Uitgangsvraag

Welk prognostisch instrument heeft de voorkeur voor het bepalen van de mate van inflammatie als er sprake is van een verhoogd risico op ondervoeding bij ouderen met een kwetsbare gezondheid in het ziekenhuis om de voedingstoestand te verbeteren?

Aanbeveling

Gebruik geen marker van inflammatie om de mate van ondervoeding te voorspellen bij ouderen met een kwetsbare gezondheid die in het ziekenhuis worden opgenomen.

 

Bepaal de waarschijnlijkheid en/of mate van inflammatie door te beoordelen op de aanwezigheid van aandoeningen geassocieerd met matige tot ernstige inflammatie.1 Indien beschikbaar kan de waarde van het CRP gebruikt worden om te kijken naar de aanwezigheid van inflammatie, gebruikmakend van de GLIM-criteria.2

Overwegingen

Balans tussen gewenste en ongewenste effecten

Voor de impactvraag over het effect van het gebruik van een prognostisch instrument met inflammatiemetingen op het maken van klinische beslissingen bij ouderen met kwetsbare gezondheid met een risico op ondervoeding, werden geen studies geïncludeerd in de literatuuranalyse. Voor de vraag over de predictieve waarde (discriminatie en overall performance) van inflammatie metingen om ondervoeding vast te stellen bij ouderen met een kwetsbare gezondheid met een risico op ondervoeding, werden 2 studies geïncludeerd. Deze studies voerden geen interne en/of externe validatie uit van het model, waardoor geen GRADE beoordeling kon worden uitgevoerd.

 

De performance van een regressiemodel met als uitkomsten gemiddelde dagelijkse energie- en eiwit-inname, waarin de CRP/albumine ratio, bovenarmomtrek en moeite met zelfstandig eten worden meegenomen, kan in de studie van Sanson (2018) worden beoordeeld als substantieel. Het model verklaart respectievelijk 43% en 44% van de variantie in de uitkomsten voor gemiddelde dagelijkse energie inname en gemiddelde dagelijkse eiwit inname. Het discriminerend vermogen van de CRP/albumine ratio ligt in de studie van Kaya (2024) net onder de klinisch relevante grens, met een area under the curve (AUC) van 0.619. De AUC van een klinisch acceptabel discriminerend vermogen ligt tussen de 0.7 en 0.8. Bij een AUC van 0.5 is er geen sprake van discriminatie.

 

Kwaliteit van bewijs

De overall kwaliteit van bewijs kon niet worden vastgesteld, omdat er geen studies zijn gevonden die het effect van het gebruik van een prognostisch instrument voor ondervoeding met CRP, albumine, ferritine en leukocytenaantal om klinische beslissingen te maken bij ouderen met een kwetsbare gezondheid onderzochten. Dit betekent dat we zeer onzeker zijn over het geschatte effect van het gebruik van deze instrumenten op de patiëntrelevante uitkomstmaten.

 

Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)

Patiënten (en eventueel naasten) zullen over het algemeen de voorkeur geven aan een zo min mogelijk belastend en pijnlijk onderzoek voor het meten van de mate van inflammatie. Wanneer een inschatting van de mate van inflammatie gemaakt moet worden dan kan hiervoor gebruik gemaakt worden van de CRP concentratie. Deze waarden zullen veelal beschikbaar zijn bij patiënten die worden geanalyseerd voor de aanwezigheid van ondervoeding of kan tijdens dezelfde bloedafname toegevoegd worden aan de bepaling. De inflammatie waarden zelf zijn onvoldoende predictief om de aanwezigheid van ondervoeding aan te tonen dan wel uit te sluiten.

 

Kostenaspecten

Het inzetten van de CRP concentratie als predictief instrument wordt niet ondersteund door de gevonden literatuur. Het alleen bepalen van de inflammatie waarden voor deze doeleinden is derhalve niet kosteneffectief. In de praktijk zullen deze waarden echter veelal bekend zijn omdat ze vaak gebruikt worden in de klinische workup van een patiënt met (het vermoeden op) ondervoeding en/of bij in instellen van een behandeling of het monitoren van de therapie effect.

 

Aanvaardbaarheid, haalbaarheid en implementatie

Er is geen bewijs gevonden voor de voorspellende waarde van het gebruik van markers voor inflammatie bij oudere patiënten met ondervoeding. Het gebruik van de markers voor inflammatie wordt om deze reden niet aanbevolen. Veelal zal een CRP waarde en/of albumineconcentratie echter wel al beschikbaar zijn vanuit de klinische analyse van een patiënt, vanuit het instellen van een therapie of het monitoren van het therapeutisch effect.

 

Rationale van de aanbeveling: weging van argumenten voor en tegen de indextest

Er is geen bewijs gevonden voor het gebruik van inflammatie waarden als prognostisch instrument bij patiënten met (een vermoeden op) ondervoeding door het ontbreken van studies en het ontbreken van interne en/of externe validatie.

 

Inflammatie parameters zullen vaak wel beschikbaar zijn in etiologische analyse volgens de GLIM criteria. Vanuit de GLIM criteria wordt geadviseerd bij acute ziekte bij een CRP waarde van 10-50 mg/l te spreken van matig ernstige inflammatie en bij een CRP waarde van >50 mg/l te spreken van ernstige inflammatie. Bij chronische inflammatie spreekt men bij een CRP waarde van 3.0-9.9 mg/l van milde inflammatie en bij een CRP waarde van 10-50 mg/l van matig ernstige inflammatie. De werkgroep neemt dit advies over.

Onderbouwing

Inflammation plays a crucial role in the pathophysiology of malnutrition. It is a key driver of disease-related malnutrition, and can lead to anorexia, reduced food intake, muscle catabolism and anabolic resistance. Furthermore, the Global Leadership Initiative on Malnutrition (GLIM) identified inflammation as an etiologic criterium of malnutrition.

 

Correct identification and/or measurement of inflammation status is crucial for both the identification of individuals at risk of malnutrition and for the optimization of the treatment plan. To date a variety of approaches can be used for the assessment of inflammation status. The likelihood of the presence of inflammation can be estimated based on the medical history where inflammation is considered likely when an inflammatory disease is present. In addition, laboratory markers can also be used to measure the inflammation burden. Frequently used markers are C-reactive protein (CRP), leukocyte count, ferritin, erythrocyte sedimentation rate and albumin.

 

To date it is not clear in which patient laboratory measurement of inflammation is necessary and if a measurement is performed, which measurement best reflects inflammation status in frail elderly with increased risk of malnutrition.

Description of studies

No studies were included in the analysis of the literature for search question 1.

 

Two studies were included in the analysis of the literature for search question 2 about the performance of a prognostic instrument. Important study characteristics and results are summarized in table 3. The assessment of the risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).

 

Kaya (2024) performed a cross-sectional study including 154 geriatric patients (age ≥65 years). Patients were admitted to a training and research hospital between May 2022 and August 2022. The authors used the Mini Nutritional Assessment (MNA) to determine nutritional status and divided patients into two groups: normal nutrition and malnourished/at risk of malnutrition. The predictive model investigated in this study was the CRP to albumine ratio (CAR). The model was not validated (internally or externally) in this study.

 

Sanson (2018) performed a prospective observational longitudinal study to identify predictive indicators of reduced food intake and poor clinical outcomes in 81 older patients. Patients had to be >65 years of age and had to be malnourished or at risk of malnutrition. The following other inclusion criteria were used: Nutritional Risk Score (NRS 2002) of ≥ 3, middle-upper arm circumference (MUAC) <23.5 cm, or impaired ability to perform/complete self-feeding. Patients were excluded if artificial nutritional support was needed any time during the study or if end-of-life care was needed. Recruitment took place between July 1st and October 31st 2015. The minimum required samples size was calculated a priori to be 80 patients based on a 10% difference between energy intake of the studied population and energy requirements of a similar at-risk population. This study identified the CRP/albumin ratio, MUAC, and impaired self-feeding as predictive indicators. The predictive indicators were not validated (internally or externally) in the study.

Kaya (2024) reported CRP/albumin ratio (CAR) in 154 geriatric patients. The Mini Nutritional Assessment (MNA) was used to determine nutritional status. Patients in the malnourishment/at risk of malnutrition group had a significantly higher CAR (n=62) compared to patients in the normal nutritional status group (n=92). A negative correlation of r = -0.196 (p = 0.015) between MNA score and CAR was found. The ROC curve indicated CAR as a significant predictor of malnourishment status, with an area under the ROC curve of 0.619, which is just below acceptable (see table 4).

 

Overall performance (R2)

Sanson (2018) reported middle-upper arm circumference (MUAC), CAR, and impaired self-feeding as indicators of impaired energy and protein intake. The final regression models could explain 43% of variance in mean daily energy intake (MEI) (R2 0.430), and 44% in mean daily protein intake (MPI)(R2 0.440). For both the MEI and MPI the performance of the final regression model, including MUAC, CAR and impaired self-feeding was considered substantial (R2≥0.26) (see table 4).

Level of evidence of the literature

The level of evidence was not graded, because none of the included studies reported validated models.

When reviewing literature, there is a hierarchy in quality of individual prognostic studies. Preferably, the effectiveness of the use of a prognostic instrument (e.g., prediction model) is evaluated in a clinical trial. Unfortunately, these studies are very rare. If not available, studies in which prediction models are developed and validated in other samples of the target population (external validation) are preferred as there is more confidence in the results of these studies compared to studies that are not externally validated. Most samples do not completely reflect the characteristics of the total population, resulting in deviated associations, possibly having consequences for conclusions. Studies validating prediction models internally (e.g., bootstrapping or cross validation) can be used to answer the first research question as well, but downgrading the level of evidence is obvious due to risk of bias and/or indirectness as it is not clear whether models perform sufficiently in target populations. The confidence in the results of unvalidated prediction models is very low. Therefore, such models will not be graded. Not grading is also applicable for prognostic factor research. The risk factors identified from such models can be used to inform patients about the risk of malnutrition; however, they are less suitable to be used in clinical decision making as the predictive value of these factors is unclear.

 

A systematic review of the literature was performed to answer the following question(s):

  1. What are the favourable and unfavourable effects of the use of prognostic instrument with CRP, albumin, ferritin and leukocyte count compared with the use of standard care to make clinical decisions for patients at risk of malnutrition?
  2. What is the predictive value of CRP, albumin, ferritin and leukocyte tests for measuring inflammation to assess malnutrition frail older people in the hospital?

A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all. Both databases were searched from 2005 to May 9th, 2025 for systematic reviews and RCTs. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) CRP, albumin, ferritin, leukocyte count; (2) inflammation; (3) malnutrition; (4) elderly. Duplicates were removed using EndNote software. After deduplication a total of 753 records were imported for title/abstract screening.

 

Initially, 9 studies were selected based on title and abstract screening. After reading the full text, all 9 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and no studies were included.

Er werd aandacht besteed aan het patiëntenperspectief door de Patientenfederatie Nederland en de KBO-PCOB uit te nodigen voor de schriftelijke knelpunteninventarisatie, maar beide hebben geen input gegeven. Daarnaast is de KBO-PCOB uitgenodigd voor de werkgroep, maar heeft afgezien van deelname. De conceptrichtlijn is tevens voor commentaar voorgelegd aan de Patientenfederatie Nederland en de KBO-PCOB en de eventueel aangeleverde commentaren zijn bekeken en verwerkt.

 

Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz

Bij de richtlijnmodule is conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uitgevoerd om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema op de Richtlijnendatabase).
