Uitgangsvraag

Wat is de optimale lokale behandeling voor patiënten met borstkanker met een lokaal recidief (hiermee wordt zowel true recurrence als second primary bedoeld in deze module) na (eerdere/salvage) mastectomie?

Aanbeveling

Aanbeveling-1 Eindoordeel: Conditionele aanbeveling voor (Doen)

Irresectabel lokaal recidief:

Zet neoadjuvante systemische therapie in om tumorreductie te bereiken wanneer er kans is dat een irresectabel recidief daardoor alsnog resectabel wordt, overeenkomstig de behandeling bij primair mammacarcinoom (zie aanbeveling-2).

 

Pas hoge dosis radiotherapie toe als het recidief irresectabel is. Dit is afhankelijk van de conditie en comorbiditeit van patiënt.

 

Voer bij een lokaal irresectabel recidief in een eerder bestraald gebied re-irradiatie met hyperthermie uit als een curatieve intentie mogelijk is, het gebied verwarmd kan worden met hyperthermie en de conditie van de patiënt dit toelaat.

 

Kies palliatieve radiotherapie (of re-irradiatie met /zonder hyperthermie) of best supportive care als curatieve behandeling niet haalbaar is, afhankelijk van de uitgebreidheid van het recidief en patiëntgebonden factoren zoals leeftijd, comorbiditeit, prognose en performance status.

 

De werkgroep ondersteunt het toepassen van samen beslissen bij deze patiëntengroepen.

 

Aanbeveling-2 Eindoordeel: Zwakke aanbeveling voor (Doen)

Behandel een resectabel lokaal recidief met hoog-risico kenmerken waarvoor nu een salvage mastectomie is verricht met postoperatieve radiotherapie thoraxwand (+/- lymfeklieren.

 

Als hoog-risico kenmerk wordt beschouwd:

    • (Focale) irradicale resectie en geen re-excisie mogelijk

Geen hoog-risico stadia zijn:

    • pT0-2N0-1
    • cT1-2N0-1--> (y)pT1-2N0

Alle andere stadia kunnen worden beschouwd als intermediate- of hoog-risico recidief. Zie overwegingen voor nadere duiding.*

 

Als het lokaal recidief is ontstaan na eerdere mastectomie (voor primair mammacarcinoom of eerder recidief): Lokale excisie met radiotherapie of re-irradiatie thoraxwand indien de thoraxwand eerder is bestraald.

 

In geval van eerdere radiotherapie kan postoperatieve re-irradiatie met of zonder hyperthermie plaatsvinden.

 

Baseer de indicatie voor hyperthermie op de radicaliteit van de resectie, uitgebreidheid van het recidief, de eerdere radiotherapiedosis, wel/geen true recurrence en -toxiciteit en patiëntgebonden factoren zoals leeftijd, comorbiditeit en performance status. Bespreek de mogelijke voor- en nadelen in multidisciplinair overleg.

 

De werkgroep ondersteunt het toepassen van samen beslissen bij deze patiëntengroepen.

Overwegingen

*Naar verwachting wordt eind 2026 de richtlijnmodule over Radiotherapie Thoraxwand gepubliceerd met een update indicaties Radiotherapie n.a.v. recente publicaties (waaronder SUPREMO en NSABP B-51/RTOG 1403). Gezien extrapolatie van postoperatieve radiotherapie-indicaties bij lokaal en locoregionaal recidief, is de aanbeveling de indicaties in dit document voor de recidief-setting aan te houden.

 

Balans tussen gewenste en ongewenste effecten

Conclusies uit de literatuursamenvatting

1. Irresectabel lokaal recidief: primaire radiotherapie of re-irradiatie en hyperthermie

Een hogere thermische dosis bij radiotherapie gecombineerd met hyperthermie bij patiënten met een lokaal recidief van borstkanker na mastectomie verbetert de overall survival ten opzichte van een lagere thermische dosis, wat klinisch relevant is. Tegelijkertijd verhoogt het echter ook de kans op thermische toxiciteit aanzienlijk, wat eveneens klinisch relevant is. Huidtemperaturen boven 43°C correleren sterk met ernstige brandwonden en blaren. Gedurende de behandeling met hyperthermie wordt de huidtemperaturen en indien mogelijk interstitiële temperaturen zorgvuldig bijgehouden om deze te voorkomen. Dit onderstreept het belang van een zorgvuldige afweging tussen effectiviteit en bijwerkingen.

 

Conclusies uit aanvullend onderzoek

2. Resectabel lokaal recidief: lokale excisie na eerdere ablatio; bij high-risk kenmerken postoperatieve re-irradiatie met of zonder hyperthermie

Aangezien er geen systematische reviews en gerandomiseerde studies zijn gevonden over de eventuele toegevoegde waarde van hyperthermie aan electieve postoperatieve re-irradiatie bij resectabel locoregionaal recidief, wordt hieronder een aantal retrospectieve studies beschreven.

 

Een retrospectieve cohortstudie van Bakker (2022) onderzocht het effect van de thermische dosis bij postoperatieve re-irradiatie met hyperthermie bij 112 patiënten met een locoregionaal recidief na eerdere ablatio of lokale excisie. Een hogere thermische dosis (CEM43T50 ≥ 7,2 min) was geassocieerd met een significant betere locoregionale controle na drie jaar (92,3% vs. 74,0%). Er werd geen verschil in overall survival gevonden tussen de groepen. Late toxiciteit graad 2, 3 en 4 werd waargenomen bij respectievelijk 56,3%, 25,0% en 0,9% van de patiënten.

 

In een fase I/II studie van Kapp (1992) werden 89 patiënten met locoregionaal recidief van borstkanker behandeld met hyperthermie in combinatie met radiotherapie. De locoregionale controle na drie jaar was 68%. Complicaties kwamen vaker voor bij een hogere temperatuur (≥ 45°C).

 

Müller (2011) voerde een retrospectieve studie uit bij 42 patiënten met een locoregionaal recidief van borstkanker, behandeld met resectie gevolgd door postoperatieve re-irradiatie of met definitieve re-irradiatie zonder chirurgie. Bij 29 patiënten werd concomitante hyperthermie toegepast. De 5-jaars overleving was 59% en de 5-jaars lokale controle was 62%. Regionale recidieven traden op bij drie patiënten en uitzaaiingen op afstand bij 12 patiënten. De gemiddelde metastasevrije overleving bedroeg 68 maanden en de ziektevrije overleving 54 maanden, beide met een betrouwbaarheidsinterval binnen vijf jaar. Bij re-irradiatie werd een toename van 12% gezien in graad 2 en 3 acute en late huidtoxiciteit in vergelijking met de eerste bestraling. Daarnaast werd milde pneumonitis (graad 1–2) bij vier patiënten en één ribfractuur gerapporteerd.

 

In een retrospectieve studie van Oldenborg (2010) werden 78 patiënten met een locoregionaal recidief van borstkanker in een eerder bestraald gebied behandeld met electieve re-irradiatie (re-RT) en hyperthermie (HT) na chirurgie of klinische complete remissie. Alle patiënten hadden eerder uitgebreide behandeling ondergaan, waaronder chirurgie, hoge dosis radiotherapie (≥50 Gy) en vaak systemische therapie. De 3-jaars en 5-jaars overleving bedroegen respectievelijk 66% en 49%. De 3-jaars en 5-jaars lokale controle waren respectievelijk 78% en 65%. Daarnaast trad acute graad 3 toxiciteit op bij 32% van de patiënten. Late graad ≥3 toxiciteit trad op bij 43% van de patiënten en het risico op late graad ≥3 toxiciteit was 24% na één jaar en 40% na drie jaar. Hyperthermie-gerelateerde bijwerkingen waren blaren (< graad 3) bij 23% en vetnecrose (< graad 3) bij 4% van de patiënten.

 

Welz (2005) voerde een prospectieve studie uit bij 50 patiënten met hoog-risico borstkanker die behandeld werden met gecombineerde hyperthermie en radiotherapie (TRT). Dertien patiënten ontvingen TRT postoperatief in een hoog-risico situatie na ablatio (groep 1), en 37 patiënten kregen TRT na resectie van een locoregionaal recidief met nauwe marges of R1-resectie (groep 2). De geschatte 3-jaars overleving was 89% (groep 1: 90%, groep 2: 89%). De 3-jaars lokale controle was 80% in de totale populatie (groep 1: 75%, groep 2: 81%). De acute huidtoxiciteit bestond uit 37% graad I, 12% graad II en 16% graad III toxiciteit. Hyperthermie werd bij 8 patiënten (15%) voortijdig beëindigd, voornamelijk door blaarvorming. In deze studies is niet beschreven welk percentage van de patiënten hoog-risico kenmerken had.

 

Kwaliteit van bewijs

De overall kwaliteit van bewijs bij studies over irresectabel en resectabel locoregionaal recidief is zeer laag. Dit betekent dat we zeer onzeker zijn over het gevonden geschatte effect van de cruciale uitkomstmaten.

 

Er is sprake van ernstige imprecisie; onnauwkeurigheid, vanwege het niet bereiken van de optimale steekproefgrootte.

 

Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)

1. Irresectabele recidieven: Het gewenste effect van toevoeging hyperthermie aan re-irradiatie is laag.

 

2. Resectabele recidieven: Het gewenste effect van toevoeging hyperthermie aan re-irradiatie is onbekend.

 

Ongewenste effecten van hyperthermie zijn met name het risico op blaarvorming en mogelijk hoger risico op subcutane vetnecrose t.o.v. re-irradiatie zonder hyperthermie.

 

Kostenaspecten

Toevoeging van hyperthermie aan re-RT is een standaardbehandeling waarvan de verwachting is dat het geen gevolgen heeft voor de kosten ten opzichte van de controle behandeling. Op basis van de beschikbare literatuur is het niet mogelijk om een kosten-batenanalyse te maken. De systematische review en meta-analyse van Datta laat zien dat toevoeging van hyperthermie aan palliatieve re-irradiatie bij irresectabele tumoren tot een hogere complete respons kan leiden (t.o.v palliatieve re-irradiatie zonder hyperthermie).

 

Voor resectabele tumoren is de toegevoegde waarde van hyperthermie aan postoperatieve radiotherapie niet duidelijk. Dit wordt uitgezocht in de RT-HYPE studie (BOOG 2023-01), waarin prospectieve data (patiënt-gerapporteerde toxiciteit, kwaliteit van leven en oncologische uitkomsten), wordt verzameld bij patiënten die worden behandeld met postoperatieve re-irradiatie met of zonder hyperthermie.

 

Gelijkheid ((health) equity/equitable)

De interventie leidt bij resectabele recidieven tot een mogelijke toename van gezondheidsgelijkheid want behandeling met hyperthermie hangt naast tumor- en patiënt-gerelateerde factoren ook af van verwijzend centrum en verwijzend arts. Tevens kan de reisafstand naar het hyperthermie-centrum bepalend zijn of de behandeling met hyperthermie haalbaar is, met name bij (oudere) patiënten in een slechtere conditie door bijvoorbeeld co-morbiditeit.

 

Aanvaardbaarheid

Ethische aanvaardbaarheid

De interventie lijkt aanvaardbaar voor de betrokkenen bij irresectabele recidieven. Er zijn geen ethische bezwaren. Bij resectabele recidieven kan de interventie worden besproken in het samen beslissen proces tussen patiënt en zorgverlener voor zover mogelijk gezien de beperkte beschikbaarheid van data over invloed van hyperthermie op oncologische uitkomsten, toxiciteit en kwaliteit van leven.

 

Duurzaamheid

Duurzaamheid heeft geen invloed op de keuze tussen wel of geen interventie.

 

Haalbaarheid

De interventie lijkt haalbaar. De interventie is over het algemeen een van de behandelopties bij irresectabele recidieven. Bij resectabele recidieven is het niet duidelijk wat de standaardzorg is en wordt zoveel mogelijk het samen beslissen proces toegepast.

 

Rationale van de 1e aanbeveling: weging van argumenten voor en tegen de interventies

Bij patiënten met irresectabele recidieven na eerdere mastectomie die ook na neoadjuvante systemische therapie niet resectabel zijn, kan radiotherapie met curatieve intentie overwogen worden mits een voldoende hoge dosis kan worden gegeven. Indien dit niet haalbaar is, kan palliatieve radiotherapie of, bij re-irradiatie, combinatie met hyperthermie worden ingezet afhankelijk van uitgebreidheid en conditie van de patiënt.

 

Rationale van de 2e aanbeveling: weging van argumenten voor en tegen de interventies

Bij operabele hoog-risico recidieven (evt. voorbehandeld met neoadjuvante systemische therapie) na salvage mastectomie of lokale excisie na eerdere mastectomie voor het primaire mammacarcinoom, is er een indicatie voor postoperatieve re-irradiatie bij hoog-risico kenmerken.

Als hoog-risico kenmerk wordt beschouwd:

De indicatie voor toevoeging van hyperthermie aan postoperatieve re-irradiatie (met/zonder neoadjuvante systemische therapie), is onduidelijk en beide behandeling worden momenteel prospectief onderzocht in het RT-HYPE onderzoek (https://www.boogstudycenter.nl/studies/studie-overzicht/rt-hype/). De indicatie voor hyperthermie kan onder andere afhankelijk zijn van eerdere radiotherapie-dosis, -schema en late toxiciteit van de eerdere behandeling. Voor- en nadelen worden besproken aan de hand van de waarden en voorkeuren in het shared decision making proces tussen patiënt en zorgverlener.

Onderbouwing

Background

Locoregional recurrence (i.e., true recurrence or second primary tumor) of breast cancer refers to the return of the disease in the ipsilateral breast, ipsilateral chest wall, or ipsilateral and contralateral regional lymph nodes after initial curative treatment. Advances in systemic therapy and locoregional management have reduced recurrence rates to 2–8% after 10 years in the Netherlands. The optimal management of locoregional recurrent breast cancer is multidisciplinary and based on various prognostic risk factors and prior treatments. From 2010, preoperative systemic therapy has been introduced and resulted in an increased number of resectable locoregional recurrences. The standard surgical treatment of local recurrences consists of salvage mastectomy following prior breast-conserving surgery and radiotherapy or second breast-conserving therapy (see module 9.2.2).. In cases of locoregional recurrence after mastectomy, local excision is indicated (+/- neoadjuvant systemic therapy) and postoperative radiotherapy. However, there is no standard of care for postoperative local/locoregional treatment, neoadjuvant systemic therapy in these patients, there is a need to update the current guidelines.

Summary of literature

Description of studies

Studies from two systematic reviews, Bakker (2019) and Datta (2016), were included in the analysis of the literature. Important study characteristics and results are summarized in table 2. The assessment of the risk of bias is summarized in the risk of bias tables.

 

Bakker (2019) performed a systematic review to determine whether the large variation in outcome observed in clinical studies for patients with recurrent breast cancer treated with radiotherapy + hyperthermia therapy (RT + HT) can be explained by variation in achieved temperatures and thermal dose during HT treatment. Single-arm, double-arm, retrospective and prospective studies (randomized and non-randomized) investigating patients with recurrent breast cancer treated with external beam RT and local HT were included. A systematic search was performed in EMBASE, PubMed, Cochrane library and clinicaltrials.gov on 3 April 2019. In total, 25 studies described in 22 articles were included in the systematic review. Four studies from Bakker (2019) were included in this summary of literature.

 

Datta (2016) conducted a systematic review and meta-analysis to evaluate the outcome of

hyperthermia (HT) and radiation therapy (RT)/re-irradiation in locally recurrent breast cancers (LRBCs). Single-arm and double-arm studies (randomized and non-randomized) were eligible for inclusion, if they fulfilled the following criteria: LRBCs treated with local HT and external RT (those using surgery, concurrent CT, and/or interstitial brachytherapy for LRBCs were excluded), treatment outcome in terms of defined complete responses were reported,

and full-text articles in English were available. A systematic search was performed in eight databases (e.g., PubMed, EMBASE, SCOPUS, and Cochrane library) on May 27, 2015. A total of 34 studies from 31 articles were included in the meta-analysis, comprising 26 single-arm studies. Eight studies from five articles were two-arm comparative trials, including five randomized and three non-randomized studies.

 

1. Irresectable locoregional recurrence: primary radiotherapy (RT) or re-irradiation (re-RT) combined with hyperthermia

Two studies describing overall survival in irresectable locoregional recurrence from the systematic review of Bakker (2018) were included, namely: Refaat (2015) and Sherar (1997).

Studies from the systematic review of Datta (2016) were included in the analysis of the literature.

 

2. Resectable locoregional recurrence: local excision after prior mastectomy; in the presence of high-risk features, postoperative re-RT with or without hyperthermia

Additionally, two studies describing toxicity in resectable locoregional recurrence from the systematic review of Bakker (2018) were included, namely: Lindholm (1995) and Linthorst (2013).

Results

1. Overall survival (critical)

1. Irresectable locoregional recurrence: primary radiotherapy (RT) or re-irradiation (re-RT) combined with hyperthermia

Two studies (Refaat, 2015; Sherar, 1997) reported on the effect of RT+HT higher thermal dose compared to RT+HT lower thermal dose on overall survival (at 18 months) in patients with irresectable locoregional recurrence. The meta-analysis showed a moderate overall survival benefit in favor of the higher thermal dose parameter (relative risk (RR) = 0.67; 95% CI 0.47 to 0.95). Patients in the high thermal dose group had, on average, a 33% lower risk of death compared to those in the low thermal dose group (see Figure 1). This difference is clinically relevant.

No studies reported on the effect of RT+HT higher thermal dose compared to RT+HT lower thermal dose on overall survival in patients with resectable locoregional recurrence.

7. Toxicity (important)

1. Irresectable locoregional recurrence: primary radiotherapy (RT) or re-irradiation (re-RT) combined with hyperthermia

Datta (2016) reported mean grade 3/4 toxicity with RT + HT of 14.4% (SD 10.7%) (based on 24 studies), and late grade 3/4 toxicity of 5.2% (SD 6.5%) (based on 21 studies) in patients with irresectable locoregional recurrence.

 

2. Resectable locoregional recurrence: local excision after prior mastectomy; in the presence of high-risk features, postoperative re-RTx with or without hyperthermia

Two studies in patients with resectable locoregional recurrence (Lindholm, 1995; Linthorst, 2013) reported on the effect of re-RT+HT using a higher thermal dose compared to a lower thermal dose on thermal toxicity. The meta-analysis showed a RR of 2.16 (95% CI 1.51 to 3.10), indicating a higher risk of thermal toxicity in the high thermal dose group. Patients receiving a high thermal dose had, on average, a 116% higher risk of experiencing thermal toxicity compared to those receiving a low thermal dose (see Figure 2). This difference is clinically relevant.

In addition, several studies have demonstrated a strong correlation between maximum skin temperature during hyperthermia treatment and the risk of thermal toxicity. Higher temperatures (>43°C) were consistently associated with an increased incidence of grade 2–3 burns and blisters (Bakker, 2017; Lindholm, 1995; Linthorst, 2013; Linthorst, 2015). Engin (1993) and Seegenschmiedt (1989) found significant thresholds at 43.7°C and 45°C, respectively, above which thermal skin damage increased. Lindholm (1995) and Linthorst (2013) confirmed that higher maximum skin temperatures were a key prognostic factor for blistering, while invasive thermal parameters had no significant effect.

What is the effect of surgery (local excision) and/or radiotherapy with/without hyperthermia, compared to each other, on overall survival, breast-cancer specific survival, local recurrence, regional recurrence, breast cancer events, local control, toxicity, side effects, quality of life, patient-reported outcomes, and cosmetic outcome in patients with a local recurrence after mastectomy?

The guideline panel considered overall survival, breast-cancer specific survival, local recurrence, regional recurrence, breast cancer events, distant metastases, and potential for de-escalation of local treatment as a critical outcome measure for decision making; and quality of life, toxicity, side effects, patient-reported outcomes, local control, and cosmetic outcome as an important outcome measure for decision making.

 

A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.

 

The guideline panel defined the management of locoregional recurrence (LRR) after prior mastectomy as follows:

  • If non-resectable recurrence (+/- after neoadjuvant systemic therapy): treatment with high dose/palliative radiotherapy or consider best supportive care depending on patient- and tumor characteristics, such as extension of locoregional recurrence, condition, age and comorbidity. Primary re-irradiation can be combined with hyperthermia (if the patient’s condition is sufficient).
  • Re-irradiation schedule combined with hyperthermia: 23x2 Gy Gy; 8x4 Gy if 23 fractions are not feasibile, e.g. due to comorbidity. Local excision is indicated for patients with LRR following previous mastectomy if the recurrence is resectable.
  • In cases of non-resectable locoregional recurrence, indication for downstaging tumor and/or axillary lymph nodes, neoadjuvant systemic therapy should be administered if this can increase the chance of resectability or results in lower radiotherapy dose.
  • After salvage mastectomy:
    • (Focal) irradical excision and no re-excision possible
    • Consider postoperative radiotherapy/re-irradiation in all clinical and pathologic stages higher than rcT1-2N0/r(y)pT1-2N0.
  • After local excision (after previous (salvage) mastectomy for primary breast cancer or recurrence):
    • Postoperative radiotherapy/re-RT.
    • Consider adding hyperthermia to postoperative re-RT based on previous radiotherapy dose schedule and toxicity, irradical excision, true recurrence and patient characteristics including age, comorbidity and performance status.

RT dose fractionation schedules:

  • (re)-RT-schedule: 15x2.67 Gy or 5x5.2 Gy
  • Re-RT combined with hyperthermia23x2Gy or 8x4Gy. According to shared decision making.

The guideline panel defined an absolute difference of 5% for all outcomes as a minimal clinically (patient) important difference.

 

Search and select (Methods)

The databases Medline (via OVID) and Embase (via Embase.com) were searched with relevant search terms from 2010 until November 14th, 2024. The detailed search strategy is depicted under the tab Methods. The systematic literature search resulted in 992 hits. Studies were selected based on the following criteria: (1) breast cancer recurrence; (2) radiotherapy OR local excision OR breast-conserving surgery OR neoadjuvant therapy OR salvage mastectomy. Titles and abstracts were screened using the ASReview software. The settings TF-IDF and Naïve bayes were used. Walstra (2019) was used as prior knowledge for inclusion. The first 10% of hits were screened by the working group and the guideline methodologist. The remaining articles were subsequently screened by the guideline methodologist, using the following stopping rule: stop after 10% of the total set subsequent exclusions. 

Based on title and abstract, four articles were initially selected. After screening the full-text articles, two studies were excluded, and two studies were included (see the table with reasons for exclusion under the tab Methods).

Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz

Bij de richtlijnmodule voerden de clusterleden conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).

Bij deze vraag horen 3 PICO’s:

Patiënten met een lokaal recidief, I = lokale excisie of radiotherapie

Patiënten met een lokaal recidief, I = borstsparende operatie

Patiënten met een regionaal recidief, I = neo-adjuvant, chirurgie of radiotherapie

 

Uit de sleutelartikelen blijkt dat er veel overlap is. Met de adviseur wordt afgesproken dat een overkoepelende zoekstrategie zal worden uitgezet, waarbij is gezocht met de concepten:

 

Breastcancer recurrence AND (radiotherapy OR local excision OR breast-conserving surgery OR neoadjuvant therapy or salvage mastectomy)

 

Omdat er veel literatuur wordt gevonden, is besloten om de selectie met ASReview te doen. Vanwege de aantallen wordt gestart met alleen de SRs (afgestemd met Tim), in de hoop dat we een bruikbare SR vinden van waaruit we een aanvulling kunnen doen naar primaire studies.

Op een later moment kunnen de clinical trials worden toegevoegd als het nodig mocht zijn. Deze zijn opgeslagen in OneDrive.
