Wat is het aanbevolen echomedium voor het uitvoeren van een echografisch onderzoek?

Bij steriel werken:
Indien de echoprobe in aanraking komt met een steriel werkveld of gedesinfecteerde huid gebruik dan niet-steriele gel in het condoom en steriele gel tussen condoom en huid.
Overwegingen
Balans tussen gewenste en ongewenste effecten
Voor deze module is er gezocht in de wetenschappelijke literatuur. Er zijn geen artikelen die voldoen aan de PICO en een directe vergelijking maakten tussen de verschillende echomedia. Hierdoor kan uit de literatuur geen conclusie getrokken worden met betrekking tot een echomedium.
Een echomedium wordt gebruikt om te zorgen voor een goede beeldkwaliteit. Echogolven kunnen zich moeilijk door de lucht verplaatsen. Een echomedium voorkomt dat er lucht tussen de echoprobe en de huid van de patiënt komt. Als echomedium wordt in de meeste gevallen steriele gel of niet-steriele gel gebruikt. Echogel wordt gebruikt zowel in het condoom, als tussen de echoprobe (al dan niet met condoom) en de patiënt.
Nota bene: Voor infectiepreventiemaatregelen rondom het uitvoeren van intra-articulaire injecties en puncties wordt verwezen naar de richtlijn Desinfectie huid en slijmvliezen plus puncties.
In de literatuur werden wel enkele studies gevonden, die een uitspraak deden over het voorkomen van infecties of contaminaties in individuele media (steriele en niet-steriele gel). Zo kon bij de uitbraak van een Burkholeria cepacia bacteriemie in 2 studies aannemelijk gemaakt worden dat deze veroorzaakt werden door gecontamineerde steriele echogel sachets (Abdelfattah, 2017; Shaban, 2017). In beide studies traden de infecties op na echogeleide invasieve echoprocedures bij patiënten op IC-afdelingen (inbrengen van centrale lijnen). Opvallend is dat bij deze beide studies tevens een condoom was gebruikt, wat in dit geval de infectie niet heeft kunnen voorkomen. Hetzelfde geldt voor een derde studie ten aanzien van een uitbraak van in dit geval Achomobacter xylosoxidans, waarbij de bron gecontamineerde niet-steriele gel uit een reservoir bleek. Het betrof hier patiënten die een transrectaal prostaatbiopt ondergingen (Olshtain-Pops, 2011).
Echogel is een voedingsbodem voor bacteriën. Echogel (zowel steriel als niet-steriel) kan besmet raken met micro-organismen door onjuist of onzorgvuldig gebruik. In uitzonderlijke gevallen kan dit leiden tot een uitbraak. Desinfectans, zoals alcohol 70% is geen voedingsbodem voor bacteriën.
Steriele gel is verpakt in individuele sachets en is duurder dan niet-steriele gel of desinfectans. Niet-steriele gel zit in verpakkingen van 250 ml of 500 ml en wordt voor diverse patiënten gebruikt totdat de fles leeg is of de houdbaarheidsdatum is bereikt, en wordt daarna weggegooid. Bovenstaande studies laten zien dat er enkele uitbraken zijn beschreven die gerelateerd zijn aan het gebruik van steriele en niet-steriele gel. Voor alcohol 70 % zijn bij de werkgroep geen studies bekend waarin een uitbraak werd beschreven gerelateerd aan alcohol 70% als echomedium.
Het is derhalve als uitvoerder van het echografisch onderzoek belangrijk te realiseren dat het gebruik van steriele gel niet automatisch veilig is. Steriele gel is een onderdeel van een bundel aan infectiepreventiemaatregelen die genomen dienen te worden bij handelingen waarbij steriel werken vereist is. Het is belangrijk dat al deze maatregelen aseptisch worden uitgevoerd. Indien de echoprobe (met condoom) in aanraking komt met een steriel werkveld of gedesinfecteerde huid dient er steriel gewerkt te worden en dient ook het echomedium tussen het steriele condoom en de patiënt steriel te zijn. De echogel die gebruikt wordt in het steriele condoom hoeft niet steriel te zijn omdat deze niet in aanraking komt met de patiënt.
Ondanks het ontbreken van degelijk onderzoek, acht de werkgroep het aannemelijk dat in situaties, waarbij een laag infectierisico bestaat, zijnde een echografisch onderzoek op de intacte huid of echografisch onderzoek waarbij de echoprobe niet in aanraking komt met een steriel werkveld, om niet-steriele gel te gebruiken als echomedium. Dit na de overweging dat echo-onderzoeken in de huidige klinische praktijk veelvuldig op deze wijze worden verricht, waarbij er geen rapportages zijn gevonden van uitbraken. Ook bij echografisch onderzoek op slijmvliezen (al dan niet met handeling) dient gebruik te worden gemaakt van niet-steriele gel omdat slijmvliezen niet steriel zijn.
Het gebruik van alcohol 70% (evt. met chloorhexidine toevoeging) kan niet worden gebruikt in het geval van echografisch onderzoek op niet-intacte huid of slijmvliezen omdat dit een irriterend agens is.
Het is onbekend in hoeverre het gebruik van een desinfectans als echomedium schade toebrengt aan een condoom bij eenmalig, kortdurend gebruik. Er zijn bij de werkgroep geen studies bekend waarin een uitbraak werd beschreven gerelateerd aan deze procedure. De werkgroep kan hierover derhalve geen advies over uitbrengen.
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
Voor patiënten is het belangrijk dat het echografisch onderzoek dat wordt uitgevoerd veilig is voor de patiënt, niet leidt tot infecties en met goede beeldkwaliteit.
Kostenaspecten
Steriele gel is van de drie opties de duurste. Niet-steriele gel en alcohol 70% (eventueel met chloorhexidine toevoeging) zijn goedkoper. Omdat er geen studies konden worden geïncludeerd kan geen uitspraak gedaan worden over de kosten in verhouding tot de effectiviteit.
Gelijkheid ((health) equity/equitable)
De interventie leidt niet tot een toe- of afname van gezondheidsgelijkheid, aangezien de procedures en producten allen binnen de reguliere zorg vallen.
Aanvaardbaarheid:
Ethische aanvaardbaarheid
De interventie lijkt aanvaardbaar voor de betrokkenen. Er zijn geen ethische bezwaren.
Duurzaamheid
Steriele gel is verpakt in individuele sachets en is minder duurzaam dan niet-steriele gel of alcohol 70%. Als het gebruik van niet-steriele gel een hogere kans geeft op infecties of uitbraken dan steriele dan zijn er daarvoor dusdanig veel materialen en middelen nodig dat dit veel minder duurzaam is.
Haalbaarheid
De interventie lijkt haalbaar. De interventie is over het algemeen al standaardzorg in de praktijk.
Rationale van de aanbeveling: weging van argumenten voor en tegen de interventies
Er konden geen studies geïncludeerd worden die voldoen aan de PICO. Echogel (zowel steriel als niet-steriel) kan besmet raken met micro-organismen door onjuist of onzorgvuldig gebruik. In uitzonderlijke gevallen kan dit leiden tot een uitbraak. Het is derhalve als uitvoerder van de echo belangrijk te realiseren dat het gebruik van steriele gel niet automatisch veilig is en op de juiste wijze gebruikt dient te worden.
De werkgroep acht het aannemelijk dat bij echografisch onderzoek op de intacte huid en echografisch onderzoek in niet-steriele omstandigheden, niet-steriele echogel als medium volstaat.
Onderbouwing
Een echomedium wordt gebruikt om een goede beeldkwaliteit te garanderen. Ultrageluidsgolven planten zich moeilijk door de lucht voort. Een echomedium voorkomt dat er lucht tussen de echoprobe en de huid van de patiënt komt.
Steriele gel en niet-steriele gel worden vaak gebruikt als echomedium tijdens echografie. In sommige gevallen wordt een desinfectiemiddel, zoals alcohol 70%, gebruikt. Het is bekend dat bacteriegroei kan optreden in echogel, zowel steriel als niet-steriel. De kosten van steriele gel zijn aanzienlijk hoger dan die van niet-steriele echogel en desinfectiemiddel, zoals alcohol 70%. Met deze module willen we het meest geschikte echomedium voor echografisch onderzoek bepalen.

Relevant outcome measures
The guideline panel considered healthcare associated infections and contamination of the gel as a critical outcome measure for decision making.
A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.
For healthcare-associated infections, the guideline panel defined a difference of 5% as a minimal clinically (patient) important difference.
For contamination, the guideline panel defined a difference of 10% as a minimal clinically (patient) important difference.
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. The detailed search strategy is available upon reasonable request via info@sri-richtlijnen.nl. Both databases were searched from inception to 25 November 2024 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from four primary search concepts: (1) ultrasound; (2) ultrasound gel; (3) alcohol 70%; (4) chlorhexidine. Duplicates were removed using EndNote software. After deduplication a total of 860 records were imported for title/abstract screening. Initially, 90 studies were selected based on title and abstract screening. After reading the full text, all studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’).

Er werd aandacht besteed aan het patiëntenperspectief door uitnodigen van Patiëntenfederatie Nederland (PFNL) voor de knelpunteninventarisatie. Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule voerde de werkgroep conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).

Te gebruiken voor richtlijntekst: A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched to 25th of November 2024 for systematic reviews, RCTs and observational studies. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from four primary search concepts: (1) ultrasound; (2) ultrasound gel; (3) alcohol 70%; (4) chlorhexidine. Duplicates were removed using EndNote software. After deduplication a total of 860 records were imported for title/abstract screening. |
