Uitgangsvraag
Wat is de optimale lokale behandeling voor patiënten met borstkanker met een lokaal recidief na borstsparende behandeling?
Aanbeveling
Eindoordeel: Zwakke aanbeveling voor
Voer een salvage mastectomie uit bij patiënten met een lokaal recidief na borstsparende behandeling (zie ook module Lokale behandeling na mastectomie).
Kies voor een tweede borstsparende behandeling indien aan de volgende voorwaarden wordt voldaan:
- Er is een sterke wens tot behoud van de borst.
- De voor- en nadelen worden met de patiënt besproken (samen beslissen)
- Het interval tussen de primaire tumor en het recidief is minimaal 24 maanden.
- Er zijn geen ernstige late radiatie-effecten, mede beoordeeld door de radiotherapeut.
- Het betreft een laag-risico recidief (kliernegatief, bij voorkeur hormoonreceptor positief, unifocale tumor (op MRI), < 2–3 cm).
- Eerdere radiotherapie met een boost, of een gehypofractioneerd schema (5 fracties) is toegestaan, mits extra aandacht voor de late toxiciteit
- De behandeling is in een multidisciplinair overleg besproken.
- Na re-borstsparende operatie wordt adjuvante radiotherapie sterk aangeraden om het risico op een nieuw lokaal recidief zoveel mogelijk te verkleinen.
- Er wordt partiële borstbestraling gegeven, met als schema 23-25x2 Gy, 15x2.67 Gy, of een equivalent hiervan (EQD2).
Overwegingen
Balans tussen gewenste en ongewenste effecten
Conclusies uit de literatuursamenvatting
Hoewel een herhaalde borstsparende behandeling een klein voordeel in 5-jaars overall survival laat zien bij patiënten met borstkanker met een lokaal recidief na borstsparende behandeling, is dit effect niet klinisch relevant. Daarentegen is de kans op een tweede lokaal recidief bij een herhaalde borstsparende behandeling zonder radiotherapie significant verhoogd, wat wel een klinisch relevant effect is. Dit benadrukt het belang van een zorgvuldige afweging van de voor- en nadelen bij de keuze voor een tweede borstsparende operatie en benadrukt het belang van re-irradiatie (Tollan, 2022).
Conclusies uit aanvullend onderzoek
Veel andere studies hebben geen vergelijking gemaakt met de huidige standaardbehandeling (salvage mastectomie). In een systematische review van Walstra (2019) zijn deze (31) studies wel meegenomen. Achttien van deze studies rapporteerden over borstsparende behandeling met radiotherapie. De 5-jaars lokale controle was 76% voor borstsparende chirurgie alleen, en 89% voor borstsparende chirurgie gevolgd door re-irradiatie (Walstra, 2019).
Re-irradiatie verkleint de kans op een lokaal recidief en heeft een acceptabel bijwerkingsprofiel. In een fase 2-studie (NRG oncology/RTOG 1014) naar uitwendige partiële borstbestraling bij een tweede borstsparende behandeling bij 65 patiënten, werd een laag risico op een tweede lokaal recidief (5%) gevonden. Late graad 3 toxiciteit trad op bij 7% van de patiënten (Arthur, 2020).
In een grote retrospectieve cohortstudie van Hannoun-Levi (2021) met 1327 patiënten werd de uitkomst vergeleken tussen patiënten behandeld met een salvage mastectomie en een tweede borstsparende behandeling (chirurgie gevolgd door re-irradiatie met brachytherapie). Na vijf jaar was er geen verschil in overall survival: 88% bij de mastectomiepatiënten en 87% bij de patiënten met een tweede borstsparende behandeling. De cumulatieve incidentie op een tweede lokaal recidief was 2,3% versus 2,8% (Hannoun-Levi, 2021).
Toxiciteit en cosmetische uitkomsten
Walstra (2019) rapporteerde over toxiciteit bij borstsparende chirurgie met re-irradiatie: in 11 studies varieerden de toxiciteitspercentages tussen de 2 en 21%. Wanneer acute en late toxiciteit gezamenlijk werden beoordeeld, waren de gewogen schattingen voor graad 3-4 toxiciteit 11% bij hele borstbestraling en 9% bij partiële borstbestraling. De cosmetische uitkomsten waren wisselend: tussen 29 en 100% van de patiënten werd beoordeeld als “excellent” tot “goed” (Walstra, 2019).
Kwaliteit van leven
Een Duitse studie (Jendrian, 2017) bij 124 patiënten (61 behandeld met mastectomie, 46 met borstsparende chirurgie) onderzocht verschillen in kwaliteit van leven na behandeling. De resultaten lieten zien dat patiënten die een tweede borstsparende behandeling ondergingen beter scoorden op het gebied van ‘Body Image’ en algemene kwaliteit van leven.
Patiëntselectie
Een tweede borstsparende behandeling kan een goed alternatief zijn voor mastectomie, met name voor vrouwen met een sterke wens tot het behoud van hun borst. Een zorgvuldige patiëntselectie is daarbij essentieel, zoals ook benoemd in de St. Gallen International Consensus Guideline (Burstein, 2021). Beschreven criteria hiervoor kunnen zijn: tumorgrootte < 2 cm, geen fixatie aan de huid of borstwand, klinisch negatieve okselklieren en geen duidelijke restverschijnselen van eerdere radiotherapie (Kurtz, 1988). Andere criteria zijn het ziektevrije interval en de histopathologie van het recidief (Lee, 2015). Kleine, unifocale tumoren (<2-3 cm) en een ziektevrij interval van >48 maanden worden voorgesteld in andere aanbevelingen (Gentilini, 2012; Harms, 2016). Een individuele aanpak in een multidisciplinair team is cruciaal (Burstein, 2021).
In een groep van 508 patiënten die een tweede borstsparende operatie met partiële borstbestraling met brachytherapie hebben ondergaan is een prognostische score gedefinieerd, de TAM score (Hannoun-Levi, 2023). De TAM score bestaat uit de combinatie van het tijdsinterval tussen de primaire tumor en het recidief (T), de GEC-ESTRO APBI classificatie (A; low-, intermediate-, high risk) en de moleculaire classificatie (M; een score bepaald aan de hand van de 3 groepen: 1. hormoonreceptor positief/HER2 negatief, 2. HER2 positief, 3. triple negatief). In multivariate analyse was de TAM score (hoog versus laag) een onafhankelijke prognostische factor voor alle oncologische eindpunten (behalve cause-specific survival).
Kwaliteit van bewijs
De overall kwaliteit van bewijs is zeer laag. Dit betekent dat we zeer onzeker zijn over het gevonden geschatte effect van de cruciale uitkomstmaten.
Er is afgewaardeerd vanwege ernstige:
- Risk of Bias: methodologische beperkingen, waaronder met name selectie en opzet van de beschikbare studies (retrospectieve studies).
- Inconsistentie: inconsistentie van de resultaten.
- Imprecisie: onnauwkeurigheid, omdat het betrouwbaarheidsinterval de grens van klinische relevantie overschrijdt.
Waarden en voorkeuren van patiënten (en eventueel hun naasten/verzorgers)
Een tweede borstsparende behandeling kan een goed alternatief zijn voor een mastectomie met name voor vrouwen met een sterke wens tot behoud van hun borst. Een gewenst effect is het behoud van de borst en hierbij een betere kwaliteit van leven. Ongewenste effecten zijn de mogelijke bijwerkingen die kunnen optreden door de re-irradiatie. Hier staat tegenover dat een mastectomie een grotere chirurgische ingreep is, met meer mogelijke complicaties.
Kostenaspecten
Het is onbekend wat de interventie qua kosten oplevert ten opzichte van de controle behandeling.
Gelijkheid ((health) equity/equitable)
De interventie leidt tot niet tot verschil in gezondheidsgelijkheid want operatie en bestraling is voor iedereen in Nederland toegankelijk.
Aanvaardbaarheid:
Ethische aanvaardbaarheid
De interventie lijkt aanvaardbaar voor de betrokkenen. Er zijn geen ethische bezwaren.
Duurzaamheid
Er zijn geen relevantie duurzaamheidsaspecten te vermelden.
Haalbaarheid
De interventie lijkt haalbaar. De interventie is over het algemeen al standaardzorg in de praktijk.
Rationale van de aanbeveling: weging van argumenten voor en tegen de interventies
Dit is een zwakke aanbeveling voor een tweede borstsparende behandeling in plaats van een salvage mastectomie, mits aan strikte selectiecriteria wordt voldaan. Hoewel de oncologische uitkomsten vergelijkbaar zijn bij zorgvuldig geselecteerde patiënten die na een borstsparende operatie tevens re-irradiatie ondergaan, is de kwaliteit van bewijs laag en moeten voordelen zoals behoud van de borst en kwaliteit van leven worden afgewogen tegen het risico op toxiciteit.
Onderbouwing
Background
Locoregional recurrence of breast cancer refers to the return of the disease in the ipsilateral breast, ipsilateral chest wall, or ipsilateral and contralateral regional lymph nodes after initial curative treatment. Advances in treatment have reduced the incidence of recurrence to 2-8% after 10 years in the Netherlands. For local recurrence (i.e., true recurrence or second primary), salvage mastectomy has traditionally been the standard treatment after prior breast-conserving therapy. However, emerging evidence suggests that repeat breast-conserving therapy, combined with partial breast re-irradiation, may be a viable option for selected low-risk patients. Additionally, neoadjuvant systemic therapies have expanded surgical options for high-risk patients. These developments highlight the need to update current guidelines.

Summary of literature
Description of studies
One study was included in the analysis of the literature. Important study characteristics and results are summarized in table 2. The assessment of the risk of bias is summarized in the risk of bias tables .
Tollan (2022) performed a systematic literature review and meta-analysis of oncological outcomes in patients treated with repeat breast-conserving surgery (rBCS) with or without repeat radiotherapy for the management of ipsilateral breast cancer recurrence (IBCR) following previous breast-conserving surgery (BCS) and radiotherapy. Studies were eligible for inclusion if they reported on second local recurrence (LR) and/or survival after rBCS for IBCR following previous BCS and radiotherapy. Both prospective and retrospective comparative and non-comparative cohort studies, as well as cross-sectional studies, were considered. Studies that did not clearly differentiate whether the reference population had been initially treated for ductal carcinoma in situ (DCIS) alone or for both DCIS and invasive breast cancer (IBC) were included in the primary analysis. A systematic search was performed in MEDLINE and EMBASE databases without chronological limitations, including studies up to 2021. In total, 42 studies were included, encompassing observational studies.

Results
1. Overall survival (critical)
Twenty studies (Alpert, 2005; Baek, 2021; Chen, 2008; Fodor, 2008; Gentile, 2021; Houvenaeghel, 2017; Kolben, 2015; Komoike, 2005; Kurtz, 1988; Lee, 2015; Li, 2021; Panet-Raymond, 2011; Salvadori, 1999; Sellam, 2019; Shah, 2012; Smanyoko, 2019; Su, 2019; Wang, 2021; Wu, 2021; Yoshida, 2016) reported on the effect of second breast-conserving surgery (with/without radiotherapy) compared to salvage mastectomy on overall survival.
The meta-analysis showed a small overall survival benefit in favor of rBCS (relative risk (RR) = 1.04 (95% CI 1.00 to 1.08); patiënts in the rBCS group had, on average, a 4% higher overall survival compared to those in the salvage mastectomy group (see Figure 1). This difference is not clinically relevant.

2. Breast-cancer specific survival (critical)
No studies reported on the effect of second breast-conserving surgery (with/without radiotherapy) compared to salvage mastectomy on breast-cancer specific survival.
3. Local recurrence (critical)
Seventeen studies (Abner, 1993; Alpert, 2005; Dalberg, 1998; Demicheli, 2013; ElSherif, 2022; Fodor, 2008; Gentile, 2021; Houvenaeghel, 2017; Kolben, 2015; Komoike, 2005; Kurtz, 1990; Salvadori, 1999; Sellam, 2019; Smanyko, 2019; Van den Bruele, 2021; Voogd, 1999; Wapnir, 2017) reported on the effect of second breast-conserving surgery (with/without re-irradiation) compared to salvage mastectomy on local recurrence. The meta-analysis reported a RR for second local recurrence of 2.10 (95% CI 1.53 to 2.88), in favor of salvage mastectomy; patiënts in the rBCS group (with/without re-irradiation) had, on average, a 10% higher risk of second local recurrence compared to those in the salvage mastectomy group (see Figure 2). This result is clinically relevant.

In the subgroup analysis of patiënts treated with rBCS followed by re-irradiation, the second local recurrence rate was 9.6%, compared to 10.3% after salvage mastectomy, and 15.7% after rBCS with and rBCS without re-irradiation.

Relevant outcome measures
The guideline panel considered overall survival, breast-cancer specific survival, local recurrence, regional recurrence, and breast cancer events as a critical outcome measure for decision making; and local control, toxicity, side effects, quality of life, patient-reported outcomes, and cosmetic outcome as an important outcome measure for decision making.
A priori, the guideline panel did not define the outcome measures listed above but used the definitions used in the studies.
The guideline panel defined an absolute difference of 5% for all outcomes as a minimal clinically (patient) important difference.
Search and select (Methods)
The databases Medline (via OVID) and Embase (via Embase.com) were searched with relevant search terms from 2010 until November 14th, 2024. The detailed search strategy is depicted under the tab Methods. A single search strategy was used for four modules addressing local treatment of locoregional recurrences. The systematic literature search resulted in 992 hits. Studies were selected based on the following criteria: (1) breast cancer recurrence; (2) radiotherapy OR local excision OR breast-conserving surgery OR neoadjuvant therapy OR salvage mastectomy. Titles and abstracts were screened using the ASReview software. The settings TF-IDF and Naïve bayes were used. Walstra (2019) was used as prior knowledge for inclusion. The first 10% of hits were screened by the working group and the guideline methodologist. The remaining articles were subsequently screened by the guideline methodologist, using the following stopping rule: stop after 10% of the total set subsequent exclusions.
Based on title and abstract, 5 articles were initially selected. After screening the full-text articles, four studies were excluded, and one study was included (see the table with reasons for exclusion under the tab Methods).

Belangenverklaringen
Een overzicht van de belangen van de clusterleden vindt u in onderstaande tabel. Alle clusterleden hebben schriftelijk verklaard of zij in de laatste drie jaar directe financiële belangen (betrekking bij een commercieel bedrijf, persoonlijke financiële belangen, onderzoeksfinanciering) of indirecte belangen (persoonlijke relaties, reputatiemanagement) hebben gehad. Gedurende de ontwikkeling of herziening van een module worden wijzigingen in belangen aan de voorzitter doorgegeven. De belangenverklaring wordt opnieuw bevestigd tijdens de commentaarfase.
Belangrijk om te benoemen dat:
- Clusterleden werken niet als enige inhoudsdeskundige aan de module;
- Een clusterlid werkt tenminste samen met een ander clusterlid met een vergelijkbare expertise in alle fasen (zoeken, studieselectie, data-extractie, evidence synthese, Evidence-to-decision, aanbevelingen formuleren) van het ontwikkelproces. Indien nodig worden clusterleden toegevoegd aan het cluster;
- In alle fasen van het ontwikkelproces is een onafhankelijk methodoloog betrokken;
- Overwegingen en aanbevelingen worden besproken en vastgesteld tijdens een clustervergadering onder leiding van een onafhankelijk voorzitter.
Wellicht ten overvloede willen wij erop wijzen dat medisch specialistische richtlijnen niet worden vastgesteld door de betreffende richtlijnwerkgroep maar door de besturen/ledenvergadering van de betrokken verenigingen.

Kwalitatieve raming van mogelijke financiële gevolgen in het kader van de Wkkgz
Bij de richtlijnmodule voerden de clusterleden conform de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) een kwalitatieve raming uit om te beoordelen of de aanbevelingen mogelijk leiden tot substantiële financiële gevolgen. Bij het uitvoeren van deze beoordeling is de richtlijnmodule op verschillende domeinen getoetst (zie het stroomschema bij Werkwijze).

Toelichting: Bij deze vraag horen 3 PICO’s:
Uit de sleutelartikelen blijkt dat er veel overlap is. Met de adviseur wordt afgesproken dat een overkoepelende zoekstrategie zal worden uitgezet, waarbij is gezocht met de concepten:
Breastcancer recurrence AND (radiotherapy OR local excision OR breast-conserving surgery OR neoadjuvant therapy or salvage mastectomy)
Omdat er veel literatuur wordt gevonden, is besloten om de selectie met ASReview te doen. Vanwege de aantallen wordt gestart met alleen de SRs (afgestemd met Tim), in de hoop dat we een bruikbare SR vinden van waaruit we een aanvulling kunnen doen naar primaire studies. Op een later moment kunnen de clinical trials worden toegevoegd als het nodig mocht zijn. Deze zijn opgeslagen in OneDrive.
