Uitgangsvraag

Preoperatief betreft:

    1. Houding en beweeg adviezen
    2. Psychologische adviezen
    3. Leefstijl adviezen/fysieke functie
    4. Preoperatieve oefentherapie

Aanbeveling

Preoperatieve adviezen

Gelijkheid ((health) equity/equitable)

Rationale aanbevelingen preoperatieve adviezen

Eindoordeel:

Sterke aanbeveling voor houding- en beweegadviezen

Zwakke aanbeveling voor psychologische adviezen

Sterke aanbeveling voor leefstijladviezen

Rationale aanbevelingen preoperatieve oefentherapie

Eindoordeel:

Zwakke aanbeveling voor preoperatieve oefentherapie

Onderbouwing

The working group aims to develop a general module for postoperative physical therapy, applicable to patients undergoing non-instrumented spinal surgery. Several modules on physical therapy already exist, including two modules: "Postoperative Physical Therapy for HNP" and "Postoperative Physical Therapy for Stenosis," which address questions that could be better consolidated into a single module. Spinal surgery is excluded from this module, as the postoperative care recommendations for these patients are fundamentally different.

The existing recommendations are accurate but lack sufficient detail and are not concrete enough to be effectively followed, resulting in significant variations in practice. For instance, physical therapy is often inadequately tailored to the patient's cognitive functioning or learning strategy, and posture advice is not addressed. Furthermore, the recommendations lack information on indications, and new information on preoperative advice is not incorporated.

Many patients underestimate the negative effects that surgery can have on daily life due to a lack of education. This includes managing expectations around spine-sparing movements and practicing activities such as getting in and out of bed correctly.

For this reason, the working group wishes to answer the question whether preoperative advices (including psychological, lifestyle and posture advice) and preoperative exercise therapy should be added to the postoperative advices which are already the standard of care in the Netherlands. Currently, only patients who happen to see a physiotherapist preoperatively (coincidentally, such as patients who already receive therapy before surgery) receive this guidance.

In this module, the working group wants to look beyond physiotherapy, also at other advice and therapies.

Description of studies

One study was included in the analysis of the literature. Important study characteristics and results are summarized in table 2. The assessment of the risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).

The systematic review of Punnoose (2023) contained five RCTs reporting on preoperative advice versus no preoperative advice in lumbar surgery. However, two of these RCTs (Lotzke, 2019; Rolving, 2015) included patients receiving cognitive behavioral therapy and more than 90% of the participants underwent instrumented surgery or fusion. These trials were therefore no match with the predefined PICO (both population- and intervention-wise) and were excluded from analysis. The study by Nielsen (2010) did only report medians (with interquartile ranges), which could not be evaluated using the GRADE framework.

The outcomes of the trial by Feninets (2022) was then added to the evidence from Punnoose (2023).

Function was reported by two studies (Lindbäck, 2018; Marchand, 2021). Both studies measured functioning using the Oswestry Disability Index (ODI, range 0-100 with higher scores indicating lower functioning). The results are presented in table 5.

3. Quality of life (important)

Health related quality of life was reported by one study (Lindbäck, 2018) using the EQ5D at 3 months after surgery (score range from −0.59 to 1, with higher scores indicating higher health utility). Mean score in the preoperative exercise group was 0.64 (SD 0.26), and mean score in the usual care group was 0.64 (SD 0.25). Mean difference was 0.00 (95% CI -0.28 to 0.28), indicating no clinically relevant difference between both groups.

4. Return to work; 5. confidence in body (important)

None of the included studies reported on return to work or confidence in body.

A systematic review of the literature was performed to answer the following question(s):

Table 1. PICO

Patients Patients undergoing non-instrumental spinal surgery
Intervention Preoperative advice: training instructions, posture and/or activities of daily living (ADL), exercise therapy, physiotherapy
Control No preoperative advice
Outcomes Pain (back and legs), ADL functioning, social functioning, return to work, confidence in body, quality of life
Other selection criteria Study design: systematic reviews and randomized controlled trials

Relevant outcome measures

The guideline panel considered pain (back and legs) and functioning: (ADL, social) as critical outcomes measure for decision making; and return to work, confidence in body and quality of life as important outcome measures for decision making.

A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.

For determining the Minimal Clinically Important Difference (MCID), the article by Ostelo (2008) was used, where an improvement of 20% to 30% compared to the baseline measurement is considered the minimal clinically relevant difference (within-group change). This roughly corresponds to a difference of 2 points on the VAS scale for pain or general health scores (0 to 10), 2 points on the NRS scale (0 to 10), 5 points on the Roland Disability Questionnaire (0 to 24), and 15 points on the Oswestry Disability Questionnaire (0 to 100). The working group applies the same value as the clinically relevant difference between groups (between-group change). When different types of outcome measures are pooled and a standardized mean difference is calculated, a difference of 0.5 is considered clinically relevant.

Search and select (Methods)

The databases Medline (via OVID) and Embase (via Embase.com) were searched with relevant search terms from 2000 until February 16th, 2024. The detailed search strategy is available upon request. The systematic literature search resulted in 622 hits. Studies were selected based on the following criteria:

  • Systematic reviews (searched in at least two databases, detailed search strategy, risk of bias assessment and results of individual studies available), or randomized controlled trials (RCTs).
  • Studies including twenty or more participants (³ ten per arm).
  • Studies according to the PICO and
  • Studies with English language full-text publication.

Initially, eight studies were selected based on title and abstract screening. After reading the full text, seven studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and one systematic review (Punnoose, 2023) was included. The systematic search by Punnoose (2023) was carried out in 2022. Since then, one new RCT was published which is also included in the summary of literature (Feninets, 2022).

Risk of Bias tables

Risk of bias table for intervention studies (randomized controlled trials; based on Cochrane risk of bias tool and suggestions by the CLARITY Group at McMaster University)

Study reference

Was the allocation sequence adequately generated?

Was the allocation adequately concealed?

Blinding: Was knowledge of the allocated interventions adequately prevented?

Was loss to follow-up (missing outcome data) infrequent?

Are reports of the study free of selective outcome reporting?

Was the study apparently free of other problems that could put it at a risk of bias?

Overall risk of bias

Feninets, 2022

Probably no;

Reason: No, allocation was based on odd/even days.

Probably no;

Reason: Due to the aforementioned reason, randomization based on dates can be predictable for both healthcare-providers and patients.

Probably yes;

Reason: the article states that patients are blinded. Blinding of other study personnel was not assumed both due to the nature of the intervention, and because the article did not state any blinding apart from patients.

Probably yes;

Reason: The study does not state any missing follow-up data.

Definitely yes;

Reason: All primary outcome measures were reported.

Probably yes;

Reason: No additional Risk of Bias was signalled.

Some concerns (all outcomes)

Lindbäck, 2017

Probably yes

Block randomization was used

Definitely yes;

Reason: allocation concealment using sealed opaque envelopes

Probably no

Reason: patients and healthcare providers were not blinded. Outcome assessor was blinded.

Probably yes

Reason: questionnaires were lost at different time points during follow-up. Imbalanced LTFU at first timepoint (19 lost questionnaires in physiotherapy group, 9 in the waiting-list group)

Definitely yes

Reason: VAS, EQ-5D, HADS, SES, FABQ-PA and PGIC were reported in the method paragraph. All were reported

Probably yes;

Reason: No additional Risk of Bias was signalled.

Some concerns (all outcomes)

Marchand, 2022

Definitely yes

Reason: Using a computer random number generator

Definitely yes;

Reason: sequentially numbered, opaque and sealed envelopes were used

Probably no

Reason:

Patients were not blinded to allocation. Outcome assessor was not blinded.

Probably not

Reason: Post-intervention, drop-out rate was 19%. This increased to 36.7% at the 6-month follow-up. LTFU seemed not imbalanced.

Probably not

Reason: NRS, ODI, EQ-5D, Tampa Scale, Beck Disability index, PGIC, Sorensen test.  EQ-5D and PGIC scores however were not reported per group with confidence intervals.

Probably yes;

Reason: No additional Risk of Bias was signalled.

Some concerns (all outcomes)

Werkwijze
