Wat is de aanbevolen diagnostische strategie om de ziekte van Creutzfeldt-Jakob vast te stellen in patiënten met snel progressieve dementie?

Overwegingen
Balans tussen gewenste en ongewenste effecten
Er is een systematisch literatuuronderzoek verricht naar de aanbevolen diagnostische strategie om de ziekte van Creutzfeldt-Jakob vast te stellen in patiënten met snel progressieve dementie. Er werden twee studies geïncludeerd (Mastrangelo, 2023; Watson, 2022). Beide onderzoeken evalueerden de diagnostische prestaties van de CSF RT-QuIC-test in vergelijking met andere beschikbare biomarkers, waaronder CSF 14-3-3-eiwit, totaal-tau (t-tau), MRI en EEG.
Mastrangelo (2023) onderzocht 1.250 patiënten en rapporteerde dat RT-QuIC de hoogste diagnostische nauwkeurigheid behaalde, met een sensitiviteit van 93.9% en specificiteit van 99.7%. De RT-QuIC was vals positief bij 1/307 patienten met een andere diagnose dan CJD, waarbij het eveneens een niet behandelbare aandoening betrof. De sensitiviteit van de andere testen varieerde van 41.2% (EEG) tot 89.9% (t-tau) en en de specificiteit van 65.3% (Total tau >1250 pg/mL) en 93.0% (EEG). Deze bevindingen ondersteunen RT-QuIC als de meest betrouwbare biomarker voor de diagnostiek van CJD.
Watson (2022) onderzocht 647 patiënten met neuropathologische bevestiging. De RT-QuIC behaalde als individuele test een sensitiviteit van 91.6% en een specificiteit van 100%. De sensitiviteit van de andere testen varieerde van 46.2% (EEG) tot 86.8% (MRI) en de specificiteit van 45.5% (CSF 14-3-3) tot 82.0% (MRI).
Beide studies tonen aan dat de RT-QuIC de hoogste diagnostische nauwkeurigheid levert bij patiënten met snel progressieve dementie. De sensitiviteit (91–94%) en specificiteit (99–100%) liggen boven de vooraf gestelde klinisch relevante drempels (≥80% sensitiviteit, ≥95% specificiteit). In vergelijking met andere diagnostische tests zoals MRI, EEG, CSF 14-3-3 en t-tau laat RT-QuIC een duidelijke absolute verbetering zien, zowel in sensitiviteit als specificiteit, wat een klinisch relevant verschil vertegenwoordigt. De meerwaarde van RT-QuIC in combinatie met andere diagnostische testen wordt onderstreept door het feit dat de gereviseerde CJD-criteria uit 2017, waarin zowel RT-QuIC als vernieuwde MRI-criteria zijn opgenomen, een hogere sensitiviteit (92% vs. 81%) hebben dan de oudere criteria, bij een vergelijkbare specificiteit (81–82%; Watson, 2022).
Hoge specificiteit is cruciaal bij CJD om fout-positieve diagnoses te vermijden, waardoor RT-QuIC de meest betrouwbare individuele biomarker is. MRI en EEG blijven aanvullend nuttig vanwege hun relatief hoge specificiteit en belang voor de diagnostiek van andere (behandelbare) oorzaken van snel progressieve dementie, terwijl 14-3-3 en t-tau alleen minder onderscheidend zijn.
Opgemerkt dient te worden dat in beide studies de t-tau/p-tau ratio niet werd beoordeeld, welke wel van toegevoegde waarde kan zijn. In een retrospectieve studie uit 2014 werd beschreven dat de specificiteit van een verhoogde t-tau/p-tau ratio (>25) in combinatie met een sterk verhoogd t-tau (>1400 pg/ml) hoger is (99%) dan van totaal tau alleen, waarbij wel dient te worden vermeld dat er waarschijnlijk sprake was van selectiebias met ondervertegenwoordiging van behandelbare oorzaken van snel progressieve dementie (Skillback, 2014). Met behulp van de t-tau/p-tau ratio kan met name onderscheid gemaakt worden tussen CJD en de ziekte van Alzheimer (Skillback, 2014; Hermann, 2022). Bij verdenking op een snel progressieve vorm van de ziekte van Alzheimer zijn, naast t-tau en p-tau, amyloid beta 40 en 42 in liquor ook relevant (Kuchenbecker, 2024). Met name bij oudere patienten (>65 jaar) is de klinische interpretatie van Alzheimerpathologie in de liquor essentieel, vanwege een relatief beperkte specificiteit voor Alzheimer dementie opin deze leeftijdscategorie (Bouwman, 2009). Alzheimerpathologie in de liquoris ook beschreven bij andere oorzaken van snel progressieve dementie, waaronder CJD en auto-immuun encefalitis (Lattanzio, 2017; Bastiaansen, 2021).
Aanvullende diagnostiek en herhaling van RT-QuIC
RT-QuIC heeft de hoogste diagnostische nauwkeurigheid voor het vaststellen van CJD (Mastrangelo, 2023; Watson, 2022). De test is echter onderdeel van een bredere diagnostische strategie bij patiënten met snel progressieve dementie, doorgaans bestaande uit een MRI-hersenen, EEG en een lumbaalpunctie. De diagnostische aanpak dient primair gericht te zijn op het vaststellen van behandelbare aandoeningen, met name auto-immuun encefalitis en andere neuro-inflammatoire aandoeningen (Day, 2022; Satyadev, 2024). Op een MRI-hersenen kunnen bij CJD op diffusie gewogen opnamen (DWI) karakteristieke afwijkingen in basale kernen en cortex worden waargenomen (Bizzi, 2020). Afwijkingen in het limbische systeem zijn zeer suggestief voor een auto-immuun encefalitis (Graus, 2016; Kelly, 2025). Op een EEG kunnen bij CJD periodieke scherpe-golf complexen worden waargenomen (Steinhoff, 2004).
De liquor t-tau/p-tau-ratio (vaak in combinatie met amyloid beta 40/42 analyses) wordt frequent bepaald als onderdeel van het standaard liquoronderzoek (Skillback, 2014). Deze ratio kan in combinatie met het t-tau helpen onderscheid te maken tussen CJD en een snel progressieve vorm van de ziekte van Alzheimer, waarbij een sterke verhoging van beiden suggestief is voor CJD (Skillback, 2014; Hermann, 2022). De kwantitatieve meerwaarde van deze bepaling is echter nog onvoldoende vastgesteld, waardoor verdere validatie wenselijk is. In de liquor kan, naast reeds besproken diagnostiek gericht op CJD en andere neurodegeneratieve ziekten, gekeken worden naar de aanwezigheid van neuronale antistoffen die geassocieerd zijn met auto-immuun encefalitis (Graus, 2024).
Het cluster acht op basis van consensus dat bij een aanhoudende verdenking op CJD ondanks een negatieve RT-QuIC-uitslag, herhaling van RT-QuIC na vier tot zes weken gerechtvaardigd is. Factoren die geassocieerd zijn met een negatieve RT-QuIC zijn onder andere een lage leeftijd en traag ziektebeloop (Rhoads, 2020; Jones, 2023). In de vroege fase van CJD is het pathogene prioneiwit mogelijk nog niet detecteerbaar; bij herhaling later in het ziektebeloop kan de test alsnog positief worden; dit is echter gebaseerd op observaties bij een zeer klein aantal patiënten (Rhoads, 2020; Jones, 2023). Benadrukt dient te worden dat er bij een negatieve RT-QuIC in de meeste gevallen geen sprake is van CJD en verder beleid in de eerste plaats gericht moet zijn op het vaststellen van andere oorzaken.
Een speciale subgroep betreft patiënten met genetische CJD, welke naar schatting 10-15% vertegenwoordigen van alle CJD patiënten. De RT-QuiC heeft een hoge sensitiviteit voor genetische CJD met veelvoorkomende mutaties E200K en V210I (~90%), maar deze is beduidend lager bij de andere genetische prionziekten, zoals fatale familiaire insomnia (FFI) en het syndroom van Gerstmann-Sträussler-Scheinker (~30-40%) (Schmitz, 2022). Bij iedere patiënt met waarschijnlijke CJD valt opslag van DNA te overwegen voor eventueel toekomstig genetisch onderzoek.
De balans tussen gewenste en ongewenste effecten is daarmee positief: de combinatie van RT-QuIC, MRI, EEG en aanvullende liquoranalyses (inclusief t-tau/p-tau-ratio) verhoogt de diagnostische zekerheid en helpt behandelbare alternatieven tijdig te identificeren, terwijl de extra belasting voor patiënt en zorgverlener gering blijft. Het cluster beschouwt deze geïntegreerde aanpak als de meest betrouwbare en haalbare diagnostische strategie bij verdenking op CJD.
Kwaliteit van bewijs
De algehele bewijskracht is moderate (zie SoF-tabel 1), wat betekent dat er redelijke zekerheid is dat het ware effect dicht bij het geschatte effect ligt, maar dat de conclusie (klinisch relevant) kan veranderen wanneer resultaten van nieuw grootschalig onderzoek aan de literatuuranalyse worden toegevoegd.
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
Het stellen van een zekere diagnose bij snel progressieve dementie is voor patiënten en hun naasten van groot belang. Duidelijkheid over de aard van de aandoening geeft richting aan vervolgstappen, begeleiding en palliatieve zorg. Het kunnen uitsluiten van CJD voorkomt onnodige angst en medische of maatschappelijke consequenties van een onterechte fatale diagnose.
De diagnostische strategie waarin RT-QuIC, MRI en aanvullende biomarker analyse (inclusief tau/p-tau-ratio) gecombineerd worden, sluit aan bij deze waarden: zij biedt zekerheid en voorkomt langdurige diagnostische onzekerheid. De belasting van de benodigde onderzoeken, waaronder lumbaalpunctie en beeldvorming, wordt door de meeste patiënten als acceptabel ervaren gezien de ernst van het ziektebeeld.
Hoewel de teststrategie in principe voor iedereen dezelfde meerwaarde heeft, kan de wens tot verdere diagnostiek verschillen: bij kwetsbare of oudere patiënten kan de nadruk liggen op comfort en beperkte belasting, terwijl bij jongere patiënten en hun familie juist behoefte bestaat aan maximale diagnostische zekerheid. De gewenste effecten zijn daarmee groot, de ongewenste effecten klein, en de teststrategie wordt over het algemeen als aanvaardbaar beschouwd.
Kostenaspecten
De RT-QuIC-test brengt hogere directe laboratoriumkosten met zich mee dan conventionele onderzoeken zoals CSF 14-3-3, totaal-tau of EEG. Het kostenverschil wordt als matig ingeschat, maar precieze bedragen verschillen per laboratorium en vergoedingsstructuur.
Daartegenover staat dat een positieve RT-QuIC-uitslag doorgaans direct diagnostische duidelijkheid geeft, waarmee onnodige vervolgonderzoeken (zoals herhaalde MRI, aanvullende liquoranalyses of langdurige klinische observatie) en gerelateerde zorgkosten kunnen worden voorkomen. De hogere testkosten worden daardoor grotendeels gecompenseerd door vermindering van aanvullende diagnostiek en consultaties, en door tijdige start van passende begeleiding en infectiepreventieve maatregelen.
Bij een negatieve RT-QuIC-uitslag blijven doorgaans aanvullende onderzoeken nodig om alternatieve oorzaken, zoals auto-immuun encefalitis of ziekte van Alzheimer, te identificeren. In die gevallen nemen de totale kosten toe, maar dit wordt gerechtvaardigd door het belang van een juiste differentiaaldiagnose en de mogelijkheid van behandelbare alternatieve aandoeningen.
Per saldo weegt het hogere kostenprofiel van RT-QuIC wel op tegen het verschil in accuratesse en de daaruit voortvloeiende besparingen in vervolgdiagnostiek en zorgtrajecten. De test wordt daarom als kosteneffectief binnen de diagnostische strategie beschouwd, mede gezien de substantiële klinische en maatschappelijke winst van vroegtijdige en zekere diagnose voor patiënt en naasten.
Gelijkheid ((health) equity/equitable)
De diagnostische strategie voor CJD - waarin RT-QuIC wordt gecombineerd met MRI en aanvullende biomarker analyse (zoals tau/p-tau-ratio) - leidt mogelijk tot een toename van gezondheidsgelijkheid. De test is beschikbaar voor alle patiënten bij wie op basis van klinisch beeld of verwijzing door een specialist een verdenking op CJD bestaat, ongeacht leeftijd, geslacht, etniciteit of sociaaleconomische status.
Aangezien de RT-QuIC-test centraal wordt uitgevoerd (momenteel in één nationaal referentielaboratorium) en op indicatie kan worden aangevraagd vanuit alle ziekenhuizen, is de toegang landelijk uniform geregeld. Eventuele reisafstand of tijdsvertraging heeft geen invloed op de mogelijkheid tot testen, omdat het om laboratoriumanalyse van een standaard verkregen liquorafname gaat.
Daarmee draagt de teststrategie bij aan gelijke toegang tot diagnostiek van hoge kwaliteit voor alle patiënten met snel progressieve dementie, ongeacht woonplaats of type ziekenhuis. Er zijn geen aanwijzingen dat specifieke patiëntengroepen (zoals ouderen, mensen met kwetsbare gezondheid of etnische minderheden) benadeeld zijn in toegang of interpretatie van de testresultaten.
Aanvaardbaarheid:
Ethische aanvaardbaarheid
De diagnostiek voor de ziekte van Creutzfeldt-Jakob, inclusief RT-QuIC, wordt als ethisch aanvaardbaar beschouwd. Er zijn geen specifieke ethische bezwaren tegen het uitvoeren van de test. Integendeel, het verkrijgen van een zekere diagnose bij een snel progressieve dementie wordt door patiënten en naasten vaak als waardevol ervaren vanwege de duidelijkheid over prognose, risico’s voor transmissie en de mogelijkheid tot tijdige begeleiding. Het gebruik van RT-QuIC draagt bij aan een zorgvuldig en verantwoord diagnostisch proces, waarin onnodige foutieve diagnoses worden voorkomen.
Duurzaamheid
Bij de uitvoering van de RT-QuIC-test spelen beperkte duurzaamheidsaspecten een rol, met name gerelateerd aan laboratoriumverbruik (zoals reagentia en verpakkingen) en afvalverwerking. In vergelijking met alternatieve testmodaliteiten (bijvoorbeeld herhaalde MRI of aanvullende laboratoriumbepalingen) is de milieu-impact naar verwachting gering en proportioneel in relatie tot de diagnostische opbrengst.
De concentratie van RT-QuIC-analyse in één nationaal referentielaboratorium bevordert efficiënt gebruik van materialen en apparatuur, wat de milieubelasting per test vermindert. Daarmee wordt de test, ook vanuit duurzaamheidsperspectief, als aanvaardbaar en verantwoord beschouwd binnen de diagnostische strategie voor CJD.
Haalbaarheid
De beschreven diagnostische strategie wordt als goed haalbaar beschouwd binnen de huidige Nederlandse zorgpraktijk. De RT-QuIC-test wordt momenteel centraal uitgevoerd in één gespecialiseerd referentielaboratorium, dat op dit moment nog ruim voldoende capaciteit heeft voor een toename van het aantal aanvragen.
De verwachte toename in testaanvragen (evenredig aan het aantal patiënten met snel progressieve dementie bij wie CJD wordt overwogen) zal geen onevenredig beslag leggen op middelen of menskracht. De benodigde diagnostische stappen - lumbaalpunctie, MRI, EEG en aanvullende CSF-bepalingen - maken reeds onderdeel uit van de standaard diagnostiek bij deze patiëntengroep.
Er worden geen wezenlijke organisatorische of logistieke belemmeringen voorzien voor landelijke implementatie. Wel is blijvende afstemming nodig tussen verwijzende centra en het referentielaboratorium om doorlooptijden te bewaken en tijdige rapportage te garanderen. Het gebruik van bestaande diagnostische infrastructuur maakt de uitvoering van deze teststrategie zowel praktisch als financieel haalbaar.
Rationale van aanbeveling-1: weging van argumenten voor en tegen de indextest
De RT-QuIC test voor het aantonen van verkeerd gevouwen prion eiwit in het hersenvocht heeft een hoge diagnostische nauwkeurigheid (zowel sensitiviteit als specificiteit) voor de meldingsplichtige diagnose CJD. RT-QuIC heeft de hoogste diagnostische nauwkeurigheid en maakt het mogelijk de diagnose CJD tijdens leven met grote zekerheid te stellen. De test draagt bij aan tijdige counseling, infectiepreventie en het vermijden van fout-positieve diagnoses.

Rationale van aanbeveling-2: weging van argumenten voor en tegen de indextest
MRI en liquoronderzoek (waaronder de t-tau/p-tau-ratio, eventueel aangevuld met amyloid beta40/42 eiwitten en neuronale antistoffen) leveren aanvullende informatie die helpt bij het onderscheiden van CJD en behandelbare alternatieve aandoeningen. De gecombineerde toepassing van deze modaliteiten vergroot de diagnostische zekerheid.

Rationale van aanbeveling-3: weging van argumenten voor en tegen de indextest voor subgroep genetische CJD
Bij patiënten met (verdenking op) genetische prionziekten anders dan CJD is de sensitiviteit van de RT-QuIC-test lager dan bij sporadische CJD. De diagnostische zekerheid kan in deze subgroep alleen worden bereikt door aanvullend genetisch onderzoek naar PRNP-mutaties.

Onderbouwing
Creutzfeldt-Jakob disease is a fatal neurodegenerative disease that is subject to mandatory registration and surveillance. The clinical presentation is heterogeneous, most commonly characterized by rapidly progressive dementia, cerebellar ataxia, and myoclonus. The primary question addresses the optimal diagnostic strategy for Creutzfeldt-Jakob disease (CJD) in patients presenting with rapidly progressive dementia. The impetus for this inquiry is the introduction of a novel cerebrospinal fluid assay, real-time quaking-induced conversion (RT-QuIC) for the prion protein (PrP), which demonstrates high sensitivity and specificity and has significant implications for current diagnostic algorithms. This advance necessitates revision of the existing guidelines, with particular emphasis on the role of RT-QuIC and a reappraisal of the diagnostic value of complementary investigations.

Description of studies
A total of two studies were included in the analysis of the literature. Important study characteristics and results are summarized in table 3. The assessment of the risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).
Mastrangelo (2023) conducted a retrospective analysis of a prospectively collected national surveillance cohort to evaluate the diagnostic impact of cerebrospinal fluid (CSF) prion Real-Time Quaking-Induced Conversion (RT-QuIC) and the amended 2017 European Union (EU) diagnostic criteria for sporadic Creutzfeldt-Jakob disease (sCJD).
Between January 2010 and December 2020, 3347 patients presenting with rapidly progressive dementia or neurological decline suspected for CJD were consecutively referred to two Italian national reference centers: the Institute of Neurological Sciences of Bologna (ISNB) and the Istituto Superiore di Sanità (ISS) in Rome. Patients were included if they met at least one of the following criteria: 1) positive or uncertain CSF 14-3-3 assay, 2) CSF total tau concentration >600 pg/mL, 3) MRI findings consistent with CJD, or 4) a clinical course compatible with possible CJD. After applying these inclusion criteria, 1250 patients were selected for analysis. Each patient was assessed at the time of first diagnostic evaluation according to both the updated WHO (2009) and the EU (2017) criteria, and final diagnoses were established based on neuropathology, PRNP gene sequencing, and/or complete follow-up data. Among the 850 CJD cases, 297 were definite sporadic CJD confirmed by neuropathology, 151 were genetic CJD confirmed by PRNP sequencing, and 402 were classified as probable sCJD based on clinical and biomarker follow-up. CSF analyses included 14-3-3 and tau protein quantification using immunoassays, while RT-QuIC was performed using either full-length (PQ-CSF) or truncated (IQ-CSF) recombinant prion proteins as substrates.
Watson (2022) conducted a multinational multicenter diagnostic study based on prospectively collected surveillance data to validate the revised 2017 International Creutzfeldt-Jakob Disease (CJD) Surveillance Network diagnostic criteria for sCJD. The study included patients presenting with rapidly progressive cognitive decline who were assessed through four national surveillance centers in the United Kingdom, France, Germany, and Italy between January 2017 and December 2019. All cases had neuropathological confirmation of diagnosis through autopsy or biopsy. A total of 501 autopsy-confirmed sCJD cases and 146 noncases with alternative neuropathological diagnoses (e.g., Alzheimer’s disease, autoimmune encephalitis, anoxic brain injury) were included. Clinical features, brain MRI, EEG, cerebrospinal fluid (CSF) 14-3-3 protein, and CSF real-time quaking-induced conversion (RT-QuIC) assay results were collected prospectively as part of routine national surveillance and retrospectively analyzed. Results
Specificity and sensitivity
Two large diagnostic accuracy studies (Watson, 2022; Mastrangelo, 2023) evaluated the performance of the cerebrospinal fluid (CSF) real-time quaking-induced conversion (RT-QuIC) assay for the detection of Creutzfeldt–Jakob disease (CJD) in patients with rapidly progressive dementia, compared with established diagnostic tests such as MRI, EEG, CSF 14-3-3 protein, and total tau. Together, these studies included more than 1,800 participants with suspected CJD.
In the study by Watson (2022), which included 501 autopsy-confirmed CJD cases and 146 non-CJD controls, the RT-QuIC assay showed a sensitivity of 91.6 % (95 % CI 87.7–94.6) and a specificity of 100 % (95 % CI 96.2–100). Comparator tests showed lower performance, with MRI yielding 86.8% sensitivity and 82.0% specificity, EEG 46.2 % and 88.1%, and CSF 14-3-3 protein 72% and 45.5%.
In the multicenter study by Mastrangelo (2023), involving 850 patients with CJD and 400 without CJD (61 with neuropathological confirmation), RT-QuIC demonstrated a sensitivity of 93.9% and a specificity of 99.7%. Comparator tests again performed less well: MRI 75.5% / 92.8%, EEG 41.2% / 93.0%, CSF 14-3-3 protein 76.8% / 71.1% and total tau 89.9% / 65.3%,

Mastrangelo (2023) compared the 2017 EU amended diagnostic criteria incorporating RT-QuIC with the previous 2009 WHO/International CJD Surveillance Network criteria.
The amended 2017 criteria demonstrated a higher sensitivity (98.8%, 95% CI 97.8–99.4) but a lower specificity (93.7%, 95% CI 90.9–95.7) compared with the previous WHO/International CJD Surveillance Network criteria (sensitivity 76.8%, 95% CI 72.7–80.5; specificity 94.0%, 95% CI 91.2–95.9).
Watson (2022) compared the revised2017 International CJD Surveillance Network diagnostic criteria, which incorporate RT-QuIC and cortical ribboning on MRI, with the prior surveillance criteria. The revised criteria showed higher sensitivity (92.2%, 95% CI 89.5–94.4 with partial investigations; 97.8%, 95% CI 94.9–99.3 with all investigations available) compared with the prior criteria (77.5%, 95% CI 73.5–81.1; 76.2%, 95% CI 70.1–81.7), while specificity remained similar between the revised (80.8%, 95% CI 72.8–87.3; 67.3%, 95% CI 52.9–79.7) and prior criteria (81.6%, 95% CI 73.7–88.0; 69.2%, 95% CI 54.9–81.3).
Positive predictive value
Both studies did not report on the outcome positive predictive value.
Negative predictive value
Both studies did not report on the outcome negative predictive value.
AUC
Mastrangelo (2023) conducted ROC analyses to assess the diagnostic accuracy of CSF RT-QuIC alone. The AUC was 0.964 (95% CI 0.940–0.980) when compared against all non-CJD cases (n = 220) and 0.966 (95% CI 0.931–0.987) when compared against definite non-CJD cases (n = 38). No AUC values were reported for MRI, EEG, 14-3-3 protein, or total tau individually. Watson (2022) did not report AUC values or ROC curve analyses. Diagnostic performance was expressed only in terms of sensitivity and specificity.
The ideal body of evidence would include studies that directly compare the test strategies under consideration (i.e., randomized trials) and the resulting interventions and consequences for patients (i.e., patient-important outcomes). Such studies would, by design, address all of the issues in the analytical framework and allow guideline panelists to apply the familiar GRADE approach for interventions’ (Schünemann, 2019). If this direct evidence does not exist, a search question on test accuracy outcomes is formulated. Connecting test accuracy to downstream consequences is required for decision-making.
A systematic review of the literature was performed to answer the following question:
What is the diagnostic value of the RT-QuIC prion test in comparison to other diagnostic methods (MRI, EEG, 14-3-3 protein, and CSF tau) for identifying Creutzfeldt-Jakob disease in patients with rapidly progressive dementia?

Relevant outcome measures
The guideline panel considered specificity as a critical outcome measure for decision making; and sensitivity, AUC, positive predictive value, negative predictive value as an important outcome measure for decision making. From a clinical perspective, high specificity is more important than sensitivity. In other words, avoiding false-positive diagnoses is more important than avoiding false-negative diagnoses, due to the absence of disease-modifying treatments and the severe clinical and ethical consequences of an incorrect CJD diagnosis (see Table 2).

A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.
The guideline panel defined the following thresholds as minimal clinically important differences for diagnostic accuracy outcomes:
- Specificity: A test specificity of ≥95% was considered clinically relevant, as avoiding false-positive diagnoses is critical given the fatal prognosis and lack of disease-modifying treatment.
• Sensitivity: A test sensitivity of ≥80% was considered clinically relevant, to ensure timely recognition and appropriate counselling.
In comparative analyses, the panel considered a ≥10% absolute improvement in specificity or sensitivity over established comparator tests (MRI, EEG, CSF 14-3-3, total tau) to represent a clinically relevant difference.
These thresholds reflect that high specificity is more important than high sensitivity in the diagnostic evaluation of CJD, where false-positive results have major clinical, psychological, and ethical consequences.
Search and select (Methods)
A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all. Both databases were searched from 2005 to August 26th, 2025, for systematic reviews, RCTs, observational and other studies. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) dementia; (2) RT-QuIC prion test; (3) diagnostic accuracy. Duplicates were removed using EndNote software. After deduplication a total of 309 records were imported for title/abstract screening.
Initially, 24 studies were selected based on title and abstract screening. After reading the full text, 22 studies were excluded (see the exclusion table under the tab ‘Evidence tabellen’), and two studies were included.

Consultancy werkzaamheden diverse bedrijven (betaald, met alle betalingen aan mijn instelling) 1. Werk voor British Journal of Anaesthesia (van sept 2011 t/m jan 2022 was ik editor). Sinds Sept 2022 “trustee” met portfolio “financial director” van de BJA Charitable Company. Ik heb nu geen invloed op het wetenschappelijk beleid; maar ben vaak reviewer van farmacologische artikelen (waarvoor ik per kwartaal een honorarium ontvang). 2. Actieve consultancy werk en PI-schap van een sponsor (Philips) – geinitieerd pijn-project. Geen conflict m.b.t. dementie/MCI/delier. Advieswerkzaamheden/medische adviesraad – met betrekking tot een voorgestelde pijnmonitor voor intra-operatief gebruik. Onbeperkte onderzoekssubsidie voor de uitvoering van een geplande studie om gegevens te verzamelen die nodig zijn voor de ontwikkeling van een dergelijke monitor. Deze studie staat op het punt te starten, waardoor deze samenwerking niet kan worden opgeschort of beëindigd. 3. Vorige consultancy werk voor Orion (> 5 jaar geleden). Consultancy advice (dexmedetomidine). 4. Vorige consultancy werk voor Ever Pharma (mbt dexmedetomidine). Hiervoor gaf ik advies over een EMA aanvraag voor registratie (van een nieuwe formulatie) en indicatie uitbreiding van dexmedetomidine voor procedurele sedatie. In dit werk was de focus alleen op het gebruik van dit middel voor sedatie. Er bestaat wetenschappelijke bewijs dat dexmedetomidine toediening tijdens een operatie onder algehele narkose de kansen van post-operatieve delier verlaagt. Ook wordt dit middel soms toegediend ter behandeling van “emergence agitation” (dwz onrust en verwardheid direct na ontwaking uit narcose). Deze indicaties zijn niet geregistreerde indicaties, en ik heb Ever Pharma dan ook geen raad hierover gegeven. De link met dementie/MCI/delier is daarom zwak. 5. Vorige consultancy werk voor Ever Pharma en PAION – hiervoor gaf ik advies aan Ever en PAION over (potentiele aankoop van een generische medicaties, en/of indicatie stellingen) van medicaties met geen (potentiele) link met dementie/MCI/delier; en niet meer actief. 6. Consultancy werk voor Becton Dickinson (Eysins, Switzerland)** en Terumo (Tokyo, Japan)* – technische advies over spuitpompen. *Deelname aan de commissie is erkenning van het potentiële belang van de vloeistof biomarkers voor de diagnostiek op zich, wat belangrijk is voor mijn vakgebied. Het is ook eervol en daardoor mogelijk goed voor mijn reputatie als wetenschapper. *Mocht de richtlijn een belangrijke rol voor vloeistofbepalingen zien, dan zou dat misschien tot meer productie in het klinische chemisch lab van VUmc kunnen leiden. Daar heb ik persoonlijk geen baat bij, evenmin als mijn onderzoeksgroep. Wel kan de grotere productie leiden tot een lagere kostprijs. *Aanvullingen op de bedrijven waar mijn lab contractresearch voor doet. Te gebruiken voor richtlijntekst: A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all. Both databases were searched from 2005 to August 26th 2025 for systematic reviews, RCTs, observational and other studies. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) dementia; (2) RT-QuIC prion test; (3) diagnostic accuracy. Duplicates were removed using EndNote software. After deduplication a total of 309 records were imported for title/abstract screening. |
