Uitgangsvraag

Wat is de optimale strategie met betrekking tot het stoppen van cholinesteraseremmers bij patiënten met dementie die cholinesteraseremmers gebruiken?

Aanbeveling

Bespreek periodiek met patiënt en naasten tijdens progressie van dementie door de ziekte van Alzheimer de voor- en nadelen over het voortzetten van de behandeling van cholinesteraseremmers.

 

Bespreek het continueren van cholinesteraseremmers bij ziekteprogressie binnen indicatiegebied van lichte tot matig ernstige dementie door de ziekte van Alzheimer gezien geen verschil in achteruitgang van cognitief en dagelijks functioneren na staken of continueren van cholinesterasremmers.

Overwegingen

Licht tot matig ernstige dementie

De gepoolde resultaten uit de studies met patiënten met licht tot matige dementie van ziekte van Alzheimer lieten op korte en middellange termijn geen duidelijke verschillen zien in cognitieve functie tussen staken of continueren van cholinesteraseremmers. De risico ratio voor het optreden van gastro-intestinale complicaties lijkt lager bij het staken van cholinesteraseremmers, hoewel dit in de gepoolde resultaten niet klinisch relevant is gezien de grote betrouwbaarheidsintervallen (Gaudig, 2011; Holmes, 2004; Scarpini, 2011; Johannsen, 2006; Moo, 2021). De onderzoekstermijn van de geïncludeerde studies bij licht tot matige dementie besloeg alleen de korte en middellange termijn. Een recent gepubliceerde target trial emulation studie (Lecerf, 2026) beschrijft een significant verschil in MMSE daling van 0.97 punt na 1 jaar en van 1.81 punt na 4 jaar tussen patiënten waarbij cholinesterasremmers wel of niet gestaakt waren, waarbij geen verschil in mortaliteit werd gezien.

 

Gevorderde dementie

De gepoolde resultaten lieten een mogelijke verslechtering op de lange termijn in cognitieve functie van gemiddeld 2,09 punten gerapporteerd middels de MMSE score zien voor de patiënten waarbij cholinesteraseremmers werden gestopt ten opzichte van patiënten waarbij deze werden gecontinueerd. Deze resultaten op middel en lange termijn zijn gebaseerd op de studie van Howard (2012), waarbij uitsluitend patiënten met gevorderde dementie van ziekte van Alzheimer (MMSE 5 tot 13) waren geïncludeerd. Hoewel deze bevinding methodologisch als klinisch relevant kan worden bestempeld, is de klinische relevantie van dit verschil van 2 punten op de MMSE score in de praktijk beperkt bij patiënten met reeds een vergevorderd stadium van dementie. Het staken van cholinesteraseremmers leidt ook niet tot toename in gedragssymptomen, gemeten met NPI score. Het beperkte klinische voordeel moet per patiënt worden afgewogen tegen het optreden van bijwerkingen, waarbij patiënten in een verder gevorderd stadium mogelijk een hoger risico hebben op het optreden van bijwerkingen gezien reeds bestaand laag lichaamsgewicht of algehele kwetsbaarheid. Het staken van cholinesteraseremmers liet in de studie van Hermann (2016), bij patiënten met een gevorderde dementie van ziekte van Alzheimer (MMSE 6-10), een verlaagde risico ratio zien voor bradycardie ten opzichte van het continueren van de cholinesteraseremmers. De risico ratio voor het optreden van gastro-intestinale complicaties lijkt lager bij het staken van cholinesteraseremmers, hoewel dit in de gepoolde resultaten niet klinisch relevant is gezien de grote betrouwbaarheidsintervallen. Wel is bekend dat gastro-intestinale bijwerkingen en gewichtsverlies vaak optreden (1-10%), met name in de titratiefase (Birks, 2015; EPHOR, 2022).

 

Algemeen

Internationale deprescribing richtlijnen benoemen ook het belang van het afwegen van resultaat en risico voor de individuele patiënt, waarbij verminderd effect van cholinesteraseremmers is benoemd bij patiënten met een verder gevorderd stadium van dementie en een beperkte levensverwachting, en vergroot risico bij patiënten met ondergewicht, kwetsbaarheid en comorbiditeiten. Het op proef staken van cholinesteraseremmers kan volgens deze aanbeveling worden overwogen bij patiënten met ziekte van Alzheimer, Lewy Body Dementie, Parkinson dementie, vasculaire dementie en samengestelde varianten van dementie nadat de medicatie voor minstens 12 maanden is gebruikt (Primary Health Tasmania, 2022).

 

De aanwezigheid van polyfarmacie geeft een additioneel risico door potentiële versterking van farmacologische effecten of bijwerkingen zoals bradycardie enerzijds of interacterende werking, bijvoorbeeld door inzet van urologische spasmolytica anderzijds. Bij patiënten met verminderde therapietrouw of moeite met innemen van medicatie is dit een potentiële extra reden om de therapie te staken. Eventueel kan bij verminderde therapietrouw besloten worden tot medicatie inname onder toezicht.

 

Kwaliteit van bewijs

De overall kwaliteit van bewijs is zeer laag. Dit betekent dat we zeer onzeker zijn over het gevonden geschatte effect van de cruciale uitkomstmaten.

Er is afgewaardeerd vanwege (zeer) ernstige:

  • Risk of Bias: methodologische beperkingen (ontbrekende informatie over de randomisatie procedure en blindering, mogelijke selectie bias, vertekening door uitval van patiënten, selectieve rapportage van uitkomsten);
  • Imprecisie: onnauwkeurigheid, omdat het betrouwbaarheidsinterval de grenzen van klinische relevantie overschrijdt en vanwege het kleine aantal patiënten. 

Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)

Parsons (2019) beschrijven in een kwalitatieve studie de perspectieven en ervaringen van mantelzorgers rondom het stoppen van cholinesteraseremmers bij patiënten met gevorderde dementie. Uit de studie komen vier verschillende perspectieven en ervaringen van mantelzorgers naar voren met betrekking tot de klinische conditie van patiënt na het staken van deze medicatie:

  1. Geen merkbare verandering in klinische conditie.
  2. Verbetering van de klinische conditie, waaronder vermindering van gedrags- en psychologische symptomen van dementie (zoals stemming, agressie en angst), evenals verbetering van de fysieke conditie en functionele status door beter eten, drinken en bewegen.
  3. Verslechtering van de klinische conditie, zowel in gedrags- en psychologische symptomen als in fysieke gezondheid.
  4. Gemengd beeld, waarbij zowel verbetering als achteruitgang wordt gezien, bijvoorbeeld verbetering van psychologische symptomen maar verslechtering van de fysieke conditie, of andersom.

Daarentegen zijn mantelzorgers vaak onzeker over het nut van cholinesteraseremmers, omdat de effecten niet objectief meetbaar zijn. Onzekerheid bestaat of achteruitgang van cognitieve functie het gevolg is van stoppen met cholinesteraseremmers of van de natuurlijke ziekteprogressie, zoals beschreven door McCloskey (2018). Besluitvorming over de medicatie wordt door mantelzorgers als complex en emotioneel belastend ervaren. Na stoppen van de cholinesteraseremmers rapporteren mantelzorgers gevoelens van zorg, stress, schuld en zelfverwijt, vooral wanneer zij denken dat hun beslissing een negatieve impact heeft gehad op hun naaste. Echter sommige mantelzorgers rapporteren daarentegen juist een gevoel van comfort of opluchting.

 

Gezien de uiteenlopende ervaringen en perspectieven van mantelzorgers rondom het stoppen van cholinesteraseremmers bij patiënten met gevorderde dementie is duidelijke communicatie tussen zorgverlener en mantelzorger over de indicatie, risico’s en baten van de medicatie essentieel. Bespreek verwachtingen van mantelzorgers hierbij rondom het staken van cholinesteraseremmers. Neem het besluit gezamenlijk, op basis van goede informatie en overleg. En zorg voor actieve begeleiding van zowel patiënt als mantelzorger na het besluit tot staken. Hierbij kan overwogen worden om 8-12 weken na staken van cholinesteraseremmers een afspraak in te plannen met patiënt en mantelzorger om het dagelijks functioneren te beoordelen. 

 

Kostenaspecten

De interventie levert naar verwachting gelijke kosten op ten opzichte van de controle behandeling. De inzet van cholinesteraseremmers bij milde tot matige dementie in een kosteneffectiviteitsanalyse met >99% waarschijnlijkheid is kosteneffectief bevonden (Bond, 2012). De in Nederland in de handel zijnde cholinesteraseremmers zijn allen uit patent en hebben een lage kostprijs van <2 euro per dag (Medicijnkosten.nl, 2025). Bij patiënten met milde tot matige dementie waarbij cholinesteraseremmers worden gecontinueerd, blijven de kosten gelijk.

 

Het is niet duidelijk wat de kosten zijn van het staken van de cholinesteraseremmers. Bij patiënten met reeds gevorderde dementie zijn de kosten met betrekking tot geleverde (chronische) zorg en mantelzorg aanzienlijk en zijn de medicatiekosten slechts een zeer gering deel. Het is niet de verwachting dat een potentiële, waarschijnlijk niet klinisch relevante verslechtering in cognitie en daarmee zorgzwaarte na staken van cholinesteraseremmers bijdraagt aan hogere kosten in deze zorg.

 

Gelijkheid ((health) equity/equitable)

De interventie leidt niet tot een toename of afname van gezondheidsgelijkheid. Het effect van het stoppen van cholinesteraseremmers wordt niet beïnvloed door ongelijkheden in gezondheid die ontstaan door factoren zoals sociaaleconomische status, opleidingsniveau, woonomgeving, ras of andere sociale determinanten. In de toekomst kan bij toenemende arbeidsmarkt tekorten beschikbaarheid van thuiszorg voor medicatie inname of opplakken rivastigmine pleister mogelijk wel regionaal verschillen opleveren.

 

Aanvaardbaarheid

Ethische aanvaardbaarheid

De interventie lijkt aanvaardbaar voor de betrokkenen. De werkgroep voorziet geen ethische bezwaren.

 

Duurzaamheid

Bij de interventie spelen verschillende duurzaamheidsaspecten een rol. Zo wordt onder andere ingezet op het voorkomen van het gebruik van cholinesteraseremmers (R1 Refuse) (NVvH, 2025), wat kan bijdragen aan een vermindering van afval afkomstig van medicatiestrips. Daarnaast leidt het terugdringen van het gebruik van cholinesteraseremmers op de langere termijn tot een lagere vraag naar grondstoffen (R2 Reduce) (NVvH, 2025). 

 

Haalbaarheid

De interventie lijkt wel haalbaar. De interventie is over het algemeen al toepasbaar in de zorg. Samen met de patiënt en naasten moeten de voor- en nadelen van het staken van cholinesteraseremmers worden afgewogen. Afhankelijk van de lokale situatie en de situatie van de patiënt kan deze interventie door verschillende zorgverleners worden uitgevoerd.

 

Rationale van de aanbevelingen

Gezien de beperkte klinische relevantie en onzekerheid van de geobserveerde uitkomsten uit de studies is het belang van gezamenlijke besluitvorming tussen patiënt, mantelzorger en arts over het staken van cholinesteraseremmers groot. We geven een sterke aanbeveling tot het samen beslissen gesprek.

Onderbouwing

In patients newly diagnosed with dementia, particularly Alzheimer’s disease, cholinesterase inhibitors (ChEIs) are often initiated as a first-line symptomatic treatment, offering modest short- to medium-term benefits. However, their long-term effectiveness is uncertain, and not all patients respond. Despite this, many patients continue on ChEIs indefinitely due to lack of clear guidance on when to reassess or discontinue therapy. The central issue is variation in practice and uncertainty about the optimal timing and criteria for stopping ChEIs.

Description of studies

A total of two studies were included in the analysis of the literature. The assessment of the risk of bias is summarized in the risk of bias tables (under the tab ‘Evidence tabellen’).

 

Parsons (2021) performed a systematic review and meta-analysis to evaluate the effects of withdrawal or continuation of cholinesterase inhibitors (ChEIs) or memantine, or both, in people with Alzheimer’s disease. A systematic search was performed up to 17 October 2020 in relevant databases using search terms appropriate for the identification of reports of trials studying ChEIs (donepezil, rivastigmine, galantamine and tacrine) and memantine. RCTs were included that compared withdrawal of ChEIs or memantine, or both, with continuation of the same drug or drugs. A total of seven studies were included. Of these, one study was not included in our analyses because the study did not differentiate between participants who were discontinuing donepezil and those discontinuing memantine for all outcome measures (Hong, 2018).

 

Moo (2021) performed an RCT to study the long-term effects of ChEIs used in the treatment of patients with various types of dementia. The study was part of a multisite Veterans Affairs (VA) Clinical Science Research and Development-funded double-blind, placebo-controlled study. Pharmacy records were used to identify participants who had been dispensed the study medications (donepezil, galantamine, or oral rivastigmine) for 1 year or more, regardless of diagnosis or symptoms. Potential participants were contacted via telephone for discussion of the study aims and procedures. Because the study was underpowered, it had to be terminated early. The manuscript focuses on a subset of participants (n=6) with Parkinson disease-associated dementia (PDD).

Results

1. Cognitive function

Seven studies reported on cognitive function (Howard, 2012; Gaudig, 2011; Herrmann, 2016; Holmes, 2004; Johannsen, 2006; Moo, 2021, Scarpini, 2011). The effect of discontinuation versus continuation of ChEIs was considered at three different timepoints, short term (up to 2 months), medium term (3 to 11 months), and long-term (over 11 months). As different cognitive outcome measures were employed by the studies, we used standardized mean differences (SMD) as the measure of effect size.

 

Data of Scarpini (2011) were not included in the analyses. Scarpini (2011) withdrew participants when they experienced a deterioration in ADAS-Cog/11 score of 4 points or more, and reported time to deterioration as the study endpoint.

 

Short-term

Five studies reported data on the short-term effect of ChEI discontinuation on cognitive function (Howard, 2012; Gaudig, 2011; Herrmann, 2016; Holmes, 2004; Moo, 2021). Johannsen (2006) also measured cognitive function at six weeks, but did not report the results. Data of Howard (2012) was adjusted for center, duration of donepezil treatment before entry, baseline sMMSE, and age. Data of Herrmann (2016) was adjusted for baseline sMMSE scores.

 

The pooled data show a SMD of -0.42 (95%CI -0.63 to -0.20) in favor of ChEI continuation (figure 1). This difference is considered not clinically relevant.

Medium-term

Three studies reported data on the medium-term effect of ChEI discontinuation on cognitive function (Howard, 2012; Holmes, 2004; Johannsen, 2006). Data of Howard (2012) was adjusted for center, duration of donepezil treatment before entry, baseline sMMSE, and age.

 

The pooled data show a SMD of -0.40 (95%CI -0.87 to 0.07) in favor of ChEI continuation (figure 2). This difference is considered not clinically relevant.

Long-term

One study reported data on the long-term effect of ChEI discontinuation on cognitive function (Howard, 2012). Data was adjusted for center, duration of donepezil treatment before entry, baseline sMMSE, and age. At week 52, the authors reported a mean (SD) change in sMMSE score from baseline of -5.89 (3.14) for the ChEI discontinuation group (n=54) and -3.80 (3.92) for the ChEI continuation group (n=54). Mean difference is -2.09 (95%CI -3.43 to -0.75) in favor of ChEI continuation. This difference is considered clinically relevant.

 

2. Bradycardia

One study reported data on bradycardia (Herrmann, 2016). The authors reported bradycardia in 0 out of 19 patients in the ChEI discontinuation group, and in 1 out of 21 patients in the ChEI continuation group. Risk ratio is 0.37 (95%CI 0.02 to 8.50) in favor of ChEI discontinuation. This difference is considered clinically relevant.

 

3. Weight loss

One study reported data on weight loss, defined as the number of people with unintentional weight loss (<1.66%) (Herrmann, 2016). The authors reported unintentional weight loss in 26.3% (5/19) of patients in the ChEI discontinuation group and in 14.3% (3/21) of patients in the ChEI continuation group. Risk ratio is 1.84 (95%CI 0.51 to 6.69) in favor of ChEI continuation. This difference is considered clinically relevant.

 

4. Gastrointestinal complications

Three studies reported on gastrointestinal complications (Howard, 2012; Scarpini, 2011; Johannsen, 2016). The pooled data show a risk ratio of 0.73 (95%CI 0.31 to 1.71) in favor of ChEI discontinuation (figure 3). This difference is considered clinically relevant.

5. Daily functioning

The pooled data show a SMD of -0.24 (95%CI -0.53 to 0.04) in favor of ChEI continuation (figure 4). This difference is considered not clinically relevant.

Medium-term

Two studies reported data on the medium-term effect of ChEI discontinuation on daily functioning (Howard, 2012; Johannsen, 2006). Data of Howard (2012) was adjusted for center, duration of donepezil treatment before entry, baseline sMMSE, and age.

 

Howard (2012) used the Bristol Activities of Daily Living Scale (BADLS) to assess daily functioning (range: 0-60). Higher scores indicate greater impairment. At week 18, Howard (2012) reported a mean (SD) change in BADLS score from baseline of -9.7 (8.73) for the ChEI discontinuation group (n=60) and -5.03 (7.23) for the ChEI continuation group (n=63). Mean difference is -4.67 (95%CI -7.51 to -1.83) in favor of ChEI continuation. This difference is considered clinically relevant.

 

Johannsen (2006) used the Disability Assessment for Dementia (DAD) scale (range: 0-100), where 100 represents maximum function and 0 represents complete dependence. At week 12, Johannsen (2006) reported a mean (SD) change in DAD score of -2.54 (15.65) for the ChEI discontinuation group (n=97) and 0.71 (15.51) for the ChEI continuation group (n=94). Mean difference is -3.25 (95%CI -7.67 to 1.17) in favor of ChEI continuation. This difference is considered not clinically relevant.

 

Long-term

One study reported data on the long-term effect of ChEI discontinuation on daily functioning (Howard, 2012). Data was adjusted for center, duration of donepezil treatment before entry, baseline sMMSE, and age. At week 52, the authors reported a mean (SD) change in BADLS score from baseline of -13.22 (8.97) for the ChEI discontinuation group (n=55) and -9.83 (8.52) for the ChEI continuation group (n=54). Mean difference is -3.38 (95%CI -6.67 to -0.10) in favor of ChEI continuation. This difference is considered not clinically relevant.

 

6. Quality of life

One study reported on quality of life (Herrmann, 2016). Data was adjusted for baseline sMMSE scores. Data of Howard (2012) were not included in the analyses, as the data reported for continuation and discontinuation groups were average values across patients who received active and placebo memantine.

 

Herrmann (2016) used the Quality of Life in late stage Dementia (QUALID) to assess quality of life (range: 11-55), with lower scores indicating a higher quality of life. Herrmann (2016) reported quality of life as change in QUALID scores from baseline to 8 weeks. A mean (SD) change in QUALID score of 0.3 (3.1) was reported for the ChEI discontinuation group (n=19), and -0.1 (4.8) for the ChEI continuation group (n=21). Mean difference is 0.40 (95%CI -2.08 to 2.88) in favor of ChEI discontinuation. This difference is considered not clinically relevant.

A systematic review of the literature was performed to answer the following question(s):

What is the effect of discontinuation of cholinesterase inhibitors in patients with dementia on cognitive function, bradycardia, weight loss, gastrointestinal complications, daily functioning, and quality of life, compared to the continuation of cholinesterase inhibitors?

Relevant outcome measures

The guideline panel considered cognitive function, bradycardia, weight loss, and gastrointestinal complications as a critical outcome measure for decision making; and daily functioning and quality of life as an important outcome measure for decision making.

 

A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.

 

For all outcome measures the guideline panel defined a 25% difference for dichotomous outcomes (0.8 ≤ RR ≥ 1.25) and 0.5 SD or -0.5 ≥ SMD ≥ 0.5 for continuous outcomes as a minimal clinically (patient) important difference.

 

Search and select (Methods)

A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline. Both databases were searched from 2005 to 29 July 2025 for systematic reviews and from 10 October 2020 to 8 August 2025 for RCTs. Systematic searches were completed using a combination of controlled vocabulary/subject headings (e.g., Emtree-terms, MeSH) wherever they were available and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) dementia; (2) cholinesterase inhibitors; (3) discontinuation. Duplicates were removed using EndNote software. After deduplication a total of 333 records were imported for title/abstract screening. Initially, three studies were selected based on title and abstract screening. After reading the full text, one study was excluded (see the exclusion table under the tab ‘Evidence tabellen’), and one systematic review and one RCT were included.

1. Werk voor British Journal of Anaesthesia (van sept 2011 t/m jan 2022 was ik editor). Sinds Sept 2022 “trustee” met portfolio “financial director” van de BJA Charitable Company. Ik heb nu geen invloed op het wetenschappelijk beleid; maar ben vaak reviewer van farmacologische artikelen (waarvoor ik per kwartaal een honorarium ontvang).

2. Actieve consultancy werk en PI-schap van een sponsor (Philips) – geinitieerd pijn-project. Geen conflict m.b.t. dementie/MCI/delier.

Advieswerkzaamheden/medische adviesraad – met betrekking tot een voorgestelde pijnmonitor voor intra-operatief gebruik.

Onbeperkte onderzoekssubsidie voor de uitvoering van een geplande studie om gegevens te verzamelen die nodig zijn voor de ontwikkeling van een dergelijke monitor.

Deze studie staat op het punt te starten, waardoor deze samenwerking niet kan worden opgeschort of beëindigd.

3. Vorige consultancy werk voor Orion (> 5 jaar geleden). Consultancy advice (dexmedetomidine). 

4. Vorige consultancy werk voor Ever Pharma (mbt dexmedetomidine). Hiervoor gaf ik advies over een EMA aanvraag voor registratie (van een nieuwe formulatie) en indicatie uitbreiding van dexmedetomidine voor procedurele sedatie. In dit werk was de focus alleen op het gebruik van dit middel voor sedatie. Er bestaat wetenschappelijke bewijs dat dexmedetomidine toediening tijdens een operatie onder algehele narkose de kansen van post-operatieve delier verlaagt. Ook wordt dit middel soms toegediend ter behandeling van “emergence agitation” (dwz onrust en verwardheid direct na ontwaking uit narcose). Deze indicaties zijn niet geregistreerde indicaties, en ik heb Ever Pharma dan ook geen raad hierover gegeven. De link met dementie/MCI/delier is daarom zwak.

5. Vorige consultancy werk voor Ever Pharma en PAION – hiervoor gaf ik advies aan Ever en PAION over (potentiele aankoop van een generische medicaties, en/of indicatie stellingen) van medicaties met geen (potentiele) link met dementie/MCI/delier; niet meer actief.

6. Consultancy werk voor Becton Dickinson (Eysins, Switzerland)** en Terumo (Tokyo, Japan)* – technische advies over spuitpompen. Niet regerelateerd aan dementie/MCI/delier; en niet meer actief.

*Deelname aan de commissie is erkenning van het potentiële belang van de vloeistof biomarkers voor de diagnostiek op zich, wat belangrijk is voor mijn vakgebied. Het is ook eervol en daardoor mogelijk goed voor mijn reputatie als wetenschapper.

*Mocht de richtlijn een belangrijke rol voor vloeistofbepalingen zien, dan zou dat misschien tot meer productie in het klinische chemisch lab van VUmc kunnen leiden.  Daar heb ik persoonlijk geen baat bij, evenmin als mijn onderzoeksgroep. Wel kan de grotere productie leiden tot een lagere kostprijs.

Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz

Bij de richtlijnmodule voerden de clusterleden conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).

Hoewel uit de toetsing volgt dat de aanbeveling(en) breed toepasbaar zijn (>40.000 patiënten), volgt uit de toetsing dat het overgrote deel van de zorgverleners al aan de norm voldoet, het geen nieuwe manier van zorgverlening betreft, het geen toename in het aantal voltijdsequivalenten of wijziging in het opleidingsniveau van zorgverleners betreft. A systematic literature search was performed by a medical information specialist using the following bibliographic databases: Embase.com and Ovid/Medline all. Both databases were searched from 2005 to July 29th 2025 for systematic reviews and from October 10th 2020 to August 8th 2025 for RCTs. Systematic searches were completed using a combination of controlled vocabulary and natural language keywords. The overall search strategy was derived from the following primary search concepts: (1) dementia; (2) cholinesterase inhibitors; (3) discontinuation. Duplicates were removed using EndNote software. After deduplication a total of 333 records were imported for title/abstract screening.
