Screeningsmodaliteiten colorectaal carcinoom
Uitgangsvraag
Op welke wijze zouden risicogroepen van 5-jaarsoverlevers van hodgkinlymfoom en diffuus grootcellig B-cellymfoom gescreend moeten worden voor colorectaal carcinoom?
Aanbeveling
Biedt surveillance aan door middel van een colonoscopie.
Verricht de colonsurveillance 5-jaarlijks, tenzij de gevonden poliepen deze termijn overrulen op basis van de geldende poliepsurveillance richtlijn.
Verricht bij voorkeur geen pre-test door middel van een ontlasting test gezien de lage specificiteit. Geef indien een overlever afziet van screening middels coloscopie de voorkeur aan jaarlijks een FIT.
Overwegingen
Voor- en nadelen van de interventie en de kwaliteit van het bewijs
Er is één studie gevonden die de diagnostische accuratesse beschreef van FIT en Mt-SDNA tests voor het detecteren van (geavanceerde poliepen bij) colorectaal carcinoom bij patiënten die zijn behandeld voor hodgkinlymfoom (Ykema, 2020). Als referentietest werd een colonoscopie gebruikt. Deze studie laat een hogere sensitiviteit zien van mt-sDNA ten opzichte van FIT. Echter is de specificiteit van FIT hoger. Er is geen relevant verschil gevonden in positief en negatief voorspellende waarde. De area under the curve (AUC) werd beschreven voor advanced neoplasia en deze was beter in Mt-sDNA (AUC (95% CI): 0.76 (0.63-0.89)) vergeleken met FIT (AUC (95% CI): 0.63(0.54-0.82). De bewijskracht voor deze bevindingen is echter laag of gemiddeld, doordat er slechts één studie met een beperkt aantal deelnemers is geïncludeerd en door imprecisie omdat het betrouwbaarheidsinterval de grens voor klinische besluitvorming omvat. Er is geen bewijs gevonden over de uitkomstmaat belasting voor de patiënt. In de Nederlandse praktijk is de mt-sDNA test niet beschikbaar, dus zal deze niet verder worden besproken en niet worden meegenomen in de aanbevelingen.
Behandeling met procarbazine-bevattende chemotherapie, al dan niet gecombineerd met infra-diafragmale radiotherapie, verhoogt het risico op colorectaal carcinoom bij hodgkinlymfoom (HL)-overlevers (Ykema, 2020). Vroege opsporing en verwijdering van geavanceerde poliepen, het voorstadium van colorectaal carcinoom, verlaagt dit risico aanzienlijk. Hoewel de FIT-test effectief is voor de algemene bevolking, is deze minder geschikt voor HL-overlevers. FIT mist namelijk vaak serrated-type poliepen, die bij HL-overlevers beduidend vaker voorkomen (6% versus 0,09% in de algemene bevolking) (Ykema, 2021; Rigter, 2018). Hierdoor is een coloscopie als screening effectiever en gerechtvaardigd.
Coloscopie kan door patiënten als belastend worden ervaren. Binnen de werkgroep is een nog niet gepubliceerde studie bekend met deze opzet en uitkomsten (Rigter et al). Uit de resultaten blijkt dat lange termijn overlevers van hodgkinlymfoom een colonoscopie als minder belastend ervoeren dan verwacht. Van de lange termijn overlevers die een colonoscopie ondergingen, nam 54% deel door middel van vragenlijsten, waarbij 43% beide vragenlijsten invulde. De meerderheid gaf aan dat de ingreep minder belastend was dan ze hadden verwacht. Vergeleken met de controlegroep uit de algemene populatie rapporteerden lange termijn overlevers vaker zowel een hogere verwachte belastbaarheid als een hogere ervaren belastbaarheid na de colonoscopie. Desondanks waren zij meer bereid een eventuele tweede colonoscopie te ondergaan.
Deze resultaten suggereren dat het haalbaar is een screeningsprogramma te starten voor specifieke lange termijn overlevers na een lymfoombehandeling. Een pre-test, zoals de Fecaal Immunochemische Test (FIT), zou het aantal onnodige coloscopieën mogelijk kunnen verminderen. Echter, bij overlevers van hodgkinlymfoom biedt FIT geen volledige zekerheid vanwege de beperkte detectie van serrated-type poliepen. Het is daarom belangrijk om patiënten te informeren over deze beperking en het nut van een colonoscopie, gezien hun verhoogde risico.
In de algemene bevolking is de effectiviteit van screening op colorectaal carcinoom uitgebreid onderzocht. Een Cochrane-review toont aan dat screening met fecaal occult bloedtests (FOBT) de sterfte door colorectaal carcinoom verlaagt (Hewitson, 2008). Volgens de Gezondheidsraad biedt de immunochemische FOBT (FIT) de beste balans tussen eenvoud, acceptatie, testprestaties en veiligheid. Deze test wordt in Nederland toegepast binnen het bevolkingsonderzoek voor mensen van 55-75 jaar.
Bij lange termijn overlevers van hodgkinlymfoom is FIT echter minder geschikt. De test spoort geen darmkanker-voorlopers op en mist vooral serrated poliepen, die in deze groep significant vaker voorkomen (6% versus 0,09% in de algemene bevolking) (Ykema, 2020; Ykema, 2022; Rigter, 2018). Hierdoor is een aanpassing van de screeningsleeftijden en -frequentie noodzakelijk, en verdient een colonoscopie als screeningsmethode de voorkeur.
In de Verenigde Staten adviseert de Children’s Oncology Group om bij overlevers die als kind zijn behandeld, elke vijf jaar een colonoscopie te doen vanaf vijf jaar na de behandeling, of vanaf 30 jaar indien er meer dan 30 Gy op de buik is gegeven, met als aanvulling een multitarget DNA-test iedere drie jaar (Prasad, 2024). Dergelijke aanbevelingen benadrukken dat Hodgkin-overlevers baat kunnen hebben bij een aangepaste en regelmatige screeningstrategie die aansluit op hun verhoogde risicoprofiel en specifieke risicofactoren.
Waarden en voorkeuren van patiënten (en evt. Hun verzorgers)
Overlevers van hodgkinlymfoom (HL) hebben door eerdere behandelingen, zoals infradiafragmale radiotherapie en chemotherapie met procarbazine, een verhoogd risico op darmkanker. Preventieve screening is daarom aanbevolen om voorstadia van darmkanker op te sporen en te verwijderen. Hierbij gaat de voorkeur uit naar een colonoscopie, omdat ontlastingstesten zoals FIT en mt-sDNA beperkte specificiteit hebben, vaak serrated-type poliepen missen en minder effectief zijn voor geavanceerde poliepen (Rigter, 2018). Bovendien ontbreekt onderzoek naar herhaalde ontlastingstesten, terwijl eerdere studies slechts éénmalige combinaties van tests en coloscopie onderzochten.
De DICHOS-trial beveelt periodieke coloscopie aan voor HL-overlevers met een hoog risico (Ykema, 2020). Hoewel gerandomiseerde gegevens over de impact op overleving ontbreken, suggereren bestaande studies dat screening mogelijk een positief effect heeft.
In het besluit om darmkankerscreening te starten, is overleg met de patiënt essentieel. Coloscopie kan door patiënten als belastend worden ervaren. Uit de Nederlandse DICHOS-studie blijkt dat 38% van de HL-overlevers opzag tegen een coloscopie (vs. 24% in de controlegroep) (Ykema, 2020). Na het onderzoek vond slechts 17% van de HL-overlevers het pijnlijk, beschamend of belastend, tegenover 7% in de controlegroep (Rigter et al, unpublished). Dit wijst op een verschil tussen de verwachte en ervaren belasting, wat in gesprekken met patiënten moet worden meegenomen.
Het is belangrijk dat artsen HL-overlevers goed voorlichten over de voor- en nadelen van ontlastingstesten versus coloscopie, met nadruk op de hogere nauwkeurigheid van coloscopie voor deze specifieke risicogroep.
Kostenaspecten
Een kosteneffectiviteitsanalyse in hetzelfde patiënten cohort (Ykema, 2022) voor HL-overlevers toonde dat een jaarlijkse FIT-ontlastingstest voor patiënten van 45-70 jaar 70% van de overlijdens door colorectaal carcinoom kan voorkomen. Bij niet-bestraalde HL-overlevers wordt een tweejaarlijkse FIT aangeraden, wat 56% van de overlijdens voorkomt. Deze screeningsmethoden kosten tussen de €13.000 en €18.000 per gewonnen levensjaar, wat binnen de acceptabele grens van €20.000 ligt. Onderzoek naar de kosten van darmkankerscreening in de algemene bevolking ondersteunde eerder deze strategie (Greuter, 2017). Zowel FIT als coloscopie blijken kosteneffectief voor HL-overlevers met verhoogd risico (Ykema, 2022).
Gelijkheid ((health) equity/equitable)
De interventie leidt niet tot een verandering in gezondheidsgelijkheid.
Aanvaardbaarheid
Ethische aanvaardbaarheid
De interventie lijkt aanvaardbaar voor de betrokkenen. Er zijn geen ethische bezwaren.
Duurzaamheid
Bij de interventie en controle spelen duurzaamheidsaspecten geen rol.
Haalbaarheid
De interventie lijkt haalbaar. De interventie is over het algemeen al standaardzorg in de praktijk.
De FIT test en coloscopie blijken beide relevant voor Hodgkin-overlevers vanwege hun verhoogde risico op colorectaal carcinoom. De FIT-test is in eerdere studies onderzocht, waaronder in populaties met vergelijkbare risico’s zoals testiskanker (Ykema, 2021; Breekveldt, 2022), en wordt gezien als kosteneffectief in combinatie met coloscopie. Toch is de lage specificiteit van de FIT-test problematisch vanwege de kans op gemiste geavanceerde poliepen. Coloscopie biedt hier een betrouwbaarder alternatief, ondanks de belasting zoals darmspoeling, sedatie en werkonderbreking. Deze procedure is in Nederland voor iedereen goed toegankelijk en grotendeels gedekt door de zorgverzekering, met uitzondering van een eventueel openstaand eigen risico, en de wachttijden zijn aanvaardbaar. Bij patiënten met ernstige comorbiditeiten of beperkte levensverwachting kan een coloscopie minder geschikt zijn, en soms is een CT van het colon als tussentijdse oplossing mogelijk. Het is wel zo dat bij een afwijkende bevinding op een CT van het colon alsnog een coloscopie wordt geadviseerd. Er is niet in studieverband onderzocht hoe frequent een coloscopie zou moeten worden verricht.
Zoals reeds eerder uitgebreid geëvalueerd door de werkgroep implementatie bevolkingsonderzoek darmkanker zijn de kosten, uitvoerbaarheid en belastbaarheid acceptabel geacht, waar de werkgroep graag in meegaat (Vleugels, 2016).
Rationale van de aanbeveling: weging van argumenten voor en tegen de diagnostische procedure
Biedt surveillance van het colon aan bij overlevers van hodgkinlymfoom die voldoen aan één van onderstaande criteria, waarbij surveillance afhankelijk is van cumulatieve dosering procarbazine en/of cumulatieve stralingsbelasting:
- behandeld met procarbazine (≥ 8.4 g/m2) bevattende chemotherapie (≥ 6 MOPP-kuren), ongeacht infradiafragmatische radiotherapie
- behandeld met ≥ 4.2 g/m2 procarbazine (≥ 3 MOPP-kuren, ≥ 6 MOPP-ABV of ≥ 6 (esc) BEACOPP-kuren) EN infradiafragmatische bestraling van ≥ 25 Gy op een deel van het colon
- behandeld met ≥ 2.8 g/m2 procarbazine (≥ 2 MOPP-kuren, ≥ 4 MOPP-ABV of ≥ 4 (esc) BEACOPP-kuren) EN infradiafragmatische bestraling van ≥ 36 Gy op een deel van het colonWat betreft onderzoek naar screening CRC bij behandeling van het DLBCL is er momenteel te weinig bewijs om hier goede aanbevelingen te doen. Patiënten met een DLBCL zijn over het algemeen ouder dan 40 jaar tijdens de behandeling dan HL-patiënten en krijgen alleen in uitzonderingsgevallen een infradiafragmatische bestralingsbehandeling, vergelijkbaar met HL-patiënten.
Overlevers van hodgkinlymfoom die een behandeling met procarbazine bevattende chemotherapie met/zonder infradiafragmale radiotherapie ondergingen hebben een verhoogd risico op het ontwikkelen van geavanceerde poliepen en colorectaal carcinoom. Dit risico neemt toe vanaf ongeveer 8 jaar na de behandeling. Daarom wordt aanbevolen om vanaf 8 jaar na het afronden surveillance te starten. De meest betrouwbare manier hiervoor is een coloscopie.
Een vooraf test (poeptest) is uit kosteneffectiviteitsperspectief een overweging. Maar deze test kent een lage specificiteit, waardoor geavanceerde poliepen kunnen worden gemist. Indien een ontlasting test als ingangstest wordt gebruikt is dit het meest kosteneffectief indien deze test jaarlijks wordt verricht. Voor surveillance met coloscopieën zonder vooraf test is niet onderzocht wat de beste frequentie is.

Onderbouwing
Hodgkin lymphoma is a type of lymph node cancer, diagnosed annually in approximately 400 new patients in the Netherlands (IKNL, 2024). Hodgkin lymphoma occurs primarily during adolescence and young adulthood. Patients who are cured generally have an excellent life expectancy. Nowadays, the majority of Hodgkin lymphoma patients are cured with combinations of chemotherapy and/or radiotherapy. However, the life expectancy and quality of life of survivors are negatively impacted by late effects of these treatment (radiotherapy, alkylating chemotherapy), such as colorectal carcinoma (Hodgson, 2007; Franklin, 2006; Swerdlow, 1997; Swerdlow, 2001; van Leeuwen, 2000; Schaapveld, 2015, de Vries, 2021). The harmful side effects that arise as a result of treatment for Hodgkin lymphoma often appear only in the long term (8-10 years) (Hodgson, 2007). In recent years, treatment for patients with HL and diffuse large B-cell lymphoma (DLBCL) has significantly improved. In addition, recent studies have been published, providing new insights into screening for colorectal carcinoma.
The risk of GI tract cancer in the general population is approximately 4.5%; this risk is determined by certain factors, such as age, genetic predisposition, lifestyle / environmental factors and preexisting conditions.
Twenty percent of all second solid tumors in survivors of Hodgkin lymphoma are of gastrointestinal origin. This risk of GI tract cancer is 4.6 times increased compared to the normal population. Especially the risk for developing colorectal cancer is 2-7 times increased, with the risk remaining elevated for up to 40 years (Schaapveld, 2015,).
Given the increased risk of developing colorectal carcinoma in survivors, it is crucial to determine the most effective screening method. The traditional gold standard for colorectal carcinoma screening is the fecal immunochemical test (FIT) used in population-based screening programs (Gezondheidsraad, 2009). In studies among survivors after alkylating-based chemotherapy and/or infra-diaphragmatic radiotherapy, the prevalence of advanced polyps appeared to be higher than in the control population (Rigter, 2018). Recent studies have examined, whether a preliminary test such as FIT is a valuable addition to the screening process for this population of Hodgkin survivors. A preliminary test namely could potentially reduce the number of colonoscopies required. This could have a beneficial effect on patient burden and the costs of care.

Description of studies
Ykema (2020) reported a prospective cohort study in Hodgkin lymphoma (HL) survivors after treatment with procarbazine containing chemotherapy and/or infra-diaphragmatic radiotherapy. The inclusion criteria were age between 16 and 50 years at HL diagnosis, at least 8 years after treatment and a life expectancy of >5 years. The exclusion criteria were proctocolectomy, colonoscopy surveillance or colonoscopy in the past 5 years. All participants had both a FIT and mt-sDNA stool test as well as a colonoscopy.
They described the sensitivity, specificity, positive and negative predictive value and area under the curve for a fecal Immunochemical tests (FIT) alone, a multi-target stool DNA-testing (Mt-sDNA) alone a combined FIT and MT-sDNA in 101 patients. The reference standard was detection of colorectal precursor lesions and cancers at colonoscopy. For the outcome “advanced neoplasia” the sensitivity (68 vs 37) and area under the curve (0.76 vs 0.63) were higher for mt-sDNA versus a FIT. However the specificity ( 70 vs 91) was lower and the positive predictive value (46 vs 58) was comparable (table 1).

Relevant outcome measures
The guideline development group considered sensitivity/negative predictive value (NPV) as a critical outcome measure for decision making; and specificity/positive predictive value (PPV) and patient burden as an important outcome.
The aim of screening colorectal cancer lesions is to easily detect advanced polyps and colorectal lesions in HL/DLBCL survivors. In such measurements, it is important to keep the number of false negatives low.
The specificity of a test is its ability to designate an individual without the outcome as negative. Sensitivity refers to a test’s ability to designate an individual with the outcome as positive. A highly sensitive test means that there are few false negatives results, and thus fewer patients with CRC are missed, which is desirable. Therefore, (high) sensitivity and (high) NPV are considered as critical outcome measures for decision making. (high) specificity and PPV are considered as important outcome measures for decision making.
A priori, the working group did not define the outcome measures listed above but used the definitions used in the studies.
The working group defined a sensitivity, specificity, NPV, PPV ≥0.8 as minimal clinically relevant.
Search and select (Methods)
The databases Medline (via OVID) and Embase (via Embase.com) were searched with relevant search terms until June 14th, 2024. The detailed search strategy is depicted under the tab Methods. The systematic literature search resulted in 46 hits. Studies were selected based on the following criteria:

Forty-six studies were initially selected based on title and abstract screening. After reading the full text, 45 studies were excluded (see the table with reasons for exclusion under the tab Methods), and one study was included.

De ontwikkeling/herziening van deze richtlijnmodule werd ondersteund door het Kennisinstituut van de Federatie Medisch Specialisten (www.demedischspecialist.nl/kennisinstituut) en werd gefinancierd uit de Stichting Kwaliteitsgelden Medisch Specialisten (SKMS). Samenstelling werkgroep

Inbreng patiëntenperspectief
Er werd aandacht besteed aan het patiëntenperspectief door het uitnodigen van patiëntenorganisaties voor de schriftelijke knelpunteninventarisatie, een afgevaardigde namens een patiëntenorganisatie in de werkgroep. De verkregen input is meegenomen bij het opstellen van de uitgangsvragen, de keuze voor de uitkomstmaten en bij het opstellen van de overwegingen. De conceptrichtlijn is tevens voor commentaar voorgelegd aan Hematon en de eventueel aangeleverde commentaren zijn bekeken en verwerkt.
Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule is conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uitgevoerd om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema op de Richtlijnendatabase).

Werkwijze
AGREE
Deze richtlijnmodule is opgesteld conform de eisen vermeld in het rapport Medisch Specialistische Richtlijnen 2.0 van de adviescommissie Richtlijnen van de Raad Kwaliteit. Dit rapport is gebaseerd op het AGREE II instrument (Appraisal of Guidelines for Research & Evaluation II; Brouwers, 2010).
Knelpuntenanalyse en uitgangsvragen
Tijdens de voorbereidende fase inventariseerde de werkgroep de knelpunten in de zorg voor patiënten met hodgkinlymfoom en/of diffuus grootcellig B-cellymfoom (DLBCL).
De werkgroep beoordeelde de aanbeveling(en) uit de eerdere richtlijnen over late effecten na hodgkinlymfoom:
- Cardiovasculaire schade na hodgkinlymfoom (NVRO, 2016)
- Fertiliteit en osteoporose na hodgkinlymfoom (NVRO, 2016)
- Miltschade na hodgkinlymfoom (NVRO, 2016)
- Nekklachten na hodgkinlymfoom (NVRO, 2016)
- Overige late effecten na hodgkinlymfoom (NVRO, 2016)
- Schildklierschade na hodgkinlymfoom (NVRO, 2016)
- Tweede tumoren na hodgkinlymfoom (NVRO, 2016)
Tevens zijn er knelpunten aangedragen tijdens een schriftelijke knelpunteninventarisatie. Een verslag hiervan is opgenomen onder aanverwante producten.
Op basis van de uitkomsten van de knelpuntenanalyse zijn door de werkgroep concept-uitgangsvragen opgesteld en definitief vastgesteld.
Uitkomstmaten
Na het opstellen van de zoekvraag behorende bij de uitgangsvraag inventariseerde de werkgroep welke uitkomstmaten voor de patiënt relevant zijn, waarbij zowel naar gewenste als ongewenste effecten werd gekeken. Hierbij werd een maximum van acht uitkomstmaten gehanteerd. De werkgroep waardeerde deze uitkomstmaten volgens hun relatieve belang bij de besluitvorming rondom aanbevelingen, als cruciaal (kritiek voor de besluitvorming), belangrijk (maar niet cruciaal) en onbelangrijk. Tevens definieerde de werkgroep tenminste voor de cruciale uitkomstmaten welke verschillen zij klinisch (patiënt) relevant vonden.
Methode literatuursamenvatting
Een uitgebreide beschrijving van de strategie voor zoeken en selecteren van literatuur is te vinden onder ‘Zoeken en selecteren’ onder Onderbouwing. Indien mogelijk werd de data uit verschillende studies gepoold in een random-effects model. Review Manager 5.4 werd gebruikt voor de statistische analyses. De beoordeling van de kracht van het wetenschappelijke bewijs wordt hieronder toegelicht.
Beoordelen van de kracht van het wetenschappelijke bewijs
De kracht van het wetenschappelijke bewijs werd bepaald volgens de GRADE-methode. GRADE staat voor ‘Grading Recommendations Assessment, Development and Evaluation’ (zie http://www.gradeworkinggroup.org/). De basisprincipes van de GRADE-methodiek zijn: het benoemen en prioriteren van de klinisch (patiënt) relevante uitkomstmaten, een systematische review per uitkomstmaat, en een beoordeling van de bewijskracht per uitkomstmaat op basis van de acht GRADE-domeinen (domeinen voor downgraden: risk of bias, inconsistentie, indirectheid, imprecisie, en publicatiebias; domeinen voor upgraden: dosis-effect relatie, groot effect, en residuele plausibele confounding).
GRADE onderscheidt vier gradaties voor de kwaliteit van het wetenschappelijk bewijs: hoog, redelijk, laag en zeer laag. Deze gradaties verwijzen naar de mate van zekerheid die er bestaat over de literatuurconclusie, in het bijzonder de mate van zekerheid dat de literatuurconclusie de aanbeveling adequaat ondersteunt (Schünemann, 2013; Hultcrantz, 2017).

Bij het beoordelen (graderen) van de kracht van het wetenschappelijk bewijs in richtlijnen volgens de GRADE-methodiek spelen grenzen voor klinische besluitvorming een belangrijke rol (Hultcrantz, 2017). Dit zijn de grenzen die bij overschrijding aanleiding zouden geven tot een aanpassing van de aanbeveling. Om de grenzen voor klinische besluitvorming te bepalen moeten alle relevante uitkomstmaten en overwegingen worden meegewogen. De grenzen voor klinische besluitvorming zijn daarmee niet één op één vergelijkbaar met het minimaal klinisch relevant verschil (Minimal Clinically Important Difference, MCID). Met name in situaties waarin een interventie geen belangrijke nadelen heeft en de kosten relatief laag zijn, kan de grens voor klinische besluitvorming met betrekking tot de effectiviteit van de interventie bij een lagere waarde (dichter bij het nuleffect) liggen dan de MCID (Hultcrantz, 2017).
Overwegingen (van bewijs naar aanbeveling)
Om te komen tot een aanbeveling zijn naast (de kwaliteit van) het wetenschappelijke bewijs ook andere aspecten belangrijk en worden meegewogen, zoals aanvullende argumenten uit bijvoorbeeld de biomechanica of fysiologie, waarden en voorkeuren van patiënten, kosten (middelenbeslag), aanvaardbaarheid, haalbaarheid en implementatie. Deze aspecten zijn systematisch vermeld en beoordeeld (gewogen) onder het kopje ‘Overwegingen’ en kunnen (mede) gebaseerd zijn op expert opinion. Hierbij is gebruik gemaakt van een gestructureerd format gebaseerd op het evidence-to-decision framework van de internationale GRADE Working Group (Alonso-Coello, 2016a; Alonso-Coello 2016b). Dit evidence-to-decision framework is een integraal onderdeel van de GRADE methodiek.
Formuleren van aanbevelingen
De aanbevelingen geven antwoord op de uitgangsvraag en zijn gebaseerd op het beschikbare wetenschappelijke bewijs en de belangrijkste overwegingen, en een weging van de gunstige en ongunstige effecten van de relevante interventies. De kracht van het wetenschappelijk bewijs en het gewicht dat door de werkgroep wordt toegekend aan de overwegingen, bepalen samen de sterkte van de aanbeveling. Conform de GRADE-methodiek sluit een lage bewijskracht van conclusies in de systematische literatuuranalyse een sterke aanbeveling niet a priori uit, en zijn bij een hoge bewijskracht ook zwakke aanbevelingen mogelijk (Agoritsas, 2017; Neumann, 2016). De sterkte van de aanbeveling wordt altijd bepaald door weging van alle relevante argumenten tezamen. De werkgroep heeft bij elke aanbeveling opgenomen hoe zij tot de richting en sterkte van de aanbeveling zijn gekomen.
In de GRADE-methodiek wordt onderscheid gemaakt tussen sterke en zwakke (of conditionele) aanbevelingen. De sterkte van een aanbeveling verwijst naar de mate van zekerheid dat de voordelen van de interventie opwegen tegen de nadelen (of vice versa), gezien over het hele spectrum van patiënten waarvoor de aanbeveling is bedoeld. De sterkte van een aanbeveling heeft duidelijke implicaties voor patiënten, behandelaars en beleidsmakers (zie onderstaande tabel). Een aanbeveling is geen dictaat, zelfs een sterke aanbeveling gebaseerd op bewijs van hoge kwaliteit (GRADE gradering HOOG) zal niet altijd van toepassing zijn, onder alle mogelijke omstandigheden en voor elke individuele patiënt.

Organisatie van zorg
In de knelpuntenanalyse en bij de ontwikkeling van de richtlijnmodule is expliciet aandacht geweest voor de organisatie van zorg: alle aspecten die randvoorwaardelijk zijn voor het verlenen van zorg (zoals coördinatie, communicatie, (financiële) middelen, mankracht en infrastructuur). Randvoorwaarden die relevant zijn voor het beantwoorden van deze specifieke uitgangsvraag zijn genoemd bij de overwegingen van de modules.
Commentaar- en autorisatiefase
De conceptrichtlijnmodule werd aan de betrokken (wetenschappelijke) verenigingen en (patiënt) organisaties voorgelegd ter commentaar. De commentaren werden verzameld en besproken met de werkgroep. Naar aanleiding van de commentaren werd de conceptrichtlijnmodule aangepast en definitief vastgesteld door de werkgroep. De definitieve richtlijnmodule werd aan de deelnemende (wetenschappelijke) verenigingen en (patiënt) organisaties voorgelegd voor autorisatie en door hen geautoriseerd dan wel geaccordeerd.
