Uitgangsvraag
Welke informatie/overwegingen moeten tijdens counseling met de ouders besproken worden betreffende de behandelopties nadat tijdens een echo spina bifida is vastgesteld?
De uitgangsvraag omvat de volgende deelvragen:
- Welke informatie dienen ouders te ontvangen en wanneer?
- Wat is de plaats van prenatale chirurgie versus een postnatale procedure?

Aanbeveling-1
Bespreek bij iedere MMC-zwangerschap de volgende 3 opties: zwangerschapsbeëindiging, prenatale chirurgie en postnatale chirurgie. Counseling vindt plaats in een SB centrum.
Deze prenatale counseling dient multidisciplinair te gebeuren door een gynaecoloog en een vertegenwoordiger van het spina bifida team.
Verwijs tijdig voor counseling over prenatale chirurgie in een vroeg stadium bij het ontbreken van expertise in eigen centrum.
Aanbeveling-2
Bespreek de gekende foetale voordelen van prenataal opereren als ook de maternale risico’s en het risico op vroeggeboorte.
Overwegingen
Balans tussen gewenste en ongewenste effecten
Bij de prenatale counseling na de diagnose van spina bifida is het van belang dat ouders op zorgvuldige, empathische en non-directieve wijze geïnformeerd worden over de behandelopties. Een multidisciplinaire behandeling is essentieel om de verschillende medische, ethische, psychosociale en praktische aspecten van deze opties te belichten. Counseling dient vooral ondersteunend te zijn en non-directief om zo tot een gezamenlijk besluit te komen. Vroege verwijzing naar gespecialiseerd centrum is sterk aanbevolen om tijdig prenatale chirurgie te kunnen bespreken.
Er is een literatuuronderzoek uitgevoerd naar de voor- en nadelen van prenatale en postnatale chirurgie. Er is 1 RCT geïncludeerd, de MOMS-trial met rapportage van uitkomsten op korte en langere termijn. Er werden klinisch relevante verschillen gevonden in het voordeel van de prenatale procedure voor de volgende kritische uitkomstmaten: het aantal drains, motorische functie en zelfkatheterisatie (CIC). In de prenatale groep kwam vroeggeboorte en complicaties tijdens de zwangerschap vaker voor. Voor mentale ontwikkeling en perinatale sterfte werden geen klinisch relevante verschillen gevonden. Op de leeftijd van 12 maanden hebben minder kinderen van de prenatale groep een orthopedische behandeling (belangrijke uitkomstmaat) ondergaan dan in de postnatale groep, maar op de leeftijd van 30 maanden was er niet langer een klinisch relevant verschil.
Aangezien het om een zeldzame aandoening gaat, en prenatale chirurgie na publicatie van de MOMS-trial als superieur wordt gezien voor bepaalde uitkomsten, is het onwaarschijnlijk dat nieuwe RCT’s zullen worden uitgevoerd. Bij ouders en professionals ontbreekt veelal de noodzakelijke equipoise, wat uitvoering van nieuwe trials bemoeilijkt.
Het aantal centra in de wereld dat prenatale chirurgie aanbiedt is wel exponentieel toegenomen na de publicatie van de MOMS-trial. De zorg is doorgaans gecentraliseerd in expertisecentra en vanuit die ervaring worden er steeds meer observationele cohorten gepubliceerd, echter veelal zonder adequate controlegroep. Over het algemeen is het wel zo dat de uitkomsten vergelijkbaar zijn met de resultaten van de MOMS-trial.
Er zijn technologische en chirurgische ontwikkelingen die relevant zijn voor de afweging van voor- en nadelen. Zo is er toenemende aandacht voor het beperken van de hysterotomie, bijvoorbeeld door toepassing van mini-hysterotomie (<3.5cm), wat geassocieerd lijkt met langere zwangerschapsduur. Daarnaast is er groeiende ervaring met foetoscopische benaderingen, met name de zogenaamde ‘open’ foetoscopie. Bij deze techniek wordt nog steeds een maternale laparotomie verricht, maar de chirurgie zelf vindt plaats via meerdere trocars in de uterus, waarbij het defect minimaal-invasief wordt gesloten. Het belangrijkste voordeel is dat er geen hysterotomie nodig is, waardoor een vaginale bevalling mogelijk blijft. In vergelijking met de resultaten van de MOMS-trial lijkt er ook een verlenging van de zwangerschapsduur. Op dit moment is echter nog onvoldoende duidelijk of de neurologische resultaten helemaal vergelijkbaar zijn met de open chirurgie. Er ontbreken vergelijkende studies van voldoende kwaliteit. Foetale centra hebben zich vaak gespecialiseerd in een bepaalde techniek en dus is het niet te verwachten dat er een RCT zal worden opgezet.
Prenatale chirurgie brengt geen directe voordelen voor de moeder zelf, maar wel reële risico’s. De moeder wordt onderdeel van het behandeltraject, wat fysieke en psychologische gevolgen kan hebben. Mogelijke complicaties zijn onder andere: vroegtijdige weeënactiviteit, uterusruptuur bij een latere zwangerschap, noodzaak tot sectio caesarea bij huidige en toekomstige bevallingen, en in enkele gevallen aanvullende chirurgische interventies. Hoewel maternale complicaties zelden levensbedreigend zijn, kunnen zij wel ingrijpend zijn en dienen zij expliciet besproken te worden tijdens counseling.
Een ander belangrijk aspect dat zeker bij de counseling benoemd moet worden is de relatief beperkte beschikbaarheid van langetermijngegevens. De MOMS2-follow-upstudie laat zien dat de verbetering in motorische uitkomsten na prenatale chirurgische behandeling ook op kinderleeftijd (5 tot 10 jaar) aanhoudt. Er bestaan echter nog onzekerheden over de duurzaamheid van deze voordelen op de langere termijn en over andere mogelijke langetermijneffecten, zoals neurologische ontwikkeling, blaasfunctie en kwaliteit van leven tijdens de adolescentie en volwassenheid. Daarnaast zijn er enkele signalen over een mogelijk verhoogde kans op tethered cord en inclusie cystes bij prenataal geopereerde kinderen, hoewel de klinische relevantie hiervan nog onbekend is.
Kwaliteit van bewijs
De overall kwaliteit van bewijs is zeer laag. Dit betekent dat we onzeker zijn over de gevonden effecten van de cruciale uitkomstmaten. Er is afgewaardeerd vanwege (zeer) ernstige:
- Risk of bias: methodologische beperkingen vanwege gebrek aan blindering. Daarnaast is de studie vroegtijdig gestopt.
- Imprecisie: onnauwkeurigheid, omdat het betrouwbaarheidsinterval de grenzen van klinische relevantie overschrijdt en omdat het data van een studie betreft.
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
Het is van belang om bij de counseling rekening te houden met de individuele waarden van patiënten. Voor sommige patiënten is een zwangerschapsbeëindiging geen optie vanwege religieuze of ethische overtuigingen. Er dient voldoende aandacht te zijn voor de emotionele aspecten met laagdrempelige beschikbaarheid van psychosociale ondersteuning van de ouders.
Prenatale therapie zal de aandoening niet volledig genezen en de impact van een zorgbehoeftig kind op het gezin kan aanzienlijk zijn. Als er sprake is van vroeggeboorte komen hier de eventuele complicaties van prematuriteit ook nog bij. De praktische aspecten spelen ook een rol gezien de ingreep momenteel in België gebeurt, over het algemeen verblijven patiënten een week in het buitenland met mogelijk financiële uitdagingen. Het is essentieel om gezinsplanning te bespreken, met name gezien de gevolgen voor toekomstige zwangerschappen.
Kostenaspecten
Zowel de prenatale als postnatale chirurgie brengen aanzienlijke kosten met zich mee gerelateerd aan de interventie. De kosten bij een prenatale ingreep zijn mogelijk hoger omwille van de complexiteit, waarbij er naast de foetale ingreep ook een maternale laparotomie en hysterotomie dient te gebeuren. Bij een preterme partus zijn er de kosten van een opname op de afdeling neonatologie. Echter, op lange termijn biedt de prenatale chirurgie belangrijke kostenbesparingen. Er is veel minder noodzaak tot het plaatsen van een ventriculoperitoneale drain en dus minder daaraan gerelateerde herinterventies en ziekenhuisopnames. De verbetering in motorische functie geeft daarbij een betere maatschappelijke participatie en verminderde zorgafhankelijkheid.
Gelijkheid ((health) equity/equitable)
De interventie leidt tot een mogelijke afname van gezondheidsgelijkheid want patiënten dienen naar het buitenland af te reizen voor de ingreep. Er zijn een aantal administratieve drempels en mogelijk enige financiële consequenties door het verblijf van naasten. De algemene impact op gezondheidsgelijkheid lijkt beperkt en dus is dit acceptabel.
Aanvaardbaarheid
Ethische aanvaardbaarheid
De interventie lijkt aanvaardbaar voor de betrokkenen. Er zijn geen ethische bezwaren. De maternale en foetale risico’s zijn welomschreven. Bij de counseling is het essentieel dat alle opties gelijkwaardig besproken worden om zo tot een gezamenlijke beslissing te komen.
Duurzaamheid
Bij de interventie spelen de volgende duurzaamheidsaspecten een rol. Door een afname in de noodzaak tot het plaatsen van een ventriculoperitoneale drain is er dus ook een vermindering in het aantal herinterventies en ziekenhuisopnames.
Haalbaarheid
De interventie lijkt haalbaar. De interventie zou over het algemeen deel moeten uitmaken van de standaard aangeboden therapeutische opties bij de diagnose van een meningomyelocele. Voor sommige patiënten vormt de verwijzing naar het buitenland een drempel. Het Erasmus MC werkt nauw samen met UZ Leuven in de zorgketen rondom prenatale behandeling voor spina bifida. Om de verwijzing naar Leuven laagdrempeliger te maken biedt het Erasmus MC de counseling en indicatiestelling met betrekking tot de ingreep aan, terwijl de prenatale ingreep zelf in UZ Leuven wordt uitgevoerd door een gezamenlijk team.
Rationale van aanbeveling-1: weging van argumenten voor en tegen de interventies
Er dient een evenwichtige counseling te gebeuren op basis van alle beschikbare literatuur en door een multidisciplinair team van behandelaars die regelmatig betrokken zijn bij de zorg rondom een spina bifida. Dit team bestaat uit prenatale behandelaars (prenatale artsen/gynaecologen) en postnatale behandelaars (neuroloog, neurochirurg, revalidatieartsen en/of physician assistant). Tijdige verwijzing naar expertisecentrum bij ontbreken ervan in eigen centrum.

Sterke aanbeveling voor.
Rationale van aanbeveling-2: weging van argumenten voor en tegen de interventies
Bij het opstellen van deze aanbeveling wordt naast de enige RCT ook rekening gehouden met grote follow-up cohortstudies. Op basis van deze gegevens lijkt het gunstige effect van prenatale chirurgie voldoende bewezen.
Eindoordeel:
We komen tot een sterke aanbeveling voor.
Onderbouwing
Wanneer spina bifida wordt vastgesteld tijdens prenatale echoscopie (door structurele echo op 13 of 20 weken zwangerschapsduur), zal men verschillende belangrijke beslissing moeten nemen. Allereerst een keuze met betrekking tot beëindigen van zwangerschap en bij continueren van de zwangerschap is er nog een keuze tussen prenatale chirurgische correctie, of postnatale chirurgische behandeling. De MOMS-trial heeft de voordelen van prenatale chirurgie duidelijk aangetoond, weliswaar met maternale risico’s. Ondanks dat is er een variatie in counseling tussen centra en ontbreekt een gestandaardiseerde, wetenschappelijk onderbouwde benadering. Door de ontwikkeling en toepassing van gestandaardiseerde counselingprotocollen en de actualisatie van langetermijngegevens over de verschillende behandelingsmodaliteiten kan de kwaliteit van zorg worden verbeterd en kunnen beter geïnformeerde beslissingen worden genomen.

Description of studies
All included studies reported on the MOMS trial, a randomized controlled trial in which eligible women were randomly assigned to undergo either prenatal surgery before 26 weeks of gestation or standard postnatal repair. The trial was performed at three maternal-fetal surgery centers in the United States. Women were included in the period 2003-2010. Women at increased risk for preterm birth were excluded. The trial was stopped early for efficacy of prenatal surgery after recruitment of 183 of the planned 200 patients.
Two primary outcomes were defined: (1): A composite outcome of fetal or neonatal death or need for a cerebrospinal fluid shunt by the age of 12 months. (2) A composite score of the Mental Development Index of the Bayley Scales of Infant Development II and the child’s motor function, with adjustment for lesion level. These outcomes were first reported by (Adzick, 2011). This initial analysis reports on the outcomes of 158 women, i.e. the cohort with available outcomes at the time of the planned interim analysis resulting in the decision to stop further inclusions. In the literature analysis the primary outcomes of the full cohort were included. Secondary outcomes at different follow-up times were reported as further described below. All these studies concerned the MOMS trial, reporting on a range of outcomes.
Only outcomes included in the literature analysis are reported. The risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).
Results
Fetal or neonatal death (Critical)
Perinatal death was reported by Tulipan (2015). In both groups 2 deaths were reported, 2/91 for the prenatal surgery group and 2/92 for the postnatal surgery group, resulting in a RR of 1.01 (95%CI 0.15 to 7.02), which is not considered clinically relevant.
Cerebrospinal fluid shunt placement (Critical)
Tulipan (2015) reported a lower number of cerebrospinal fluid shunt placements at the age of 12 months, i.e. (40/91) 44% in the prenatal group and (77/92) 83.7% in the postnatal group. Resulting in a clinically relevant RR of 0.53 (95%CI 0.41 to 0.67) in favor of the prenatal group.
Mental development (critical)
Farmer (2018) reported on mental development at the age of 30 months. A higher percentage of the children in the prenatal repair group scored above 70 on the Bayley Scales of Infant Development (BSID II) mental development index compared to the postnatal repair group (RR=1.05 (95%CI 0.93 to 1.19)). This difference is not clinically relevant. Also, when using other cut-off values, no clinically relevant effects were observed (these findings are not included in the SoF-table). The mean score in the prenatal group was 89.5 (SD=15.0) versus 86.2 (SD=18.1) in the postnatal group. This difference is not considered clinically significant (not included in SoF-table).
Houtrow (2020) reported on mental development at school age (age 5-10 years). Children in the prenatal group scored 7.4 points higher on the Kaufman Brief Intelligence Test. This difference is smaller than 0.5 SD and therefore not considered clinically relevant. Comparable findings were found for other tests, with small, not clinically relevant, differences observed in favor of the prenatal groups. See Table, these additional findings are not included in the SoF table.

Motor function (critical)
Farmer (2018) reported on the difference between the anatomical and functional levels of the lesion at the age of 30 months. In the prenatal surgery group 36% of the children had a motor level worse than anatomic compared to 59% of the children in the postnatal surgery group. This difference is considered clinically relevant in favor of the prenatal group (RR=0.60 (95%CI 0.43 to 0.84)). Also, at school age (age 5-10 years) less children in the prenatal group had a motor level worse than anatomic, with an observed RR of 0.44 (95%CI 0.25 to 0.77) (Houtrow, 2021). This difference, in favor of the prenatal group, is also considered clinically relevant. In the table also some additional results are shown (not included in the SoF-table).

Urologic outcomes (critical)
A supplementary urology study of the MOMs trial was performed (Brock, 2015). The primary outcome for this study was death or the need for clean intermittent catheterization (CIC) through 30 months of age. The necessity for CIC was based on a set of criteria. However, we report the actual performed CIC. The observed RR of 0.74 (95%CI 0.48 to 1.12) is in favor of the prenatal group and this effect is considered clinically relevant. A comparable effect was observed at older age (>6 years) (Brock, 2019). At this age 62% vs 87% in the prenatal and postnatal surgery groups, respectively, were placed on clean intermittent catheterization (RR 0.71 (95% CI 0.58 to 0.86)). This clinically relevant effect estimate is in favor of the prenatal surgery group.

Obstetrical outcomes (critical)
Obstetric outcomes were reported by Johnson (2015). Gestational age at birth was 34.0 weeks (SD=3.0) for the prenatal repair group and 37.3 weeks (SD=1.1) for the postnatal repair groups. The mean difference of 3.3 weeks is a clinically relevant difference in favor of postnatal surgery. The RR for a preterm birth (<37 weeks) was 4.99 (95%CI 3.11 to 8.01) in favor of the postnatal group. This also reflects a clinically relevant effect. Pregnancy complications occurred more often in the prenatal repair group (see Table, not included in the SoF-table).

Orthopedic outcomes (important)
Swarup, 2022 reported on the frequency of treatment for orthopedic conditions at the age of 12 and 30 months. At 12 months 38% in prenatal and 54% of the postnatal surgery group underwent treatment, a clinically relevant difference in favor of the prenatal surgery group (RR= 0.71 (95%CI: 0.51 to 1.00)). Also, at the age of 30 months a lower percentage of children in the prenatal group underwent treatment (78% versus 90%). This difference is small and therefore not considered clinically relevant (RR=0.87 (95%CI 0.76 to 0.99)). Prevalence of conditions are shown in the additional table (not included in the SoF-table).

Houtrow (2020) reported on the quality of life based on the caregiver-reported Parkin Spina Bifida Health-Related Quality of Life. Parents. The mean z-score in the prenatal group was 0.15 (SD=0.68) and 0.11 (SD=0.73) in the postnatal group. This difference is not considered clinically relevant.

Relevant outcome measures
The guideline panel considered fetal and neonatal death, shunt placement, mental development, motor function, urologic and obstetric outcomes as critical outcomes measure for decision making; and orthopedic outcomes and quality of life as important outcome measures for decision making.
A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.
The guideline panel defined a 1% difference for death (RR < 0.99 or > 1.01) as a minimal clinically (patient) important difference. For the other outcomes, a 25% difference
for dichotomous outcomes (RR < 0.8 or > 1.25) and 0.5 SD for continuous outcomes was taken as minimal clinically (patient) important difference.
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 2005 to the 24th of March 2025 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from two primary search concepts: (1) spina bifida; (2) prenatal closure. Duplicates were removed using EndNote software. After deduplication a total of 648 records were imported for title/abstract screening. The systematic reviews and RCTs were included in the title and abstract screening. Initially, 30 studies were selected based on this screening. After reading the full text, 22 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and 8 studies were included.

Voor het ontwikkelen van de richtlijnmodule is in 2024 een multidisciplinair cluster ingesteld. Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule voerden de clusterleden conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).
