Latente tuberculose-infectie (LTBI) en tuberculose

Tuberculose wordt veroorzaakt door Mycobacterium tuberculosis, en wordt aerogeen verspreid. Na blootstelling aan een besmettelijke patiënt kunnen mensen besmet raken; er is dan sprake van een zogenaamde latente tuberculose-infectie (LTBI), die asymptomatisch en niet besmettelijk is. Bij 5-10% van de geïnfecteerden ontwikkelt de infectie zich tot actieve tuberculose. Het risico op ziekte is het hoogst in de eerste twee jaar na besmetting. Bij jonge kinderen en mensen met een immuunstoornis zoals een hiv-infectie of gebruik van immuunsuppressiva is de kans op progressie van (latente) infectie naar ziekte verhoogd.

LTBI wordt gedefinieerd als de aanwezigheid van een cellulaire immuunrespons tegen M. tuberculosis complex, bij een persoon zonder klinische, bacteriologische of radiologische aanwijzingen voor actieve tuberculose. De tuberculinehuidtest (THT) en de interferon-gamma release assay (IGRA) zijn testen voor het aantonen van een infectie met M. tuberculosis complex. Ze worden 6-8 weken na besmetting positief op basis van M. tuberculosis-antigenen die bij reeds geïnfecteerde personen een T-cel gemedieerde immuunrespons veroorzaken. Bij een positieve THT is een induratie in de huid meetbaar. Bij een positieve IGRA wordt in het bloed een significante hoeveelheid IFN-γ waargenomen. Geen van beide testen kunnen onderscheid maken tussen een LTBI en actieve tuberculose.
