable to the Dutch population.

Description of studies
Three randomized controlled trials (RCTs) were included in the analysis of the literature. Important study characteristics and results are summarized in table 2. The assessment of the risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).
Javaheri (2006) performed a randomized double-blind, placebo-controlled, cross-over study in the USA, to determine the effect of acetazolamide versus placebo (taken 1 hour before bedtime for 6 nights with 2 weeks washout period) on central sleep apnea (CSA) associated with heart failure. Male patients with systolic heart failure whose initial polysomnograms showed Cheyne-Stokes breathing with an apnea–hypopnea index (number of episodes/hour) of more than 15/hour were eligible for inclusion in the study. Exclusion criteria included female patients; unstable cardiovascular status; significant intrinsic pulmonary, renal, or liver disorders; and use of morphine derivatives, benzodiazepines, or theophylline.
Naghan (2019) performed a double-blind, placebo-controlled, cross-over trial in Iran, to determine the effect of acetazolamide versus placebo on CSA. Adult opioid abusers with CSA of >5 episodes/hour and more than 50% of apnea-hypopnea index (AHI) determined by clinical polysomnography were included in the study. Central hypopnea was considered as an enrollment criterion but due to low accuracy of Randerath ’s criteria, i.e., 68%, the researchers decided not to distinguish central from obstructive hypopneas. Inevitably, all hypopnea was considered as a single unit.
Ginter (2020) performed a randomized placebo-controlled, cross-over trial in the USA, to determine if acetazolamide would decrease the propensity to develop hypocapnic central apnea and decrease the apneic threshold. Inclusion criteria were not clearly described, but participants included both able-bodied and spinal cord injury (at or above the T6 level) patients with evidence of sleep-disordered breathing AHI≥5 events/hour. Subjects younger than 17 years of age and pregnant or breast-feeding women were excluded, as well as subjects with sulfa-drug allergies.

Results
Apnea-hypopnea index (AHI)
Javaheri (2006) reported, in 12 patients with systolic heart failure, a lower AHI after acetazolamide compared to placebo after 4 weeks, with a mean ± SD of 57 ± 28 events/hour after placebo and 34 ± 20 events/hour after acetazolamide. Mean difference could be calculated as +3.6% after placebo and -38.2% after acetazolamide.
Naghan (2019) reported, in 10 adult opioid abusers, a lower AHI after acetazolamide compared to placebo after 6 days, with a median (interquartile range) of 20.0 (7.1-61.6) events/hour after placebo and 6.4 (3.1-34.0) events/hour after acetazolamide. Mean difference could not be calculated, because results were presented as median (IQR).
Ginter (2020) also reported, in 8 able-bodied and 8 spinal cord injury patients, a lower AHI after acetazolamide compared to placebo after 3 days, with a mean ± SD of 39.3 ± 24.1events/hour after placebo and 28.8 ± 22.9 events/hour after acetazolamide. Mean difference could not be calculated, because baseline AHI was not presented for the total group (only presented separately for able-bodied and SCI patients).
Results could not be pooled due to inconsistency in study populations and inconsistency in outcome measures.

Relevant outcome measures
The guideline panel considered apnea-hypopnea index (AHI), cardiovascular mortality and all-cause mortality as a critical outcome measures for decision making; and quality of life as an important outcome measure for decision making.
A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.
The guideline panel defined the following values per outcome as a minimal clinically (patient) important difference.
- Apnea-hypopnea index (AHI): decrease by 50%;
- Cardiovascular mortality: Risk difference of 5% (0.95≥RR≥1.05);
- All-cause mortality: Risk difference of 5% (0.95≥RR≥1.05);
- Quality of life (QoL): Difference of ≥ 10 points on the MLHFQ scale (Zhu 2023); Difference of ≥ 5 points on the SF-36 scale (Ogura 2020).
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 2005 to the 19th of May 2025 for systematic reviews and RCTs. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from two primary search concepts: (1) central sleep apnea; (2) acetazolamide. Duplicates were removed using EndNote software. After deduplication a total of 103 records were imported for title/abstract screening. Initially, six studies were selected based on title and abstract screening. After reading the full text, three studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and three studies were included.

Manuel Sastry (vz.) |
