Systemische therapie bij niet lokaal behandelbare metastasen

Uitgangsvraag

Voor welke systemische therapie komt een patiënt met een gemetastaseerd colorectaal carcinoom zonder lokale behandelopties in aanmerking?

 

De uitgangsvraag omvat de volgende deelvragen:

  1. Welke doelgerichte therapie in welke lijn?
  2. Combinatie chemotherapie versus sequentiële chemotherapie?
  3. Welke chemotherapie schema in welke lijn?

Aanbeveling

Maak samen met de patiënt een zorgvuldige afweging over de systemische behandeling. Bespreek de wensen en verwachtingen van de patiënt, de te verwachten therapeutische winst. Leg het vast in het dossier.

 

Bepaal voorafgaand aan de start van systemische behandeling van metastasen de KRAS (exon 2, 3 en 4), NRAS (exon 2, 3 en 4) en BRAF mutatiestatus als ook de mismatch repair status van de tumor.

 

Bespreek de optie van het toevoegen van bevacizumab aan chemotherapie in de eerstelijn.

 

Geef oxaliplatin niet langer dan 6 cycli (3-wekelijks schema) of 8 cycli (2-wekelijks schema) ter voorkoming van ernstige neurotoxiciteit, en bespreek met patient de voor- en nadelen om fluoropyrimidine met of zonder bevacizumab als onderhoudsbehandeling te continueren. Voeg de oxaliplatin weer toe bij progressie indien er geen contra-indicatie is. Geef geen bevacizumab monotherapie.

 

Overweeg triple chemotherapie (FOLFOXIRI) plus bevacizumab als eerstelijns behandeling bij patiënten met een rechtzijdige primaire tumor en/of KRAS of NRAS gemuteerde tumor. Neem in deze overweging de klinische conditie, leeftijd en de zijdigheid van de tumorlocatie mee.

 

Geef pembrolizumab als eerstelijns behandeling aan patienten met een gemetastaseerd MSI of dMMR colorectaal carcinoom.

 

Bespreek de optie van anti-EGFR therapie in tweede lijn in combinatie met chemotherapie of in derde lijn als monotherapie na een bevacizumab-bevattend schema in 1e lijn bij KRAS, NRAS en BRAFV600E wildtype, linkszijdige primaire tumoren.

 

Overweeg bij patiënten die geen kandidaten zijn voor combinatie chemotherapie (bijvoorbeeld oudere en/of kwetsbare patiënten) fluoropyrimidine monotherapie + bevacizumab.

 

Vervang capecitabine of continue 5FU infusie door S-1 bij ernstige klachten van handvoetsyndroom of cardiale klachten.

 

Geef irinotecan bij voorkeur in combinatie met 5 FU (FOLFIRI) of als monotherapie in 2e lijn na oxaliplatin houdende eerstelijns behandeling.

 

Geef combinatietherapie met cetuximab en encorafenib bij patiënten met een BRAFV600E-gemuteerde tumor die progressie hebben op een eerstelijns behandeling.

 

Overweeg trifluridine-tipiracil als laatste lijns behandeling bij patiënten die nog in redelijke conditie zijn.

Overwegingen

Op grond van de thans beschikbare gegevens hebben cetuximab en panitumumab een vergelijkbare effectiviteit en toxiciteit, en de resultaten van beide geneesmiddelen kunnen dus worden gebruikt om de plaats van anti-EGFR therapie vast te stellen (Price, 2014).

 

In Nederland wordt het effect van de volgende behandelingen als klinisch onvoldoende relevant beschouwd om als standaardbehandeling te worden toegepast: het continueren van bevacizumab na eerste progressie tijdens 2e lijns therapie met een irinotecan- of oxaliplatin-bevattend schema, de toevoeging van aflibercept (een “decoy” receptor van VEGF) aan (FOLF)IRI in de 2e lijn na eerdere oxaliplatin-bevattende chemotherapie met of zonder bevacizumab, de toevoeging van ramucirumab (antilichaam tegen de VEGF-receptor2) aan FOLFIRI in 2e lijn, en behandeling met regorafenib (een tyrosine multikinase inhibitor) in laatste lijn.

 

Chemotherapie en bevacizumab

De toevoeging van bevacizumab aan chemotherapie in 1e lijns behandeling geeft ten opzichte van alleen chemotherapie een winst in één of meerdere eindpunten (tumor respons, progressie-vrije overleving en/of totale overleving) (Hurwitz, 2004; Saltz, 2008, Kabbinavar, 2008; Cunningham, 2013; Tebbutt, 2010). Bij patiënten met KRAS gemuteerde, niet resectabele levermetastasen toonde de toevoeging van bevacizumab aan FOLFOX in een gerandomiseerde fase 3 studie een significante winst in het percentage R0 resecties, tumorrespons, progressie-vrije overleving en totale overleving (Tang, 2020).

 

Resultaten van een prospectieve en 2 grote observationele studies tonen geen chemotherapie-schema van voorkeur voor combinatie met bevacizumab (Parikh, 2018, Kozloff, 2009; Van Cutsem, 2009).

De interpretatie van de meerwaarde van bevacizumab in eerste lijn wordt bemoeilijkt door heterogeniteit van de gepubliceerde studies. Bij meerdere studies waarin combinatie chemotherapie werd gegeven voldeed de winst niet aan de huidige PASKWIL criteria (Baraniskin 2019). De duur van behandeling met bevacizumab speelde hierin een belangrijke rol. Het belang van de behandelingsduur van bevacizumab is in een prospectieve studie bevestigd (Simkens, 2015) en kan een verklaring zijn voor minder positieve resultaten van eerdere studies (Saltz, 2008).

 

Behandeling met FOLFOXIRI + bevacizumab toont in een meta-analyse van 5 gerandomiseerde studies in vergelijking met FOLFIRI of FOLFOX + bevacizumab een significante verbetering van de mediane OS (28.9 versus 24.5 maanden, HR 0.81, p<0.001), van de mediane PFS (12.2 versus 9.9 maanden, HR 0.74, p < 0.001) en response rate (64.5% versus 53.6%, p< 0.001) (Cremolini, 2020). In een subgroep analyse werd dit voordeel vooral waargenomen bij patiënten met een rechtszijdige primaire tumor en/of RAS gemuteerde tumor, maar niet bij patiënten met een BRAFV600E gemuteerde tumor. Het merendeel van patiënten had een WHO performance score van 0-1 en een leeftijd < 75 jaar. Behandeling met FOLFOXIRI + bevacizumab veroorzaakte significant vaker graad 3/4 neutropenie (45.8% versus 21.5%), febriele neutropenie (6.3% versus 3.7%) en graad 3/4 diarree (17.8% versus 8.4%). Vanwege de toxiciteit van triple chemotherapie wordt geadviseerd om conform de studies dit schema niet boven de leeftijd van 75 jaar toe te passen in de dagelijkse praktijk.

 

In verschillende studies is onderhoudsbehandeling met capecitabine +/- bevacizumab bestudeerd. Het geeft een significante verlenging met 3.2 maanden van de mediane PFS ten opzichte van observatie bij patiënten die stabiele ziekte of beter hebben bereikt na 18 weken inductie behandeling met CAPOX + bevacizumab (HR 0.67, p<0.0001), met een trend voor de winst van 3.5 maanden in het secundaire eindpunt van mediane OS (HR 0.83, p 0.06) (Simkens, 2015). De conclusie van een non-inferiority studie met 5FU + bevacizumab onderhoudsbehandeling versus observatie versus bevacizumab na 24 weken inductie FOLFOX + bevacizumab is dat onderhoudsbehandeling met 5FU + bevacizumab de voorkeur heeft (Hegewisch-Becker, 2015). Een gepoolde analyse van beide studies bevestigt de meerwaarde van onderhoudsbehandeling van een fluoropyrimidine + bevacizumab (PFS HR 0.70) (Goey, 2017). In een recente meta-analyse (Bassam Sonbol, 2020) van studies aangaande onderhoudsbehandeling werden recentelijk 12 studies geïncludeerd, alhoewel PFS significant beter was voor onderhoudsbehandeling zoals mag worden verwacht, werd geen OS voordeel gezien (Hazard Ratio, 0.91; 95% CI, 0.83 tot 1.01). De auteurs van deze meta-analyse concludeerden dat een onderhoudsstrategie met een fluoropyrimidine, al dan niet met toevoeging van bevacizumab, de voorkeur heeft, maar dat gezien het ontbreken van een OS-voordeel een therapiepauze te overwegen is in samenspraak met de patient.

 

Het gebruik van bevacizumab monotherapie heeft geen meerwaarde (Aparicio, 2018; Giantonio, 2007).

 

Na progressie op 1e lijns therapie met oxaliplatin en 5FU/LV is het 2 wekelijkse schema met irinotecan + 5FU/LF alswel een driewekelijks schema met irinotecan te overwegen met vergelijkbare OS (hazard ratio (HR) of 0.91 (95% CI: 0.81 tot 1.02), waarbij de PFS in het voordeel van de combinatie werd vastgesteld in een, waarschijnlijk op basis van de heterogeniteit van inclusie, waarbij ongevoeligheid voor 5FU/LV (al dan niet in combinatie met oxaliplatin) niet was vereist (HR: 0.68, 95% CI: 0.53 tot 0.87) beschreven in een meta-analyse gebaseerd op 5 klinische studies (Wulaningsih, 2016).

 

Daarnaast is na progressie op 1e lijns therapie met irinotecan en 5FU/LV in de E3200 studie een significante winst van 2.1 maanden gevonden voor 2e lijns FOLFOX + bevacizumab ten opzichte van FOLFOX of bevacizumab monotherapie in het primaire eindpunt van OS (HR 0.75, p 0.001) als ook van 3.9 maanden van de mediane PFS (HR 0.61, p < 0.0001) (Giantonio, 2007).

 

Chemotherapie en EGFR remming

Bij patiënten met een (K)RAS wildtype als ook linkszijdige lokalisatie (flexura lienalis tot en met rectum) van de primaire tumor is een klinisch relevante winst aangetoond van anti-EGFR therapie in 1e of 2e lijn in combinatie met chemotherapie of in latere lijn als monotherapie (Tejpar, 2016; Boeckx, 2017; Arnold, 2017; Holch, 2017; Brulé, 2015). Hoewel de analyses van eerstelijns studies enig voordeel tonen voor behandeling van chemotherapie + anti-EGFR antilichaam ten opzichte van chemotherapie + bevacizumab bij patiënten met linkszijdige tumoren wordt dit resultaat sterk beïnvloed door de FIRE-3 studie (Heinemann, 2014). De FIRE-3 studie (1e lijn FOLFIRI+cetuximab versus FOLFIRI+bevacizumab bij KRAS wildtype tumor) toont geen verschil in response rate (primaire eindpunt) en mediane PFS, wel een winst van 3.7 maanden in mediane OS (HR 077, p 0.017) voor FOLFIRI+cetuximab. Deze winst in mediane OS kon niet bevestigd worden door de CALGB 80405 studie (HR 0.92, p 0.34) (Venook, 2017).

 

De meerwaarde van anti-EGFR therapie bij patiënten met een BRAFV600E mutatie is zeer beperkt of afwezig, en is in deze setting niet geïndiceerd (Pietrantonio, 2014; Rowland, 2014). Gemetastaseerde colorectale tumoren met BRAF mutaties buiten codon 600 lijken een veel minder ongunstig klinisch beloop te hebben (Cremolini, 2015; Jones, 2017).

 

In de BEACON-studie wordt bij patiënten met een gemetastaseerd BRAF-V600E -gemuteerd CRC bij progressie na een of twee lijnen therapie na een mediane follow-up voor OS van 7,8 maanden een statistisch significant langere overleving gezien na palliatieve behandeling met encorafenib en cetuximab dan na standaardbehandeling met irinotecan of FOLFIRI en cetuximab (HR: 0,60 (95%-BI: 0,45 tot 0,79); p < 0,001). De toevoeging van binimetinib resulteerde niet in een verbetering van dit resultaat maar veroorzaakte wel meer bijwerkingen (Kopetz, 2019). Een studie in 1e lijn is gaande. 

 

Anti-EGFR therapie dient niet te worden gecombineerd met capecitabine vanwege een verhoogde kans op toxiciteit en (waarschijnlijk hiermee samenhangende) verminderde effectiviteit (Maugham, 2011). Er zijn geen gegevens over de combinatie van anti-EGFR-therapie met fluoropyrimidine monotherapie, en er zijn weinig gegevens van anti-EGFR-therapie met triple chemotherapie. Er zijn uit andere studies geen aanwijzingen dat anti-EGFR-therapie minder werkzaam zou zijn na eerdere therapie met bevacizumab (Peeters, 2010; Bennouna, 2018).

 

Een winst in PFS, maar niet OS, is aangetoond van de toevoeging van anti-EGFR-therapie aan 2e lijns chemotherapie ten opzichte van alleen chemotherapie (Peeters, 2010; Seymour, 2013), en in PFS en OS in late lijn van anti-EGFR-monotherapie ten opzichte van observatie (Karapetis, 2008; Amado, 2008). Ook voor deze indicaties dient de toediening van anti-EGFR-therapie inmiddels beperkt te worden tot patiënten met een KRAS, NRAS, BRAFV600E wildtype en linkszijdige primaire tumor.

 

Overige gegevens over chemotherapie

FOLFOX toont vergelijkbare effectiviteit ten opzichte van FOLFIRI in de eerstelijnsbehandeling (Tournigand, 2004; Colucci, 2005).

 

FOLFOX toont vergelijkbare effectiviteit ten opzichte van CAPOX, met vaker optreden van thrombopenie, diarree en handvoetsyndroom bij gebruik van CAPOX en vaker neutropenie bij FOLFOX (Arkenau, 2008). FOLFIRI toont vergelijkbare effectiviteit ten opzichte van CAPIRI, met vaker optreden van ernstige bijwerkingen tijdens CAPIRI (Fuchs, 2007; Ding, 2015; Souglakos, 2012). Een CAPIRI schema met gereduceerde dosering van capecitabine en irinotecan geeft minder kans op toxiciteit (Schmiegel, 2013) maar de werkzaamheid van dit schema is in de Westerse populatie niet prospectief vergeleken met FOLFIRI. Capecitabine heeft ook een vergelijkbare effectiviteit in vergelijking met 5-fluorouracil als monotherapie.

 

In verschillende studies heeft S-1 monotherapie of in combinatie met oxaliplatin met of zonder bevacizumab vergelijkbare werkzaamheid ten opzichte van 5FU of capecitabine (Kwakman, 2016). S-1 +/- bevacizumab toont in Nederlandse patiënten een significant lagere incidentie van handvoetsyndroom ten opzichte van capecitabine +/- bevacizumab (graad 3: 4% versus 21%, p 0.003; alle graderingen 45% versus 73%, p 0.0005, SALTO studie), met vergelijkbare antitumor activiteit (Kwakman, 2017). Bij de meerderheid van patiënten met ernstig handvoetsyndroom ten gevolge van capecitabine die verder behandeld worden met S tot 1 treedt een duidelijke verbetering of zelfs verdwijnen van de symptomen op (Kwakman, 2017). Cardiale toxiciteit (vnl. coronairspasmen) is een zeldzame bijwerking van capecitabine en continue 5FU infusie. Recidief van cardiale toxiciteit wordt bij minder dan 1% van patiënten waargenomen bij wie de behandeling vanwege deze toxiciteit was omgezet naar S-1 (Kwakman, 2017; Österlund, 2020)

 

Monotherapie met trifluridine-tipiracil toont ten opzichte van placebo een winst in het primaire eindpunt van mediane OS bij patiënten met een ECOG performance status 0 tot 1 die tenminste 2 eerdere systemische behandelingen hadden gekregen en progressief waren of ernstige bijwerkingen hadden die verdere behandeling onmogelijk maakten (Mayer, 2015; Xu, 2017).

 

Bij patiënten met een MSI gemetastaseerd colorectaal carcinoom toonden niet-gecontroleerde studies van beperkte omvang een positief effect van pembrolizumab (Le, 2015) en nivolumab met of zonder ipilimumab (Overman, 2017; Overman, 2018). Een fase 3 studie met pembrolizumab versus chemotherapie in eerste lijn bij patiënten met een MSI gemetastaseerd colorectaal carcinoom toonde een significante winst in het primaire eindpunt van PFS (16.5 versus 8.2 maanden, HR 0.60 (André, 2020).

 

Goede resultaten zijn ook getoond voor behandeling met larotrectinib, een selectieve tropomyosine receptor kinase (TRK) inhibitor, bij patiënten met een TRK fusie-positief gemetastaseerd colorectaal carcinoom (Drilon, 2018). TRK remmers zijn in Nederland nog niet geregistreerd voor deze indicaties.

 

Samenvatting van nationale data en populatiestudies

Er zijn geen nationale data en/of populatiestudies geselecteerd voor deze vraagstelling.

Onderbouwing

Nog altijd ontwikkelen patiënten die behandeld zijn voor een lokaal colorectaal carcinoom metastasen op afstand of hebben deze reeds bij eerste diagnose die niet meer voor lokale therapie toegankelijk zijn. Hiervoor zijn de afgelopen decennia meerdere systemische behandelingen beschikbaar gekomen, bestaande uit chemotherapie en doelgerichte therapie. In deze module wordt getracht een rationele volgorde en combinatie van systemische behandelingen te geven. Uiteraard zullen alle voor-, en nadelen van het starten van een behandeling met palliatieve systeemtherapie voorafgaand met de patiënt moeten worden besproken.

Samenvatting van internationale richtlijnen en consensusdocumenten

De aanbevelingen ten aanzien van het gebruik van de besproken geneesmiddelen zijn overeenkomstig de ESMO consensus guidelines for the management of patients with metastatic colorectal cancer (Van Cutsem, 2016), de NCCN guidelines colon cancer (Benson, 2018), en de guidelines van ASCO, American Society for Clinical Pathology, College of Am Pathologists, Association for Molecular Pathology en de American Society of Clinical Oncology (Sepulveda, 2017). Gezien de implicaties voor de keuze van behandeling dient voorafgaand aan de systemische behandeling van metastasen de KRAS, NRAS en BRAF mutatiestatus en MMR status van de tumor te worden bepaald (Sepulveda, 2017).

 

Samenvatting van systematische reviews en meta-analyses

PICO 1 (toegevoegde waarde van anti-EGFR antilichaamtherapie aan chemotherapie)

Een meta-analyse toont meerwaarde voor anti-EGFR therapie bij KRAS wildtype tumoren als monotherapie in 3e lijn en in combinatie met chemotherapie in 1e en 2e lijn (Vale, 2012). Meta-analyses tonen weinig tot geen overlevingswinst van anti-EGFR therapie bij patiënten met RAS wildtype, BRAF V600E gemuteerde tumoren (Pietrantonio, 2015) (HR PFS 0.88, p=0.33, HR OS 0.91 p=0.63); (Rowland, 2015) (HR PFS=0.86, HR OS=0.97). De locatie van de primaire tumor heeft prognostische, en voor het gebruik van anti-EGFR therapie predictieve waarde (Holch, 2017).

 

PICO 2 (toegevoegde waarde van bevacizumab aan chemotherapie)

Meta-analyses en systematische reviews tonen meerwaarde aan van de toevoeging van bevacizumab aan 1e lijns chemotherapie (Welch, 2010; Zhang, 2015; Qu, 2015; Botrel, 2016; Baraniskin, 2019). Heterogeniteit van de gebruikte chemotherapie schema’s en wisselende kwaliteit van studies worden bij de interpretatie als complicerende factoren genoemd.

 

PICO 3 (doublet versus triplet chemotherapie schema’s)

Een meta-analyse toont meerwaarde voor eerstelijns triple chemotherapie (FOLFOXIRI) plus bevacizumab ten opzichte van doublet chemotherapie (FOLFIRI of FOLFOX) plus bevacizumab, behoudens bij patiënten met een BRAFV600E gemuteerde tumor (Cremolini, 2020).

FOLFIRI toont vergelijkbare effectiviteit ten opzichte van CAPIRI, met vaker optreden van diarree bij CAPIRI (Ding, 2015).

Voor het beoordelen van de actualiteit van deze richtlijn is de werkgroep in stand gehouden voor een periode van 3 jaar, met optionele verlenging van 3 jaar. De werkgroep zal halfjaarlijks bijeen komen voor een herbeoordeling van de geldigheid van de modules in deze richtlijn. De geldigheid van richtlijnmodules zal bij herbeoordeling komen te vervallen wanneer nieuwe ontwikkelingen aanleiding geven om een herzieningstraject te starten. De Nederlandse Vereniging voor Heelkunde (NVvH) is regiehouder van deze richtlijn en eindverantwoordelijke op het gebied van de actualiteitsbeoordeling van de richtlijnmodules. De andere aan deze richtlijn deelnemende wetenschappelijke verenigingen of gebruikers van de richtlijn delen de verantwoordelijkheid en informeren de regiehouder over relevante ontwikkelingen binnen hun vakgebied. Per module is aangegeven welke wetenschappelijke vereniging de eerstverantwoordelijke is voor de actualiteitsbeoordeling (zie de Algemene inleiding).

De richtlijnontwikkeling werd ondersteund door het Kennisinstituut van de Federatie Medisch Specialisten en werd gefinancierd uit de Stichting Kwaliteitsgelden Medisch Specialisten (SKMS). De financier heeft geen enkele invloed gehad op de inhoud van de richtlijn.

Doel en doelgroep

Doel

Het beschrijven van het beleid bij patiënten met een colorectaal carcinoom: gestandaardiseerd en op hun wensen afgestemd in alle fasen van de ziekte.

 

Doelgroep

Deze richtlijn is geschreven voor alle leden van de beroepsgroepen die betrokken zijn bij de zorg voor patiënten met colorectaal carcinoom in de tweede en derde lijn.

Samenstelling werkgroep

Voor het ontwikkelen van de richtlijn is in 2017 een multidisciplinaire werkgroep ingesteld, bestaande uit vertegenwoordigers van alle relevante specialismen die betrokken zijn bij de zorg voor patiënten met colorectaal carcinoom te maken hebben. De werkgroepleden zijn door hun beroepsverenigingen gemandateerd voor deelname. De werkgroep is verantwoordelijk voor de integrale tekst van deze richtlijn.

Belangenverklaringen

De KNMG-code ter voorkoming van oneigenlijke beïnvloeding door belangenverstrengeling is gevolgd. Alle werkgroepleden hebben schriftelijk verklaard of zij in de laatste drie jaar directe financiële belangen (betrekking bij een commercieel bedrijf, persoonlijke financiële belangen, onderzoeksfinanciering) of indirecte belangen (persoonlijke relaties, reputatiemanagement, kennisvalorisatie) hebben gehad. Een overzicht van de belangen van werkgroepleden en het oordeel over het omgaan met eventuele belangen vindt u in onderstaande tabel. De ondertekende belangenverklaringen zijn op te vragen bij het secretariaat van het Kennisinstituut van Medisch Specialisten.

De werkgroep heeft bij aanvang van het richtlijnontwikkelingstraject geconstateerd dat sommige werkgroepleden betrokken zijn bij wetenschappelijk onderzoek dat wordt gefinancierd door overheid, onafhankelijke subsidieverstrekkers, of commerciële partijen (in het laatste geval: unrestricted scientific grants). Om te voorkomen dat indirecte belangen (waaronder reputatie en kennisvalorisatie) de richtlijnaanbevelingen onbedoeld zouden kunnen beïnvloeden, heeft de werkgroep besloten dat geen van de modules/aanbevelingen worden opgesteld door één werkgroeplid met dergelijke indirecte belangen. Tenminste één ander werkgroeplid met hetzelfde specialisme of tenminste één ander werkgroeplid met vergelijkbare inhoudelijke kennis van het betreffende onderwerp zijn verantwoordelijk voor de inhoud (vier-ogen-principe).

 

De werkgroep zag in de gemelde (on)betaalde nevenfuncties geen aannemelijke reden voor onbedoelde beïnvloeding door belangenverstrengeling ten aanzien van de onderwerpen die in de richtlijn aan bod komen.

Inbreng patiëntenperspectief

Er werd aandacht besteed aan het patiëntenperspectief door het uitnodigen van de Stichting voor Patiënten met Kanker aan het Spijsverteringskanaal (SPKS) voor de invitational conference. Daarnaast hebben dr. L. Meerman en mw. A. van Heijst (Stomavereniging, Nederlandse Federatie van Kankerpatiëntenorganisaties, NFK) modules meegelezen en de werkgroep van feedback vanuit het patiëntenperspectief voorzien. De conceptrichtlijn is tevens ter commentaar voorgelegd aan SPKS en NFK. In verbrand met de zeer verslechterde gezondheidstoestand en het overlijden van mw A. van Heijst wordt vanaf maart 2020 feedback vanuit het patiëntperspectief gegeven door dr. L. Meerman en mw. M. v.d. Bosch-Scholts.

Methode ontwikkeling

Evidence based

Implementatie

In de verschillende fasen van de richtlijnontwikkeling is rekening gehouden met de implementatie van de herziene richtlijnmodules en de praktische uitvoerbaarheid van de aanbevelingen. Daarbij is uitdrukkelijk gelet op factoren die de invoering van de richtlijn in de praktijk kunnen bevorderen of belemmeren. Het implementatieplan is te vinden bij de aanverwante producten. De werkgroep heeft geen interne kwaliteitsindicatoren ontwikkeld, omdat interne en externe kwaliteitsindicatoren al ontwikkeld worden binnen de Dutch Colorectal Audit (DCRA), de clinical audit board betreffend colorectaal carcinoom onder de Dutch Institute for Clinical Auditing (DICA).

Werkwijze

AGREE

Deze richtlijn is opgesteld conform de eisen vermeld in het rapport Medisch Specialistische Richtlijnen 2.0 van de adviescommissie Richtlijnen van de Raad Kwaliteit. Dit rapport is gebaseerd op het AGREE II instrument (Appraisal of Guidelines for Research & Evaluation II; Brouwers, 2010), dat een internationaal breed geaccepteerd instrument is.

 

Knelpuntenanalyse

Tijdens de voorbereidende fase inventariseerden de voorzitter van de werkgroep en de adviseurs de knelpunten. De werkgroep beoordeelde de aanbevelingen uit de eerdere richtlijn (NVRO, 2014) op noodzaak tot revisie. De werkgroep stelde vervolgens een long list met knelpunten op en prioriteerde de knelpunten op basis van: (1) klinische relevantie, (2) de beschikbaarheid van nieuwe evidence, (3) en de te verwachten impact op de kwaliteit van zorg, patiëntveiligheid en (macro)kosten. Tevens zijn er knelpunten aangedragen door verschillende partijen via een invitational conference. Een verslag hiervan is opgenomen onder aanverwante producten.

 

Uitgangsvragen en uitkomstmaten

Op basis van de uitkomsten van de knelpuntenanalyse zijn door de voorzitter en werkgroepleden concept-uitgangsvragen opgesteld. Gedurende het richtlijnherzieningstraject heeft de werkgroep de definitieve uitgangsvragen in een hernieuwd raamwerk (de inhoudsopgave, zie ook de algemene inleiding) van de richtlijn vastgesteld. Vervolgens inventariseerde de werkgroep per uitgangsvraag welke uitkomstmaten voor de patiënt relevant zijn, waarbij zowel naar gewenste als ongewenste effecten werd gekeken. De werkgroep baseerde de keuze voor uitkomstmaten mede op basis van de “core outcome set” gepubliceerd door Zerillo (2017). De werkgroep waardeerde deze uitkomstmaten volgens hun relatieve belang bij de besluitvorming rondom aanbevelingen, als cruciaal (kritiek voor de besluitvorming), belangrijk (maar niet cruciaal) en onbelangrijk. Tevens definieerde de werkgroep tenminste voor de cruciale uitkomstmaten welke verschillen zij klinisch (patiënt) relevant vonden. Hiervoor sloot de werkgroep zich, waar mogelijk, aan bij de grenzen voor klinische besluitvorming volgens de PASKWIL criteria uit 2016. Wanneer er geen grenzen voor klinische besluitvorming conform de PASKWIL criteria geformuleerd konden worden (bijvoorbeeld bij non-inferioriteitsvraagstukken) werden de standaard GRADE grenzen gehanteerd, tenzij anders vermeld.

 

Strategie voor zoeken en selecteren van literatuur

De werkgroep raadpleegde voor de afzonderlijke uitgangsvragen reeds gepubliceerde systematische reviews (SRs). SRs kwamen in aanmerking indien: i) systematisch was gezocht in één of meerdere databases; ii) transparante in- en exclusiecriteria zijn toegepast en iii) idealiter de kwaliteit van de geïncludeerde literatuur was beoordeeld. De voor de uitgangsvragen relevante internationale richtlijnen en consensusdocumenten werden eveneens geraadpleegd. Overige literatuur, waar niet systematisch naar werd gezocht, is als bewijsvoering in de overwegingen opgenomen.

 

Relevante beschikbare populatiegegevens uit Nederland werden gebruikt voor het bepalen van knelpunten, praktijkvariatie en externe validiteit van internationale richtlijnen en gepubliceerde literatuur.

 

Kwaliteitsbeoordeling individuele studies

De (risk of bias) kwaliteitsbeoordelingen van de individuele studies uit de gebruikte SRs werden gebruikt, tenzij deze afwezig waren of onbruikbaar waren voor het beoordelen van de kracht van het wetenschappelijke bewijs. Indien afwezig of onbruikbaar werd aangenomen dat er een risico op bias aanwezig zou kunnen zijn. In enkele gevallen, wanneer studies informatie verschaften over de cruciale uitkomstma(a)t(en), werd er een nieuwe kwaliteitsbeoordeling van individuele studies uitgevoerd.

 

Samenvatten van de literatuur

De relevante onderzoeksgegevens van de geselecteerde SRs werden weergegeven in ‘Summary of Findings’ tabellen. De belangrijkste bevindingen uit de literatuur werden kort beschreven onder “Samenvatting literatuur” (kopjes “Samenvatting van internationale richtlijnen en consensusdocumenten” en “Samenvatting van systematische reviews en meta-analyses”) en onder “Overwegingen”. Bestaande meta-analyses uit de gebruikte systematische reviews werden beschreven. Bij een voldoende aantal studies en overeenkomstigheid (homogeniteit) tussen de studies werden de gegevens ook kwantitatief samengevat (meta-analyse) met behulp van Review Manager 5.

 

Beoordelen van de kracht van het wetenschappelijke bewijs

A) Voor interventievragen (vragen over therapie of screening)

De kracht van het wetenschappelijke bewijs werd bepaald volgens de GRADE-methode. GRADE staat voor ‘Grading Recommendations Assessment, Development and Evaluation’ (zie http://www.gradeworkinggroup.org/).

 

GRADE onderscheidt vier gradaties voor de kwaliteit van het wetenschappelijk bewijs: hoog, redelijk, laag en zeer laag. Deze gradaties verwijzen naar de mate van zekerheid die er bestaat over de literatuurconclusie (Schünemann, 2013). Het resultaat van de gradering van het wetenschappelijk bewijs werd opgenomen in de ‘Summary of Findings’ tabellen.

 

GRADE

Definitie

Hoog

  • er is hoge zekerheid dat het ware effect van behandeling dichtbij het geschatte effect van behandeling ligt zoals vermeld in de literatuurconclusie;
  • het is zeer onwaarschijnlijk dat de literatuurconclusie verandert wanneer er resultaten van nieuw grootschalig onderzoek aan de literatuuranalyse worden toegevoegd.

Redelijk*

  • er is redelijke zekerheid dat het ware effect van behandeling dichtbij het geschatte effect van behandeling ligt zoals vermeld in de literatuurconclusie;
  • het is mogelijk dat de conclusie verandert wanneer er resultaten van nieuw grootschalig onderzoek aan de literatuuranalyse worden toegevoegd.

Laag

  • er is lage zekerheid dat het ware effect van behandeling dichtbij het geschatte effect van behandeling ligt zoals vermeld in de literatuurconclusie;
  • er is een reële kans dat de conclusie verandert wanneer er resultaten van nieuw grootschalig onderzoek aan de literatuuranalyse worden toegevoegd.

Zeer laag

  • er is zeer lage zekerheid dat het ware effect van behandeling dichtbij het geschatte effect van behandeling ligt zoals vermeld in de literatuurconclusie;
  • de literatuurconclusie is zeer onzeker.

*in 2017 heeft het Dutch GRADE Network bepaalt dat de voorkeursformulering voor de op een na hoogste gradering ‘redelijk’ is in plaats van ‘matig’.

 

B) Voor vragen over diagnostische tests, schade of bijwerkingen, etiologie en prognose

De kracht van het wetenschappelijke bewijs werd eveneens bepaald volgens de GRADE-methode: GRADE-diagnostiek voor diagnostische vragen (Schünemann, 2008), en een generieke GRADE-methode voor vragen over schade of bijwerkingen, etiologie en prognose. In de gehanteerde generieke GRADE-methode werden de basisprincipes van de GRADE methodiek toegepast: het benoemen en prioriteren van de klinisch (patiënt) relevante uitkomstmaten, een systematische review per uitkomstmaat, en een beoordeling van bewijskracht op basis van de vijf GRADE-criteria (startpunt hoog; downgraden voor risk of bias, inconsistentie, indirectheid, imprecisie, en publicatiebias).

 

Formuleren van de conclusies

Voor elke relevante uitkomstmaat werd het wetenschappelijk bewijs samengevat in een of meerdere literatuurconclusies waarbij het niveau van bewijs werd bepaald volgens de GRADE-methodiek (te vinden in de ‘Summary of Findings’ tabellen). De werkgroepleden maakten de balans op van elke interventie (overall conclusie). Bij het opmaken van de balans werden de gunstige en ongunstige effecten voor de patiënt afgewogen. De overall bewijskracht wordt bepaald door de laagste bewijskracht gevonden bij een van de cruciale uitkomstmaten. Bij complexe besluitvorming waarin naast de conclusies uit de systematische literatuuranalyse vele aanvullende argumenten (overwegingen) een rol spelen, werd afgezien van een overall conclusie. In dat geval werden de gunstige en ongunstige effecten van de interventies samen met alle aanvullende argumenten gewogen onder het kopje Overwegingen.

 

Overwegingen (van bewijs naar aanbeveling)

Om te komen tot een aanbeveling zijn naast (de kwaliteit van) het wetenschappelijke bewijs ook andere aspecten belangrijk en worden meegewogen, zoals de expertise van de werkgroepleden, de waarden en voorkeuren van de patiënt (patient values and preferences), kosten, beschikbaarheid van voorzieningen en organisatorische zaken. Deze aspecten worden, voor zover geen onderdeel van de literatuursamenvatting, vermeld en beoordeeld (gewogen) onder het kopje Overwegingen.

 

Formuleren van aanbevelingen

De aanbevelingen geven antwoord op de uitgangsvraag en zijn gebaseerd op het beschikbare wetenschappelijke bewijs en de belangrijkste overwegingen, en een weging van de gunstige en ongunstige effecten van de relevante interventies. De kracht van het wetenschappelijk bewijs en het gewicht dat door de werkgroep wordt toegekend aan de overwegingen, bepalen samen de sterkte van de aanbeveling. Conform de GRADE-methodiek sluit een lage bewijskracht van conclusies in de systematische literatuuranalyse een sterke aanbeveling niet a priori uit, en zijn bij een hoge bewijskracht ook zwakke aanbevelingen mogelijk. De sterkte van de aanbeveling wordt altijd bepaald door weging van alle relevante argumenten tezamen.

 

Aanbevelingen uit de richtlijn Colorectaal carcinoom 2014

De werkgroep beoordeelde alle aanbevelingen in de richtlijn Colorectaal carcinoom uit 2014 op geldigheid. De werkgroep heeft aanbevelingen gehandhaafd die als nog steeds geldig werden beschouwd. De werkgroep heeft deze aanbevelingen tekstueel opgefrist met als doel de implementeerbaarheid te vergroten. De aanbevelingen zijn niet inhoudelijk veranderd. De werkgroep heeft in het modulair onderhoudsschema vastgesteld wanneer herziening van deze modules gepland is. De werkgroep verwacht dat eind 2020 de gehele richtlijn is herzien.

 

Randvoorwaarden (Organisatie van zorg)

In de knelpuntenanalyse en bij de ontwikkeling van de richtlijn is expliciet rekening gehouden met de organisatie van zorg: alle aspecten die randvoorwaardelijk zijn voor het verlenen van zorg (zoals coördinatie, communicatie, (financiële) middelen, menskracht en infrastructuur). Randvoorwaarden die relevant zijn voor het beantwoorden van een specifieke uitgangsvraag maken onderdeel uit van de overwegingen bij de bewuste uitgangsvraag. Meer algemene, overkoepelende, of bijkomende aspecten van de organisatie van zorg worden behandeld in de module Organisatie van Zorg.

 

Indicatorontwikkeling

Er werden geen interne kwaliteitsindicatoren ontwikkeld om het toepassen van de richtlijn in de praktijk te volgen en te versterken. De werkgroep conformeert zich aan de SONCOS normen en de DICA-DCRA indicatoren.

 

Kennislacunes

Tijdens de ontwikkeling van deze richtlijn is systematisch gezocht naar onderzoek waarvan de resultaten bijdragen aan een antwoord op de uitgangsvragen. Bij elke uitgangsvraag is door de werkgroep nagegaan of er (aanvullend) wetenschappelijk onderzoek gewenst is om de uitgangsvraag te kunnen beantwoorden. Een overzicht van de onderwerpen waarvoor (aanvullend) wetenschappelijk van belang wordt geacht, is als aanbeveling in de Kennislacunes beschreven (onder aanverwante producten).

 

Commentaar- en autorisatiefase

De conceptrichtlijn werd aan de betrokken (wetenschappelijke) verenigingen en patiëntorganisaties voorgelegd ter commentaar. De commentaren werden verzameld en besproken met de werkgroep. Naar aanleiding van de commentaren werd de conceptrichtlijn aangepast en definitief vastgesteld door de werkgroep. De definitieve richtlijn werd aan de deelnemende (wetenschappelijke) verenigingen en (patiënt) organisaties voorgelegd voor autorisatie en door hen geautoriseerd dan wel geaccordeerd.
