Waar gaat deze richtlijn over?

Opioïden zijn medicamenten met een krachtige analgetische werking. Ze zijn onmisbaar bij de behandeling van pijn. Aan opioïden kleven echter ook nadelen. Naast de bijwerkingen op korte termijn zijn er ook ernstige bijwerkingen op lange termijn, zoals verslaving en opioïden geïnduceerde hyperalgesie (OIH).

 

In deze module staan een aantal generieke aanbevelingen voor volwassen operatieve patiënten en patiënten met acute pijn. In de aanbeveling wordt een ziekenhuisbreed opioïden programma beschreven met daarin tenminste de volgende elementen:

 

  • Standaard pijnmedicatieopdrachten opgenomen in het kwaliteitssysteem gericht op het restrictief voorschrijven van opioïden
  • Educatie van alle voorschrijvers van opioïden
  • Voorlichting voor alle patiënten die een mogelijke indicatie hebben voor opioïden
  • Aandacht voor de verstandige keuzes rondom opioïden (zie bijlage 1)
  • Afbouwschema volgens de Handreiking afbouw opioïden

 

Deze module gaat niet in op aandoeningsspecifieke aanbevelingen, dit hoort thuis in de individuele richtlijnen. Dit geldt ook voor de concrete invulling van voorschrijfadviezen.

 

De werkgroep heeft er bewust voor gekozen om voor andere patiëntengroepen, patiënten met chronische pijn, oncologische patiënten en palliatieve patiënten te verwijzen naar bestaande kwaliteitsdocumenten en alleen generieke organisatorische aanbevelingen op te stellen over het doorverwijzen van patiënten.

 

Voor wie is deze richtlijn bedoeld?

Deze richtlijn is bedoeld voor ziekenhuisbestuurders en alle mogelijke voorschrijvers van opioïden in de tweede en derdelijn.
