Wat is de effectiviteit van de combinatie anticonceptiepil (OAC) (alleen of in combinatie met andere middelen) bij de behandeling van hormonale en klinische symptomen bij adolescenten en volwassenen met PCOS?
Aanbeveling
Anticonceptiepil alleen
Adviseer volwassen vrouwen met PCOS een combinatie anticonceptiepil voor de behandeling van hirsutisme en/of onregelmatige menstruaties.
Overweeg ook bij adolescenten met een duidelijke diagnose van PCOS het gebruik van een combinatie anticonceptiepil voor de behandeling van klinische symptomen van hirsutisme en/of onregelmatige menstruaties.
Overweeg ook bij adolescenten met een verhoogd risico op PCOS zonder definitieve diagnose het gebruik van een combinatie anticonceptiepil voor de behandeling van hirsutisme en/of onregelmatige menstruaties.
Hanteer de reguliere richtlijnen voor de dosering van progestagenen, oestrogenen bij het voorschrijven van een combinatie anticonceptiepil bij adolescenten en volwassenen met PCOS.
Beschouw de 35 microgram ethinyloestradiol plus cyproterone acetate preparaten niet als eerste keuze behandeling voor PCOS vanwege de bijwerkingen, waaronder het risico op diepe veneuze trombo-embolie volgens de reguliere richtlijnen.
Bij het voorschijven van een combinatie anticonceptiepil:
- Er wordt bij vergelijk tussen verschillende combinatie anticonceptiepillen geen verschil gezien in het effect op hirsutisme.
- Gebruik bij voorkeur de laagste effectieve dosis van oestrogenen (bijvoorbeeld 20-30 microgram ethinylestradiol of gelijkwaardig) en natuurlijke oestrogenen preparaten; weeg daarbij de risico’s af tegen het metabole risicoprofiel, de bijwerkingen, kosten en beschikbaarheid.
- Wees bewust van de beperkte evidence over het gebruik van combinatie anticonceptiepillen bij vrouwen met PCOS. De beschikbare kennis berust veelal op richtlijnen voor de algemene populatie.
- Neem relatieve en absolute contra-indicaties en bijwerkingen in overweging bij het maken een individueel behandeladvies.
- Neem PCOS-specifieke risicofactoren zoals een verhoogd BMI, hyperlipidemie en hypertensie mee in het individuele behandeladvies.
Anticonceptiepil in combinatie met andere middelen
Overweeg het toevoegen van Metformine naast een combinatie anticonceptiepil bij vrouwen met PCOS voor de behandeling van metabole symptomen, wanneer de combinatie anticonceptiepil en leefstijlinterventies onvoldoende effect hebben gehad.
Overweeg het toevoegen van Metformine naast een combinatie anticonceptiepil bij adolescenten met PCOS met een BMI ≥ 25kg/m2 als de combinatie anticonceptiepil en leefstijlinterventies onvoldoende effect hebben gehad.
Metformine heeft mogelijk de grootste toegevoegde waarde in de populatie PCOS-vrouwen met een hoog metabool-risico waaronder diegenen met risicofactoren voor diabetes, verminderde glucosetolerantie en in specifieke etnische groepen met een verhoogd risico.
Overweeg behandeling met antiandrogenen voor de behandeling van hirsutisme bij vrouwen met PCOS waarbij na 6 maanden gebruik van een combinatie anticonceptiepil samen met een cosmetische behandeling onvoldoende effect hebben gehad.
Overweeg behandeling met antiandrogenen in combinatie met een combinatie anticonceptiepil bij patiënten met PCOS met androgeen-gerelateerde alopecia.
Gebruik antiandrogenen bij vrouwen met PCOS alleen in combinatie met sluitende anticonceptie om het risico op verminderde foetale mannelijke virilisatie te voorkomen.
Met betrekking tot deze medicatie geldt variërende regelgeving (off label gebruik) en beschikbaarheid, voor sommige middelen geldt een verhoogd risico op levertoxiciteit.

Anticonceptiepil alleen
Voor- en nadelen van de interventie en de kwaliteit van het bewijs
Ook al worden combinatie anticonceptiepillen (OAC) veelvuldig gebruikt, ze zijn niet zonder risico’s. Met name als er additionele risicofactoren zijn zoals overgewicht, een verhoogd familie risico op trombose en roken.
De keuze voor een specifiek OAC kan worden bepaald op basis van het risicoprofiel van het progestageen en de bijwerkingen. Gecombineerde OAC met levonorgestrel, norethisterone en norgestimate worden geassocieerd met het laagste tromboserisico. De effecten van het pilgebruik op klinische symptomen en metabole kenmerken van PCOS zijn wisselend in verschillende studies.
De belangrijkste evidence is afkomstig van studies en ervaring van OAC gebruik in de algemene populatie, niet specifiek bij PCOS.
Waarden en voorkeuren van patiënten (en evt. hun verzorgers)
Het gebruik van gecombineerde OAC als anticonceptivum is breed geaccepteerd. Dit is niet separaat onderzocht bij vrouwen met PCOS.
Kosten (middelenbeslag)
De pil wordt bij vrouwen boven de 18 niet compleet vergoed in de basisverzekering tenzij er een medische indicatie is zoals menorragie of endometriose; PCOS is hier momenteel niet voor geregistreerd.
Anticonceptiepil in combinatie met metformine
Voor- en nadelen van de interventie en de kwaliteit van het bewijs
Het gebruik van de Metformine al dan niet in combinatie met een gecombineerd OAC versus een gecombineerd OAC is onderzocht in verschillende RCT’s, met name bij volwassenen. De kwaliteit van de beschikbare evidence was desondanks overwegend laag. Uit de onderzoeken blijkt met name een metabool effect in de BMI > 25 subgroep. Het effect van Metformine in combinatie met een gecombineerde OAC lijkt niet sterker dan een gecombineerde OAC met lifestyle interventies. Metformine wordt daarom pas geadviseerd als deze combinatie niet afdoende blijkt. Het voornaamste effect wordt verwacht bij vrouwen met PCOS en obesitas.
Metformine heeft met name gastro-intestinale bijwerkingen; zie de adviezen hierover in aanbeveling 4.4.5.
Aanvaardbaarheid, haalbaarheid en implementatie
Het off-label voorschrijven van Metformine door gynaecologen met aandachtsgebied endocrinologie en zeker ook de huisarts zal in de praktijk afstemming vragen. In module 4.4 wordt nader ingegaan op indicaties en doseringen van Metformine waarbij binnen het lokale zorgnetwerk zal moeten worden afgestemd wie uiteindelijk het voorschrijven zal doen.
Anticonceptiepil in combinatie met antiandrogeen

Er is zeer beperkte bewijskracht voor het gebruik van antiandrogenen zonder uitgesproken voordelen ten opzichte van een gecombineerde OAC. Met betrekking tot antiandrogenen geldt variërende regelgeving en beschikbaarheid, voor sommige middelen geldt een verhoogd risico op levertoxiciteit. Zij dienen altijd gecombineerd te worden met sluitende anticonceptie.

Combined contraceptives, including oral contraceptive pills, are commonly prescribed for adults and adolescents with PCOS to ameliorate the clinical symptoms and associated hormonal disturbances. The effects of COCPs on menstrual cycle, hirsutism, weight loss, waist/hip ratio, testosterone concentrations, lipid profile and blood sugar levels are
variably reported and depend on type of COCP used, duration of use, severity of presentation/phenotype, adherence to the regimen, among other factors. Different combinations of COCPs are available with heterogeneous estrogen and progestin preparations with varying pharmacological and clinical properties. Thus, the efficacy and consequences of COCPs in PCOS may vary. Some preparations also comprise natural estrogen instead of synthetic ethinyloestradiol (EE) with benefits and contraindications considered similar.

One randomised controlled trial (RCT) was identified to address this comparison in adolescents (Hoeger, 2008). There was a statistically significant improvement with COCP (compared to placebo) for high-density lipoprotein (HDL) in this very low quality study with low certainty. No statistically significant differences were found for outcomes: body mass index (BMI) (kg/m2); Waist (cm); Total testosterone (ng/dl); sex hormone-binding globulin (SHBG) (nmol/liter); free androgen index (FAI); Hirsutism (FG score); Total cholesterol (mg/dl); LDL (mg/dl); Triglycerides (mg/dl); Fasting insulin (IU/ml); Fasting blood sugar (mg/dl); CRP (mg/l); PAI-1. COCP versus lifestyle
One RCT was identified to address this comparison in adolescents (Hoeger, 2008). There was a statistically significant improvement with lifestyle (compared to COCP) for low-density lipoprotein (LDL) in this very low quality study with very low certainty. No statistically significant differences were found for: BMI (kg/m2); Total testosterone (ng/dl); SHBG (nmol/liter); FAI; Hirsutism (FG score); Total cholesterol (mg/dl); HDL (mg/dl); Triglycerides (mg/dl); Fasting insulin (IU/ml); Fasting blood sugar (mg/dl); CRP (mg/l); PAI-1. COCP versus metformin
A systematic review including four RCTs that address this comparison in adolescents was identified (Al Khalifah, 2016). The evidence team conducted additional analysis of outcomes not addressed in the systematic review. While a statistically significant improvement was found in BMI and LDL with use of metformin over COCP; and a statistically significant improvement was found in menstrual regulation with use of COCP over metformin, we remain cautious due to very low certainty in effect estimates and the quality of evidence. A statistically significant improvement in dysglycemia (OGTT) was found with the use of metformin over COCP, however it should be noted that there is low certainty in the effect estimates and the quality of evidence. No statistically significant differences were found for: Hirsutism; Total Testosterone (nmol/L); Triglyceride (mg/dL); Total Cholesterol (mg/dL); HDL (mg/dL); Weight (kg); Fasting insulin; SHBG; FAI; Fasting blood sugar (mg/dL); CRP (mg/L); PAI-1. Side effects included weight gain with COCP; and side effects were not specified with metformin.
COCP versus metformin + anti-androgen
One RCT was identified to address this comparison is adolescents (Ibanez, 2004). Due to the lack of direct comparisons between groups (no p values reported for between groups) it is uncertain whether there were any differences in this very low quality study with very low certainty for outcomes: BMI (kg/m2); Hirsutism (FG score); Glucose/insulin ratio; SHBG (ug/dl); Testosterone (ng/dl); Triglycerides (mg/dl); HDL (mg/dl); LDL (mg/dl); Cycle regularity; Weight. Research evidence – ADULTS
COCP versus metformin
Nine RCTs were identified to address this comparison (Dardzinska, 2014; Harborne, 2003; Kilic, 2011; Meyer, 2007; Morin-Papunen, 2003; Morin-Papunen, 2000; Moro, 2013; Rautio, 2005; Wu, 2008). There were statistically significant improvements with metformin (compared with COCP) for fasting insulin, including for both BMI subgroups. Metformin improved HDL in the BMI > 25 subgroup but not in the BMI < 25 subgroup or when all participants were combined; and improved triglycerides when all participants were combined, in the BMI > 25 subgroup and in the subgroup where BMI was not defined, but not in the BMI < 25 subgroup. There were statistically significant improvements with COCP (compared with metformin) for SHBG, FAI, total testosterone and irregular cycles, including for all BMI subgroups. COCP improved LDL in the BMI > 25 subgroup but not in the BMI < 25 subgroup or when all participants were combined. No statistically significant differences were found for: Weight; Clamp (M value); homeostatic model assessment (HOMA) (change from baseline); BMI (kg/m2); waist-hip-ratio (WHR); Hirsutism [FG score]; Fasting glucose [mmol/l], Total cholesterol [mmol/l]. Metformin use increased GI-related events, whereas the COCP group had none. While one of the included studies was of moderate quality and certainty, the majority of studies in these meta-analyses were of low to very low certainty in effect estimates and the quality of evidence and therefore all findings should be interpreted with caution.
COCP versus COCP + metformin
Six RCTs were identified to address this comparison (Cibula, 2005; Elter, 2002; Essah, 2011; Feng, 2016, Moro, 2013; Wu, 2008; ). There was a statistically significant improvement with COCP alone (compared with COCP plus metformin) for triglycerides. COCP alone improved SHBG in the BMI < 25 subgroup but not in the BMI > 25 subgroup or when all participants were combined. There were statistically significant improvements with COCP plus metformin (compared with COCP alone) for FAI. COCP plus metformin improved testosterone when all participants were combined but not in BMI subgroups; improved hirsutism and fasting glucose when all participants were combined and in the BMI > 25 subgroup but not in the BMI < 25 subgroup; improved SHBG and fasting insulin in the BMI < 25 subgroup but not when all participants were combined or in the BMI > 25 subgroup; and improved total cholesterol in the BMI > 25 subgroup but not when all participants were combined or in the BMI < 25 subgroup. No statistically significant differences were found for: HOMA; Weight (kg); BMI (kg/m2); WHR; HDL [mmol/l]; LDL [mmol/l]. The addition of metformin to COCP increased GI-related events, whereas the COCP alone group had none. COCP (DRP-containing COCs) + metformin + lifestyle versus COCP (CPA-containing COCs) + metformin + lifestyle
One study was identified to address this comparison in adults (Wang, 2016). There were no statistically significant differences reported between the two interventions (differing by the combination in the COCP) in this very low quality study of very low certainty for outcomes: WHR; Fasting plasma glucose (mmol/L); HbA1c (%); Total cholesterol (mmol/L); LDL (mmol/L). Side effects were not reported.
Summary of narrative review evidence
Evidence on COCP use from the general population also informed recommendations. Consideration of adverse effects is needed before prescribing COCPs. Absolute contraindications for COCP use according to world health organisation (WHO) include a history of migraine with aura, deep vein thrombosis (DVT)/pulmonary emboli (PE), known
thrombogenic mutations, multiple risk factors for arterial cardiovascular disease, history of ischemic heart disease or stroke, complicated valvular heart disease, breast cancer, neuropathy, severe cirrhosis and malignant liver tumours (World Health Organization, 2015). Other risk factors for DVT need consideration including postpartum immobility, transfusion at delivery, BMI > 30 kg/m2, postpartum haemorrhage, immediately post-caesarean delivery, preeclampsia or smoking. Current evidence suggests that COCPs containing levonorgestrel, norethisterone and norgestimate are associated with the lowest relative risk of DVT. Also, WHO recommends that COCPs with 35 micrograms of EE and cyproterone acetate should only be used when treating moderate to severe hirsutism or acne due to higher DVT risk. For contraception, irregular menstrual cycles and mild to moderate hirsutism, other lower risk preparations are recommended first line (World Health Organization, 2018).

Justifications
Although relatively safe, COCPs have absolute and relative contraindications and risks and benefits in the general population that need consideration by health professionals and women.
Although combined metformin and COCP offers additional benefits, these did not surpass the impact of COCP plus lifestyle intervention. Hence the combination is indicated where COCP and lifestyle have failed to meet goals. A combination regime may also lead to increased mild gastrointestinal side effects, which can impact on adherence. Strategies to reduce side effects are available (see metformin recommendations below). With metformin therapy in addition to COCP, women with PCOS and obesity may yield the greatest benefit. The PCOS phenotype, BMI, ethnicity and the informed preference of the individual with PCOS need to be considered when recommending pharmacological agents for the treatment of PCOS.
COCPs, metformin and anti-androgens are off label treatments specifically for treatment of PCOS. However, use is evidence-based for the treatment of clinical features of PCOS and is generally not restricted for use in PCOS. Women should be informed of the benefits and risks and the regulation status of relevant medications. The combination of off label treatments with the COCP is not routine practice and will require education and integration into algorithms. This is anticipated to significantly change practice. Due to subgroup differences in recommendations, the personal characteristics of all women need to be considered.

De ontwikkeling van deze richtlijn werd ondersteund door het Kennisinstituut van de Federatie Medisch Specialisten (www.demedischspecialist.nl/kennisinstituut) en werd gefinancierd uit de Kwaliteitsgelden Medisch Specialisten (SKMS). De financier heeft geen enkele invloed gehad op de inhoud van de richtlijnmodule.

Alle werkgroepleden hebben schriftelijk verklaard of zij in de laatste drie jaar directe financiële belangen (betrekking bij een commercieel bedrijf, persoonlijke financiële belangen, onderzoek financiering) of indirecte belangen (persoonlijke relaties, reputatiemanagement) hebben gehad. Inbreng patiëntenperspectief
Er werd aandacht besteed aan het patiëntenperspectief door het afgevaardigden van patiëntvertegenwoordigers van Stichting PCOS en Freya in de werkgroep (zie Samenstelling van de werkgroep). Zij hebben deelgenomen aan diverse werkgroepvergaderingen waarin inbreng van patiëntvertegenwoordigers is gevraagd. De input die zij hebben geleverd op de overwegingen (patiëntenperspectief) is verwerkt door de betreffende auteurs van de module. De richtlijn is ter commentaar voorgelegd aan stichting PCOS en Freya en de aangeleverde commentaren zijn verwerkt.
Werkwijze
Adaptatie
Bij dit adaptatietraject voldoet de te ontwikkelen richtlijn aan de adviezen uit het adviesrapport MSR 2.0. Hierbij werden de volgende domeinen getoetst, gebaseerd op het ADAPTE-tool en het AGREE-II instrument: (1) onafhankelijkheid van de richtlijn, (2) methodologische kwaliteit, (3) projectomvang, toetsing raamwerk en plan van aanpak. Omdat een aantal onderdelen van het reguliere richtlijntraject al door de internationale werkgroep was uitgevoerd, is het stappenplan ‘adaptatietraject’ uit het adviesrapport MSR 2.0 gevolgd voor het adapteren van een internationale richtlijn naar een Nederlandse versie.
AGREE
Deze richtlijnmodule is opgesteld volgens de eisen vermeld in het rapport Medisch Specialistische Richtlijnen 2.0 van de adviescommissie Richtlijnen van de Raad Kwaliteit. Dit rapport is gebaseerd op het AGREE II instrument (Appraisal of Guidelines for Research & Evaluation II; Brouwers, 2010).
Knelpuntenanalyse en uitgangsvragen
Uit inventarisatie van de knelpunten door de commissie van de NVOG bleek dat er een noodzaak was voor het adapteren van deze richtlijnmodules. Ook zijn tijdens een schriftelijke knelpunteninventarisatie knelpunten aangedragen door aanpalende stakeholders inclusief patiëntenorganisaties. Een verslag kan opgevraagd worden bij secretariaat@kennisinstituut.nl.
Uitkomstmaten
Op basis van de internationale richtlijn en input uit de knelpuntenanalyse zijn door de werkgroepleden en de adviseur uitgangsvragen opgesteld. De internationale werkgroep heeft onderzocht welke uitkomstmaten voor de patiënt relevant zijn. De internationale werkgroep waardeerde deze uitkomstmaten volgens hun relatieve belang bij de besluitvorming rondom aanbevelingen. De Nederlandse werkgroep heeft volgens het stappenplan Adaptatietraject gevolgd en bepaald welke knelpunten er voor de Nederlandse situatie zijn om tot een Nederlandse versie van de internationale richtlijn te komen.
Methode literatuursamenvatting
De internationale werkgroep heeft aan de hand van specifieke zoektermen gezocht naar gepubliceerde wetenschappelijke studies in verschillende elektronische databases. Ook werd aanvullend gezocht naar studies aan de hand van de literatuurlijsten van de geselecteerde artikelen. In eerste instantie werd gezocht naar studies met de hoogste mate van bewijs. De internationale werkgroepleden selecteerden de via de zoekactie gevonden artikelen op basis van vooraf opgestelde selectiecriteria. De geselecteerde artikelen werden gebruikt om de uitgangsvraag te beantwoorden. De geselecteerde databases waarin is gezocht en de gehanteerde selectiecriteria zijn te vinden in de module met desbetreffende uitgangsvraag. De zoekstrategie is opvraagbaar bij de internationale richtlijnwerkgroep. De Nederlandse werkgroep zag geen aanleiding om de zoekstrategie van de internationale werkgroep aan te passen of te updaten.
Beoordelen van de kracht van het wetenschappelijke bewijs
De internationale werkgroep volgde bepaalde kracht van het wetenschappelijke bewijs volgens de GRADE-methode. GRADE staat voor ‘Grading Recommendations Assessment, Development and Evaluation’. De basisprincipes van de GRADE-methodiek zijn: het benoemen en prioriteren van de klinisch (patiënt) relevante uitkomstmaten, een systematische review per uitkomstmaat, en een beoordeling van de bewijskracht per uitkomstmaat op basis van de acht GRADE-domeinen (domeinen voor downgraden: risk of bias, inconsistentie, indirectheid, imprecisie, en publicatiebias; domeinen voor upgraden: dosis-effect relatie, groot effect, en residuele plausibele confounding). GRADE onderscheidt vier gradaties voor de kwaliteit van het wetenschappelijk bewijs: hoog, redelijk, laag en zeer laag. Deze gradaties verwijzen naar de mate van zekerheid die er bestaat over de literatuurconclusie, in het bijzonder de mate van zekerheid dat de literatuurconclusie de aanbeveling adequaat ondersteunt (Hultcrantz, 2017; Schünemann, 2013).

Bij het beoordelen (graderen) van de kracht van het wetenschappelijk bewijs in richtlijnen volgens de GRADE-methodiek spelen grenzen voor klinische besluitvorming een belangrijke rol (Hultcrantz, 2017). Dit zijn de grenzen die bij overschrijding aanleiding zouden geven tot een aanpassing van de aanbeveling. Om de grenzen voor klinische besluitvorming te bepalen moeten alle relevante uitkomstmaten en overwegingen worden meegewogen. De grenzen voor klinische besluitvorming zijn daarmee niet één op één vergelijkbaar met het minimaal klinisch relevant verschil (Minimal Clinically Important Difference, MCID). Met name in situaties waarin een interventie geen belangrijke nadelen heeft en de kosten relatief laag zijn, kan de grens voor klinische besluitvorming met betrekking tot de effectiviteit van de interventie bij een lagere waarde (dichter bij het nuleffect) liggen dan de MCID (Hultcrantz, 2017).

Adapteren van overwegingen
De Nederlandse werkgroep heeft de internationale ‘justifications’ kritisch beoordeeld en aangepast naar de Nederlandse situatie. Om te komen tot een aanbeveling zijn naast (de kwaliteit van) het wetenschappelijke bewijs ook andere aspecten belangrijk en worden meegewogen, zoals de expertise van de werkgroepleden, de waarden en voorkeuren van de patiënt, kosten, beschikbaarheid van voorzieningen en organisatorische zaken. Deze aspecten worden, voor zover geen onderdeel van de literatuursamenvatting, vermeld en beoordeeld (gewogen) onder het kopje ‘Overwegingen’.
Adapteren van aanbevelingen
De Nederlandse werkgroep heeft de internationale ‘recommendations’ kritisch beoordeeld en aangepast naar de Nederlandse situatie. De aanbevelingen geven antwoord op de uitgangsvraag en zijn gebaseerd op het beschikbare wetenschappelijke bewijs en de belangrijkste overwegingen, en een weging van de gunstige en ongunstige effecten van de relevante interventies. De kracht van het wetenschappelijk bewijs en het gewicht dat door de werkgroep wordt toegekend aan de overwegingen, bepalen samen de sterkte van de aanbeveling. Volgens de GRADE-methodiek sluit een lage bewijskracht van conclusies in de systematische literatuuranalyse een sterke aanbeveling niet a priori uit, en zijn bij een hoge bewijskracht ook zwakke aanbevelingen mogelijk. De sterkte van de aanbeveling wordt altijd bepaald door weging van alle relevante argumenten tezamen.
In de GRADE-methodiek wordt onderscheid gemaakt tussen sterke en zwakke (of conditionele) aanbevelingen. De sterkte van een aanbeveling verwijst naar de mate van zekerheid dat de voordelen van de interventie opwegen tegen de nadelen, gezien over het hele spectrum van patiënten waarvoor de aanbeveling is bedoeld. De sterkte van een aanbeveling heeft duidelijke implicaties voor patiënten, behandelaars en beleidsmakers (zie onderstaande tabel). Een aanbeveling is geen dictaat, zelfs een sterke aanbeveling gebaseerd op bewijs van hoge kwaliteit (GRADE-gradering HOOG) zal niet altijd van toepassing zijn, onder alle mogelijke omstandigheden en voor elke individuele patiënt.

Organisatie van zorg
In de knelpuntenanalyse en bij de ontwikkeling van de richtlijnmodule is expliciet aandacht geweest voor de organisatie van zorg: alle aspecten die randvoorwaarden zijn voor het verlenen van zorg (zoals coördinatie, communicatie, (financiële) middelen, mankracht en infrastructuur). Randvoorwaarden die relevant zijn voor het beantwoorden van deze specifieke uitgangsvraag zijn genoemd bij de overwegingen.
Commentaar- en autorisatiefase
De richtlijnmodules werden aan de betrokken (wetenschappelijke) verenigingen en (patiënt) organisaties voorgelegd ter commentaar. De commentaren werden verzameld en besproken met de werkgroep. Naar aanleiding van de commentaren werd de richtlijnmodule aangepast en definitief vastgesteld door de werkgroep. De definitieve richtlijnmodule werd aan de deelnemende (wetenschappelijke) verenigingen en (patiënt) organisaties voorgelegd voor autorisatie en door hen geautoriseerd dan wel geaccordeerd.
