IgG onderzoek bij diagnose H. pylori infectie

Uitgangsvraag

Serologie/urine/speeksel IgG bij de diagnose van H. pylori infectie bij kinderen van 0-18 jaar.

Aanbeveling

Testen gericht op het opsporen van antilichamen (IgG, IgA) tegen H. pylori-infectiein bloed, urine en speeksel zijn niet betrouwbaar bij kinderen en worden daarom afgeraden.

Overwegingen

Bij deze uitgangsvraag werden geen overwegingen geformuleerd.

Onderbouwing

Hoog

Testen die gericht zijn op het opsporen van antilichamen (IgG, IgA) tegen  H. pylori-infectiein bloed, urine en speeksel zijn niet betrouwbaar voor gebruik in de klinische setting bij kinderen.

Koletzko et al. (6) geeft de aanbevelingen dat testen die gericht zijn op het opsporen van antilichamen (IgG, IgA) tegen H. pylori-infectiein bloed, urine en speeksel niet betrouwbaar zijn voor gebruik in de klinische setting. Dit komt met name door de lage sensitiviteit in de lagere leeftijdscategorieën en de variabiliteit per test. Sinds Koletzko zijn er geen SRs en/of originele artikelen over dit onderwerp gepubliceerd die voldeden aan onze inclusiecriteria.

In de modulen worden per uitgangsvraag de beschikbare literatuur, conclusies, overige overwegingen en aanbevelingen besproken. We zijn uitgegaan van de recente internationale richtlijn van Koletzko et al. (6) en hebben alleen artikelen geselecteerd en beschreven die gepubliceerd werden nadat de zoekactie van Koletzko et al. (6) eindigde (van september 2009 tot maart 2011).

 

De met AGREE vastgestelde domeinscores van de richtlijn van Koletzko et al. (6) waren: onderwerp en doel: 50%, betrokkenheid van belanghebbenden: 33%, methodologie: 69%, helderheid en presentatie: 71%, toepassing: 0%, onafhankelijkheid van de opstellers: 42%. De richtlijn is ontwikkeld door een internationale werkgroep van grotendeels kinderartsen MDL, maar ook epidemiologen, microbiologen en pathologen waren betrokken. De leden waren van Europese en Noord-Amerikaanse afkomst. Er werd een systematische literatuursearch gedaan en de werkgroep kwam bijeen om aanbevelingen te scoren. Artikelen werden geëvalueerd met behulp GRADE, zie tabel 2. Kanttekening bij het gebruik van deze richtlijn als uitgangspunt is dat deze niet direct op de Nederlandse situatie van toepassing is vanwege een andere prevalentie en afwijkend resistentiepatroon.

De ontwikkeling van de richtlijn H. pylori-infectie bij kinderen is gefinancierd door Stichting Kwaliteitsgelden Medisch Specialisten (SKMS)

Doel en doelgroep

Doel

De richtlijn is geschreven voor alle behandelaren die te maken hebben met deze groep kinderen in zowel de eerste-, tweede-, als derdelijnsgezondheidszorg. Het doel van deze richtlijn is te komen tot een betere herkenning, optimalisering van de diagnostiek en behandeling van kinderen van 0 tot 18 jaar met H. pylori-infectie. Om hierover gefundeerde aanbevelingen te kunnen doen, wordt een samenvatting gegeven van de wetenschappelijke stand van zaken en kennis uit de praktijk betreffende H. pylori-infectie bij kinderen volgens de methode van evidence-based richtlijnontwikkeling (4). De richtlijn kan dienen als ondersteuning in het besluitvormingsproces, moet leiden tot een verbetering van de kwaliteit en doelmatigheid van het medisch handelen en moet meer uniformiteit in de praktijkvoering teweegbrengen. De aanbevelingen die in de richtlijn staan vermeld, zijn te vertalen naar lokale protocollen die zijn toegespitst op de plaatselijke situatie. De behandelaar heeft de autonomie om, wanneer hij/zij dat nodig acht, beargumenteerd van de geformuleerde richtlijn af te wijken.

De doelgroep omvat alle kinderen van 0 tot 18 jaar met verdenking H. pylori-infectie.

De werkgroepleden hebben een belangenverklaring ingevuld waarin ze hun banden met de farmaceutische industrie hebben aangegeven. De verklaringen liggen ter inzage bij de NVK. 

Inbreng patiëntenperspectief

Het perspectief van patiënten wat betreft de zorg rondom H. pylori-infectie zou een waardevolle aanvulling zijn bij de totstandkoming van deze richtlijn. Er is echter geen patiëntenvereniging specifiek voor kinderen met H. pylori-infectie.

Implementatie en indicatorontwikkeling

In de verschillende fasen van de ontwikkeling van het concept van de richtlijn is zoveel mogelijk rekening gehouden met de implementatie van de richtlijn en de daadwerkelijke uitvoerbaarheid van de aanbevelingen. De definitieve richtlijn is onder de verenigingen verspreid en via de website van de NVK (www.nvk.nl) elektronisch beschikbaar gesteld. Op wetenschappelijke bijeenkomsten van de betrokken wetenschappelijke verenigingen zijn de aanbevelingen van de richtlijn gepresenteerd. Verder zal er patiëntenvoorlichtingsmateriaal worden ontwikkeld ter ondersteuning van de richtlijn.

Om de implementatie en evaluatie van deze richtlijn te stimuleren, zijn interne indicatoren ontwikkeld aan de hand waarvan de implementatie steekproefsgewijs kan worden gemeten. Indicatoren geven in het algemeen de zorgverleners de mogelijkheid te evalueren of zij de gewenste zorg leveren. Zij kunnen daarmee ook onderwerpen voor verbeteringen van de zorgverlening identificeren. De interne indicatoren die bij onderhavige richtlijn zijn ontwikkeld worden behandeld in aanverwant product 'Indicatoren'.

Werkwijze

Gedurende een jaar (november 2010-december 2011) is aan de ontwikkeling van de richtlijn gewerkt door leden van de werkgroep en de kernwerkgroep. Allereerst werd door door de leden van de werkgroep een knelpuntanalyse uitgevoerd om de huidige werkwijze ten aanzien van de diagnostiek en behandeling van kinderen met H. pylori-infectie in de eerste-, tweede-, en derdelijnsgezondheidszorg in Nederland in kaart te brengen. Op basis van de resultaten van de knelpuntenanalyse werden met de werkgroep de uitgangsvragen opgesteld.

 

Vervolgens werd samen met de richtlijnontwikkelaar van de NVK volgens de methode van Evidence-Based Richtlijn Ontwikkeling (EBRO) per vraag een uitgebreid literatuuronderzoek verricht. In eerste instantie werd gezocht naar evidence-based richtlijnen. Hierbij werd gebruik gemaakt de volgende databases: SUMSEARCH, Clinical evidence van BMJ, Scottish Intercollegiate Guidelines Network en de TRIP DATABASE. De gevonden richtlijnen werden op kwaliteit beoordeeld door de kernwerkgroepleden met behulp van het AGREE-instrument. Wanneer er een valide richtlijn werd gevonden werd de evidence uit de richtlijn gebruikt om de vragen te beantwoorden. De met AGREE vastgestelde domeinscores werden gebruikt als houvast voor de beoordeling van de richtlijn. Wanneer er geen geschikte richtlijn werd gevonden, werd gezocht naar systematische literatuuroverzichten in Medline en Embase.

 

Voor H. pylori-infectie werden de volgende zoektermen gebruikt: “Helicobacter pylori or Helicobacter infection or pylori”. Verder werd zoveel mogelijk gebruik gemaakt van zoekstrategieën opgesteld door Cochrane (zoals die voor kinderen, systematische reviews en gerandomiseerd onderzoek). Ook de systematische reviews werden op inhoudelijke en methodologische kwaliteit beoordeeld door de kernwerkgroepleden. Vervolgens werd naar aanvullende originele studies gezocht vanaf het moment waar de zoekactie in de review eindigde.

 

Aan elk geselecteerd artikel werd een mate van bewijskracht toegekend volgens tabel 1a. Er werd een samenvattend oordeel gegeven over de kwaliteit van de beschouwde evidence. Aan elke conclusie werd een niveau van bewijskracht toegekend volgens tabel 1b. Ook werden aanbevelingen geformuleerd. De volledige (evidence-based) uitwerking van de vragen met de daarbij geformuleerde conclusies werd geheel voorbereid door de kernwerkgroepleden. Vervolgens formuleerde de gehele werkgroep de definitieve aanbevelingen. Naast de evidence werden hierbij “overige overwegingen’’ uit de praktijk, die expliciet genoemd werden, meegenomen.

Kostenimplicaties

Door de toenemende aandacht voor de kosten in de gezondheidszorg neemt het belang van richtlijnen die doelmatig handelen bevorderen toe. In deze richtlijn is nog geen analyse gemaakt van de verwachte effecten op de kosten.
