Wat is de prognose van SOLK?

Overwegingen

Onderzoek in tweedelijnspopulaties bij mensen met lichamelijk onverklaarde buikpijn of met functionele syndromen laat zien dat de prognose veel ongunstiger is. Vaak duren de klachten vijf jaar of langer. Voor een deel hangt dit samen met het feit dat bij deze populatie de klachten ernstiger zijn en dat - bij de functionele syndromen per definitie - sprake is van meer klachten. Dit zijn de twee factoren die samenhangen met een ongunstig beloop bij SOLK in de eerste lijn.

Onderbouwing

Er is maar weinig prospectief onderzoek verricht naar de prognose van SOLK. Lastig bij het interpreteren van de gegevens van onderzoek naar de prognose is dat de startsitu- atie in de verschillende onderzoeken vaak verschillend is. Wanneer en op welke gronden is vastgesteld dat er sprake is van SOLK? In welk stadium van het proces bevonden de patiënten zich?

Niveau 1

Bij 50-75% van de mensen met SOLK in een eerstelijnspopulatie nemen de klachten af in de loop van het eerste jaar tot vijftien maanden. Bij 10-30% nemen de klachten echter toe in de tijd.

De auteurs van een meta-analyse identificeerden na een uitgebreide search slechts 6 onderzoeken naar de prognose van SOLK in een brede patiëntenpopulatie, dat wil zeggen een populatie die overeenkomt met een eerstelijnspopulatie (Olde Hartman e.a., in druk). Uit deze onderzoeken blijkt dat bij 50 tot 75% van de patiënten de klachten verbeteren tijdens de follow-upperiode die varieert van 6 tot 15 maanden. 10-30% van de patiënten verslechtert echter tijdens de follow-upperiode. Bij de meeste onderzoeken is het onduidelijk in hoeverre er sprake is geweest van behandeling. Uit deze 6 onderzoeken blijkt dat een groter aantal symptomen bij de eerste meting een slechtere prognose heeft. Dat geldt ook voor de ernst van de symptomen; hoe ernstiger de klachten bij de eerste meting hoe groter de kans dat de klachten niet verdwijnen. Bij psychiatrische comorbiditeit bleek er sprake van tegengestelde effecten, daar is op basis van deze onderzoeken geen uitspraak over te doen.

De multidisciplinaire richtlijn Somatisch Onvoldoende Verklaarde Lichamelijke Klachten en Somatoforme Stoornissen is op initiatief en onder auspiciën van de Landelijke Stuur­groep Multidisciplinaire Richtlijnontwikkeling in de GGZ en de daaronder ressorterende Commissie Cliëntenparticipatie tot stand gebracht door de werkgroep Somatoforme Klachten en Stoornissen, waarin de deelnemende verenigingen en organisaties hebben samengewerkt.

Methodologische en organisatorische ondersteuning en begeleiding werden verzorgd door het Trimbos-instituut en het Kwaliteitsinstituut voor de Gezondheidszorg CBO.

Trimbos-instituut

Het Trimbos-instituut is het landelijk kennisinstituut voor de geestelijke gezondheids­zorg, verslavingszorg en maatschappelijke zorg en lichamelijke ziektes en legt zich toe op de ontwikkeling, verspreiding, toepassing en evaluatie van kennis. De centrale vraag is op welke manieren deze kennis kan worden ingezet om de geestelijke gezondheid en de kwaliteit van leven van de Nederlandse bevolking te verbeteren.

Het Trimbos-instituut werkt hierbij nauw samen met partners in de wetenschap, praktijk en politiek.

Kwaliteitsinstituut voor de Gezondheidszorg CBO

Het Kwaliteitsinstituut voor de Gezondheidszorg CBO, gevestigd in Utrecht, heeft tot doel de individuele beroepsbeoefenaren, hun beroepsverenigingen en zorginstellingen te ondersteunen bij het verbeteren van de patiëntenzorg. Sinds zijn oprichting in 1979 heeft het CBO zich ontwikkeld tot een toonaangevend en ook internationaal erkend instituut dat via programma's en projecten ondersteuning en begeleiding geeft bij systematisch en gestructureerd meten, verbeteren, herontwerpen en borgen van kwaliteit van de patiëntenzorg.

 

 

 

Doel en doelgroep

De richtlijn is bedoeld voor alle hulpverleners die betrokken zijn bij patiënten/cliënten- zorg aan mensen met SOLK en somatoforme stoornissen in Nederland. Met de richtlijn beogen we houvast te bieden bij bejegening, herkenning, diagnostiek, voorkómen van verergering, behandeling, verwijzing en begeleiding van mensen met SOLK en somatoforme stoornissen. Patiënten kunnen de richtlijn gebruiken bij het overleg met hulpverleners over passende diagnostiek en behandeling.

De richtlijn biedt aanknopingspunten voor hiervan afgeleide richtlijnen per discipline en voor zorgorganisatie, bijvoorbeeld transmurale afspraken en locale protocollen. Daar­naast geeft de richtlijn kennislacunes aan en doet aanbevelingen voor wetenschappelijk onderzoek. Ten slotte bevat de richtlijn aanbevelingen voor implementatie.

Implementatie

De richtlijn wordt verspreid onder de leden van de aan de ontwikkeling van de richtlijn deelnemende wetenschappelijke verenigingen en beroepsverenigingen. Daarnaast wordt een samenvatting van de richtlijn gepubliceerd in relevante tijdschriften.

Werkwijze

Deze paragraaf is opgesteld aan de hand van het ‘Appraisal of Guidelines for Research & Evaluation' (AGREE) instrument. Dit instrument is in een Europees verband opgesteld om de kwaliteit van richtlijnen te kunnen beoordelen. Door de aspecten van AGREE te verwerken in de inleiding van de richtlijn, wordt duidelijk aan welke kwaliteitseisen is voldaan.

 

De werkgroep werkte gedurende twee en een half jaar (14 vergaderingen) aan de totstandkoming van de richtlijn.

De werkgroep heeft uitgangsvragen geformuleerd die gebaseerd zijn op knelpunten die door professionals in de zorg worden ervaren. De werkgroep heeft door middel van systematisch literatuuronderzoek onderzoeksgegevens verzameld ter beantwoording van de uitgangsvragen. Dit leverde een zeer grote hoeveelheid potentieel relevante onderzoeken. De beperkte tijd die voor het maken van deze richtlijn stond, dwong de werkgroep prioriteiten te stellen. Vervolgens schreven de werkgroepleden een paragraaf of hoofdstuk waarin de beoordeelde literatuur werd verwerkt. Tijdens vergaderingen lichtten zij hun teksten toe, dachten mee en discussieerden over andere hoofdstukken.

Een redactiecommissie heeft de teksten uiteindelijk tot een samenhangend geheel geredigeerd.

De richtlijn heeft van december 2008 tot maart 2009 op de website van het Trimbos-instituut en het CBO gestaan om beroepsverenigingen en professionals in de gelegenheid te stellen commentaar te leveren. De commentaren zijn door de werkgroep beoordeeld en waar van toepassing in de richtlijn verwerkt.

 

Wetenschappelijke onderbouwing

De richtlijn is voor zover mogelijk gebaseerd op bewijs uit gepubliceerd wetenschappe­lijk onderzoek. We zochten in de databases van Medline (PubMed), Embase, PsycINFO en Cochrane naar publicaties in het Nederlands, Engels en Duits. Indien we een erg

groot aantal treffers vonden, hebben we ons beperkt tot publicaties die tussen 1997 en 2007 zijn verschenen. Voor de gehanteerde zoektermen verwijzen we naar bijlage II: Systematische literatuursearch. Tevens verkregen we artikelen uit referentielijsten van opgevraagde literatuur. Daarnaast raadpleegden we ook andere richtlijnen voor soma- tofome stoornissen en SOLK. De werkgroepsleden hebben de geselecteerde artikelen beoordeeld op kwaliteit van het onderzoek en ingedeeld naar mate van bewijs. De hierbij gebruikte indeling staat in tabel 2.

 

Format

De beoordeling van de verschillende artikelen is in de verschillende teksten terug te vinden onder het kopje ‘Wetenschappelijke onderbouwing'. Het wetenschappelijke bewijs is vervolgens kort samengevat in een conclusie, met daarbij een niveau van bewijs (zie tabel 1 en 2). Om te komen tot een aanbeveling zijn er naast het wetenschappelijk bewijs nog andere aspecten van belang, zoals patiëntenvoorkeuren, kosten, en beschik­baarheid (in verschillende echelons). Deze aspecten worden vermeld onder het kopje ‘Overige overwegingen'. Deze overwegingen, geformuleerd door werkgroepleden, zijn dus niet gebaseerd op wetenschappelijk onderzoek. De aanbevelingen zijn het resultaat van het beschikbare bewijs en de overige overwegingen.

Deze classificatie is alleen van toepassing in situaties waarin om ethische of andere redenen gecontroleerde trials niet mogelijk zijn. Zijn die wel mogelijk dan geldt de classificatie voor interventies.

 

Afbakening in de richtlijn

In de richtlijn ligt de nadruk op algemene principes van herkenning, bejegening, diagnos­tiek en behandeling van patiënten met SOLK en somatoforme stoornissen. De richtlijn is uitdrukkelijk van toepasssing voor alle SOLK en somatoforme stoornissen.

 

De voornaamste functie van de richtlijn is het bieden van een ‘moederrichtlijn' waarin alle hulpverleners het generieke beleid kunnen vinden voor SOLK of somatoforme stoornis. Iedere discipline kan deze moederrichtlijn gebruiken om van daaruit haar eigen disciplinespecifieke richtlijn te specificeren.

De werkgroep heeft derhalve besloten de aandacht voor klachtspecifieke aspecten beperkt te houden. Immers, voor klachtspecifieke diagnostiek en beleid bestaan al veel klachtspecifieke richtlijnen, zoals de huisartsgeneeskundige NHG-standaarden, de verzekeringsgeneeskundige protocollen, de interdisciplinaire CBO-richtlijnen en Gezondheidsraadrapporten en de Landelijke Eerstelijns Samenwerkings Afspraken (LESA's). Voor CVS en fibromyalgie is een multidisciplinaire richtlijn in ontwikkeling. Derhalve zal dat hier niet worden behandeld. Ook is besloten om de chronische pijn niet uitgebreid aan bod te laten komen. De werkgroep adviseert om voor chronische pijn een aparte richtlijn te ontwikkelen.Hypochondrie is eerder beschreven in de MDR angststoornissen. We hebben echter besloten deze stoornis toch op te nemen in de richtlijn, omdat hypochondrie in de DSM gerekend wordt tot de somatoforme stoor­nissen. Bovendien zijn er sinds het verschijnen van de richtlijn angststoornissen nieuwe studies verschenen die voor ons reden waren tot aanpassing van de aanbevelingen.

De richtlijn gaat niet over SOLK en somatoforme stoornissen bij kinderen. Deze problematiek, met de grote invloed van ontwikkelingsfasen en de rol van ouders en andere opvoeders, wettigt ons inziens een aparte richtlijn.

Bij het zoeken naar literatuur is besloten om primair te zoeken op de algemene term ‘lichamelijk onverklaarde klacht' (en zijn vele varianten), op de namen van functionele syndromen (zoals prikkelbaredarmsyndroom, en niet-cardiale thoraxpijn) en op DSM-IV somatoforme stoornissen.

Tot slot heeft de werkgroep ervoor gekozen in het hoofdstuk behandeling voor één groep klachten, namelijk buikklachten, de klachtspecifieke aspecten wél uit te werken. Aan de hand van enkele gynaecologische pijnsyndromen en het prikkelba- redarmsyndroom wordt de rol van verschillende klachtspecifieke aspecten bij buikpijn geïllustreerd.

 

Opzet van de richtlijn

Keuzes

Hoewel de werkgroep ervan uitgaat dat de richtlijn zo veel mogelijk gebaseerd moet zijn op resultaten van wetenschappelijk onderzoek, bleek gaandeweg dat de grote hoeveel­heid onderzoeksgegevens slechts zeer fragmentarische kennis representeert. Enkele voorbeelden: over werkgerelateerde factoren bij rugpijn bestaat veel onderzoek, terwijl over de rol van de factor werk bij SOLK en somatoforme stoornissen in het algemeen heel weinig bekend is; kortetermijneffecten van antidepressiva worden vaak onderzocht, terwijl relevanter langetermijnonderzoek ontbreekt; naar bepaalde kruiden is verrassend veel onderzoek gedaan; over organisatie van zorg (stepped care, disease-management, effectiviteit van dezelfde therapie uitgevoerd door verschillende disciplines of in verschil­lende settings) bestaat vrijwel geen onderzoek. De meeste uitgangsvragen bleken niet of slechts zeer gedeeltelijk op basis van bestaand onderzoek te beantwoorden. De werk­groep heeft daarom op veel punten haar eigen praktische expertise laten spreken.

Ter wille van de transparantie van de richtlijn noemen we hier enkele belangrijke keuzes die de werkgroep gemaakt heeft op basis van haar eigen ervaring en consensus:

  • De werkgroep beschouwt SOLK en somatoforme stoornissen als een continuüm, lopend van enerzijds een enkelvoudige lichamelijk onvoldoende verklaarde klacht die nog niet al te lang bestaat en weinig functionele beperkingen met zich meebrengt, via langdurige SOLK met functionele beperkingen en vaak meerdere klachten naar soma­toforme stoornissen die voldoen aan de DSM-criteria, met als ernstigste de somatisa- tiestoornis. Daarbij zien we SOLK vooral als het gevolg van gebrekkige communicatie en verschillen in klachtinterpretatie tussen arts en patiënt, terwijl bij somatoforme stoornissen de rol van psychische stoornissen en aangeboren kwetsbaarheid bij de patiënt toeneemt. Dit leidt tot aanbevelingen over communicatietraining voor alle hulpverleners.
  • De werkgroep ziet een consequent tweesporenbeleid in de eerste lijn als de basis van het beleid bij patiënten met SOLK en somatoforme stoornissen.
  • De werkgroep hanteert stepped care als basisprincipe voor de organisatie van zorg. Dit houdt in dat begonnen wordt met de lichtste vorm van behandeling, en dat alle patiënten die hierop niet verbeteren een tweede en zo nodig derde stap aangeboden krijgen. Het is nog onduidelijk of patiënten met een complexe lang bestaande soma- toforme stoornis met grote belemmeringen in het dagelijks leven er baat bij hebben de eerste stappen over te slaan en direct een hooggespecialiseerde behandeling aangeboden te krijgen.
  • De werkgroep benadrukt het belang van tijdige herkenning en adequate hulp bij SOLK en somatoforme stoornissen. De werkgroep beschouwt somatoforme stoor­nissen, inclusief de somatisatiestoornis, als goed behandelbare aandoeningen, waarbij nog niet te zeggen is welk percentage van de patiënten chronisch last van hun soma- toforme stoornissen zullen houden als zij meer dan nu tijdig adequate hulp krijgen.
  • Werken met patientprofielen die het risico op iatrogene schade dan wel een chronisch beloop aangeven biedt de mogelijkheid om mensen die een hoog risico hebben op ongunstig beloop te identificeren en adequaat te behandelen en begeleiden, door de patient op geleide van diens profiel ergens in de stepped care behandeling in te laten stappen.

Bovenstaande keuzes vormen, naast de wetenschappelijke literatuur, de peilers onder de opzet van de richtlijn.

 

De indeling van de richtlijn

De indeling van de hoofdstukken volgt de uitgangsvragen.

 

Slotopmerking

Daar waar de werkgroep in deze richtlijn ‘hij' schrijft, wordt ook ‘zij' bedoeld.

Daar waar de richtlijn ‘patiënt' schrijft, erkent de werkgroep dat de term ‘cliënt' in de geestelijke gezondheidszorg veel wordt gebruikt. Omwille van de eenheid binnen deze richtlijn en binnen alle GGZ-richtlijnen is echter de term patiënt aangehouden.
