Uitgangsvraag

Hoe ziet de diagnostiek van respiratoire complicaties bij patiënten met myotone dystrofie eruit?

Aanbeveling

Verricht de volgende diagnostiek om respiratoire complicaties te onderzoeken:

  • verricht bij elke MD1 patiënt tenminste eenmaal een spirometrie als uitgangswaarde.
  • neem jaarlijks de Respicheck vragenlijst af (hoofdbehandelaar)
  • verwijs naar longarts op basis van Respicheck vragenlijst of noodzaak voor aanvullende diagnostiek bij verdenking op hoestinsufficëntie en/of hypoventilatie.
  • longarts vraagt aanvullend onderzoek aan met spirometrie (zit/lig), evt. respiratoire spierkrachtmeting, bloedgasanalyse en poly(somno)grafie.
  • doorverwijzing naar een Centrum van Thuisbeademing bij (vermoeden op) hypoventilatie of verminderde hoestkracht (met respiratoire infecties) of wens tot voorlichting over beademing. 

Overwegingen

Voor- en nadelen van de interventie en de kwaliteit van het bewijs

Er werden geen studies gevonden die voldeden aan de kwaliteitseisen van diagnostische studies en de bewijskracht is daardoor zeer laag en niet met GRADE beoordeeld. Factoren die geassocieerd waren met respiratoire complicaties werden wel beschreven in de tekst.

 

Spirometrie is momenteel de meest gebruikte methode om te screenen op pulmonale achteruitgang, maar de gemiddelde jaarlijkse daling van 34±60mL (Thil et al. 2017) is relatief gering, en spirometrische metingen in zittende en liggende houding weerspiegelen niet altijd de aanwezigheid of ernst van respiratoir falen. Dit maakt een jaarlijkse standaardcontrole met spirometrie (zit/lig) bij de longarts weinig efficiënt, terwijl het wel belastend is voor zowel de patiënt als diens mantelzorgers. De afgelopen jaren zijn er nieuwe inzichten verkregen over screeningsvragenlijsten, zoals de Respicheck vragenlijst, die na vertaling en validatie door het MUMC+ binnenkort ook in het Nederlands beschikbaar zal zijn. Daarnaast zijn er meer voorspellende factoren geïdentificeerd die kunnen worden meegenomen in een risico-assessment voor respiratoire achteruitgang. Op basis van deze nieuwe inzichten stellen de auteurs een driestappenplan voor, waarbij eerst screeningsparameters worden beoordeeld door de hoofdbehandelaar, gevolgd door aanvullende respiratoire diagnostiek bij een longarts indien geïndiceerd, en ten slotte een beoordeling voor chronische thuisbeademing via een centrum voor thuisbeademing indien geïndiceerd.

 

Jaarlijkse screening door hoofdbehandelaar

Tijdens het jaarlijkse evaluatiemoment met de hoofdbehandelaar van MD1 (neuroloog/revalidatiearts) wordt aanbevolen om standaard de Respicheck vragenlijst af te nemen. Deze korte ja/nee vragenlijst kan binnen enkele minuten door de patiënt worden ingevuld en beoordeelt de negen belangrijkste respiratoire symptomen: orthopneu, dyspneu d’effort, slaapapneu, slechte slaap, ochtendhoofdpijn, afgenomen concentratie en aandacht, slaperigheid overdag, vermoeidheid en behandelde longinfecties. Een score ≥4 punten duidt op grote kans op respiratoire betrokkenheid en vormt een indicatie voor aanvullend onderzoek (de Mattia et al 2020).

 

Daarnaast moeten bij de beslissing om verder respiratoir onderzoek uit te voeren ook de volgende factoren worden meegewogen: een langere CTG-repeat, hogere leeftijd, verhoogde BMI, een MIRS score >3, de aanwezigheid van cardiale comorbiditeit, of een klinische verslechtering na een doorgemaakt cardiopulmonaal event.

 

De auteurs van deze richtlijn zijn van mening dat het de voorkeur verdient om bij elke patiënt met MD1 tenminste eenmaal vroegtijdig een longfunctie meting te doen om een uitgangssituatie vast te leggen. Indien een patiënt begeleid wordt door een fysiotherapeut met expertise op het gebied van neuromusculaire aandoeningen zoals MD1, kan er ook voor gekozen worden om een screenend longfunctie onderzoek te doen met een handheld spirometer: deze is laag belastend en kan gebruikt worden als screenings- of monitoringsinstrument (Dacanay 2013, Gao 2025).

 

Verwijzing naar longarts / centrum voor thuisbeademing (CTB)

Verwijzing naar de longarts voor aanvullend onderzoek moet laagdrempelig overwogen worden vanwege de hoge incidentie van respiratoire betrokkenheid met kans op overlijden bij MD1. De ervaring leert dat MD1 patiënten, in tegenstelling tot patiënten met andere neuromusculaire aandoeningen, zelf vaak weinig klachten ervaren of rapporteren. Desondanks kunnen er wel (ernstige) nachtelijke ademhalingsstoornissen of dysfunctie van de ademspieren bestaan. (Romigi et al., 2011).

 

Verwijzing naar een longarts voor verdere evaluatie van de longfunctie of het nachtelijk ademhalingspatroon is in ieder geval geïndiceerd bij:

  • Uitkomst Respicheck CAT score ≥ 4.
  • Recidiverende luchtweginfecties of aspiratie.
  • Indicatie voor aanvullende onderzoeken bij verdenking op slaapapneu, hypoventilatie en/of hoestinsufficiëntie.

Indien een patiënt begeleid wordt door een revalidatiearts met expertise op het gebied van neuromusculaire aandoeningen zoals MD1 en er lokale mogelijkheden zijn tot het verrichten van aanvullend onderzoek zoals longfunctie en bloedgasanalyse in het revalidatiecentrum kan er besloten worden om deze onderzoeken daar te verrichten waarbij laagdrempelig overleg wordt aanbevolen met het CTB.

 

Afhankelijk van de lokale afspraken en wensen van de patiënt kan een verwijzing naar een algemeen longarts of direct naar het CTB plaatsvinden.

 

Overzicht van onderzoeken

Na verwijzing kan de longarts de patiënt beoordelen aan de hand van de thema’s in de Respicheck vragenlijst en op longfunctioneel vlak. Hiervoor wordt gebruik gemaakt van:

  • spirometrie bij voorkeur in zowel zittende als liggende houding. Kenmerkend voor MD1 patiënten wordt een restrictief patroon gezien (FVC <75% bij Tiffeneau-index >70%). Een daling in vitale capaciteit van 20% in liggende houding is suggestief voor diafragma uitval.
  • Optioneel: Respiratoire spierkrachtmeting (MIP en MEP). Aangezien MIP en MEP slechts matig betrouwbare tests zijn worden ze niet routinematig aangeraden om als solitaire test in te zetten voor het meten van respiratoire spierkracht. Een normale MIP/MEP kan vrij goed respiratoire spierzwakte uitsluiten maar een lage waarde is geen betrouwbare bevestiging van respiratoire spierzwakte. MIP en MEP kunnen eventueel als aanvulling op spirometrie ingezet worden (Silveira, 2024).  
  • Arterieel of capillair bloedgas. Type 2 chronische respiratoire insufficiëntie (hypercapnie) is het meest voorkomende type van respiratoire insufficiëntie bij patiënten met een spierziekte zoals MD1. Invasief meten van de CO2 is de meest betrouwbare manier voor het vaststellen van hypoventilatie (op dat moment). Chronische beademing kan gestart worden bij de aanwezigheid van (nachtelijke) hypercapnie in combinatie met klachten van hypoventiliatie. Eventueel kan een nachtelijke transcutane CO2 meting plaatsvinden ter beoordeling van nachtelijke hypoventilatie (in geval van normocapnie in bloedgas).  

In geval van afwijkende bloedgas meting en/of longfunctie meting en er nog geen reden is om te verwijzen naar het CTB verdient het de aanbeveling om patiënten onder controle te blijven houden en regelmatig het onderzoek te herhalen.

 

Kanttekening: MD1 patiënten met een normale respiratoire spierkracht presenteren zich regelmatig toch met een hypercapnie. Dit is waarschijnlijk het gevolg van een verstoorde regulatie van de ademhaling vanuit het ademhalingscentrum. Het is discutabel of deze patiënten behandeld moeten worden met chronische beademing. Overleg met een centrum voor thuisbeademing of met een longarts van het expertisecentrum MD1 kan laagdrempelig overwogen worden.

 

  • Polygrafie / polysomnografie. Het wordt aanbevolen laagdrempelig een slaaponderzoek te verrichten om een evt (ernstig) slaapapneu vast te stellen. Bij MD1 patiënten is het vaak moeilijk om een onderscheid te maken of de excessieve vermoeidheid en slaperigheid een expressie zijn van een slaapgerelateerde ademhalingsstoornis of intrinsiek zijn aan MD1. De prevalentie van slaapapneu bij MD1 patiënten varieert tussen 16-75%. Pincherle et al., 2012; Kiyan et al., 2010 ). Ook de prevalentie van overmatige slaperigheid overdag is hoog, maar dit is niet altijd geassocieerd met het voorkomen van slaapapneu (Hoxhaj, 2024). Zonder het verrichten van een poly(somno)grafie is het niet te bepalen of de overmatige slaperigheid al dan niet een gevolg is van slaapapneu.

Beoordeling Centrum voor Thuisbeademing

Verwijzing naar een centrum voor thuisbeademing kan (direct) plaatsvinden bij:

  • Aanwijzingen voor hypoventilatie, zowel nachtelijk als overdag op basis van arterieel bloedgas of transcutane CO2 meting.
  • Afgenomen hoestkracht (al dan niet i.c.m. recidiverende respiratoire infecties).
  • Sterke afname in vitale capaciteit, en zeker indien VC <50%..
  • Aanwijzingen voor diafragma dysfunctie met 20% daling in vitale capaciteit van zittende naar liggende houding. 
  • Op verzoek van patiënt wanneer chronische beademing (evt. op  termijn) overwogen wordt (zie voor adressen www.vsca.nl).

Aanvullende onderzoeken in centrum voor thuisbeademing

  • Beoordeling hoestkracht. Spierzwakte van middenrif en hulpademhalingsspieren  kunnen leiden tot een hoestinsufficiëntie waardoor MD1 patiënten een verhoogd risico hebben op sputumstase, atelectase en terugkerende of fatale longontsteking. Bij een peak cough flow onder de 270L/min kunnen hoest bevorderende technieken ingezet worden zoals ‘air-stacking’ (Morrow, 2021). De centra voor thuisbeademing kunnen hierin faciliteren. Revalidatiecentra die hiermee bekend zijn, kunnen ook faciliteren bij het inzetten van hoest bevorderende technieken.
  • Nachtelijke transcutane CO2 meting is een betrouwbare manier om gedurende een hele nacht het CO2 gehalte op een non-invasieve wijze te monitoren en is met name een aanvulling op een bloedgasmeting overdag omdat het nachtelijke alveolaire hypoventilatie kan detecteren bij een normocapnisch bloedgas overdag. De aanwezigheid van nachtelijke hypoventilatie in combinatie met bijpassende klachten van vermoeidheid en slaperigheid vormt indicatie voor het starten van chronische beademing.

Behandeling

Behandeling van respiratoire betrokkenheid bij MD1 patiënten is geen onderdeel van deze module. Hiervoor wordt verwezen naar de laatste versie van de landelijke veldnorm voor chronische beademing (Vereniging samenwerkingsverband Chronische Ademhalingsondersteuning).

 

Waarden en voorkeuren van patiënten (en evt. hun verzorgers)

Patiënten geven de voorkeur aan onderzoeken die weinig belastend zijn en eenvoudig kunnen worden uitgevoerd tijdens een regulier polibezoek. De Respicheck-vragenlijst sluit goed aan bij deze wens, omdat deze snel in te vullen is en geen fysieke inspanning vereist. Jaarlijkse spirometrie wordt vaak als vermoeiend en tijdrovend ervaren, zeker bij patiënten met beperkte mobiliteit. Patiënten waarderen een stapsgewijze aanpak waarbij aanvullend onderzoek alleen wordt ingezet bij aanwijzingen voor respiratoire betrokkenheid. Heldere uitleg over het doel van de screening en gezamenlijke besluitvorming over vervolgonderzoek versterken de therapietrouw en tevredenheid. Bij deze patiëntengroep speelt ook dat cognitieve, frontale stoornissen er geregeld toe leiden dat de gegeven behandeling niet opgevolgd wordt. Door de initiatiefloosheid en het zwart/wit denken is de therapietrouw aan de chronische beademing bij veel patiënten matig/laag. Het is belangrijk dat het CTB/de longarts zich hiervan bewust is en de intensiteit van de begeleiding zo nodig aanpast.

 

Kosten (middelenbeslag)

Het gebruik van jaarlijkse spirometrie bij alle patiënten is relatief kostbaar en weinig doelmatig, gezien de beperkte opbrengst en de belasting voor patiënt en zorg. De inzet van de Respicheck-vragenlijst als eerste screeningsinstrument is daarentegen laagdrempelig en kostenefficiënt. Door aanvullend onderzoek alleen te verrichten bij patiënten met een verhoogd risico kan onnodige diagnostiek worden voorkomen en kunnen middelen gerichter worden ingezet.

 

Aanvaardbaarheid, haalbaarheid en implementatie

De voorgestelde aanpak met de Respicheck-vragenlijst als eerste screeningsinstrument is goed aanvaardbaar voor zowel patiënten als zorgverleners. De methode is eenvoudig, snel uitvoerbaar en vergt minimale training van professionals. Implementatie is goed haalbaar binnen bestaande zorgstructuren, bijvoorbeeld tijdens jaarlijkse controle bij de neuroloog of revalidatiearts. Succesvolle invoering vraagt om landelijke afstemming over gebruik van de vragenlijst en duidelijke afspraken over verwijzing naar de longarts of het centrum voor thuisbeademing.

 

Rationale van de aanbevelingen: weging van argumenten voor en tegen de diagnostische procedure

De kwaliteit van het bewijs is zeer laag; er zijn geen studies die voldoen aan de vereisten voor diagnostisch onderzoek. Jaarlijkse spirometrie blijkt weinig efficiënt en belastend, terwijl nieuwe screeningsmethoden, zoals de Respicheck-vragenlijst en risicofactorgebaseerde beoordeling, een praktischer en patiëntvriendelijker alternatief bieden. Daarom wordt een stapsgewijze benadering met gerichte verwijzing naar de longarts of het CTB aanbevolen.

Onderbouwing

Due to respiratory muscle weakness and central dysregulation of breathing, patients with myotonic dystrophy can develop chronic respiratory insufficiency. Additionally, these factors increase the risk of pneumonia, partly due to dysphagia, aspiration, and reduced cough strength. There is an increased prevalence of sleep apnea. Pneumonia and respiratory failure are most common causes of death, approximately in 50% of DM1 patients. Currently, diagnostic approaches vary across different centers and previous advices are based on expert opinion rather than evidence-based.

Based on the annual decline of the FVC, DM1 is slowly progressive with regard to respiratory impairment.

In contrast, during infections pulmonary function can deteriorate rapidly, resulting in respiratory failure.

Studies suggest that CTG repeat size, disease severity (MIRS score), BMI, and baseline pulmonary function and the presence of cardiac comorbidities are associated with respiratory decline in DM1.

Respicheck may be a useful tool for assessing respiratory impairment in DM1 patients, with a cut-off score of 4  optimizing sensitivity and specificity across studies when compared with standard respiratory tests.

Description of studies

Eight studies were included.

 

No systematic reviews or multifactorial cohort studies with internal or external validation were available. To give some direction retrospective cohort studies and cross-sectional studies are described here, see table 3 for an overview. Six studies were cohort studies that described factors associated with respiratory decline and two studies described the Respicheck questionnaire. Factors associated with respiratory decline were studied in several (retrospective) cohort studies. These factors were not tested by internal or external validation but are described here to give some direction.

 

Factors associated with respiratory decline were described in a prospective cohort of 283 patients that were followed for at least 5 years. These factors were higher number of CTG repeats, older age at first evaluation and higher baseline BMI (Boussaid 2018). Similar factors were mentioned by Hartog (2021) in a retrospective cohort study of 110 patients: greater CTG repeat size, higher MIRS rating, and longer disease duration were all correlated with lower baseline FVC but not with annual rate of change.

 

Rossi (2019) found several factors associated in univariate models: CTG expansion size in leukocytes, clinical muscle severity, most functional parameters of respiratory muscle involvement, presence of cardiac conduction disturbances,  pacemaker (PMK), exertion dyspnoea, obstructive sleep apnoea, and indication and compliance to NIV were all  significantly associated with restricted syndrome (Forced Vital Capacity <80% of predicted). In a multivariate model only CTG repeat and MIRS score remained. Vivekanada (2019) also found that MIRS score was a predictor of requirement of nocturnal ventilation in a cohort of 126 patients.

 

Lizio (2023) described that arterial blood gas analysis, spirometry, and nocturnal oximetry prediction models were suitable to distinguish people that needed nocturnal invasive ventilation from those who did not.

 

With regard to the natural course of respiratory decline, Thil (2017) described that the annual decline in FVC was -34 ± 60ml (-0.72 ± 1.7% predicted). Boussaid (2018) found an annual decline in VC of 1.8% ± 7.0% of predicted. The study of Hartog (2021) confirmed the slowly progressive annual decline of lung function in DM1 patients.

 

Respicheck

Two studies, De Mattia (2020) and Gutschmidt (2023), tested the Respicheck questionnaire to assess respiratory impairment in DM1 patients. The questionnaire consists of eight symptoms: orthopnea, dyspnea during daily activities, apnea, poor sleep, morning headache, decreased concentration and attention, daytime sleepiness, and fatigue (including treated chest infections). Patients respond with "yes" (score = 1) or "no" (score = 0) for each symptom.

 

De Mattia (2020) validated the Italian translation of Respicheck in 58 consecutive DM1 patients. Eligible participants were over 18 years old with adult-onset symptoms, while congenital and childhood-onset forms, as well as major unrelated comorbidities, were excluded. The mean total Respicheck score was 3.45 out of 8 (SD ±2.35). Higher scores correlated with more severe respiratory impairment, as measured by forced vital capacity (FVC) and respiratory muscle pressures (MIP, MEP), with an overall p-value of 0.01. Patients with worse respiratory health and quality of life (QoL) reported higher scores. The study compared Respicheck scores with respiratory test results, categorizing patients as having either no respiratory involvement or impairment requiring clinical intervention. A score of 5 was identified as the optimal cut-off, yielding a sensitivity of 90% and a negative predictive value of 77%. The area under the ROC curve (AUC) was 0.7214, as shown in Figure 2 of De Mattia (2020).

 

Gutschmidt (2023) applied the Respicheck questionnaire in 74 DM1 patients, who completed it before undergoing pulmonary function tests. The study found that the Respicheck SUM score significantly correlated with FVC % predicted (p = 0.013; r = −0.287). Similarly, the Respicheck CAT score correlated with FVC % predicted (p = 0.006; r = −0.314) and MIP % predicted (p = 0.046; r = −0.234). The study also calculated positive predictive value, sensitivity, and specificity for a Respicheck cut-off score of ≥4 compared to FVC, MIP, and MEP as percentages of predicted values. For FVC % predicted, the positive predictive value was 50.0%, sensitivity was 87.1%, and specificity was 37.2%. For MIP % predicted, the positive predictive value was 86.8%, sensitivity was 79.3%, and specificity was 53.3%. For MEP % predicted, the positive predictive value was 94.3%, sensitivity was 76.9%, and specificity was 62.5%, as summarized in Table 2.

Factors influencing respiratory dysfunction at baseline were

We examined various  parameters of respiratory functional status, and found FVC (% predicted) correlated  best with other measures of disease severity. Annual change in FVC was −1.42  (std error = 0.381). Greater CTG repeat size, higher MIRS rating, and longer disease  duration were all correlated with lower baseline FVC but not with annual rate of change.

22 (27.5%) had restricted respiration at inclusion, 32 (40%) were hypoxemic, and 19 (24%) were hypercapnic. The figures at final evaluation were 52 (65%), 50 (63%), and 19 (24%), respectively.

 

Annual declines in FVC (P < 0.01; b 5–30.27),  FEV1 (P < 0.04; b 5–1.06), and TLC (P < 0.03;  b 5–15.26) were positively associated with BMI.

Control: Pulmonary function assessments consisted of spirometry (forced vital capacity, FVC) and manometry (maximal inspiratory pressure, MIP; maximal expiratory pressure, MEP) in a sitting position, adjusted for sex and age, the FVC, MIP and MEP are presented in the predicted% of normal.

Cross-sectional, respicheck before pulmonary function assessments

Respicheck score:, it consists of nine symptom categories with three questions each, recording clinical symptoms that indicate respiratory involvement, resulting in 27 questions with dichotomous answering options (orthopnea, dyspnea when performing activities of daily living, poor sleep, morning headache, apnea, decreased cognitive performance, fatigue, excessive daytime sleepiness, treated chest infections).

 

And spirometry ( FVC) and manometry (MIP; MEP); presented in the predicted% of normal.

For DM1, the Respicheck SUM score correlated significantly with the results of the lung function assessment in FVC %pred ( p = 0.013;

r = −0.287), as well as the Respicheck CAT score with FVC %pred

( p = 0.006; r = −0.314) and MIP %pred ( p = 0.046; r = −0.234).

DM2 Respicheck SUM score significantly correlated with MEP %pred

( p = 0.032; r = −0.254).

Intervention: The Respicheck was assessed for reliability including internal consistency and reproducibility. The internal consistency of the questionnaire was assessed using Cronbach’s alpha.

Forty-four patients were asked to complete the Respicheck at baseline and after 3 weeks.

 

Control: Forced spirometry (FVC sitting and supine and the difference between these two measures) and respiratory muscle pressures (MIP, MEP) assessments were performed according to the American Thoracic Society (ATS) standards

Considering the analysis regarding the internal consistency of the Respicheck questionnaire, a Cronbach’s alpha of 0.7 was reported.

 

Respicheck total scores significantly correlated with the severity of respiratory impairment based on the laboratory and clinical classification (overall p = 0.0105). The higher scores were obtained from patients with the worst respiratory health status And QoL perception.

The level of evidence regarding the outcome measure diagnostic accuracy was not graded as the included studies did not fullfill the methodological requirements of these diagnostic accuracy studies.

Relevant outcome measures

The guideline development group considered the need for home mechanical ventilation and the incidence of pneumonia as critical outcome measures for decision making; and positive and negative predictive value, sensitivity, specifity, AUC, odds ratio, likelihood ratio, and incidence of hypercapnia, as an important outcome measures for decision making.

 

The guideline panel defined 25% difference as a minimal clinically (patient) important difference for dichotomous outcomes (RR< 0.8 or >1.25) and 0.5 SD for continuous outcomes.

 

Search and select (Methods)

A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 2010 to 9th of July 2024 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from primary search concepts for: (1) ‘myotonic dystrophy’ (2) and ‘respiratory failure’. After deduplication a total of 1066 records were imported for title/abstract screening. Initially, 2 systematic reviews and 17 studies with another design were selected based on title and abstract screening. After reading the full text, nine studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and eight studies were included.

Eight studies were included in the analysis of the literature. Important study characteristics and results are summarized in the evidence tables. The assessment of the risk of bias is summarized in the risk of bias tables.

Samenstelling werkgroep

Voor het ontwikkelen van de richtlijnmodule is in 2023 een multidisciplinair cluster ingesteld. Dit cluster bestaat uit vertegenwoordigers van alle relevante organisaties die betrekking hebben op de zorg voor patiënten met myotone dystrofie.

 

Het cluster Myopathie bestaat uit meerdere richtlijnen, zie [hier] voor de actuele clusterindeling. De stuurgroep bewaakt het proces van modulair onderhoud binnen het cluster. Inbreng patiëntenperspectief

De werkgroep besteedde aandacht aan het patiëntenperspectief door deelname aan het cluster myopathie waarin deze module werd ondergebracht. De richtlijnmodule werd ter commentaar voorgelegd aan Spierziekten Nederland en de Patiëntenfederatie. De door hen aangeleverde commentaren werden verwerkt.

 

Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz

Bij de richtlijnmodule voerden de clusterleden conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).
