Search date: October 27 and 31, and November 11, 2016.

Databases: OVID Medline, Pubmed, Embase, Cochrane Library, Cinahl (see appendix for search strings).

Search limits:

• Publication date: 2006-2016;
• English and Dutch only;
• Study design: systematic reviews, RCTs, observational studies.

a.   Excluded studies

For the 3 searches, 1916, 316 and 192 unique hits, respectively, were screened on title and abstract (Table 1). Of these, 1853, 310 and 192 were excluded. The most important reasons for exclusion were:

1. Other population: patients without cancer, or general cancer population
2. Other intervention: interventions other than those specified
3. Wrong study design: e.g. narrative reviews
4. Of the remaining 69 papers, the full-text was retrieved. Based on the full-text, all 69 papers were excluded. Table 2 provides an overview of the excluded studies.
