Risico op uitlokken suïcidaal gedrag

Uitgangsvraag

Kan het onderzoek naar suïcidaal gedrag een suïcide uitlokken?

Aanbeveling

Het is aan te bevelen om naar suïcidaal gedrag te vragen als daar aanleiding voor is. Patiënten waarderen daarbij een betrokken en deskundige houding van de professional. Het kan nodig zijn de gesprekstechniek af te stemmen op specifieke gevoeligheden van de patiënt wanneer deze uit het gesprek blijken.

Overwegingen

Een algemeen klinisch uitgangspunt is dat diagnostiek en behandeling van een probleem pas mogelijk is als daarover met de patiënt wordt gesproken. Het niet bespreekbaar maken van suïcidegedachten die er wel zijn ontneemt de professional de mogelijkheid om hulp te bieden. Het kan niet worden uitgesloten dat sommige patiënten emotioneel kunnen reageren op vragen over suïcidaal gedrag of dat zij van hun suïcidale gedragingen een strijdpunt maken. Bij deze patiënten moet de professional zijn gesprekstechniek aan de patiënt aanpassen.

Onderbouwing

De professional kan zich in het vragen naar suïcidaal gedrag geremd voelen door de vrees dat vragen naar suïcidegedachten het gedrag in de hand werkt. In de literatuur is gezocht naar empirisch bewijs voor deze vrees.

 opstelling van professional die uitleg geeft over de hulp die wordt verleend.

In een gerandomiseerd onderzoek naar iatrogene risico’s van een  screeningprogramma bij 2.342 jongeren werd geen verschil gevonden tussen groepen die waren blootgesteld aan vragen over suïcidaal gedrag en een controlegroep waarin dit niet gebeurde (Gould et al., 2005). In hoogrisicogroepen van jongeren met depressieve symptomen of met een voorgeschiedenis van suïcidaal gedrag waren spanningsklachten en  suïcidegedachten in de interventiegroep significant afgenomen. Cukrowicz et al. (2010) rapporteerden een klein onderzoek bij 21 personen met een depressieve stoornis die deelnamen aan een intensief onderzoek naar  suïcidegedachten. In de follow-ups na 1 en 3 maanden rapporteerde geen van de respondenten dat suïcidegedachten waren toegenomen. Bij de eerste follow-up waren de gedachten bij 7 deelnemers gelijk gebleven en bij 14 afgenomen. Bij de tweede follow-up waren dat respectievelijk 9 en 12. Uit psychotherapie is bekend dat als een professional denkt dat zijn patiënt suïcidegedachten heeft, dit vaak blijkt te kloppen (Hendin et al., 2006). In een review van 31 onderzoeken naar de ervaring met professionals van personen met zelfbeschadigend gedrag komt naar voren dat patiënten een betrokken en deskundige houding van professionals waarderen (Taylor et al., 2009). Wat patiënten misten, was een gevoel van betrokkenheid en duidelijke uitleg over de verleende hulp.

Conclusie: er zijn geen aanwijzingen voor iatrogene schade van het vragen naar suïcidaal gedrag onder middelbare scholieren.

Doel en doelgroep

Het doel van deze richtlijn is concrete aanbevelingen te geven voor diagnostiek en behandeling van suïcidaal gedrag, op basis van het beste beschikbare bewijs. De opstellers van deze richtlijn zijn psychiaters, psychologen, huisartsen en verpleegkundigen. Wij bevelen de richtlijn aan de volgende beroepsgroepen aan: artsen, verpleegkundigen, psychologen en psychotherapeuten die werkzaam zijn in de gezondheidszorg. De werkgroep is van mening dat het herkennen van suïcidaal gedrag en



het uitvoeren van een oriënterend onderzoek tot de basisvaardigheden behoort van alle artsen, verpleegkundigen, psychologen en psychotherapeuten in de gezondheidszorg.

Deze richtlijn is niet opgesteld vanuit het perspectief van niet-big-geregistreerde professionals binnen en buiten de gezondheidszorg die geregeld met suïcidaal gedrag te maken krijgen, zoals onderwijspersoneel, politie en geestelijk verzorgers. Hoewel deze professionals een belangrijke rol kunnen spelen in de zorg bij suïcidaal gedrag, zijn voor hen geen richtlijnen beschikbaar en er is nauwelijks empirisch bewijs om het handelen op te baseren. De werkgroep adviseert deze professionals om samenwerking te zoeken met de genoemde artsen, verpleegkundigen, psychologen en psychotherapeuten die werkzaam zijn in de gezondheidszorg. Een richtlijn is niet bedoeld als uitputtende en limitatieve beschrijving van alle vormen van zorg die bij individuele patiënten kunnen worden toegepast. Richtlijnen zijn dan ook geen standaarden die in alle omstandigheden van toepassing kunnen worden verklaard. De professional heeft de verantwoordelijkheid om op basis van onderzoek te komen tot een indicatiestelling voor behandeling. De professional komt tot een oordeel op basis van alle beschikbare informatie, rekening houdend met de actuele mogelijkheden en beperkingen. Door deze richtlijn te volgen, draagt u bij aan zorgvuldig klinisch handelen bij patiënten met suïcidaal gedrag, maar het biedt geen garantie voor een gunstige uitkomst bij elke patiënt. Ook bij zorgvuldig klinisch handelen zullen er patiënten zijn die suïcide plegen.

Werkwijze

Empirische evidentie

Bij het opstellen van deze richtlijnen zijn twee uitgangspunten gehanteerd die soms met elkaar op gespannen voet stonden. Ten eerste is als werkwijze gekozen voor de methodiek van de evidence-based richtlijnontwikkeling (ebro), ontwikkeld door het Centraal Begeleidingsorgaan Kwaliteitsinstituut voor de Gezondheidszorg (Van Everdingen et al., 2004). De richtlijn is voor zover mogelijk gebaseerd op bewijs uit gepubliceerd wetenschappelijk onderzoek, voor een deel samengevat in de apa-richtlijn (apa, 2003). Daarnaast werden relevante artikelen gezocht door het verrichten van systematische zoekacties. Er werd gezocht in de Cochrane-database, Medline, Psychinfo, en bij vragen waar dit relevant is ook in Cinahl. Op verzoek zijn de volledige zoekstrategieën beschikbaar. Daarnaast werden artikelen geëxtraheerd uit referentielijsten van opgevraagde literatuur. De beoordeling van de artikelen vindt u in de verschillende teksten terug onder het kopje ‘wetenschappelijke onderbouwing’. Het wetenschappelijke bewijs is vervolgens kort samengevat in een ‘conclusie’. De belangrijkste literatuur waarop deze conclusie is gebaseerd, staat bij de conclusie vermeld, inclusief de mate van bewijs. In bijlage 7 vindt u de definities van de classificatie van onderzoek (A1, A2, B, C of D) en die van de bewijsniveaus (1 tot en met 4), met een toelichting.

Voor het formuleren van een aanbeveling zijn er naast het wetenschappelijke bewijs vaak nog andere aspecten van belang, bijvoorbeeld: patiëntenvoorkeuren, kosten, beschikbaarheid (in verschillende echelons) of organisatorische aspecten. Deze aspecten worden, voor zover niet wetenschappelijk onderzocht, vermeld onder het kopje ‘overige overwegingen’. In de overige overwegingen spelen de ervaring en de mening van de werkgroepleden een belangrijke rol. De ‘aanbeveling’ is het resultaat van de integratie van het beschikbare bewijs met de weergegeven overige overwegingen. Van de in conclusies gebruikte literatuur zijn zogeheten evidencetabellen oftewel bewijstabellen gemaakt, die zijn opgenomen in deze richtlijn (bijlagen 8 en 9).

Ten tweede is in de doelstelling van de richtlijn vastgelegd dat deze aansluit bij vragen uit het werkveld. Veel van deze vragen zijn echter niet eenduidig te beantwoorden op basis van empirisch onderzoek. Het empirisch onderzoek naar suïcidaal gedrag heeft namelijk twee essentiële, methodologische beperkingen. In de eerste plaats is suïcide naar verhouding een zeldzame gebeurtenis waardoor prospectief onderzoek met suïcide als uitkomstmaat vrijwel onmogelijk is. In de tweede plaats is suïcide een ongewenste gebeurtenis die voor zover mogelijk door ingrijpen zal worden afgewend. Nagenoeg alle resultaten van onderzoek waarin suïcidaal gedrag als uitkomstmaat geldt, zullen hierdoor vertekend zijn.

 

Professioneel oordeel via werk- klankbord- en focusgroepen

Omdat de evidentie op wetenschappelijk basis beperkt is, is het oordeel van deskundigen op het gebied van suïcidaal gedrag zwaar meegewogen. Een brede vertegenwoordiging van professionals was nauw betrokken bij de ontwikkeling van deze richtlijn, waarbij gezocht is naar consensus over kennis en inzichten. Professionals waren vertegenwoordigd in een werkgroep, de klankbordgroep en twee focusgroepen. Voor het samenstellen van de werkgroep en een klankbordgroep is de betrokken beroepsverenigingen gevraagd om een afvaardiging. Tevens zijn vertegenwoordigers van patiëntenverenigingen en groepen en personen met specifieke deskundigheid uitgenodigd.

De uitgangsvragen en teksten zijn voorbereid door de werkgroep en voor commentaar voorgelegd aan de klankbordgroepen. Met de klankbordgroepen werd driemaal een invitational conference belegd. Met de verpleegkundige focusgroep zijn drie bijeenkomsten belegd.

Om het patiëntenperspectief tot zijn recht te laten komen, werd tweemaal overlegd met vertegenwoordigers van de Ivonne van de Ven Stichting. Deze stichting van nabestaanden richt zich vanuit ervaringsdeskundigheid op het bevorderen van suïcidepreventie. Dit overleg werd later uitgebreid en er werd een focusgroep patiëntenparticipatie geformeerd, waarin vertegenwoordigers van Stichting Ex6, Stichting 113Online en het Landelijk Platform ggz vertegenwoordigd waren.

 

Literatuuronderzoek

Bestaande internationale richtlijnen zijn gebruikt als startpunt voor het literatuuronderzoek. Het betreft de richtlijnen van de American Psychiatric Association (apa, 2003), van de New Zealand Guideline Group (2003), van het National Institute for Clinical Excellence (nice, 2004), The Royal Australian and New Zealand College of Psychiatrists (ranzcp, 2004) en The Royal College of Psychiatrists (2004; 2010). Daarnaast is gebruikgemaakt van twee uitgebreide literatuuronderzoeken van de Schotse overheid (McLean et al., 2008; Leitner et al., 2008). Voor hoofdstuk 4 tot en met 9 is aanvullend literatuuronderzoek uitgevoerd teneinde het literatuuroverzicht te actualiseren. De zoekopdrachten zijn opgenomen in de bewijstabellen, die zijn toegevoegd als bijlagen (bijlage 8 en 9). De verpleegkundige focus-groep heeft voor diverse uitgangsvragen aanvullend gezocht in de verplegingswetenschappelijke literatuur.

De werkgroep sluit zich aan bij de voor ggz-richtlijnen veel toegepaste interpretatie van tabelB9.1, waarbij ook single blind rct’s in de richtlijn een A-niveau toegekend krijgen. Omdatonderzoek naar psychologische interventies niet dubbelblind uitgevoerd kan worden, is deindeling van methodologische kwaliteit anders niet volledig van toepassing op onderzoek naarpsychologische interventies.
